Anterior Surgical Approach in Recurrent Cervical Neurenteric Cyst - Case Report - (재발한 경추 신경장성 낭종의 전방경유 치험 - 증 례 보 고 -)

  • Intraspinal neurenteric cyst is rare congenital lesion derived from disorder of notochord formation. Most of these are located ventral to the spinal cord and conventional posterior approach is considered to be effective method as initial treatment modality. This cyst can recur, but the risk of reccurence after partial removal through long term follow-up has not been determined. We experienced one case of cervical neurenteric cyst which recurred after partial removal through laminectomy. The magnetic resonance imaging and postmyelography computerized tomography revealed an intradural extramedullary cystic lesion anterior to the cervical cord at the fifth cervical vertebra level. We performed anterior cervical corpectomy and cyst was totally removed. The patient's neurological symptom was improved postoperatively. Neurenteric cyst located ventrally to the cervical spinal cord should be removed through anterior route for direct visualization of the relationship between the cyst wall and the spinal cord.

Capillary Hemangioma of the Thoracic Spinal Cord

  • Capillary hemangiomas are common soft tissue tumors on the skin or mucosa of the head and neck in the early childhood, but very rare in the neuraxis. A 47-year-old man presented with one month history of back pain on the lower thoracic area, radiating pain to both legs, and hypesthesia below 17 dermatome. Thoracic spine MRI showed $1{\times}1.3{\times}1.5\;cm$, well-defined intradural mass at T6-7 disc space level, which showed isointensity to spinal cord on T1, heterogeneous isointensity on T2-weighted images, and homogeneous strong enhancement. The patient underwent T6-7 total laminotomy, complete tumor removal and laminoplasty. Histologically, the mass showed a capsulated nodular lesion composed of capillary-sized vascular channels, which were tightly packed into nodules separated by fibrous septa. These features were consistent with capillary hemangioma.

  • Ferrokinetcs by Pollycove's method on 2 cases of Chronic myelogenous leukemia were presented in order to study the erythropoietic function and pathogenesis of anemia. The results are as follows. (1) The daily hemoglobin syntheses were slightly elevated. Both the mean effective hemoglobinization time and mean erythron life span were shortened. The maximum net ROC incorporations of $^{59}Fe$ weere decreased. The in vivo surface measurements of radioactivity over liver, spleen, and sacral bone marrow revealed findings consistant with hemolysis and extramedullary hematopoies. (2) Based upon those findings it is assumed that anemia in chronic myelogenous leukemia may result from hemolysis and inadequate compensation to the developing anemia.

Chronic Ovine Copper Toxicosis in Korea (국내에서 발생한 면양의 만성 동중독증 예의 관찰)

  • Ten sheeps from one farm had sudden onset of anorexia, jaundice and hemoglobinuria and died within 3 days after showing clinical signs during 3 months period. Postmortom examination was performed on one case and copper concentrations in the livers kidney and serum of the necmpsied minim were analysed. Grossly, the conjunctive, subcutaneous tissue and abdominal fat were generally icteric. The liver was enlarged with yellowish orange in color. The kidney was enlarged with dark red in color and the urinary bladder was filled with dark red urine. Histopathologically, centrilobular hepatocellular necrosis, neutrophilic infiltrations bile stasis and aggregation of fine granules-laden macrophages in the portal area were noted in the liver. Most of the Bowman's spaces and renal tubules were filled with homogenous eosinophilic fluid. Extramedullary hematopoiesis was noted in the submandibular lymph node. Copper concentrations in serum, liver and kidney of the necropsied animal were 25.0, 2732.8 and 471.3 ppm respectively. Based on the gross and histopathological findings and the high copper concentrations in the organs, this case was diagnosed as chronic copper toxicosis. Possible etiology on this outbreak is also discussed.

Subacute Toxicity of Recombinant Human Erythropoietin (rHu-EPO) in Beagle Dogs (비글개에서 인체 재조합 적혈구 조혈인자, rHu-EPO의 아급성정맥독성시험)

  • A recombinant human erythropoietin (rHu-EPO), was administered intravenously to beagle dogs at doses of 100, 500 and 2, 500IU/kg/day for 30 days. There were no significant clinical signs such as body weight, food intake, physical and opthalmic examination, urine analysis, etc. Any toxic response was not observed except for enlarged spleen and extramedullary hematopoiesis. No observed adverse effect level (NOAEL) of rHu-EPO for 30 days was considered to be 100IU/kg/day in beagle dogs.

  • The subchronic toxicity study of rHuEPO, a newly developed recombinant erythropoietin, was investigated for 13 weeks in Beagle dogs intravenously treated with doses of 100, 500 and 2,500 lU/kg/day. There were no significant changes in body weight, food intake, physical and opthalmic examination, urine analysis, etc. Any toxic response was not observed except for enlarged spleen and extramedullary hematopoiesis. These results indicate that the no-observed adverse effect level (NOAEL) of rHuEPO is 100 lU/kg in Beagle dogs.

Intraparenchymal Myeloid Sarcoma and Subsequent Spinal Myeloid Sarcoma for Acute Myeloblastic Leukemia

  • Myeloid sarcoma is a solid, extramedullary tumor composed of leukemic myeloblasts or immature myeloid cells. Intraparenchymal myeloid sarcoma without the involvement of the skull or meninges is extremely rare. Here, we present the case of a 49-year-old man who developed intraparenchymal myeloid sarcoma on the left cerebellum after allogeneic bone marrow transplantation (BMT). He received radiotherapy after complete removal of intraparenchymal myeloid sarcoma, but he was diagnosed spinal myeloid sarcoma three month later. Nine months after the operation, new intracranial and spinal myeloid sarcoma were diagnosed and the patient's condition had been worsened rapidly. Although the spinal myeloid sarcoma was not histologically diagnosed, this report provides valuable insights into the clinical course of progression of intraparenchymal myeloid sarcoma.

Cervical Schwannoma Presenting with Acute Intracranial Subarachnoid Hemorrhage

  • A subarachnoid hemorrhage (SAH) associated with negative finding on four-vessel angiography is seen in 5 to 30% of patients with intracranial SAH. A previously silent lesion in the spinal canal may be responsible for the angiographically negative finding for cause of intracranial SAH. We report a case of upper cervical (C1-2) intradural schwannoma presenting with acute intracranial SAH. Repeated cerebral angiographic studies were negative, but cervical magnetic resonance imaging study and tissue pathology revealed a intradural-extramedullary schwannoma in C1-2 level. This case illustrates the importance of a high index of clinical suspicion for spinal disease in angiographically negative intracranial SAH patients.

  • Primary melanotic schwannoma in spinal cord is a very uncommon disorder, eight cases of melanotic schwannoma in spinal cord have been found in the literature. We present a case report of a patient with an intradural, extramadullary melanotic schwannoma in cervical spine and the literature on melanotic schwannoma is reviewed. The proposed theories on the origin of these tumors and treatment are discussed.
