견갑골 내측 상부의 발통점에 체외충격파와 초음파 적용시 통증과 피부두께, 근력변화에 미치는 영향 (The Effects of ESWT and US on Muscle Power, Pain and Muscle Tension at Trigger Point of the Scapular Inter Angle)

  • Purpose : This study aims to investigate into the change in muscle power, pain and muscle tension upon application of extracorporeal shock wave therapy (ESWT) and ultrasound (US) at trigger point (TrP) of the Scapular inter angle. Methods : In total of 24 human subjects that experience pain upon stimulation at the scapular inter angle were recruited. They were divided into three groups: control, ESWT and US group. Each group was subjected to appropriate stimulation three times per week for four weeks except the control group which had no restrictions in lifestyle. The ESWT group was subjected to Extracorporeal Shock Wave Therapy with a 2 $cm^2$ head, 20 times at speed of application of 60 pulses/min and depth of intrusion of 25 mm Whereas the US group was subject to Ultra sound therapy with 1$cm^2$ head, 1 MHz in frequency, 1.5 W/$cm^2$ of strength with five minutes of continuous application. Measurements were performed immediately prior to and at the end of the experiment. Results : There was no statistical significance in the change in muscle tension and muscle power as well as the difference between both groups before and after the experiment. VAS and VRS, all of which indicates changes in pain level, had statistically significant differences between them in pretest posttest comparisons. However, ESWT was superior to US in terms of decreasing muscle tension and pain while increasing muscle power. Conclusion : This study has made it evident that ESWT is far more reasonable to apply to TrP.

Extracorporeal Shock Wave Therapy for Hypertrophic Scars

  • Background Hypertrophic scars cause aesthetic concerns and negatively affect the quality of life. A gold standard treatment for hypertrophic scars has not been established due to various responses of modalities. Extracorporeal shock wave therapy (ESWT) is a noninvasive and affects scar remodeling by fibroblast regulation. This study investigated the effectiveness of ESWT for hypertrophic scars. Methods Twenty-nine patients were enrolled. All patients underwent ESWT once a week for 6 consecutive weeks. Their scars were assessed using the Patient and Observer Scar Assessment Scale (POSAS), erythema index, melanin index, and scar pliability before treatment and again 4 weeks after treatment completion. Results Thirty-four hypertrophic scars in this study had persisted for between 6 months and 30 years. Most scars developed after surgical incision (55.88%). The chest and upper extremities were the predominant areas of occurrence (35.29% each). Most of the POSAS subscales and total scores were significantly improved 4 weeks after treatment (p < 0.05). Furthermore, the pain, itching, and pigmentation subscale were improved. The pliability, melanin index, and erythema index were also improved, but without significance. The patients were satisfied with the results and symptoms alleviation, although subjective score changes were insignificant. No serious adverse events were found. The patients reported pruritus in 62.5% and good pain tolerance in 37.5%. Subgroup analyses found no differences in scar etiologies or properties at different parts of the body. Conclusion The ESWT is a modality for hypertrophic scar treatment with promising results. Most of POSAS subscales were significantly improved.

체외충격파치료가 주관절 외측상과염 환자의 손목 신전근의 근력과 통증에 미치는 영향 (Effect of Wrist Extensor Strength and Pain on Extracorporeal Shock Wave Therapy of the Lateral Epicondylitis)

  • Purpose : The purpose of this study was to assess the effectiveness of Extracorporea Shock-wave therapy(ESWT) on the pain and improvement of wrist extensor strength which is one of the available medical treatment in patients with lateral epicondylitis. Object and Methods : The subjects of this study were consisted of 15 patients with lateral epicondylitis, These medical treatments was conducted total 6 time for 3 weeks. In the ESWT application group. To investigate the effectiveness of the treatment after ESWT treatment, Pain was measured by visual analogue scale(VAS) which is divided into 10 ranks and wrist extensor strength was measured by Aneroid phygmomanometer Results : 1. It was showed that VAS and wrist extensor strength before treatment and after two treatment(1 week) were no significant differences(p<0.05). 2. It was showed that VAS and wrist extensor strength before treatment and after four treatment(2 week) were significant differences(p<0.05). 3. It was showed that VAS and wrist extensor strength before treatment and after six treatment(3 week) were significant differences(p<0.05). Conclusion : We think that ESWT for the patients with lateral epicondylitis. is effective to wrist extensor strength and decrease of pain.

  • Lateral epicondylopathy represents pain in the lateral (radial aspect) of the elbow caused by degeneration in the common extensor tendon. Calcium deposit frequently developes in lateral epicondylopathy, with the prevalence up to 46%. There are considerable debates on its treatment protocols for lateral epicondylopathy so far, likewise, the effective treatment method for calcific lateral epicondylopathy has not been established. We report here a case of chronic intractable calcific lateral epicondylopathy treated with ultrasound-guided barbotage and focused type extracorporeal shock wave therapy (ESWT). To our knowledge, this is the first report of calcific lateral epicondylopathy treated by ultrasound-guided barbotage combined with ESWT.

주관절 외상과염의 체외 충격파 시술에서 에너지량에 따른 치료 효과 - 전향적 무작위적 이중 맹검 대조군 연구 - (Dose-related Effect of Extracorporeal Shock Wave Therapy for Lateral Epicondylitis - Prospective Randomized Double Blind Comparative Study -)

  • 목적: 주관절 외상과염 환자에서 체외 충격파 시술의 에너지량에 따른 치료 효과를 전향적 무작위적 이중 맹검 비교 방법으로 분석해 보고자 하였다. 대상 및 방법: 6개월 이상의 비수술적 치료로 호전이 없어, 체외 충격파 시술을 시행한 주관절 외상과염 환자들을 무작위적으로 각각 15명씩 저에너지군과 고에너지군으로 나누어 전향적 이중 맹검법으로 분석하였다. 모든 환자들은 1주 간격으로 총 3회 동안 체외 충격파 시술을 시행받았고, 평균 에너지량은 저에너지군이 $0.12 mJ/mm^2$, 고에너지군이 $0.24 mJ/mm^2$ 이었다. 시술 전과 시술 1개월, 3개월, 6개월에 각각 Upper extremity function score, Mayo elbow score를 측정하여 임상적 결과를 비교하였다. 결과: 체외 충격파 시술 후 저에너지군과 고에너지군 모두에서 유의한 임상적 호전을 보였으며, 특히 시술 후 6개월에서 고에너지군이 통증의 호전이 유의하게 높았다(p=0.019). 체외 충격파 치료에 의한 치료 효과는 1개월 내의 단기 효과보다는 시술 후 1개월에서 6개월 사이에 주로 나타났다. 결론: 주관절 외상과염에서 체외충격파 치료는 에너지량에 상관없이 모두 유의한 치료 효과를 보였고, 특히 고에너지의 사용이 통증 조절에 보다 효과적이었다.

탄도형 충격파 치료기의 음향 출력 시험을 위한 Dry Test Bench의 신뢰성 및 유용성 (Reliability and utility of a Dry Test Bench for testing the acoustic output from a ballistic shock wave therapeutic device)

  • 탄도형 체외 충격파 치료기의 출력 에너지를 측정하는 Dry Test Bench(DTB)의 신뢰성을 검증하기 위해, 상용 탄도형 충격파 치료기에 대해 Laser Doppler Vibrometer(LDV)로 측정된 충격파의 음향 에너지와 비교했다. 실험 결과, DTB로 측정된 역학적 에너지는, 동일한 출력 설정에서, 5 % 이내의 변동성을 보이며, 치료기의 전 출력 설정 범위에서 LDV로 측정된 충격파의 음향 에너지와 선형적인 상관성(adj. R2 = 0.991)을 확인했다. 두 측정 방법의 상관성과, LDV을 이용하여 공기 및 수중에서 측정된 충격파 음향 에너지의 상관성(adj. R2 = 0.995)을 통합하면, DTB 측정으로부터 수중에서 발생된 충격파의 energy flux density를 평균 7.85 % 오차로 추정된다. DTB는 치료기의 출력에너지에 대한 정보만 제공하기 때문에, IEC61846 및 IEC63045에서 요구하는 다양한 충격파의 음향 출력을 시험하는 도구로 적합하지 않다. 그러나 측정 원리가 단순하고 사용이 용이한 DTB는 제조사 및 사용자가 탄도형 Extracorporeal Shock Wave Therapy(ESWT) 치료기의 성능을 관리하는 목적으로 유용하게 사용될 수 있을 것으로 기대된다.

체외충격파 치료가 동결견 환자의 통증과 견관절 기능에 미치는 영향 (Effect of Pain and Shoulder Joint Function on Extracorporeal Shock Wave Therapy of Patients with Frozen Shoulder)

  • Purpose: The purpose of this study was to assess the effectiveness of Extracorporea Shock - Wave Therapy (ESWT) on the pain and improvement of shoulder joint function which is one of the available medical treatment in patients with frozen shoulder. Object and Methods: 26 patients with frozen shoulder, with 26 patients assigned to two groups; a control treatment group (n=12) and a ESWT application group (n=14). In the ESWT application group, the patients received general physical therapy, and then they received ESWT. In the control treatment group, the patients received only general physical therapy. These medical treatments was conducted total 6 time for 3 weeks. To investigate the effectiveness of the treatment after the each group's treatment. Pain was measured by Visual Analogue Scale (VAS) which is divided into 10 ranks and shoulder joint function was measured by CSA(Constant Shoulder Assessment scale). Results: 1. There was a statistical significance of measured by shoulder pain degree in both group but, the result of a ESWT application group shows more significance than that of a control treatment group. 2. There was a statistical significance of measured by CSA in both group but, the result of a ESWT application group shows more significance than that of a control treatment group. Conclusion: We think that treatment with General physical therapy and ESWT together for the patients with frozen shoulder is more effective and more safe method to shoulder joint function improvement and decrease of pain.

유방암 환자의 복합 상지병증에서 체외충격파치료의 유용성 (Feasibility of Extracorporeal Shock Wave Therapy for Complex Upper Limb Morbidity in Breast Cancer Patient)

  • Upper limb morbidity including shoulder pain, lymphedema, adhesive capsulitis, myofascial pain syndrome, and limited range of motions (LROMs) occurs frequently in breast cancer patients. Especially restricted upper limb motion aggravated by chronic lymphedema results in functional limitation and impaired activities of daily living (ADL). However, conventional rehabilitation therapy including complex decongestive therapy and stretching exercise is insufficient for managing advanced breast cancer patients with complex upper limb morbidity. Here, we report a case of breast cancer-related chronic lymphedema in a 62-years old female with severe LROM in all joints of the affected upper extremity that caused severe disability in ADL. Significant improvements in range of motion and upper extremity function were observed after serial extracorporeal shock wave therapy (ESWT), combined with conventional rehabilitation therapy. Therefore, ESWT might be recommended for managing the extensive limited joint motion which causes functional decline in a patient of chronic lymphedema with trophic skin change.

장시간 스마트폰 사용으로 VDT증후군을 가진 20대 성인의 체외충격파적용이 목-어깨의 복합적 통증지수에 대한 효과 (The Effects of the Extracorporeal Shock Wave Therapy on Complex Pain Scales of Neck-Shoulder in 20s Adults with VDT Syndrome due to prolonged Smartphone Use)

  • 본 연구는 스마트폰을 장시간 사용으로 VDT증후군(Visual Display Terminal Syndrome)을 가진 20대 성인들을 대상으로 체외충격파 치료(ESWT)를 통해 목-어깨의 근육의 복합적 통증지수에 미치는 영향을 알아보고자 한다. VDT증후군으로 목-어깨통증을 가지고 있는 대상자 20명을 무작위로 분류해서 실험군 10명과 대조군 10명으로 나눠서 실험에 참여하였다. 실험대상자들은 4주간, 주 5일간 실시하였다. 실험군은 1회 5 Hz, 2.0 bar의 강도로 체외충격파치료를 2000회씩 실시하였고, 대조군은 1회 30분간 자가신장운동을 실시하였다. 실험대상자들은 실험전과 실험후에 VAS와 PPT로 목-어깨통증을 측정하여 비교하였다. 자료분석은 SPSS win 18.0으로 실험전과 실험후에 실험군과 대조군의 결과값을 분석하기 위해 독립비교를 실시하였다. 실험 후 실험군이 목-어깨근육의 VAS, PPT에서 많이 감소하였다. 하지만 대조군은 큰 차이가 없었다. 이런 결과를 통해 체외충격파 치료는 치료전보다 치료후에 통증감소가 나타났지만 자가신장운동은 치료후 통증감소의 차이가 없었다. 앞으로도 VDT증후군의 원인으로 목-어깨통증을 감소시킬 수 있는 다양한 치료법으로 이용될 것으로 사료된다.

Applying Focused and Radial Shock Wave for Calcific Tendinitis of the Shoulder : Randomized Controlled Study

  • Objective: Extracorporeal shock wave therapy (ESWT) is a nonsurgical treatment alternative to surgery for various musculoskeletal diseases that have traditionally been difficult to treat conservatively, including calcific tendinitis, tennis elbow, and plantar fasciitis. This study evaluated the effect of focused and radial shock wave therapy for calcific tendinitis of the shoulder. Design: Randomized controlled study Methods: Forty participants with calcific tendinitis were randomized into focused shock wave therapy (FSWT, n=20) and radial shock wave therapy (RSWT, n=20) groups. Patients were examined before and one week after treatment. Pain intensity was subjectively assessed using the visual analogue scale and function was assessed using the Constant-Murley score (CMS) and range of motion (ROM). Results: The results showed a significant decrease in pain and significant increase in shoulder mobility and function in both groups. However, FSWT was significantly more effective than RSWT, based on CMS and ROM assessment. Conclusions: Although it is possible to raise the energy intensity of RSWT to increase the depth at which the energy becomes dispersed, higher energy intensity is associated with a greater risk of severe neurovascular damage, and that high-intensity stimulation can cause adverse effects such as pain and petechiae. Therefore, FSWT is considered to be a safe and effective method for treating tendinous lesions while minimizing adverse effects. In conclusion, both FSWT and RSWT can reduce pain and increase mobility and function. FSWT can be considered as an alternative for calcific tendinitis of the shoulder.
