Objective: Extracorporeal shock wave therapy (ESWT) is a nonsurgical treatment alternative to surgery for various musculoskeletal diseases that have traditionally been difficult to treat conservatively, including calcific tendinitis, tennis elbow, and plantar fasciitis. This study evaluated the effect of focused and radial shock wave therapy for calcific tendinitis of the shoulder. Design: Randomized controlled study Methods: Forty participants with calcific tendinitis were randomized into focused shock wave therapy (FSWT, n=20) and radial shock wave therapy (RSWT, n=20) groups. Patients were examined before and one week after treatment. Pain intensity was subjectively assessed using the visual analogue scale and function was assessed using the Constant-Murley score (CMS) and range of motion (ROM). Results: The results showed a significant decrease in pain and significant increase in shoulder mobility and function in both groups. However, FSWT was significantly more effective than RSWT, based on CMS and ROM assessment. Conclusions: Although it is possible to raise the energy intensity of RSWT to increase the depth at which the energy becomes dispersed, higher energy intensity is associated with a greater risk of severe neurovascular damage, and that high-intensity stimulation can cause adverse effects such as pain and petechiae. Therefore, FSWT is considered to be a safe and effective method for treating tendinous lesions while minimizing adverse effects. In conclusion, both FSWT and RSWT can reduce pain and increase mobility and function. FSWT can be considered as an alternative for calcific tendinitis of the shoulder.
It is an important issue that cavitation is always deal with shock wave. During operation with ESWL(Extracorporeal Shock Wave Lithotriptor), the cavitation bubbles are generated at the focal region. This phenomenon is an important problem because the cavitation bubbles have a harmful effect on the disintegration ability and the human body. In this paper, therefore, we investigate the relation between the cavitation and the radiated sound due to the ESWL.
Purpose: The purpose of this study was to investigate the effects of extracorporeal shock-wave therapy (ESWT) on pain, grip strength, and upper-extremity function in patients diagnosed with lateral epicondylitis and to provide an effective intervention method for lateral epicondylitis. Methods: Twenty patients with lateral epicondylitis were randomly assigned to the ESWT group (n = 10) and the stretching exercise group (n = 10). Interventions in both groups were performed six times twice a week for three weeks. The visible analog scale (VAS) was used to measure pain change. A dynamometer was used to measure grip strength (GS). Patient-rated tennis elbow evaluation (PRTEE) was used to measure the upper-extremity function. Results: There were significant differences in pain, grip strength, and upper-extremity function in both groups before and after intervention (p < 0.05). There were also significant differences in pain, grip strength, and upper-extremity function between the groups after intervention (p < 0.05). Conclusion: This study showed very positive improvement in pain, grip strength, and upper-extremity function after ESWT in patients with lateral epicondylitis. Therefore, ESWT can be recommended for patients with lateral epicondylitis.

The incidence of heterotopic ossification (HO) was reported to be higher in combat-injured patients than in civilian trauma patients. HO is often considered a possible cause of residual limbs pain in amputee. Here, we report the case of a 21-year-old male, who underwent a traumatic right transfemoral and left transtibial amputation with two segments of painful HO around his left amputation site. We report the effect of extracorporeal shock wave therapy (ESWT) on size and pain associated with HO. After ESWT, the visual analog scale score decreased from 5~6 to 0~1 and the size of two masses decreased from 13.1 × 6.7 mm and 12.5 mm to 11.9 × 4.7 mm and 12.2 mm, respectively. To the best of our knowledge, this is the first case that has reported on the treatment of HO using ESWT for a traumatic transtibial amputation patient. The case suggests that ESWT could serve as a complementary treatment for HO in traumatic amputation patient.
Objective: The purpose of this study is to compare the effects of extracorporeal shock wave therapy (ESWT) and injection therapy through qualitative and quantitative analysis by synthesizing randomized controlled trials (RCTs) conducted on patients with various shoulder diseases. Design: A systematic review and meta-analysis of randomized controlled trials Methods: This review conducted a literature search through international electronic databases in January 2023 to compare the effects of ESWT and injection therapy. Qualitative analysis was performed as a risk of bias tool, and quantitative analysis was synthesized with a random effect model to show the effect size as a standardized mean difference (SMD). Results: Five RCTs involving 298 individuals with shoulder disorders were included in this systematic review and meta-analysis. ESWT showed a moderate effect on pain (SMD=-0.44; 95% CI, -0.95 to 0.06) and a large effect on shoulder function (SMD =-0.81; 95% CI, -1.70 to 0.07) than injection therapy. A significant difference was found in the shoulder range of motion, showing a large effect size (SMD=1.50; 95% CI, 0.58 to 2.43). Conclusions: When considering treatment options for the management of patients complaining of shoulder disorders, it is appropriate to recommend ESWT first rather than injection therapy to increase the range of motion of the joint. In addition, ESWT is safe for pain control and shoulder function improvement, and a positive prognosis could be expected.

Calcific tendinitis is characterized by inflammation around calcium hydroxyapatite crystal deposits. Minimally invasive extracorporeal shock wave therapy (ESWT) has been postulated to be an effective treatment option for treating calcific tendinitis. In clinical practice, shock waves usually are aimed at the painful area after palpation and not focused. It has been known that exact fluoroscopic focusing of ESWT at the calcific deposit for treatment of calcifying tendinopathy is highly effective. Ultrasound is a simple, inexpensive and radiation-free diagnostic tool that has been used to demonstrate tendinopathy including calcific tendinitis. However, focusing of shock wave under ultrasound is less well established. We present a patient in whom large calcific tendinitis of gluteus medius was completely resolved by exact focusing of ESWT by ultrasound with literature review.

Purpose : The purpose of this study was to examine the effects of tools (i., extracorporeal shock wave therapy, massage gun, and foam roller) on range of motion, muscle tone and pain threshold among patients with hamstring stiffness. Methods : Fourteen participants with hamstrings stiffness were recruited. Interventions were performed 6 times, and each session was for 30 seconds using the three tools. The range of motion, muscle tone, and pain threshold were measured. The order of the use of the three tools was randomly determined. The foam roller was made to move from the bottom of the hip crease to the upper part of the back of the hamstring. Additionally, velocity 5 vibration stimulation was performed on the hamstring using a massage gun. Moreover, vibration stimulation was performed on the hamstring with extracorporeal shock wave therapy 5 minutes, 5 Hz, and 1,500 strokes. The flexibility of the posterior thigh muscle was based on maintaining the knee and hip joints in a 90 ° bend in the supine position. The joint angle of the knee was measured, when the knee was actively extended, at the maximum point where the posterior thigh muscle was stretched. The elasticity of the posterior thigh muscle was measured while the subject was prone and in a relaxed state without any force. Measurements were made at the muscle abdominal area of the semitendinosus muscle of the posterior femur, and the area to be measured was marked with a pen. The measurement of the tenderness threshold of the posterior femur was measured using a tenderness meter(Commander Algometer, J-Tech, USA). The force value at the point at which the pressure sensation change to pain was measured after applying vertical pressure to the posterior femur muscle, which was the halfway point between the ischial tuberosity and the popliteal surface of the subject lying on their stomach. Results : The extracorporeal shock wave therapy increased stiffness and, muscle tone, and caused changes in the pain threshold, whereas the other two tools had no effect on these indices. Conclusion : Extracorporeal shock wave therapy has important effects on range of motion and muscle stiffness and can be used in warmup protocols.
The purpose of this study was to determine the effect of extracorporeal shock wave therapy for the neck-shouler muscles of complex pain scales of the 20s adults with VDT Syndrome to prolonged smartphone use. Twenty subjects with neck pain due to VDT syndrome participated in the experiment. They were randomly divided into 10 experimental groups and 10 control groups. Experimental subjects were conducted for 4 weeks, 5 days a week. The experimental group was subjected to extracorporeal shock wave therapy 2000 times at an intensity of 5 Hz and 2.0 bar once, and the control group performed self-stretching exercise for 30 minutes once. Subjects measured and compared neck-shoulder pain with VAS and PPT before and after the experiment. Data analysis was performed with SPSS win 18.0, and correspondence comparison was performed to analyze the results of the experimental group and the control group before and after the experiment. After the experiment, the experimental group decreased more in VAS and PPT of the neck-shoulder muscle. but the control group was no difference. Though these results, extracorporeal shock wave therapy was showed a reduction in pain after treatment than before treatment but there are no difference in the pain reduction after self-stretching exercise. Thus, extracorporeal shock wave therapy is expected to be used as a variety of treatments to reduce neck-shoulder pain as a cause of VDT syndrome.

In order to verify the reliability of Dry Test Bench (DTB) used for testing the output energy from ballistic extracorporeal shock wave therapeutic devices, the measurements with DTB were compared with the acoustic energy measured with a Laser Doppler Vibrometer (LDV) for a commercial ballistic ESWT device. It was shown that the mechanical energy detected with DTB had variability maintained within 5 % at the same output power setting and also had a linear correlation (adj. R2 = 0.991) with the acoustic energy measured with the LDV for the entire output power settings. Using the correlation between the two methods and the correlation on the acoustic energy measured in between air and water with the LDV, the DTB measurement can be used to estimate the energy flux density in water with an average error of 7.85 % for the entire output power settings of the ballistic shock wave generator considered in the experiment. DTB provides information limited to the output mechanical energy and therefore it is not suitable for testing the various acoustic output parameters required in IEC61846 and IEC63045. However, DTB that is simple in measurement principles and easy to use is expected for manufacturers and clinical users to monitor the performance of ballistic Extracorporeal Shock Wave Therapy (ESWT) devices.

Survey was carried out on the acoustic outputs from 12 shock wave fields produced by the 10 extracorporeal shock wave lithotriptors whose technical documents are available, among the 33 devices approved by the Ministry of Food & Drug Safety (MFDS).The results show that the acoustic outputs (P+, P-, efd, and E), critical to the therapeutic efficacy and the patient safety, are largely different between the devices. The maximum values of P+, P-, efd, and E vary up to 2.08, 3.72, 3.89, and 15.98 times, respectively. The acoustic output parameters are not thoroughly provided in the technical documents, and some of data (eg. efd) are suspected to be abnormal outside usual ranges. The large device to device differences in the shock wave outputs are likely to undermine equivalence between the ESWL devices approved for the same indication. To verify the reliability of the data in the technical documents of the approved devices and to confirm if the acoustic outputs from the devices in clinical use are the same as those in their technical documents, an authorized test laboratory should be available. A postapproval monitoring led by the regulatory agency is suggested to maintain the acoustic outputs from the ESWL devices that suffer from degrading in performance due to aging.
