Although breast implants have been in clinical use for almost 6 decades and have undergone considerable development during this time, implant rupture is still a dreaded long-term complication. Some obvious external factors, such as trauma, can lead to implant rupture, but many studies have reported a high rate of "spontaneous" implant rupture. Herein, we present two cases with the aim of raising awareness of a new possible cause of "spontaneous" implant rupture: mechanical irritation by bony protrusions.
Purpose: This study was to propose the clear understanding for stress distribution of supporting bone by use of staggered buccal offset tripodal placement of fixtures of posterior 3 crown implant partial dentures. We realized posterior 3 crown implant fixed partial dentures through finite element modeling and analysed stress effect of implant arrangement location to supporting bone under external load using finite element method. Method: To understand stress distribution of 3 crown implant fixed partial dentures which have 2 different arrangement by finite element analysis. In each model, for loading condition, we applied $45^{\circ}$ oblique load to occlusal surface of crown and applied 100 N for 3 crown individually(total 300 N) for imitating possible oral loading condition. at this time, we calculated Von Mises stress distribution in supporting bone through finite element method. Result: When apply $45^{\circ}$ oblique load to in-line arrangement model, maximum stress result for 100 N for each 3 crown 47.566MPa. In tripodal placement, result for 1mm buccal offset tripodal placement implant model was maximum distributed load 51.418MPa, so result was higher than in-line arrangement model. Conclusion: In stress distribution result by placement of implant fixture, the most effective structure was in-line arrangement. The tripodal placement does not effective for stress distribution, gap cause more damage to supporting bone.
The purpose of this paper was to investigate the significance of splinted and non-splinted implant-supported restorations with an internal connection for multiple consecutively missing teeth. Upon examination of the effects of fixture-abutment connection, the distribution of occlusal load was favorable in splinted implant-prosthesis with an external connection, but effect of strain distribution was not significant in splinted implant-prosthesis with an internal connection. In splinted implant-prostheses for short implants, strain distribution was not affected by the method of retention. For cement-retained prostheses, the effect of strain distribution due to splinting was not significant. In clinical studies, non-splinted prostheses with an internal connection for multiple consecutively missing teeth showed high survival rate, mild marginal bone loss, and stable periodontal condition. However, failure to achieve optimal proximal contact between single-unit prostheses may lead to food impaction, and veneer fracture may be inevitable when the framework provides inadequate support in the proximal region. In conclusion, splinted implant-prosthesis is not an indication in all cases, and clinical consideration of its use should be based on the patient's oral condition, such as location and number of implants, formation of proximal contact, canine guidance, existence of parafunctional habit, and oral hygiene, when multiple consecutively missing teeth are replaced by internal connection type implant.

A comparative study of stress distributions in the maxillary bone with three different types of abutment was conducted. Finite element analysis was adopted to determine stress generated in the bone with the different implant systems with micro threads (Onebody type implant, Internal type implant, and External type implant). It was found that the types of abutments and the number of micro threads have significant influence on the stress distribution in the maxillary bone. They were due to the difference in the load transfer mechanism and the size of contact area between abutment and fixture. Also the maximum effective stress in the maxillary bone was increased with increasing inclination angle of load. It was concluded that the maximum effective stress in the bone was the lowest by the internal implant among the maximum effective stresses by other two types of implants and by appropriate number of micro threads, and that the specific number of micro thread was existed to decrease the maximum effective stress in the maxillary bone due to different implant systems and loading conditions.
Purpose: Patients who treated implant immediate loading within a week after implant placement at Wonkwang University Dental Hospital Implant Center were evaluated marginal bone resorption. These retrospective analyses are intended to reinforce the clinical evidence for the implant immediate loading. Materials and methods: Medical history and radiographic data were investigated, which were the patients' who treated implant immediate loading and restoration with provisional prostheses between January 2005 and June 2016, at Wonkwang University Dental Hospital Implant Center. Total number of implants was 70, marginal bone resorption was measured according to implant length, diameter and connection type. To measure marginal bone resorption, periapical radiographs were taken when the implants were placed and after 6 month. Statistical analysis was done in Mann-whitney U test and Kruskal-wallis test with SPSS 22.0 software (P<.05). Results: Mean marginal bone resorption around immediately loaded implants according to implant connection type was shown $1.24{\pm}0.72mm$ in internal hexagon connection type and $1.73{\pm}1.27mm$ in external hexagon connection type. There was no statically significant difference in marginal bone resorption with implant length and diameter. Conclusion: Implants with immediated loading in internal hexagon connection type showed less marginal bone resorption significantly than in external hexagon connection type.

Objectives: In this study, we determined the incidence and pattern of screw loosening in patients who received dental implants. Materials and Methods: Patients who received implants between January 2008 and October 2013 and completed their prosthetic rehabilitation were evaluated for the incidence, frequency, and onset of screw loosening using dental charts and radiographs. The association between each factor and screw loosening was analyzed using the chi-square test and a multivariate analysis with binary logistic regression models (P<0.05). Results: Total 1,928 implants were placed in 837 patients (448 males, 389 females), whose follow-up period after loading varied from 0.25 to 70 months (mean period, 31.5 months). Screw loosening occurred in 7.2% of implants. Most cases occurred less than six months after loading. Among those, 22.3% experienced recurrent screw loosening. Screw loosening was most common in the molar region (8.5%) and frequently associated with an implant diameter of ≥5 mm (14.2%). External implant-abutment connections (8.9%) and screw-retained implant prostheses (10.1%) showed higher incidence of problems than internal implant-abutment connections and cement-retained implants, respectively. Screw loosening was most common in implant prostheses with single crowns (14.0%). Conclusion: Within the limits of the current study, we conclude that the incidence of screw loosening differs significantly according to the position of implant placement, the type of implant and manufacturer, implant diameter, the type of implant-abutment connection, the type of retention in the implant prosthesis, and the type of implant prosthesis.
Purpose: In this study, the loosening torque test was conducted with three implant products that are produced, approved and sold in Korea, which are manufactured in different fixture and abutment tightening methods (internal submerged type, internal morse taper type, and external type) to examine the loosening torque of the screw according to the method of tightening the implant fixture and abutment. Methods: In the loosening torque test, the three types of fixtures and abutments with different tightening methods were tightened by rotating them clockwise with a $30N{\cdot}cm$ force using a driver equipped with an electric torque meter. The results of the test are as follows. Results: The loosening torque values of the internal submerged type, internal morse taper type and external type implants were $24.10{\pm}0.742N{\cdot}cm$, $29.10{\pm}1.710N{\cdot}cm$, and $26.60{\times}1.636N{\cdot}cm$, respectively. Conclusion: The screw loosening torque values of the three fixture and abutment tightening methods were analyzed via Kruskal Wallis test layout, and they were significantly different (p<0.05).
Statement of Problem: To conduct a successful function of implant prosthesis in oral cavity for a long time, it is important that not only structure materials must have the biocompatibility, but also the prosthesis must be designed for the stress, which is occurred in occlusion, to scatter adequately within the limitation of alveolar bone around implant and bio-capacity of load support. Now implant which is used in clinical part has a very various shapes, recently the fixture that has tapered form of internal connection is often selected. However the stress analysis of fixtures still requires more studies. Purpose: The purpose of this study is to stress analysis of the implant prosthesis according to the different implant systems using finite element method. Material and methods: This study we make the finite element models that three type implant fixture ; $Br{\aa}nemark$, Camlog, Frialit-2 were placed in the area of mandibular first premolar and prosthesis fabricated, which we compared with stress distribution using the finite element analysis under two loading condition. Conclusion: The conclusions were as follows: 1. In all implant system, oblique loading of maximum Von mises stress of implant, alveolar bone and crown is higher than vertical loading of those. 2. Regardless of loading conditions and the type of system. cortical bone which contacts with implant fixture top area has high stress, and cancellous bone has a little stress. 3. Under the vertical loading, maximum Von mises stress of $Br{\aa}nemark$ system with external connection type and tapered form is lower than Camlog and Frialit-2 system with internal connection type and tapered form, but under oblique loading Camlog and Frialit-2 system is lower than $Br{\aa}nemark$ system.

Purpose: The purpose of this study was to measure the tightening torque for dental implant in dental laboratory and to analyze of the effects of different tightening torque. Materials and Methods: The tightening torque for dental implant in dental laboratory were measured by digital torque gauge. The length of abutment and analog were measured as tightening torque of manufacturer's instructions and the measured value. And the data were statically analyzed. Results: The mean tightening torque of implant screw in dental laboratory was $1.563{\pm}0.332Ncm$. The external type implant system of total length were showing no significant differences but the internal type implant system had difference significant (P < 0.05) when compared with tightening torque. Conclusion: The implant prosthesis should be made under manufacturer's instructions especially as tightening torque of screw. For the fidelity of implant prosthesis, dental technician should learn how to use the torque gauge.

Statement of problem. One of the common problems of dental implant prosthesis is the loosening of the screw that connects each component, and this problem is more common in single implant-supported prostheses with external connection, and in molars. Purpose. The purposes of this study were: (1) to compare the initial abutment screw detorque values of the six different implant-abutment interface designs, (2) to compare the detorque values of the six different implant-abutment interface designs after cyclic loading, (3) to compare the detorque values of regular and wide diameter implants and (4) to compare the initial detorque values with the detorque values after cyclic loading. Material and methods. Six different implant-abutment connection systems were used. The cement retained abutment and titanium screw of each system were assembled and tightened to 32Ncm with digital torque gauge. After 10 minutes, initial detorque values were measured. The custom titanium crown were cemented temporarily and a cyclic sine curve load(20 to 320N, 14Hz) was applied. The detorque values were measured after cyclic loading of one million times by loading machine. One-way ANOVA test, scheffe’s test and Mann-Whitney U test were used. Results. The results were as follows : 1. The initial detorque values of six different implant-abutment connections were not significantly different(p>0.05). 2. The detorque values after one million dynamic cyclic loading were significantly different (p<0.05). 3. The SS-II regular and wide implant both recorded the higher detorque values than other groups after cyclic loading(p<0.05). 4. Of the wide implants, the initial detorque values of Avana Self Tapping Implant, MIS and Tapered Screw Vent, and the detorque values of MIS implant after cyclic loading were higher than their regular counterparts(p<0.05). 5. After cyclic loading, SS-II regular and wide implants showed higher detorque values than before(p<0.05).
