목적: 총상에 의한 상완골 골절 증례를 통해 치료의 원칙에 대하여 고찰한다. 대상 및 방법: 28세 남자가 좌측 상완부의 총상으로 수상 후 5일에 응급실로 내원하였다. 상지의 신경 및 혈관 손상의 징후 및 타장기의 손상은 없었으며, 생체징후는 안정적이었다. 수상 당시 타 병원에서 총상에 대한 변연 절제술 및 세척술 후에 일차적 봉합술을 시행받았으며, 상완부의 전외측에 5 cm 가량의 오염된 상처가 있었다. 본원에서 변연절제술 및 탄환 파편의 제거술을 시행하고 외고정을 통하여 해부학적 정렬을 유지하였다. 술 후 8일까지 상처에 대한 무균적 소독을 시행하였으며, 정맥 항생제를 유지하였다. 술 후 9일째에 광범위 변연 절제술 및 항생제를 섞은 시멘트 구슬 삽입술을 시행하였다. 감염이 호전되는 소견을 보여, 시멘트 구슬 삽입 2주째에 외고정 장치를 제거하고 외고정 핀 삽입부의 피부 봉합술을 시행하였다. 1주 후에, 금속판을 이용하여 내고정을 하였다. 결과: 유합술 후 3 개월째, 만족스러운 상완골의 정렬과 골 유합을 얻었다.
Pilon fractures involving distal tibia remain one of the most difficult therapeutic challenges that confront the orthopedic surgeons because of associated soft tissue injury is common. To introduce and describe the diagnosis, current treatment, results and complications of the pilon fractures. In initial assessment, the correct evaluation of the fracture type through radiographic checkup and examination of the soft tissue envelope is needed to decide appropriate treatment planning of pilon fractures. Even though Ruedi and Allgower reported 74% good and excellent results with primary open reduction and internal fixation, recently the second staged treatment of pilon fractures is preferred to orthopedic traumatologist because of the soft tissue problem is common after primary open reduction and internal fixation. The components of the first stage are focused primarily on stabilization of the soft tissue envelope. If fibula is fractured, fibular open reduction and internal fixation is integral part of initial management for reducing the majority of tibial deformities. Ankle-spanning temporary external fixator is used to restore limb alignment and displaced intraarticular fragments through ligamentotaxis and distraction. And the second stage, definitive open reduction and internal fixation of the tibial component, is undertaken when the soft tissue injury has resolved and no infection sign is seen on pin site of external fixator. The goals of definitive internal fixation should include absolute stability and interfragmentary compression of reduced articular segments, stable fixation of the articular segment to the tibial diaphysis, and restoration of coronal, transverse, and sagittal plane alignments. The location, rigidity, and kinds of the implants are based on each individual fractures. The conventional plate fixation has more advantages in anatomical reduction of intraarticular fractures than locking compression plate. But it has more complications as infection, delayed union and nonunion. The locking compression plate fixation provides greater stability and lesser wound problem than conventional implants. But the locking compression plate remains poorly defined for intraarticular fractures of the distal tibia. Active, active assisted, passive range of motion of the ankle is recommended when postoperative rehabilitation is started. Splinting with the foot in neutral is continued until suture is removed at the 2~3 weeks and weight bearing is delayed for approximately 12 weeks. The recognition of the soft tissue injury has evolved as a critical component of the management of pilon fractures. At this point, the second staged treatment of pilon fractures is good treatment option because of it is designed to promote recovery of the soft tissue envelope in first stage operation and get a good result in definitive reduction and stabilization of the articular surface and axial alignment in second stage operation.
외고정장치를 이용한 골절 치료에서, 골절 부위에서의 절골편간 운동은 골절의 치유과정에 다양한 영향을 미친다고 알려져 있고, 이 미세 운동은 외고정장치-절골편 시스템의 강성과 밀접한 관련이 있다. 그렇기 때문에 최적의 골절 치유 과정을 부여하기 위하여 외고정장치 시스템의 구성 인자와 강성과의 연관성에 대한 이해가 필요하다. 본 연구에서는 장골 골절의 외고정술에 대한 유한요소 해석에서 외고정장치 시스템의 구성인자가 시스템 강성에 미치는 영향을 조사하였다. 이를 위하여, 시스템 정렬, 재료 비선형성, 조인트 유연성 및 가골형성 등을 해석 인자로 선정하였다. 개발된 유한요소 모델은 실험 결과와 유사한 시스템 강성을 나타내었고, 조인트 유연성 및 재료 비선형성의 고려는 시스템 강성 결과를 더욱 정확하게 반영하였다. 시스템 비정렬상태, 조인트 유연성 및 재료 비선형성은 시스템 강성의 감소를, 그리고 골절 부위의 가골형성은 시스템 강성의 증가를 나타내었다. 본 연구 결과는 외고정술시 시스템 강성을 증가시키기 위한 프레임 구성이나 기구 설계 등의 도구로 사용될 수 있을 것으로 기대된다.
Purpose: Ankle fractures with dislocations and pilon fractures at the distal tibia are usually associated with soft tissue damage caused by high-energy damage. Recently, a two-stage operation to perform internal fixation after the application of external fixation devices for stabilizing soft tissues has been accepted as the treatment of choice. This paper reports the clinical result of these injuries treated with threaded trans-calcaneal pin external fixation devices. Materials and Methods: Thirty-three patients diagnosed with ankle fractures with dislocations or tibial pilon fractures without open wounds. They underwent surgical treatment with threaded trans-calcaneal pin external fixation from January 2008 to February were enrolled in this study. This study evaluated the visual analogue scale (VAS), foot function index (FFI), and Olerud & Molander score as well as whether complications occurred. Results: The average VAS showed a meaningful decrease (p<0.001) from 7.4 before surgery to 2.6 after application of the external fixation device, and 1.4 at 12 months after surgery. The FFI also decreased significantly from 84.3 preoperatively to 20.3 at 12 months postoperatively (p<0.001). The Olerud & Molander score averaged 71.4 points, showing good clinical results. Complete bone union was observed in all patients. One patient each underwent debridement due to wound necrosis and infection in the pin insertion site. At the final follow-up, seven patients had posttraumatic ankle joint arthritis, according to a radiological examination. Conclusion: Manual reduction and external fixation using a threaded trans-calcaneal pin is a suitable surgical technique that is easy to perform and shows good clinical outcomes in stabilizing soft tissue damage in fractures and dislocations of ankle fracture or tibia pilon fractures in foot and ankle injury.
This study was conducted to evaluate and analyze fractures types in wild mammals in Korea. Wild mammals treated at 3 wildlife centers in Korea were included. Of the 63 cases included in this study, water deer was the most common species encountered with fractures. We determined 85 different fracture sites, with maximum lumber vertebral fractures followed by femoral, tibial and pelvic fractures. External skeletal fixator (ESF)-intramedullary (IM) pin (IM+ESF tie-in fixation) was the most common method applied for the surgical treatment of mammals. Lumbar vertebrae and femur were the most common bones involved. The findings of this study are expected to provide information for veterinarians regarding the frequency and characteristics of fractures in wild mammals and serve as a basic database for the types and outcomes of fractures in wild mammals.
좌측 대퇴골 원위부 골육종 진단 받은 9세 여아로 광범위 절제수술과 재활용 자가골 이식술을 받은 후 하지 부동과 성장판 손상이 있어 확장형 종양 대치물로 치환수술을 시행하였다. 그 후 종양 대치물에 심부 감염 발생하여 종양 대치물 제거 후 감염을 조절하기 위한 시멘트와 골수내정을 이용한 한시적 공간 대치물을 삽입하였다. 감염이 조절된 후 외고정 장치를 이용한 하지 부동(10 cm)에 대한 교정을 시행하였고, 마지막으로 종양 대치물 재삽입을 통한 관절 기능 보존을 시행한 1례를 보고 하고자 한다.

A dog aged two years and seven months and a cat aged seven years were referred owing to fractures of long bones. Preoperative radiographs revealed comminuted bone fractures close to joints. Conventionally, long-bone fractures are treated using intramedullary pins, plate and screw systems, or an external fixator system. In cases of non-reducible fractures, various graft materials have been used in fracture treatments to stimulate bone repair. Here, recombinant human bone morphogenetic protein-2 (rhBMP-2) and a collagen membrane were applied. Four weeks after surgery, fractured bone fragments began to unite and the bone union was observed using radiography four months after surgery. No complications occurred related to grafted materials. We successfully applied rhBMP-2 and collagen membranes in two different species to support the healing process of comminuted fractures, according to the concept of guided bone regeneration.
Purpose: Ankle fracture fixation is the gold standard of treatment but it does have its own complications. There is inadequate data regarding the comparative effectiveness of early vs. delayed fixation for open ankle fracture outcomes. This study compares the clinical and functional outcomes of open ankle fractures treated by early or delayed definitive fixation and identifies the limitations of both methods. Materials and Methods: All 73 patients enrolled in the study underwent surgical intervention within 24 hours of injury. The early fixation group (group A) consisting of 39 patients underwent definitive fixation as an index procedure, while the delayed fixation group (group B) consisted of 34 patients who underwent debridement and external fixator application as an index procedure and definitive fixation when soft tissue condition was conducive. All patients were evaluated at 2, 6, and 12 weeks postoperatively and then three monthly for a year. Results: Enneking and American Orthopaedic Foot and Ankle Society scores were markedly higher in the early fixation group at 6 months postoperatively (p-values <0.001 and 0.011, respectively). However, no discernible intergroup difference was evident at 12 months postoperatively. Between 6 and 12 months, group functional outcome scores were significantly different. At 6 months, there was a substantial difference in dorsiflexion between the two groups (p-values 0.001 and <0.001, respectively), but no difference was observed at 12 months postoperatively. At 6 and 12 months, group average plantar flexions were non-significantly different. Conclusion: Early definitive fixation of complex ankle fractures using a targeted approach produced promising results for lower grade open fractures (grades 1 and 2), and delayed definitive fixation, after initial external fixation to allow for soft tissue stabilization, produced promising results for higher grade open fractures (grades 3A and 3B). At 12-month follow-ups, clinical and functional outcomes achieved using these strategies were equivalent.
목적: 종양 대치물을 이용한 사지 구제 술 후 발생한 심부감염, 해리, 또는 하지 단축 등의 합병증으로 종양 대치물을 제거한 후 단계별 치료를 하는 과정에서 연부 조직 구축으로 인해 초래되었거나 또는 합병증 치료 시작 당시부터 있었던 하지 단축에 대하여 연부조직 연장 만을 위하여 외고정 기기를 한시적으로 사용한 결과를 평가하고 그 유용성을 알아 보고자 하였다. 대상 및 방법: 악성 골 종양으로 종양대치물을 이용하여 사지구제술을 받은 후 발생한 합병증으로 연부 조직 구축 및 하지 단축이 발생되어, 연부 조직 연장을 위해 외고정 기기(Dyna-extor$^{(R)}$)를 사용하였던 5명의 환자 6예를 대상으로 하였다. 외고정 기기는 골격 결손부위의 근위와 원위 정상 뼈 부분에 금속핀을 삽입 후 설치 하였다. 외부 고정기 연장은 술 후 약 3일부터 하였으며, 신경학적 증상이 나타나지 않는 범위에서 하루에 2~3 mm 이상씩 시행하였다. 결과: 치료 부위는 원위 대퇴골이 3예, 근위 경골이 2예였다. 최초 사지 구제술시의 평균연령은 13.4세(9~19세)였으며, 외부 고정기를 설치할 때의 평균연령은 22.2세(15~29세)로 최초 사지구제술 후 합병증으로 하지 단축이 발생되어 외부 고정기를 설치할 때까지의 기간은 평균 8.8년(3~14년)이었다. 4명의 환자 5예에서 평균 착용 기간은 37일(25~50일)에 평균 5.8 cm을 연장 하였으며, 1명의 환자는 150일 동안 착용하여 7.2 cm을 연장하였다. 외부 고정기 제거 3명의 환자에서 종양 대치물을 재삽입 하였고, 2예의 환자는 슬관절 유합술을 시행하였다. 이후 평균 추시 기간은 22개월이었으며 감염은 재발하지 않았다. 결론: 종양 대치물을 이용한 사지 구제술 후의 합병증으로 또는 합병증의 치료 과정에서 초래되는 연부 조직 구축과 하지 단축에 대하여 외고정 기기를 한시적으로 사용하여 효과적으로 하지 길이를 연장할 수 있었으며, 추후 종양 대치물 재 삽입이나 관절 고정술의 수술을 용이하게 해줄 수 있는 유용한 방법이라고 사료된다.
