A case of neonatal peroneal neuropathy with intrauterine onset (신생아 종아리신경병증 1례)

  • Peroneal neuropathy presenting at birth is a rare disorder. Although neonatal mononeuropathies may be related to obstetrical complications, prenatal mechanisms should be also considered. We describe an infant who was born at term by cesarean section due to breech presentation with a unilateral footdrop. Lack of compound muscle action potential in the peroneal nerve and denervation potentials confined to the tibialis anterior and the extensor hallucis longus muscles in the electrophysiological studies on the fourth day of life strongly suggest an isolated peroneal neuropathy of intrauterine onset. Early and sequential electrodiagnostic studies will be important to provide better temporal and pathophysiologic definitions, the better timing of onset and prognosis for mononeuropathies presenting in newborn infants.

A Myological Study of Hand Great Yin Lung Meridian Muscle System and Comparison with Deep Front Line in Anatomical Train (수태음폐경근의 근육학적 고찰 및 심부상지전방선과의 비교)

  • Objectives : This study was aimed to widen range of comprehesion about meridian muscle system through myological study of meridian muscle system and comparison with deep front arm line in anatomical train Methods : We have studied the similarity and difference between Hand Great Yin Lung Meridian Muscle System and Deep Front Line in Anatomical Train through Principles of Meridians & Acupoints, publications about myology, Anatomical trains. Results : I. Like another advanced studies, muscular system of hand great yin showed similarity to deep front line in anatomical train. II. It is considered that muscular system of hand great yin contains Musculus abductor pollicis brevis, Musculus extensor hallucis longus, Musculus brachioradialis, Musculus biceps brachii, Musculus subclavius, Musculus pectoralis major. III. Comparing muscular system of hand great yin to deep front arm line in anatomical train it showed similarity to part of muscles and pathological symptoms. But it showed difference to part of muscles and pathological symptoms. Conclusions : Hand Great Yin Lung meridian muscle system showed similarity and difference to deep front arm line in anatomical train. Further studies would be needed.

Reverse Dorsalis Pedis Flap Based on the Distal Communicating Artery of the Dorsalis Pedis Artery for the Reconstruction of the Forefoot Defect (원위교통동맥 기저 역행성 족배피판을 이용한 전족부 결손의 재건)

  • A 31-year-old female patient presented with a skin and soft tissue defect measuring $8{\times}6cm$ in size with exposure of the extensor hallucis longus tendon and the first metatarsal bone after metatarsal lengthening for brachymetatarsia. The defect was covered with a distally based dorsalis pedis flap based on the distal communicating branch of the dorsalis pedis artery. Secondary defect was covered by a split thickness skin graft. There was congestion of the flap tip after the operation; however, it was resolved using medical leeches and anti-coagulants. No necrosis or infection was encountered and the contour of the flap was satisfactory. There was no donor site morbidity. Reverse dorsalis pedis flap has not been commonly used due to the anatomical variation and uncertainty, which is different from the reverse radial forearm flap. However, when faced with the challenge of a moderate soft tissue defect of the distal forefoot, we believe that the reverse dorsalis pedis flap offers a good option with various advantages.

Tendon Transfer with a Microvascular Free Flap in Ijured Foot of Children (건 이전술 및 유리피편 이식술을 이용한 소아 외상성 족부 손상의 재건)

  • We peformed tendon transfer with a microvascular free flap for recovery of handicapped function and reconstruction for the skin and soft tissue loss. We review the clinical data of 11 children who underwent these operation due to injured foot by pedestrian accident from January, 1986 to June, 1994. The mean age of patients was 5.6 years old(3-8). Five cases underwent tendon transfer and microvascular free flap simultaneously. Another 6 cases underwent operations separately. The time interval between tendon trasnfer and microvascular free flap was average 5.6 months(2-15 months). The duration between initial trauma and tendon transfer was average 9.6 months(2-21 months). The anterior tibial tendon was used in 6 cases. Among these, the technique of splitting the anterior tibial tendon was used in 5 cases. The posterior tibial tendon was used in 3 cases and the extenosr digitorum longus tendon of the foot in 2 cases. Insertion sites of tendon transfer were the cuboid bone in 3 cases, the 3rd cuneiform bone in 3 cases, the 2nd cuneiform bone in 1 case, the base of 4th metatarsal bone in 1 case, and the remnant of the extensor hallucis longus in 3 cases. The duration of follow-up was average 29.9 months(12-102 months). The clinical results were analysed by Srinivian criteria. Nine cases were excellent and 2 cases were good. The postoperative complications were loosening of the tranferred tendon in 2 cases, plantar flexion contracture in 1 case, mild flat foot deformity in 1 case and hypertrophic scar in 2 cases. So we recommend the tendon transfer with a microvascular free flap in the case of injured foot of children combined with nerve injury and extensive loss of skin, soft tissue and tendon.

  • 한국재래산양(韓國在來山羊) 12마리의 후지근(後肢筋)을 절개하여 관찰하였던 바 다음과 같은 결과를 얻었다. 1. 한국재래산양(韓國在來山羊)의 후지근(後肢筋)에서는 다음과 같은 근(筋)들을 관찰할 수 있었다 : 소요근(小腰筋) M. psoas minor, 대요근(大腰筋) M. psoas major, 장골근(腸骨筋) M. iliacus, 요방형근(腰方形筋) M. quadratus lumborum, 대퇴근막장근(大腿筋膜張筋) M. tensor fasciae lata, 중둔근(中臀筋) M. gluteus medius, 심둔근(深臀筋) M. gluteus profundus, 둔이두근(臀二頭筋) M. gluteobiceps, 반건양근(半腱樣筋) M. semitendinosus, 반막양근(半膜樣筋) M. semimbranosus, 봉공근(縫工筋) M. sartorius, 박근(薄筋) M. gracilis, 치골근(恥骨筋) M. pectineus, 내전근(內轉筋) M. adductor, 대퇴방형근(大腿方形筋) M. quadratus femoris, 외폐쇄근(外閉鎖筋) M. obturatorius externus, 내폐쇄근(內閉鎖筋) M. obturatorius internus, 쌍자근(雙子筋) M. gemelli, 대퇴사두근(大腿四頭筋) M. quadriceps femoris, 제삼비골근 M. fibularis tertius, 내측지신근(內側趾伸筋) M. extensor digitorum medialis, 장지신근(長趾伸筋) M. extensor digitorum longus, 전경골근(前脛骨筋) M. tibialis cranialis, 장비골근 M. fibularis longes, 외측지신근(外側趾伸筋) M. extensor digitorum lateralis, 비복근 M. gastrocnemius, 가제미근(筋) M. soleus, 천지굴근(淺趾屈筋) M. flexor digitorum superficialis, 심지굴근(深趾屈筋) M. flexor digitorum profundus, 슬와근(膝窩筋) M. popliteus, 골간근(骨間筋) M. interosseus medius. 2. 천둔근(淺臀筋)의 전부(前部)는 대퇴근막장근(大腿筋膜張筋)과 융합된 것 같고, 후부(後部)는 대퇴이두근(大腿二頭筋)과 융합된 것 같다. 그러나 천둔근(淺臀筋)의 후부(後部)와 대퇴이두근(大腿二頭筋)이 결합된 것으로 생각되는 부분에는 완전융합이 일어나지 않고 천둔근(淺臀筋)을 구분(區分)할 수 있을 정도로 표면으로 2근(筋)을 분리(分離)할 수 있었다. 3. 외측지신근(外側趾伸筋)과 내측지신근(內側趾伸筋)의 건(腱)은 부전골의 원위(遠位) 1/3부(部)에서 서로 건막성(腱膜性)띠에 의하여 서로 연결 되었는데, 이 건막성(腱膜性) 띠는 건섬유(腱纖維)의 방향(方向)으로 보아 외측지신근(外側趾伸筋)의 건(腱)에서 분리(分離)되어 나온 한 가지 (branch)가 내측지신근(內側趾伸筋)의 건(腱)으로 이행되고 있었다. 4, 양(羊)에서 볼 수 있는 이상근(梨狀筋) M. piriformis과 장모지신근(長母趾伸筋) M. extensor hallucis longus은 나타나지 않았다.

Tenodesis after Tendon Lengthening for Irreparable Tibialis Anterior Tendon Avulsion Injury: A Case Report (단순 건고정술이 불가능한 전경골건 견열 손상에 대한 건연장술 이후 건고정술: 증례 보고)

  • Anterior tibialis ruptures are a rare type of injury related to the foot and ankle. Specifically, chronic and non-traumatic ruptures are related to preexisting chronic tendinopathic conditions and anatomical factors. These ruptures may cause persistent pain and functional impairments if neglected. Chronic tibialis anterior ruptures are frequently diagnosed late because the symptoms are not distinct. In cases with chronic or non-traumatic tibialis anterior tendon ruptures, tendons often become irreparable. Hence, various surgical options have been introduced to address this issue. The current surgical treatment options are as follows: free sliding anterior tibialis graft, extensor hallucis longus tendon transfer, and reconstruction with an allograft tendon. To date, there have been few reports about the reconstruction technique using Z-plasty for irreparable tibialis anterior tendon ruptures. In this report, we present a rare case of the application of the tibialis anterior tendon reconstruction technique using Z-plasty and tenodesis for a middle-aged man with an irreparable avulsion injury rupture. We also present the management plan and prognostic outlook, as well as a subsequent review of the relevant literature.

Effects of potassium channel modulators on the fatigue velocity of mouse skeletal muscle (K+ 통로 조절 약물이 마우스 골격근의 피로현상에 미치는 영향)

  • The density of ATP-sensitive potassium($K_{APT}$) channels, that open as intracellular ATP concentration falls below a critical level, is very high in skeletal muscle surface membrane and those high density may imply that $K_{ATP}$ channels have very important physiological roles. To elucidate a role of $K_{ATP}$ in relation to fatigue, the modulating effects of potassium channel openers and blockers on the fatigue velocity(FV) of mouse extensor hallucis longus muscle(EHL) were investigated in vitro. Twitch contraction was induced by an electrical field stimulation (EFS: 24-48V, 20ms, 0.2-4Hz) and resulting contraction force was isometrically recorded. The twitch forces were gradually decreased to 25% of initial contraction force(ICF) in $37.52{\pm}1.55sec$($mean{\pm}s.e.m.$, n=135), indicating the fatigue phenomena. The mean velocity for development of the fatigue was measured during the period that twitch force decreased to half($FV_{0/0.5}$) and during the period from half to 25%($FV_{0.5/0.25}$) of ICF. The fatigue was induced once every one hour and the tissue response was stable for up to 4 hours. In control condition, ICF was $5.8{\pm}0.12g$ (n=144) and decreased to 50% of ICF with the mean fatigue velocity of $0.182{\pm}0.006g/sec$($FV_{0/0.5}$, n=135) and from 50% to 25% of ICF with $0.084{\pm}0.004g/sec$($FV_{0.5/0.25}$, n=135). Cromakalim($50{\mu}M$) significantly increased $FV_{0.5/0.25}$(n=4). Glibenclamide($IC_{50}>50{\mu}M$), $Ba^{2+}$($IC_{50}=10{\mu}M$), 4-aminopyridine($FV_{0/0.5}$, $IC_{50}=0.5mM$; $FV_{0.5/0.25}$, $IC_{50}=2mM$) decreased both $FV_{0/0.5}$ and $FV_{0.5/0.25}$ concentration-dependently up to 75%. $TEA^+$(30mM), E-4031($10{\mu}M$), tolbutamide(1mM) decreased $FV_{0.5/0.25}$, but apamin(300nM) and $TEA^+$(10mM) showed no significant effects. Our results suggest that activation of the $K_{ATP}$ channels may be major cause of $K^+$ outflux during development of the fatigue and the isolated EHL muscle could be an useful experimental preparation in studying the fatigue phenomena in skeletal muscle. In addition, the possibility of activation of delayed rectifier during the fatigue development remains to be studied further.
