The purpose of this study was to characterize the impact shock wave and its attenuation, and the kinematic response of the lower extremity's joints to the impact shock during downhill running in which the lower extremity's extensor acts dominantly. For this study, fifteen subjects(mean age:$27.08{\pm}4.39$; mass:$76.30{\pm}6.60$; height:$177.25{\pm}4.11$) were required to run on the 0% grade treadmill and downhill grades of 7%, and 15% in random at speed of their preference. When the participant run, acceleration at the tibia and the sacrum and kinematic data of the lower extremity were collected for 20s so as to provide at least 5 strides for analysis at each grade. Peak impact accelerations were used to calculate shock attenuation between the tibia and sacrum in time domain at each grade. Fast Fourier transformation(FFT) and power spectral density(PSD) techniques were used to analyze impact shock factors and its attenuation in the frequency domain. Joint coordinate system technique was used to compute angular displacement of the ankle and knee joint in three dimension. The conclusions were drawn as fellows: 1. Peak impact accelerations of the tibia and sacrum in downhill run were greater than that of 0% grade run, but no significant between conditions. Peak shock of PSD resembled also in pattern of peak impact acceleration. The wave of impact shock attenuation between the tibia and sacrum decreased with increasing grade, but didn't find a significant difference between grade conditions. 2. Adduction/abduction, flexion/extention, and internal/external rotation of the ankle and knee joints at support phase between grade conditions didn't make much difference. 3. At grade of 7% and 15%, there were relationship between the knee of the flexion/extension movement and peak impact acceleration during heel strike and found also it in the ankle of plantar/dorsiflexion at grade of 15%.
The purpose of this study was to investigate the biomechanical effects of an application of whole body vibration during strengthening exercise. Every participant performed four weeks exercise program using general leg-press versus vibrating leg-press. Participants did legpress exercise three sets of 25 repetitions with the load of 25 percent of 1RM during first week, three sets of 20 repetitions with 40 percent of 1RM during second week, three sets of 15 repetitions with 60 percents of 1RM during third week, and three sets of 15 repetitions with 80 percent of 1RM during last fourth week. The vibration(25Hz, 5mm) was applied only to the vibration exercise group. A three dimensional virtual lower extremity model for one of subject and virtual leg-press model were generated. The knee extensor muscle forces were analyzed using the virtual model and the knee joint torque(maximum extension torque) was measured using an isokinetic device. Calculated muscle forces were smaller in vibrating leg-press exercise than in general leg-press exercise. An increase of the maximum knee extension joint torque was 2.14 times larger approximately after the four week vibration leg-press exercise program was performed.

De Quervain's disease is a stenosing tenosynovitis that affects the synovial sheath of the tendons of the abductor pollicis longus and extensor pollicis brevis; it can be considered a work-related disease, since it frequently is associated with highly repetitive movements in jobs using the hands. The usual sign and symptom of de Quervain's diseases are positive Finkelstein's sign and pain and tenderness around the radial styloid process. The majority of de Quervain's disease has a good results to conservative treatment, but surgery can be performed in case of recurrence or pain last more than 6 months without improvement in spite of conservative treatment. We diagnosed a patient with atypical de Quervain's disease by ultrasonography, treated by local steroid injection and pain was relieved. We report a case of atypical de Quervain's disease evaluated by ultrasonography with brief of literatures.
Objective: The aim of this study was to investigate the influence of seat heights for optimizing the ingress/egress performance in the elderly people. Background: Recently elderly users have been increasing in number and the ease of ingress/egress of the vehicle becomes an important issue. Method: Seven elderly subjects participated in this study(age: 71.7${\pm}$3.6yr, height: 167.7${\pm}$5.4cm, weight: 68.1${\pm}$11.5kg). Each subject performed the sitting and rising task from comfortable seated position on the chair under the following conditions: (1) with a lumbar support and (2) without a lumbar support. We measured EMG activities of seven muscles(tibialis anterior, soleus, gastrocnemius, vastus lateralis, rectus femoris, biceps femoris and erector supinae) and ranges of motion in lower limb during sitting and standing from three different heights (400mm, 500mm and 600mm from ground) of slide-up seat. Results: Muscle activities and angular movements of hip and knee joints during standing-up and sitting-down with a high seat height(600mm) were reduced mean 30.4% in extensor muscles, 57.11% in flexor muscle, 18.74% in erector muscle and 31.0% in joints compared with a low seat height(400mm). Conclusion: Muscle activities and joint movements in hip and knee were reduced when rising/sitting from a high seat height(600mm) compared with a low seat height(400mm). Application: This study can be used to design vehicle that are easy to get in and out of by older peoples with or without impairments.

De Quervain's disease is caused by stenosing tenosynovitis of the first dorsal compartment of the wrist. The first dorsal compartment at the wrist includes the tendons of the abductor pollicis longus (APL) and the extensor pollicis brevis (EPB). Patients with De Quervain's usually report pain at the dorsolateral aspect of the wrist with referral of pain toward the thumb and/or lateral forearm. This condition responds well to nonsurgical treatment. Objective : The aim of this study was to find out the therapeutic effect of the Cervus elaphus Herbal-acupuncture in the treatment of patients with De Quervain's Disease. Design : A prospective study of Cervus elaphus Herbal-acupuncture was conducted. Setting : The study was conducted in the Kang-nam Oriental Hosp. of Dong-Guk University. Patients : The study comprised 4 patients with De Quervain's Disease. Methods : We investigated 4 cases patients the De Quervain's Disease. We evaluated wrist function that before and after Cervus elaphus Herbal-acupuncture treatment by VAS score, tenderness and Finkelstein test. the treatment is operated every other day. Results : Pain reduction(VAS) was found out in the case treated with Cervus elaphus Herbal-acupuncture. Conclusions : Clinical results show that Cervus elaphus Herbal-acupuncture has a good analgesic effect in De Quervain's Disease.
Background: Despite muscle latency times and patterns were used as broad examination tools to diagnose disease and recovery, previous studies have not compared the dominant arm to the non-dominant arm in muscle latency time and muscle recruitment patterns during reaching and reach-to-grasp movements. Objects: The present study aimed to investigate dominant and non-dominant hand differences in muscle latency time and recruitment pattern during reaching and reach-to-grasp movements. In addition, by manipulating the speed of movement, we examined the effect of movement speed on neuromuscular control of both right and left hands. Methods: A total of 28 right-handed (measured by Edinburgh Handedness Inventory) healthy subjects were recruited. We recorded surface electromyography muscle latency time and muscle recruitment patterns of four upper extremity muscles (i.e., anterior deltoid, triceps brachii, flexor digitorum superficialis, and extensor digitorum) from each left and right arm. Mixed-effect linear regression was used to detect differences between hands, reaching and reach-to-grasp, and the fast and preferred speed conditions. Results: There were no significant differences in muscle latency time between dominant and non-dominant hands or reaching and reach-to-grasp tasks (p>.05). However, there was a significantly longer muscle latency time in the preferred speed condition than the fast speed condition on both reaching and reach-to-grasp tasks (p<.05). Conclusion: These findings showed similar muscle latency time and muscle activation patterns with respect to movement speeds and tasks. Our findings hope to provide normative muscle physiology data for both right and left hands, thus aiding the understanding of the abnormal movements from patients and to develop appropriate rehabilitation strategies specific to dominant and non-dominant hands.
Spasticity, an abnormal increase in resting muscle tone, is one of the most common symptoms of stroke, and its management is becoming a major issue in rehabilitation. The aims of this study are to determine the effects of electroacupuncture(EA), TENS and neuromuscular electric stimulation(NMES) on spasticity, as well as the possibility of tissue comliance method as a spasticity scale. 45 stroke patients participated in a study of the effects on hemiplegic spasticity of EA, TENS and NMES. Spasticity was measured by modified Ashworth scale on the upper extremity and tissue compliance measurement, penetration mm/kg, on Susamni(LI10) area at just before and after stimulation, and 30 minutes, 1 hour, 2 hours and 24 hours after stimulation. The acupuncture points were applied to Kokchi(LI11), Susamni(LI10), Hapkok(LI4) and Oegwan(TE5) of the affected limb. The electrodes were placed unilaterally on LI11 to LI10 and TE5 to LI4. EA with biphasic wave, 60Hz, 0.4 msec pulse duration and low intensity was applied continuously for 20 minutes. TENS with high frequency, low intensity was applied. NMES was applied with spasticity program for 20 minutes. Each electric stimulation was done on extensor muscles group of forearm for 20 minutes. EA and NMES groups were found to produce a statistically significant decrease(p〈0.05) of spasticity and these effects lasted up to 30 minutes after stimulation. There was no definite correlation between the modified Ashworth scale and tissue compliance measurement. But tissure compliance method was found to be possible as a quantitative measurement on spasticity. There was no significant correlation between the effects of EA and NMES and the characteristics of patient, but significant correlation between the effects of EA and NMES and the modified Ashworth scale.
Purpose: To find out the priority of which procedure has had a better outcome both clinically and radiographically between the two groups, one is treated by primary repair and the other by modified Brostr$\ddot{o}$m's procedure, by comparing the postoperative ankle joint stability and the patient's degree of satisfaction. Material and methods: 16 cases were taken into consideration whose number of severed ligaments were at least two or more of the lateral collateral ligaments of the ankle, and also were confirmed intraoperatively. Among them, 8 cases were treated with primary repair and the other 8 cases were treated with primary repair and the other 8 cases by modified Brostr$\ddot{o}$m's procedure. Results: There was no distinguishable difference for the patient's degree of satisfaction between the two procedures above mentioned. In 3 cases treated with primary repair, functional instability was observed. In case of postoperative ankle joint stability, 7 of 8 cases treated by modified Brostr$\ddot{o}$m's procedure has revealed increased joint stability. And 3 of 8 cases which were treated by primary repair have showed postoperative residual instability. Conclusion: Actually, the severed ligament can not maintain its normal strength though several months has elapsed, and possible residual instability could be remained. Therefore, it can be expected that modified Brostr$\ddot{o}$m's procedure also would be a .good method in obtaining suitable ankle joint stability as well as subtalar joint stability because of its reinforcement using extensor retinaculum.
The purpose of this study is (a) to estimate effective moment arms of quadriceps forces and (b) to compare the $d_e$ between the ACL-reconstructed and uninjured knees from the same individual. One female (20 yrs old, 2 yrs post-op, hamstring tendon autograft) and two males (22 yrs old, 2 yrs post-op; 28 yrs old, 4 yrs post-op; Patellar tendon autografts for both). Sagittal view radiographs were obtained for 6-7 different angles $(range\;5^{\circ}-110^{\circ})$ from each knee. The do was determined by the method of Chow et al. (1999a). The results showed that the maximum de values ranged from 4.61 to 5.59cm and 4.59 to 4.89cm for the ACL-reconstructed and uninjured knees, respectively. The maximum $d_e$ occurred between $35^{\circ}\;and\;50^{\circ}\;and\;20^{\circ}\;and\;50^{\circ}$ for the ACL-reconstructed and uninjured knees, respectively. The minimum do values ranged from 4.12 to 4.35 cm and 3.12 to 3.63cm for the ACL-reconstructed and uninjured knees, respectively. The effective moment arm of the knee extensor affects the loads on knee ligaments during knee-extension exercises. Because apparent differences in the moment arm of the quadriceps in different participants, it is very important to use personalized knee joint geometry for the computation of knee joint force. In the present study, no noticeable bilateral difference was found in the male subjects. However, apparent bilateral differences in de were observed in the female subject. This suggests that the effects of ACL reconstruction surgery on patellar mechanism deserve further investigation.
Purpose: Hexadactyly without thumb is a rare congenital anomaly of the hand where six triphalangeal digits are symmetrically distributed without thumb. Contrary to mirror hands, triphalangeal six digits are symmetrically distributed on each side at the midline with well - differentiated carpal bones, extensor tendons, one ulnar and one radius. The authors developed a new surgical technique based on a three - dimensional concept to correct the hexadactyly and applied to 2 cases of hexadactyly with good functional and aesthetic results. Here we document the surgical technique and its result. Methods: A 16 month old male patient visited our clinic with chief complaints of bilateral hexadactyly deformity. On physical examination most radial first and second digits showed no opposition and adduction motion on both side hands. Radiography showed 6 triphalangeal digits with normal development of carpal, radial and ulnar bone. Right side abnormality was corrected by removal of most radial side extra - digit, rotation and migration of 2nd ray to thumb position and creation of 1st web by transposing a mid - palm based rectangular palmar flap as in Snow & Littler procedure which has been being applied for correction of 1st web syndactyly in cleft hand deformity. Seven months later, left side abnormality was also corrected with the same procedure. Results: Postoperative appearances of the both hands were satisfactory. Flexion, extension, opposition and grasping were possible with the pollicized 2nd ray. Pinching power was 3.0 kg 15 months after surgery and 2.5 kg 22 months after in right hand respectively. Conclusion: In correction of hexadactyly deformity, satisfactory aesthetic and relevant functional results can be expected with authors' newly developed technique: removal of most radial digit, rotation and migration of 2nd digit to thumb position as well as creation of the 1st web space by transposition of mid - palm based rectangular flap.
