Distribution and Exposure Prevalence of Carbohydrate-based Food Intake among Obese Korean Adults Based on the Health Examinees (HEXA) Study (한국 성인 비만 집단의 탄수화물 급원 식품 섭취 분포 및 노출 위험도: 대규모 한국인유전체 역학 자료 활용)

  • Objectives: The purpose of this study was to estimate the distribution and exposure prevalence of total carbohydrate intake and the carbohydrate-based foods such as rice, noodles, sweet potatoes, sweet drinks, milk and fruits and to characterize intake patterns among obese Korean adults. Methods: Subjects included 137,363 adults aged 40 years or older who participated in a Health Examinees (HEXA) Study. Multiple regression analysis of data from Semi-Quantitative Food Frequency Questionnaire (SQFFQ) identified food sources of carbohydrates. Weight, height and waist circumstance (WC) were measured, and the body mass index (BMI) was calculated. Obesity was defined as $BMI{\geq}25kg/m^2$ and abdominal obesity as $WC{\geq}90cm$ and ${\geq}85cm$ for males and females, respectively. Results: Obese adults appeared to have a higher total carbohydrate intake in the univariate analysis but had eaten less after adjustment for general and lifestyle factors, compared to normal weight adults (OR 0.78, 95% CI 0.73-0.82 for general obesity; OR 0.79, 95% CI 0.74-0.85, for abdominal obesity; P trend < 0.0001, respectively). Based on advance analysis for the food sources of carbohydrates, obese subjects had lower intake of rice (OR 0.86, 95% CI 0.68 -1.09 for general obesity; OR 0.87, 95% CI 0.67-1.13, for abdominal obesity; P trend < 0.0001, respectively) and higher intake of noodles (OR 1.21, 95% CI 1.16-1.27 for general obesity; OR 1.25, 95% CI 1.19-1.32, for abdominal obesity; P trend < 0.0001 respectively). With regard to other food sources of carbohydrates such as milk and fruits, intake was lower among obese compared to normal weight subjects. Conclusions: In the Korean middle-aged and older obesity groups, the intake of carbohydrates and the related foods was lower than in normal weight subjects, except for noodles. We conclude that a higher intake of noodles may enhance weight-gain. Therefore, this study suggested that the guidelines should consider the types of carbohydrate sources and the amount consumed from foods in order to provide proper guidance with regard to control and prevent obesity among Korean adults.

Comparison of Meta-analysis Results with and Without Adjustment for Healthy Worker Effect on the Association Between Occupational Exposure to Trichloroethylene and Cancer Risk (건강근로자효과 보정 전후의 메타 분석 결과 비교 -직업적 트리클로로에틸렌 노출과 암의 연관성-)

  • Objectives: By conducting a meta-analysis of cohort studies reporting standardized mortality ratios (SMRs) for workers exposed to trichloroethylene, we attempted to adjust for healthy hired effect by applying the same methods as described in a recent report from the Agricultural Health Study. Methods: Among all cohort studies that evaluated the association between all cancer, non-Hodgkin's lymphoma (NHL), kidney cancer, liver cancer and occupational exposure to trichloroethylene, a total of 10 studies reporting SMR values were selected. A random-effects model was used to estimate the summary SMRs or rSMRs and 95% confidence intervals. Relative SMR ($rSMR=SMR_x/SMR_{not\;x}$) was calculated comparing observed and expected counts for all cancer, NHL, kidney cancer, and liver cancer with an independent referent set of values consisting of the observed and expected counts for other causes. Results: The SMR values for all causes ranged from 0.68 to 1.03, suggesting moderate to weak healthy worker effect for the selected studies. When the healthy worker hire effect was taken into account, the summarized risk became statistically significant; the summary SMR of all cancer was 0.95 (0.91-1.00) and the summary rSMR of all cancer was 1.10 (1.04-1.15). The summary SMR of NHL was 1.04 (0.93-1.14) and the summary rSMR of NHL was 1.23 (1.04-1.46). The summary SMR of kidney cancer was 1.08 (0.88-1.33) and the summary rSMR of kidney cancer was 1.23 (1.02-1.49). The summary SMR of liver cancer was 0.88 (0.78-0.99), and the summary rSMR of liver cancer was 0.95 (0.84-1.07). Conclusion: The rSMR method is useful to determine summary risk adjusted for healthy worker effect through meta-analysis.

A Time-Series Study of Ambient Air Pollution in Relation to Daily Death Count in Daejeon, 1998-2001 (대전 광역시 대기오염과 일별 사망자 수의 상관성에 관한 시계열적 연구(1998년~2001년))

  • This study is performed to examine the relationship between air pollution exposure and mortality in Daejeon for the years of 1998 - 2001. Daily counts of death were analyzed by general additive Poisson model, with adjustment for effects of seasonal trend, air temperature, humidity, and day of the week as confounders in a nonparametric approach. Daily death counts were associated with CO(4 day before), $O_3$(current day), $PM_10$(4 day before), $NO_2$(6 day before), $SO_2$(2 day before). Increase of $31.07{\mu}g/m^3$(interquartile range) in $PM_10$ was associated with 2.0 % (95% CI = 0.5 % - 3.5 %)) increase in the daily number of death. This effect was greater in children(less than 15 aged) and elderly(more than 65 aged). We concluded that Daejeon had 2 - 4 % increase in mortality in association with IQR in air pollutants. Daily variations in air pollution within the range currently occurring in Daejeon might have an adverse effect on daily mortality. These findings also support the hypothesis that air pollution at levels below the current ambient air quality standards of Korea except PM10, is harmful to sensitive subjects, such as children or elderly.

Plasma Stress Responses in Juvenile Red-Spotted Grouper (Epinephelus akaara) exposed to Abrupt Salinity Decrease

  • The objective of the current study was to determine acute plasma stress responses in two size groups of juvenile Epinephelus akaara (average body weight: $8.4{\pm}2.1$ and $3.3{\pm}0.6g$; 150 and 120 days after hatch, respectively) exposed to abrupt salinity drops (from 34 practical salinity unit, PSU seawater to 18, 10 PSU (experiment 1) or 26, 18, 10 PSU (experiment 2), respectively). Plasma glucose, glutamic oxalate transaminase, glutamic pyruvate transaminase, red blood cell counts, and gill histology were determined during 72 h exposure. Significantly increased plasma glucose, glutamic oxalate transaminase levels, and red blood cell counts were observed in fish exposed to 18 or 10 PSU. Histological changes, such as hyperplasia and lifting of epithelium in the gill secondary lamellae, were also observed in fish exposed to 18 or 10 PSU at 72 h post-drop. E. akaara exposed to sudden salinity drops to 18 or 10 PSU still seems to undergo the primary adjustment phase before fish reaches a new homeostasis, whereas fish exposed to 26 PSU seems to mount osmotic changes. Therefore, the no observed adverse effect levels for 72 h acute salinity challenge was 26 PSU in our study, and salinity drop to 18 PSU and below can possibly cause acute adverse effect, in which fish could be vulnerable to additional stresses such as a temperature changes or handling stress.

Relationship Between Serum Concentrations of Organochlorine Pesticides and Metabolic Syndrome Among Non-Diabetic Adults

  • Objectives: This study was performed to investigate if organochlorine pesticides (OCPs) were associated with metabolic syndrome and insulin resistance among non-diabetes. Methods: Among subjects who participated in a community-based health survey, 50 non-diabetic subjects with metabolic syndrome and 50 normal controls were selected. Insulin resistance was measured by the homeostasis model assessment (HOMA-IR). Eight OCPs were selected. Results: After adjusting for confounders except for body mass index (BMI), beta-hexachlorocyclohexane ($\beta$-HCH) and heptachlor epoxide were positively associated with metabolic syndrome. Odds ratios across tertiles of $\beta$-HCH and heptachlor epoxide were 1.0, 3.2 and 4.4, and 1.0, 4.0 and 6.0, respectively (p for trend = 0.01 and <0.01). After additional adjustment for body mass index (BMI), heptachlor epoxide still showed an increasing trend with adjusted odds ratios of 1.0, 4.1, and 4.6 (p for trend = 0.10). When the five components of metabolic syndrome (with the definition of high fasting glucose ($\geq$ 100 mg/dL) were separately analyzed, all components were positively, but not significantly, associated with heptachlor epoxide. As the serum concentration of heptachlor epoxide increased, HOMA-IR increased significantly in subjects with metabolic syndrome even after adjusting for BMI (p value <0.05 and <0.01). Conclusions: Despite the small sample size, this study suggests that the background exposure to some OCPs may be associated with metabolic syndrome.

  • Factors influencing the fusion frequency of protoplasts were investigated with auxotrophic mutants of Pleurotus florida and Pleurotus ostreatus. Immediately after the polyethylene glycol (PEG) solution was added, the protoplasts adhered firmly and shrank. During the subsequent dilution with 0.6 M sucrose, the protoplasts regained their normal size and larger bodies were observed. Interspecific heterokaryons were obtained by fusion of the nutritionally complementing protoplasts. Hyphae of the heterokaryotic fusants formed true clamp connections. The optimum conditions were a total of 1 to 15 million protoplasts per ml, 30% polyethylene glycol 8000 solution with adjustment to pH 8.0 and 0.6 M sucrose stabilized regeneration medium. Other parameters such as $CA^{++}$, glycine, exposure time and temperature influenced mainly the viability of the protoplasts.

An Association Between Air Pollution and the Prevalence of Allergic Rhinitis in the Ulsan Metropolitan Region (울산지역 대기오염과 알레르기 비염 유병률과의 관계)

  • This study aims to investigate the relationship between air pollution exposure and the prevalence of allergic rhinitis in a young population in the Ulsan metropolitan region (UMR). Data on physician-diagnosed allergic rhinitis (past 12 months) in 1,449 infants and children aged 1-18 years who lived within 1.5 or 2 km of air quality monitoring sites were collected in a cross-sectional health interview survey conducted between January-February 2006 in the UMR. Comparisons of the spatial distribution of the prevalence rates for allergic rhinitis and annual average pollutant concentrations over the region showed that a relatively high prevalence rate occurred around the coastal industrial area, with high PM10 concentrations. A linear correlation analysis demonstrated a positive correlation relationship between them (R = 0.680, p = 0.04). Multiple linear regression analysis revealed that the combined effect of the PM10 and $SO_2$ variables accounts for approximately 81% of the variance (R-square: 0.81) in the prevalence rate. From the multiple logistic regression analysis after adjustment by age, sex, and air-pollutant factors, the PM10 and $SO_2$ which were mainly from industrialrelated emissions were found to be significantly associated with an increased risk of allergic rhinitis (aOR: 1.76, 95% CI: 1.15-2.70 for PM10 ; aOR: 1.63, 95% CI: 1.12-2.35 for SO2).

Blood Lead Concentration and Hypertension in Korean Adults Aged 40 and Over According to KNHANES IV (2008) (40세 이상의 한국성인의 혈중 납 농도와 고혈압 - 2008년 국민건강영양조사를 바탕으로 -)

  • Objectives: The purpose of this study was to examine the cross-sectional relationship between low blood lead levels and increasing blood pressure among Korean adults using a nationally representative sample of the Korean population: the Korea National Health and Nutrition Examination Survey (KNHANES) 2008. Methods: A total of 918 subjects aged 40 and older and not currently being treated for hypertension participated in this study. Information about age, gender, smoking status, alcohol consumption, education level, and the use of anti-hypertensive medication was collected. The blood pressure was defined as the mean of the second and the third measurements after three time measurements. Lead levels were determined by an analysis of blood samples. Multiple linear and logistic regression analyses were implemented after adjusting for covariates including age, gender, educational level, smoking status, alcohol consumption, and BMI. Results: This study showed that the average differences in systolic and diastolic blood pressure comparing the lowest to highest quintile of blood lead were 4.33 mmHg (95% CI, 0.66-8.00; p for trend = 0.027) and 2.66 mmHg (95% CI, 0.26-5.06; p for trend = 0.021), respectively. After multivariate adjustment for covariates, the prevalence odds ratio (POR) of subjects in the highest quintile was associated with a 1.70-fold increase in the risks of hypertension (95% CI, 0.83-3.49; p for trend test = 0.112) over those in the lowest quintile of blood lead concentration, However, it was not statistically significant. Conclusions: This study provided evidence for an association between low- levels of blood lead and elevations in blood pressure and risk for hypertension in the general population of Korea.

Daily counts of death were analyzed by general additive Poisson model, with adjustment for effects of seasonal trend, air temperature, humidity, and day of the week as confounders in a nonparametric approach. Daily death counts were associated with CO(1 day before), O$_3$(2 day before), PM$_{10}$(1 day before), NO$_2$(1day before), SO$_2$(1 day before). Increase of 32.21 ${\mu}$g/m$^3$(interquartile range) in PM$_{10}$ was associated with 1.9 % (95% CI = 0.8 % - 2.9 %) increase in the daily number of death. This effect was greater in children(less than 15 aged) and elderly(more than 65 aged). We concluded that Incheon had 2 - 4 % increase in mortality in association with IQR in air pollutants. Daily variations in air pollution within the range currently occurring in Incheon might have an adverse effect on daily mortality. These findings also support the hypothesis that air pollution, at levels below the current ambient air quality standards of Korea, is harmful to sensitive subjects, such as children or elderly.

Results of Protocol-based Perioperative Management in Off-Pump Coronary Artery Bypass Grafting for Patients with Non-dialysis-dependent Chronic Kidney Disease

  • Background: Recent studies have demonstrated the benefits of off-pump coronary bypass grafting over the on-pump technique in patients with chronic kidney disease (CKD). To further reduce the risk of acute kidney injury and the need for renal replacement therapy, even in patients undergoing off-pump coronary artery bypass grafting, we adopted protocol-based perioperative management for patients with CKD. Methods: From December 2012 to March 2015, 265 patients underwent isolated off-pump coronary artery bypass grafting. To analyze renal function in a stable condition, we excluded 12 dialysis-dependent end stage renal failure and 10 emergency or urgent cases. Among the remaining 243 patients, 208 patients had normal kidney function (normal group), and 35 patients had CKD (CKD group). Minimizing contrast exposure, ensuring adequate hydration, using strict drug dosage adjustment, and optimizing hemodynamic status were key elements of the protocol for the CKD group. Results: The risk of acute kidney injury was about ${\times}3$ higher in the CKD group than in the normal group (p=0.01). Estimated glomerular filtration rates and serum creatinine levels deteriorated until the third postoperative day in the CKD group. However, by adopting protocol-based perioperative management, this transient renal dysfunction recovered to preoperative levels by the fifth postoperative day without requiring renal replacement therapy in all cases. Conclusion: Off-pump coronary bypass surgery combined with this protocol-based perioperative management strategy in patients with non-dialysis-dependent CKD could mostly be performed without renal replacement therapy.
