Renal scar formation after urinary tract infection in children

  • Urinary tract infection (UTI) is a common bacterial illness in children. Acute pyelonephritis in children may lead to renal scarring with the risk of later hypertension, preeclampsia during pregnancy, proteinuria, and renal insufficiency. Until now, vesicoureteral reflux (VUR) has been considered the most important risk factor for post-UTI renal scar formation in children. VUR predisposes children with UTI to pyelonephritis, and both are associated with renal scarring. However, reflux nephropathy is not always acquired; rather, it reflects reflux-associated congenital dysplastic kidneys. The viewpoint that chronic kidney disease results from renal maldevelopment-associated VUR has led to questioning the utility of any regimen directed at identifying or treating VUR. Despite the recognition that underlying renal anomalies may be the cause of renal scarring that was previously attributed to infection, the prevention of renal scarring remains the goal of all therapies for childhood UTI. Therefore, children at high risk of renal scar formation after UTI should be treated and investigated until a large clinical study and basic research give us more information.

Effect of Breathing Exercise Using Panflutes on the Postoperative Compliance, Pulmonary Infections and Life Satisfaction in Elderly Patients Undergoing Spinal Surgery (척추수술을 받는 노인에게 시행한 수술 전 팬플룻 호흡운동 훈련의 효과)

  • Purpose: The purpose of this study was to examine the effects of breathing exercises performed using panflutes in elderly patients undergoing spinal surgery. Methods: The study design was a nonequivalent control group non-synchronized pre-post test. The study included 24 patients in both the experimental group and the control group. The experimental group completed a daily breathing exercise regimen using panflutes for 30minutes after meals, whereas the control group was provided standard preoperative education, including breathing exercises using incentive spirometers. After the exercise regimen, breathing exercise compliance, pulmonary infections, and life satisfaction were measured in both groups, and the data were analyzed using the SPSS/WIN program. Results: The compliance rate of breathing exercises was significantly higher in the experimental group. The experimental group presented no pulmonary infections in the later period, whereas the control group presented higher pulmonary infection rates in the same period. In addition, the life satisfaction score in the experimental group significantly increased. Conclusion: The breathing exercise program using panflutes for elderly patients undergoing spinal surgery enhanced their breathing exercise compliance and their daily life satisfaction in addition to reducing their pulmonary infection rates.

Infection Characteristics of Korean Trichinella Isolate to Some Kind of Experimental Animals

  • Present study was performed to investigate the host-parasite relationship of the Korean Trichinella isolate (KTI). In the experiment to observe the infectivity of KTI to several kinds of animals, the reproductive capacity index (RCT) was highest in cats, and that in mice, hamsters and rats was followe4 in descending order. However, birds, i.e. wild goose and chicken, did not infect with KTI. The number of larvae per a gram of muscle (LPG: 377) was highest in the tongue of cats experimentally infected with KTI larvae. LPG in the diaphragm, anterior leg, back, posterior leg and abdominal muscles were 313, 246, 234, 225 and 170 respectively. Muscle larvae recovered at 55 days after infection were revealed the highest infectivity (RCI: 137.2) in mice. RCI was comparatively low in the mice infected with less than 25 day-old and more than 300 day-old larvae. In the experiment to observe the susceptibility of KTI by the mouse strain, ICR (RCI: 137.2), C57BL/6 (RCI: 108.8), DBA/2 (RCI: 107.1), C3H (RCI: 98.7), BALB/c (RCI: 96.9), FVB (RCI: 96.1) and B6C3F1 (RCI: 85.3) were very susceptible. However, BDF1 (RCI: 57.7) and CBA (RCI. 57.1) were revealed the moderate susceptibility, and B6CBAF1 (RCI: 23.1) was shown the lowest. The infection sites of adults were posteriorly transferred in the small intestine of experimental mice according to the infection periods of muscle larvae. The infection characteristics of KTI observed in this study may be useful as the basic data in the advanced studies, furthermore in the study of other Trichinella isolates.

THE CHANGES OF SERUM C-REACTIVE PROTEIN IN THE HEAD AND NECK INFECTION (두경부 감염 환자에서 혈청 내 C-Reactive Protein의 변화)

  • The purpose of this study is to determine the value of serum concentration of C-reactive protein(CRP) in comparison with ESR and leukocyte as the tools for diagnosis and follow-up study of infection. For this study, thirty-one patients with head and neck infection and thirty-two patients performed the orthognathic surgery were selected for experimental and control groups each other. we analyzed the blood sample daily to measure CRP, ESR and leukocytosis. The results of this study are as the following : 1. Serum C-Reactive Protein levels in the experimental group were higher than control group and the pattern of CRP changes continued to decline in both groups(P<0.001). 2. ESR changes in both groups were scattered without any special pattern. 3. Correlation between CRP and Leukocyte was higher than others(r=0.664, P<0.01). 4. In the Receiver Operating Characteristic(ROC) Curve of experimental group, rapid increase of CRP curve and Area under the curve(AUC) value, 0.774, indicate the high accuracy of estimation. 5. In experimental group, sensitivity of CRP, ESR, and Leukocytosis on recovery day were 83%, 17% and 71%. Based on the results of this study, we could conclude that determination of CRP is more useful method to diagnosis and follow-up study of infection than other commonly used variables in oral and maxillofacial region.

The Effect of Smartphone Video Educational Program on Educational Satisfaction of Patients in Rehabilitation Units at a University Hospital (스마트폰 동영상 교육 프로그램이 일개 대학병원에서의 재활병동 입원환자 교육 만족도에 미치는 영향)

  • Purpose: This study was to determine the effect of a smartphone video educational program on educational satisfaction of patients in rehabilitation units at a university hospital. Methods: The subjects of this study were 60 stroke patients recruited and divided into two groups; experimental group (n=30) and control group (n=30). Subjects in the experimental group and control group conducted conventional physical and occupational therapy. In addition, only the experimental group provided smartphone video information for a total of 15 sessions. The video consisted of five episodes (10 minutes each) viewed a total of 15 times (for each video three times). The outcome was assessed by educational satisfaction. Result: After inervention, both groups revealed significant increased awareness of prevention of infection educational satisfaction and prevention from falling educational satisfaction and exercise program educational satisfaction. However, the experimental group was more effective than the control group relative to heightened awareness of increasing prevention of infection prevention from falling and exercise program. Conclusion: We suggest that a smartphone video educational program may be effective in improving awareness of the prevention of infection, prevention from falling, and exercise program educational satisfaction. Therefore, video-based information is beneficial for such patients. Further studies are needed for better understanding of the effectiveness of a smartphone video educational program in rehabilitation units at a university hospital.

  • Experimental infection and histopathological study using a deleterious aquatic fungus (Saprolegnia diclina type I) were carried out in the eels (Anguilla japonica) to know what pathological changes the fungus would produce in the affected eels. The eels in group A and B which were treated with fish net were not susceptible to experimental infection. In group C which the cuticle was scraped by artificial treatment, the aquatic fungi were invading all the eel bodies including the desquamated lesions. Histopathologically, the affected eels were markedly reduced in number of goblet cells and showed moderate damage of the clavate cells. Nodular formation involving ecchymotic hemorrhage was seen in some affected areas. The eels survived to 25 days after experimental infection.

Effects of Daily Chlorhexidine Bathing on the Acquisition of Multidrug-resistant Organisms and Healthcare-associated Infection in an Intensive Care Unit (중환자실에서 클로르헥시딘 목욕이 다제내성균 획득과 의료관련감염에 미치는 효과)

  • Purpose: The aim of this study was to verify the effects of daily 2% chlorhexidine gluconate (CHG) bathing on the acquisition of multidrug-resistant organisms (MDRO) and healthcare-associated infection (HAI) in a medical intensive care unit (MICU). Methods: The study was a randomized controlled group posttest only design, involving 91 patients in MICU at a tertiary hospital (47 patients in the experimental group and 44 patients in the control group). The 2% CHG bathing was performed daily according to bathing protocol to the patients in the experimental group, and traditional bath was performed every three days to those in the control group. Fisher's exact test and x2 test were used to analyze the data. Results: MDRO were found in 6 patients of the experimental group and in 15 patients of the control group. The difference was statistically significant (p= .016). HAI occurred in 2 patients of the experimental group and in 7 patients of the control group. The difference was not statistically significant (p= .084). Conclusion: The results confirmed that daily bathing with CHG was effective in reducing the incidence of MDRO acquisition. Therefore, it is expected that daily bathing with CHG will be used as an effective nursing intervention to reduce the incidence of MDRO acquisition.

THE EFFICACY OF PROPHYLACTIC ANTIBIOTICS FOR MANDIBULAR THIRD MOLAR EXTRACTION (하악 제3대구치 발치를 위한 예방적 항생제의 효용성)

  • This study analyzed the incidence of wound infection after the operation of mandibular third molar extraction in relation with antibiotic prophylaxis with the object of young and healthy patients. The study object was 1,177 mandibular third molars of 850 men of 20 to 25 years old without any specific systemic disease. Three methods of preventive antibiotic medication were selected according to the preventive antibiotic medication previously reported; three experimental groups were selected based on them, and the antibiotic used was amoxicillin($^{(R)}Kymoxin$, Yuhanyanghaeng, Seoul). The group 1 includes the patients that took the antibiotic orally before the operation(one hour earlier, 500mg) and for three days after the operation(250mg per time, three times/day), the group 2 is the ones that took the same antibiotic orally only once about one hour before the operation(500mg), and the group 3 did not take any antibiotics before and after the operation. And to compare the difficulties and the degrees of extraction during operations which can be possibly related to the wound infection after the operations, the mandibular third molars' impacted depths and extraction methods were investigated as well. To check if the wound was infected, observations with an internal of one week were performed twice after the operation, and the meaningfulness of the infection incidence was verified through Chi-square test using SPSS program(SPSS Inc., IL, USA). There was no statistically significant difference between the antibiotic medication methods and the wound infection incidence after the operation among the experimental groups. As examining the relations between the mandibular third molar operation methods and the wound infection incidence after the operation, there existed a statistically meaningful difference in the infection incidence according to the operation methods(p=0.020). And there was no statistically significant difference in the wound infection incidence according to the impacted depth of the mandibular third molar. Therefore, it is thought that there exists little necessity of prophylatic antibiotics medication when extracting the mandibular third molar of young and healthy men without any systemic disease in general; however, in case when it is expected that the possibility of infection will be high or the wound on the tissue will be severe, it is sure that the prophylactic antibiotics medication will be necessary.
