Comparison of the Effects of Physiotherapy vs. Corticosteroid Injection in Patients with Chronic Plantar Fasciitis (만성 발바닥 근막염 환자에게 물리치료와 코르티코스테로이드 주사가 미치는 효과 비교)

  • Purpose : This study was to compare the effect changes after physical therapy (extracorporeal shock wave therapy + high-load strengthening exercise) or corticosteroid injection in patients diagnosed with chronic plantar fasciitis. Methods : A total of 40 patients were randomly assigned to each group of 20. According to the intervention method, "Group 1. Physiotherapy" was performed for 12 weeks, and "Group 2. Corticosteroid injection" was performed only once. As a pre-intervention test, plantar fascia thickness, pain intensity I, II (What is the most painful moment of the day?, How painful is the first step in the morning?), and functional performance were measured. To compare the effects of each group, the tests 3, 6, and 12 weeks after were also performed using the same measurement method. Also, after 12 weeks, patient satisfaction was also compared. Results : There was no significant difference between the groups in the change in the thickness of the plantar fascia during all periods. However, pain intensity I, II was significantly lower in Group 1 than in Group 2 at only 12 weeks and functional performance was also significantly increased in Group 1 compared to Group 2 at only 12 weeks. Also, there was no significant difference between groups in patient satisfaction. Conclusion : The physiotherapy protocol, which consisted of extracorporeal shock wave therapy and high-load strengthening exercise, showed excellent results, especially after 12 weeks, compared to corticosteroid injection. It is recommended as a more effective treatment method as it is possible to safely return to daily life by reducing pain and improving functional performance.

The Development of Prediction Equation for Estimating VO2max from the 20 m PSRT in Korean Middle-School Girls. Exercise Science (20 m 점증 왕복달리기 검사를 이용한 여중생의 VO2max 추정식 개발)

  • The purpose of this study was to develop and validate regression models to estimate maximal oxygen uptake (VO2max) from the 20 m Progressive Shuttle Run Test (20 m PSRT) in Korean middle-school girls aged 13-15 years. The 20 m PSRT and VO2max were assessed in a sample of 194 participants. The sample was randomly split into validation (n=127) and test-retest reliability (n=99, 32 out of 127 participants also performed validity test) groups. 127 participants performed a graded exercise test (GXT, stationary gas analyser) and the 20 m PSRT (portable gas analyser) once to develop a VO2max prediction model and to analyze the validity of the modified 20 m PSRT protocol (starting at 7.5 km/h and increasing by 0.5 km/h every 1 min). 99 participants performed the 20 m PSRT twice for test-retest reliability purpose. Mean measured VO2max (39.2±5.1 ml/kg/min) from the potable gas analyzer was significantly increased from that measured during the GXT from stationary gas analyzer (37.7±5.7 ml/kg/min, p=.001) using the modified 20 m PSRT protocol. But it was a narrow range (1.5 ml/kg/min). The measured VO2max from the potable and stationary gas analyzers correlated at r=.88(p<.001). Test-retest of the 20 m PSRT yielded comparable results (Laps r=.88 & final speed r=.85). New regression equations were developed from present data to predict VO2max for middle-school girls: y=.231×Laps-.311×weight(in kg)+46.201 (r=.74, SEE=4.29 ml/kg/min). It is concluded that (a) the modified 20 m PSRT protocol is a valid and reliable test and (b) this equation developed in this study provides valid estimates of VO2max of Korean middle-school girl aged 13-15 years.

  • Hemorheology plays an important role in atherosclerosis. Hemorheologic properties of blood include whole blood viscosity, plasma viscosity, hemaocrit, RBC deformability and aggregation, and fibrinogen concentration in plasma. Blood flow is determine by three parameters (pressure, lumen diameter, and whole blood viscosity), whole blood viscosity is one of the key physiological variables. However, the significance of whole blood viscosity has not yet not been fully appreciated. Whole blood viscosity has a unique property, non-Newtonian shear-thinning characteristics, which is primarily due to the presence of RBCs. Hence, RBC deformability and aggregation directly affect the magnitude of blood viscosity, and any factors or diseases affecting RBC characteristics influence blood viscosity. Therefore, on can see that whole blood viscosity is the causal mechanism by which traditional risk factors such as hypertension, hyperlipidemia, smoking, exercise, obesity, age, and gender are related to atherogenesis. In this regard, we included whole blood viscosity in the three key determinants of injurious pulsatile flow that results in mechanical injury and protective adaptation in the arterial system. Because whole blood viscosity is a potential predictor of cardiovascular diseases, it should be measured in routine cardiovascular profiles. Incorporating whole blood viscosity measurements into a standard clinical protocol could improve our ability to identify patients at risk for cardiovascular disease and its complications.

  • The purpose of this study is to investigate the method of trace gas technique by G.C and the effects of the type of blouse on ventilation. The experimental system employed a trace gas technique in which ventilation was monitored to determine oxigen exchange rate. The experimental variables were tested with three types of blouse depending on the sizes of neckline, armhole and waist line. Exercise conditions include standing and walking position. The results and discussion forcused on the construct validity of the apparatus/testing protocol. the effect of experimental variables on ventilation was also discussed.

Physical Therapy Approaches for Respiratory Diseases (호흡계 질환의 물리치료적 접근방법)

  • Background: Various functional factors should be incorporated during assessment and intervention for patient rehabilitation. Stable respiratory function is one of required factors for functional restoration. To maximize respiratory physical therapy intervention outcome, it is required to understand clinical features of respiratory diseases and physical therapy approaches. Methods: Previous studies were systematically reviewed through computerized search. Methodological qualities of selected studies were evaluated and the levels of recommendations were determined. Results: Assessment for respiratory pattern and thoracic mobility is of importance to improve cardiopulmonary fitness during physical reconditioning. Application of optimal therapeutic protocol can increase thoracic mobility and respiratory function. Interdisciplinary communication is critical during rehabilitation for respiratory patients. Health care provider should have professional knowledge and experience for cardiopulmonary fitness and obligation to endeavor for patients' respiratory rehabilitation. It is necessary to standardize therapeutic intervention, and rehabilitative respiratory exercise should be applied to confirm the effects of intervention. Conclusion: Respiratory diseases that may reduce patients' quality of life and cardiopulmonary fitness should be resolved through physical therapy approaches. Through conducting research, effect of evidence-based and patients' function-oriented intervention can be determined.

  • This study aims to expand the future study methodology and to develop a methodology of future-oriented curriculum analysis with future skills needs derived from patent analysis. With the case of information security, the methodology is applied to the 16 universities, which have information security department in undergraduate course. From the results, the followings are suggested : 1) for the increasing importance area including hacking, infiltration and PC security, a practical exercise should be emphasized; 2) for the convergence area including security policy, security legislation and OS security, proper faculties should be filed with recruiting field-based experts; 3) for the increasing importance area including professional area including security audit and information security protocol, the advanced curriculum related to graduate level should be provided.

  • Scince $^{201}$TI was introduced as a myocardial perfusion imaging agent in the early 1970s, scintigraphic evaluation of myocardial perfusion for the diagnosis of coronary artery disease is a valuable noninvasive diagnostic imaging modality. Stress radionuclide myocardial perfusion imaging is widely accepted to have high diagnostic and prognostic use in the assessment of patients with known or suspected coronary artery disease. With wise use of this nonivasive imaging technique, more patients are referred for stress perfusion imaging. Until now various protocols for stress testing and myocardial imaging were developed and used in worldwide. This article presented various protocols of stress testing and myocardial imaging for clinical use.

Is Short-term Exercise a Therapeutic Tool for Improvement of Cardioprotection Against DOX-induced Cardiotoxicity? An Experimental Controlled Protocol in Rats

  • Background and Objective: Cardiotoxicity and oxidative stress is a life-threatening side effect of doxorubicin (DOX). We investigate the effects of short-term exercise as therapeutic tool for improvement of cardioprotection against DOX-induced cardiotoxicity in the rat. Methods: Wistar males (weighing $257{\pm}28g$) were divided into six groups: (1) control+placebo (2) control+DOX $10mg.kg^{-1}$ (3) control+DOX $20mg.kg^{-1}$ (4) training+placebo (5) training+ DOX$10mg.kg^{-1}$ (6) training+DOX $20mg.kg^{-1}$. Cardiotoxicity was induced by DOX (10 and $20mg.kg^{-1}$). The rats in groups 4, 5 and 6 experienced treadmill running of 25 to $39min.day^{-1}$ and 15 to $17m.min^{-1}$, 5 days/wk for 3 wk. At the end of the endurance training program, rats in the 1 and 4 groups, in the 2 and 5 groups and in the 3 and 6 groups received saline solution, DOX $10mg.kg^{-1}$ and DOX $20mg.kg^{-1}$, respectively. Result: DOX administration (10 and $20mg.kg^{-1}$) caused significant increase in MDA and Apelin, an insignificant increase in NO and a significant decrease in SOD, as compared to the C+P group. Three weeks of the pretreatment endurance exercise resulted in a significant increase of Apelin and SOD, an insignificant increase of NO and an insignificant decrease of MDA, as compared to the C+P group. Furthermore, after three weeks of endurance training and DOX treatment with $10mg.kg^{-1}$ and $20mg.kg^{-1}$, a significant increase in apelin and SOD, and a significant decrease in MDA were detected in comparison to C+DOX10 and/or C+DOX20 groups. There was a significant difference between DOX$10mg.kg^{-1}$ and DOX$20mg.kg^{-1}$ treatments in MDA levels only. Conclusion: Pretreatment exercise may improve myocardial tolerance to DOX-induced cardiotoxicity by inhibition of oxidative stress and up-regulation of antioxidants in heart tissue.

Influence of Rotation Number in the Effect of PC6 on the Heart Rate: A Pilot Study (내관 자침 시 염전 횟수가 심박 변이도에 미치는 영향: 예비연구)

  • Objectives : In the acupuncture treatment of Korean Medicine, the Tonifying or Purging depending on how many times the needle is rotated has been used widely. However, there is little evidence about the optimal number of rotation. This study, therefore, was aimed to investigate the optimal number of rotation. Methods : Heart rate variation was measured before and after exercise in 20 healthy adults. Acupuncture was performed at PC6 immediately after exercise according to the protocol of each group without remaining. Pre-exercise and post-exercise measurements were compared and the rotation number that produced statistically significant difference was investigated. Results : Significant differences were found in the heart rate variation and R-R interval Average, between non-rotation group (control) and both of 15 times rotation group of tonifying and 6 times rotation group of purging. Conclusions : Significantly effective number of rotation in the effect of PC6 on the HR and RRAv was 15 times in the tonifying and 6 in the purging respectively.

Effects of Herbal Sports Drinks Containing Prunus mume Fruit Extract on the Plasma Lipid Profile and Endurance of Rats (매실을 첨가한 한방스포츠음료가 흰쥐의 혈중지질 및 운동수행력 증강에 미치는 영향)

  • This study examined the effects of four herbal sports drinks containing Prunus mume, Liriope platyphylla, and Acanthopanax senticosus fruit extracts on body fat and endurance of rats trained by a progressive loaded exercise program. Forty male Sprague-Dawley rats (6 weeks old) were divided into five groups and fed experimental diets for 6 weeks according to the following protocol: C, exercise-trained control group (n=8); A, exercise group with Acanthopanax senticosus extract (n=8); L, exercise group with Liriope platyphylla extract (n=8); PA, exercise group with Prunus mune fruit extract plus Acanthopanax senticosus extract (n=8); PL, exercise group with Prunus mune fruit extract plus Liriope platyphylla extract (n=8). Endurance was measured by a progressive loaded exercise test using a treadmill. PA and PL supplementation significantly extended time to exhaustion compared to the control group (p<0.05). Further, the four herbal sports drinks all significantly reduced epididymal and interscapular white adipose tissue weights compared to the control group (p<0.05). Plasma triglyceride concentration was significantly lower in the A group compared to the control group. Plasma free fatty acid concentration was higher in the A group compared to the control group. On the other hand, Acanthopanax senticosus fruit extract supplementation tended to reduce the plasma glucose concentration compared to the control group. PA and PL supplementation significantly increased gastrocnemius muscle LDH activities compared to the control group (p<0.05). These results show that sports drink containing Acanthopanax senticosus improved endurance capacity, plasma lipids, and glucose concentrations. Sports drink with Prunus mume and Liriope platyphylla or Acanthopanax senticosus was synergistically improved endurance.
