The Effect of a Community-Based Group Exercise in Chronic Stroke

  • Community-based group exercise programs for people with chronic stroke are relatively uncommon in Korea. In addition, it is currently not known whether a community-based group exercise program is effective or not. The purpose of this study was to evaluate an 8 week community-based functional exercise program for its effects on balance performance and occupational performance in persons with chronic stroke. Twenty-five community-dwelling individuals with stroke participated in this program. Outcome of the program was assessed by the Berg Balance Scale and the Canadian Occupational Performance Measure (COPM). The functional exercise program lasted for eight weeks, with a 1-hour program twice per week and it consisted of mobility, stability, balance, functional strength, and gait training. The subjects were trained by one physical therapist but were under one-to-one supervision from students. The data of sixteen individuals who scored more than 24 on the Mini-Mental State Examination (MMSE) were analyzed. There was a significant effect both in terms of the COPM Performance Score & the Satisfaction Score (p=.002) and with the Berg Balance Score (p=.001). It was found that a short-term community-based exercise program could improve both performance of activities and balance. Further, all subjects reported that they were satisfied with this program.

The Effect of Pelvic Muscle Exercise Program on Women with Stress Urinary Incontinence in the Degree and Amount of Urinary Incontinence and Maximum Vaginal Contraction Pressure (골반저근육 운동 프로그램이 긴장성 요실금 여성의 요실금정도, 요실금량 및 최대 질수축압에 미치는 영향)

  • The purpose of this study was to help the women with stress urinary incontinence lead more comfortable life, by letting them to do pelvic muscle exercise and to learn by direct experience the effect that urinary incontinence is controlled. The research design was a one-group pretest-posttest design. The subjects were 16 middle-aged women(over 38) with stress urinary incontinence. The study was conducted from August 1, to October 11, 1999. Women trained themselves for muscular strength and endurance, every the other day for each exercise for six weeks at home(that means each exercise for more than three days a week) in the pelvic muscle exercise program which was developed by Dougherty et al., and filled out exercise participation card every week. They visited laboratory once a week to get visual feedback, by means of Perineometer, of how the pelvic muscle exercise was going on. Stress Urinary Incontinence Scale that Lee, Young-Sook revised and supplemented Hendrickson's original scale was used for measuring the frequency and situation scores of urinary incontinence, "30 minute pad test" was carried out for measuring the amount of urinary incontinence, and Perineometer was used for measuring maximum vaginal contraction pressure. Percentage, mean, standard deviation and Wilcoxon signed ranks test were used for data analysis by means of SPSS/PC+ WIN 9.0 program. The results were summarized as follows : The hypothesis 1 that after carrying out pelvic muscle exercise program on women with stress urinary incontinence, the weekly degree of urinary incontinence would reduce compared to before the exercise was supported. The hypothesis 1-1 that after carrying out pelvic muscle exercise program on women with stress urinary incontinence, the weekly frequency of urinary incontinence would reduce compared to before the exercise was supported (p=.003). The hypothesis 1-2 that after carrying out pelvic muscle exercise program on women with stress urinary incontinence, the situation scores of urinary incontinence would reduce compared to before the exercise was supported(p=.044). The hypothesis 2 that after carrying out pelvic muscle exercise program on women with stress urinary incontinence, the amount of urinary incontinence would reduce compared to before the exercise was supported(p=.001). The hypothesis 3 that after carrying out pelvic muscle exercise program on women with stress urinary incontinence, the maximum vaginal contraction pressure would increase compared to before the exercise was supported (p=.012). These results suggest that pelvic muscle exercise program has an effect on women with stress urinary incontinence in the degree and amount of urinary incontinence and maximum vaginal contraction pressure. So it is judged that training women with stress urinary incontinence for pelvic muscle exercise is an effective nursing intervention strategy in order to care urinary incontinence.

Verification of the Effectiveness of a combined Exercise Program for Improving the Health of the Elderly (노인건강증진을 위한 복합운동프로그램의 효과 검증)

  • This study was conducted to verify the effectiveness of a combined exercise program to improve the health of the elderly. For this purpose, 10 people in the exercise group and 10 in the control group were selected among seniors aged 65 years or older. The subject of this study was a 12-week combined exercise program in which aerobic and anaerobic exercise were performed three times a week for 60 minutes. Before participating in exercise, blood lipid and inflammatory index tests were conducted, and blood lipid and inflammatory index tests were conducted upon completion of the combined exercise program. The test results were verified with basic statistics and treatment effects between groups using the SPSS statistical program. Based on this, the following conclusions were obtained. First, the combined exercise program for elderly health promotion showed significant differences in TC, LDL, and TG in blood lipids between the exercise group and the control group, and there was an interaction effect on the treatment program. Second, the combined exercise program for improving the health of the elderly showed significant differences in inflammatory indicators IL-6 and TNF-a between the exercise group and the control group, and there was an interaction effect with the treatment program.

  • A great deal of attention has been paid to the quality of life in citizens due to the increased number of long term life expectancy. reported as effective in the health promotion of the senior citizens. In this study, the principal investigator developed an exercise program for the senior citizens, which could be educated and managed by nursing staff. This kind approach may support the need of exercise program on regular base via the social organization, which may mean the intentional change of life style. The exercise program consisted of exercise, health education, and a direct the population of the senior A regularexercise has been nursing care. A quasi -experimental study was conducted to investigate the effects of excercise program on health of the elderly in senior citizen's center. Independent variable was a 9 weeks exercise program. Dependent variables were: 1) physical health [e.g. physical fitness(cardiopulmonary endurance, back muscle strength, grip strength, muscular tolerance, flexibility and body fat proportion), physiologic parameters(blood pressure, pulse, respiration. and blood glucose), and perceived physical health status; 2) mental health measured by depression score; and 3) cognitive perception of exercise measured by usefulness and self-efficacy. A total of 37 subjects was randomly assigned into either the control group (without the exercise program: n=18) or the intervention group(with the exercise program: n=19). The results of the study analyzed using a SAS, were as follows: 1) In physical fitness of physical health, there was a significant improvement in cardiopulmonary tolerance. back muscle strength. muscular tolerance and flexibility in the intervention group, compared to the control group, while no difference in grip strength and body fat proportion. The exercise program resulted in decreases in blood pressure, pulse, respiration and blood glucose within normal range and improvement of perceived physical health status in the intervention group. 2) There was a slight increase of mental health(depression score) in the intervention group compared to the control group. but without statistical significance. 3) There was a significant improvement in cognitive perception of exercise(e.g. usefulness and self -efficacy) in the intervention group, compared to the control group. It was concluded that the exercise program employed in this study was appropriate for women senior citizens and had a positive effect on health in general.

The Effect of Exercise Program on pain of Patients with Chronic Low Back Pain (운동프로그램이 만성요통 환자의 통증에 미치는 효과)

  • To investigated the effect of the exercise program in patients with chronic low back pain, 40 patients paticipated in this experiment, who were applied for the simple exercise program and the complex exercise program respectively. The interval change of?pain?from pre-test and post-test over 7 weeks and the difference between two experimental groups are the follows : 1) There was statistically significant differences at comparison with pain in simple exercise group during weeks except first week(p<.05). 2) In complex exercise group, there was statistically significant differences at comparison with pain during each week. (p<.05) 3) There was not significant difference between simple exercise group and complex exercise group in pain by exercise period. 4) There was more significant change in complex exercise group than simple exercise group. but there was not statically significant differences between them.

The Relationships among Network Centrality, Psychological Well-being, and Intention to Exercise Maintenance in Participants of an Aquatic Exercise Program (수중운동 프로그램 참여자의 네트워크 중심성과 심리적 안녕감, 운동지속의도와의 관계)

  • Purpose: The purpose of this study was to identify the relationships among network centrality, psychological well-being (PWBS), and intention to exercise maintenance in participants of an aquatic exercise program. Methods: Using a single-experimental design, 17 osteoarthritis patients participated in an aquatic exercise program. The questionnaire to connect the network of members was used to peer nomination by Moreno (1953). Data were analyzed with the UCINET using centrality (degree, closeness, betweenness) and SPSS using descriptive statistics, wilcoxon signed ranked test, and spearman's rho. Results: Closeness centrality, PWBS, and intention to exercise maintenance were significantly different between 4 weeks and 8 weeks. At 4 weeks, PWBS was positively correlated with closeness centrality. Intention to exercise maintenance was positively correlated with degree, closeness, and betweenness centrality. At 8 weeks, PWBS was positively correlated with closeness centrality. Intention to exercise maintenance was positively correlated with closeness centrality. Conclusion: The aquatic exercise program can be effective in increasing closeness centrality, psychological well-being, and intention to exercise maintenance. This was the first study attempted to analyze construction of member relationships in osteoarthritis patients participating an exercise program by using social network analysis.

Effects of Exercise Program for Workers in Small Enterprises (운동프로그램의 효과 -소규모사업장 근로자를 대상으로-)

  • Physical activity is one of important factors of the chronic disease. It was reported that Most of Korean workers didn't take exercise regularly. The purpose of this study is testing effects of exercise program, based on the Theory of Planned Behavior. 30 subjects participating in this study are workers in small enterprises and have not taken exercise regularly. Exercise program, which includes education, support, monitoring, and counseling, was conducted for 6 weeks. Effects of this program was evaluated by the change of exercise, attitude, subjective norm, perceived control. intention of exercise, perceived health states, and fatigue. The results of this study can be summarized as follows: 1. There was the significant difference between pre and post-test in the subjective norm, perceived control. and intention of exercise, but not the significant difference in the attitude of exercise. 2. 53.3% of the subject of this study had taken exercise regulary in the post-test. 3. There was not the significant difference in the perceived health states and fatigue, From the results, it can be concluded that the exercise program is effective, leading to the change of intention and exercise rate. On the other hand, it was not effective in Physiological aspects. It seems that these result was due to short term program. Thus, it can be recommended that studies are needed with longer term program and larger sample.

The Effects of Walking Exercise Program on Aging-related Hormones in Elderly Men (걷기운동 프로그램이 남성 노인들의 노화관련 호르몬에 미치는 영향 - 경산지역 노인들을 대상으로 -)

  • Objectives: The purpose of this study was to evaluate the effects of 12 weeks walking exercise program participation on changes in aging-related hormones concentration in elderly men. Methods: 16 elderly men(above 65 years old) were participated in 12 weeks walking exercise program by group types(exercise group ; n=8 & control group ; n=8), and their aging-related hormones(GH, testosterone and melatonin) levels were assessed before program(0 week) and after program(12 weeks). Regularly walking exercise program(field walking exercise) performed for 12 weeks(3days/week, 60~70min/day, HRR 55~65%). All data were expressed as mean and standard deviation by using SPSS/$PC^+$(ver. 13.0) program, and to evaluate the differences before and after treatment by groups, data were analyzed by paired t-test(a${\leq}$.05) and 2-way ANOVA was performed to examine the differences between groups and within groups by variables. Results and Conclusion: The conclusions obtained from this study were as follows ; In growth hormone. testosterone and melatonin response after 12 weeks exercise was increased significantly in walking exercise group(p<.01; p<.001; p<.001). And in result of 2-way ANOVA analysis for group, exercise, group${\times}$exercise, as compared with the exercise, there is significant difference in all items. Interaction effect of between group and exercise is difference significantly on GH(F=5.915, p<.05), testosterone(F=15.117, p<.01) and melatonin(F=8.131. p<.01) in all groups. We observed a direct relationship(positive improvement) between the regular physical activity and aging-related hormones(growth hormone, testosterone and melatonin concentrations) in elderly men.

Effect of Aquatic Exercise Program on Lower Limbs, Muscle Strength, Knee Joint Flexion and Pain Changes of Arthritis Patients (수중운동이 관절염 환자의 하지근력, 관절각도 및 통증에 미치는 영향)

  • In an attempt to investigate the effect of long-term aquatic exercise program on lower limbs' muscle strength, knee Joint flexion, pain reduction and weight changes with aquatic exercise program. Forty three women with arthritis were pre and post tested for changes of muscle strength, range of motion, weight and pain. This data was collected from April 1, 1997 to February 24, 1998. The mean age was 52.5. Statistically significant in lower limbs' muscle strength with an aquatic exercise program by age, BMI, education, rheumatic drug use, illness duration until 12 weeks after experiment. Statistically significant in knee joint flexion with aquatic exercise program by age, BMI, education, rheumatic drug use, illness duration until 12 weeks after experiment. Statistically significant in pain reduction with aquatic exercise program by age, BMI, education, rheumatic drug use, illness duration until 12 weeks after experiment. Statistically significant in weight changes with aquatic exercise program by age, BMI, education, rheumatic drug use, illness duration until 6 weeks after experiment. As a conclusion, aquatic exercise programs for the patients with arthritis require at least 12 weeks and a variety of aquatic exercise programs for the effective control should be developed.

The Effect of Aerobic Exercise Program for Chronic Disease Patients (성인병 환자를 위한 유산소성 운동프로그램의 효과)

  • The purpose of this study is to determine the effects of exercise programs on the treatment of chronic diseases. For this purpose, nine middle-aged women suffering from such chronic diseases as obesity, hypertension and coronary heart disease(CHD) were sampled to perform a 4-month-long exercise program which required the lactate threshold(LT) level corresponding to 70% of maximal oxygen uptake(VO2max). Thereupon, their physiques, physical fitness, blood components and physiological varibles were compared pre and post of exercise program. The results were summarized as follows ; 1) After the 4-month-long exercise program, %fat was significantly decreased, while such variables as grip strength, side step, trunk flexion and leg balance were significantly increased. 2) Blood components such as TG, TC, HDL-C, LDL-C, hemoglobin and hematocrit did not show and change in after exercise program. 3) Physiological varables such as HR, SBP, DBP, HR/LT, SBP/LT, DBP/LT, RPE/LT, VE/LT, VO2/LT and VO2 max showed significant increase after 4-month exercise program. These results suggest that regular aerobic exercise in a long period might help to decrease % fat and to increase aerobic capacity in middle-aged women suffering from chronic diseases.
