관절염 환자의 운동행위 결정요인 (Determinant factors of Exercise behaviors in Patients with Arthritis)

  • The aims of this study were to understand and to predict the determinant factors affecting the exercise behaviors and physical fitness by testing the Ponder's health promotion model, and to help the patients with rheumatoid arthritis and osteoarthritis perform the continuous exercise program, and to help them maximize the physical effect such as muscle strength. endurance, and fuctional status and mental effects including self efficacy and quality of life, and improve the physical and mental wellbeing, and to provide a basis for the nursing intervention strategies. We analyzed the clinical records of 208 patients with rheumatoid arthritis and degenerative arthritis who visited the outpatient clinics at H university hospital in Seoul between October 5, 1999 and October 24, 1999. Data were composed of self reported questionnaire and good of fitness score which were obtained by pedalling the ergometer of bicycle for 9 minutes. SPSS Win 8.0 and Window LISREL 8.12a were used for statistical analysis. 24 Of 54 hypothetical paths were supported in modified model, which was considered as a proper model with improved fit index. The physical fitness was directly influenced by exercise participation behavior and education level, and indirectly by physical fitness, while fatigue, physical disability, pastexercise behavior, life-style, self-efficacy, which explained 20% of physical fitness. The exercise participation were directly influenced by perceived benefits and self-efficacy, and indirectly influenced by life-style, fatigue and physical disability, and directly and indirectly by past exercise behavior, which explained 53% of exercise participation. Exercise score were directly affected by perceived health status, perceived benefits, self efficacy, and past exercise behavior, and were indirectly affected by fatigue, physical disability, and life-style, which explained 50%. Perceived health status were directly influeced by level of education, depression, sleep disorder, and physical disability, which explained 34% of perceived health status. Perceived benefit was directly influenced by fatigue, sleep disorder, physical disability, and life-style, which explained 45%. Perceived barriers was directly influenced by fatigue, sleep disorder, and lifestyle, which explained 9%. Self- efficacy was directly influenced by fatigue, physical disability, past exercise behavior, and level of education, which explained 61%. In conclusion, important variables for physical fitness were exercise participation and level of education, and variables affecting exercise participation were perceived self-efficacy, benefits, and past exercise behavior. Perceived self-efficacy of exercise was a significant predictor of exercise participation. Life-style, fatigue, and physical disability showed direct effects on perceived benefit, perceived barriers, and self-efficacy, and indirect effects on exercise behavior. Therefore, disease related factor should be minimized for physical performance and well being in nursing intervention for patients with rheumatoid arthritis, and plans to promote and continue exercise should be soaked to reduce disability. In addition, Exercise program should be planned and performed by the exact evaluation of exercise according to the ability of the patients and the contents to improve the importance of exercise and self efficacy in self control program, dedicated educational program should be involved.

근린 체육활동시설이 운동시간과 주관적 건강 수준에 미치는 영향 분석 - 국민생활체육참여실태조사(2013~2015) 자료를 중심으로 - (The Impacts of Neighborhood Sport Activity Facility on Exercise Time and Subjective Health Level - Focused on the National Sport Participation Survey (2013-2015) in Seoul, Korea -)

  • The purpose of this study is to examine the impacts of neighborhood sports activity facilities on the exercise time and subjective health level using the National Sport Participation Survey data(2013-2015) in Seoul, Korea. We conducted path analysis including exercise time, subjective health level and various environmental factors. From the survey data, we extracted individual's characteristics including subjective health and exercise time. In addition, we calculated physical environment variables of neighborhood units. The results of this study are as follow. First, most of moderating variables such as gender, age, income and job showed expected relationships with exercise time or subjective health level. Second, the perceived level regarding neighborhood sports facilities such as public sports facilities had a strong positive association with exercise time and subjective health level. Third, the area of public sports facility, neighborhood park and the number of schools had positive effects on the exercise time and subjective health level. Lastly, land use mix and the number of intersections showed a positive effects on exercise time. To sum up, the results indicated that public sports facility, neighborhood park and perceived environment on neighborhood sports facility are critical factors of residents' exercise time and subjective health level.

The Effects of Exercise Commitment of University Water Sport Participants on Re-participation Intentions

  • Purpose: Today, participation rate of water sport has been dramatically increased due to expansion of leisure time and concerns. The purpose of this study is to identify the effect of the exercise commitment of university water sport participants on re-participation intentions and to induce insights for expanding water sport industry. Research design, data, and methodology: Subjects were collected from undergraduate students who participated water sports class in University. The sample was extracted by purposive sampling of nonprobability sampling method. To achieve the purpose of the research, survey was processed to 281 subjects. SPSS Version 21.0 was used to analyze the data. All statistical significance level was set to p<.05. In terms of measurement, items of demographic information, exercise commitment, and re-participation intentions were utilized. Results: The results are as follows. Frist, in the difference in exercise commitment and re-participation intention according to the grade, only the re-participation intention was significant. The effects of the exercise commitment of university water sport participants on re participation intentions were statistically significant. Conclusions: The findings indicate the function of sport club participants' commitment towards water sports in promoting their intentions to re-participation. The study suggested insights and future directions for expanding water sport markets and related industry.

여자 대학 태권도 선수들의 선수 경력과 대회 참가빈도가 운동몰입에 미치는 영향 (Effects of Athlete Career and Competition Participation Frequency on Exercise Commitment of Women University Taekwondo Athletes)

  • 본 연구의 목적은 여자 대학생 태권도 선수들의 선수 경력, 대회 참가빈도가 운동몰입에 미치는 영향을 분석하는 데 있다. 본 연구의 대상은 여자 대학 태권도 선수 20명이다. 선수 경력과 대회 참가빈도는 4점 척도로 조사하였고, 1점에서 4점으로 점수가 높으면 높을수록, 선수 경력과 대회 참가빈도가 높음을 의미한다. 운동몰입에 대한 평가는 운동몰입척도를 활용하였다. 운동몰입척도는 총 8문항으로 행위 몰입 4문항과 인지 몰입 4문항으로 구성되어있고, 5점 리커트 척도를 활용하여 평정하며, 점수가 높으면 높을수록 운동몰입 수준이 높음을 의미한다. 그 결과 대회 참가 빈도와 운동몰입 간에 정적 (+) 상관관계와 인과관계가 나타났다. 선수 경력은 부적 (-) 인과관계를 보였다. 이러한 결과는 대회 참가빈도가 높을수록 운동몰입 수준이 증가하며, 선수 경력이 길수록 운동몰입 수준이 감소한다고 설명할 수 있다. 따라서, 여자 대학 태권도 선수들의 운동몰입 수준을 향상시키기 위해 대회 참가 빈도와 선수 경력을 고려해야 할 것이다.

  • The aims of this study were to understand and to predict the determinent factors affecting the exercise behaviors and physical fitness by testing the Pender's revised health promotion model, and to help the patients with rheumatoid arthritis and osteoarthritis perform the continous exercise program, and to help them maximize the physical effect such as muscle strength, endurance, and functional status and mental effects including self efficacy and quality of life, and improve the physical and mental well being, and to provide a basis for the nursing intervention strategies. Of the selected variables in this study, the endogenous variables included the physical fitness, exercise score, exercise participation, perceived benefits of action, perceived barriers of action to exercise, activity-related affect(depression) and perceived self-efficacy, interpersonal influences(family support), situational factors(duration of arthritis, fatigue) and the exogenous variables included personal sociocultural factor(education level), personal biologic factor(body mass index), personal psychologic factor(perceived health status) and prior related behavior factors(previous participation in exercise, life-style). The physical fitness was directly influenced by life-style, perceived health status, education level, family support, fatigue, which explained 12% of physical fitness. The exercise participation were directly influenced by life-style, education level, past exercise behavior, perceived benefits of action, perceived barriers of action, depression and duration of arthritis, which explained 47% of exercise participation. Exercise score were directly affected by perceived self efficacy. BMI, life-style, past exercise behavior, perceived benefits of action, family support, perceived health status. perceived barriers of action, and fatigue, which explained 70%. Perceived benefits of action was directly influenced by BMI, life-style, which explained 39%. Perceived barriers of action were directly influeced by past exercise behavior, perceived health status, which explained 7%. Perceived self efficacy were directly influeced by level of education, perceived health status, life-style, which explained 57%. Depression were directly influeced by past exercise behavior, BMI, life-style, which explained 27%. Family support were directly influeced by life-style, perceived health status, which explained 29%. Fatigue were directly influeced by BMI, life-style, perceived health status. which explained 41%. Duration of arthritis were directly influeced by life-style, past exercise behavior, BMI, which explained 6%. In conclusion, important variables for physical fitness were life-style, and variable affecting exercise participation were life-style. Perceived self-efficacy of exercise was a significant predictor of exercise score. BMI, Life-style, perceived benefits of action, family support, past exercise behavior showed direct effects on perceived self-efficacy. Therefore, disease related factor should be minimized for physical performance and well being in nursing intervention for patients with rheumatoid arthritis, and plans to promote and continue exercise should be seeked to reduce disability. In addition, Exercise program should be planned and performed by the exact evaluation of exercise according to the ability of the patients and the contents to improve the importance of exercise and self efficacy in self control program, dedicated educational program should be involved. This study suggest that the methods to reduce the disease related factors, the importance of daily life-style, recognition of benefit of exercise, and educational program to promote self efficacy should be considered in the exercise behavior promotion and nursing intervention for continous performance. The significance of this study is also thought to provide patients with chronic arthritis the specific data for maximal physical and mental well being through exercise, chronic therapeutic procedure, daily adaptation and confrontation in nursing intervention.

간호대학생의 운동 자기-도식과 운동행동에 관한 융합적 연구 (The Study of Exercise Self-Schema and Exercise Behavior in Nursing Students)

  • 본 연구는 간호대학생의 운동 자기-도식과 운동행동간의 관계를 파악하여 간호대학생의 운동실천정도에 영향을 미치는 요인을 파악하고자 시도되었다. 본 연구는 2018년 11월 1일부터 11월 30일까지 J시에 소재한 대학의 간호대학생 178명에게 실시하였다. 수집된 자료는 SPSS/WIN 21.0 프로그램을 이용하여 서술통계, t-test, ANOVA, Pearson's correlation coefficiention 및 다중회귀분석을 실시하였다. 연구결과 운동행동에 영향을 미치는 요인은 행동적 운동자기도식, 운동실천의도, 인지.감정적 운동자기도식 순이었으며, 설명력은 63.8%였다(F=104.59, p<.001). 본 연구를 바탕으로 간호대학생의 운동실천향상을 위해서는 올바른 운동자기도식을 형성시킬 수 있는 건강관련 교육과정 개발이 필요하며, 다양한 신체활동 프로그램을 운영할 수 있도록 대학차원의 적극적인 지원이 필요한 것으로 생각된다.

성별과 운동참여 형태에 따른 노인의 우울증과 인지기능 (Depression and cognitive function according to gender and exercise participation types in the elderly)

  • 본 연구의 목적은 성별과 운동참여 형태에 따라 노인의 우울증과 인지기능 수준에 차이가 존재하는지 규명하는 것이다. 본 연구의 대상은 2017년에 보건복지부 한국보건사회연구원에서 진행한 노인실태조사에 참여한 만65세 이상의 노인 10,059명이다. 연구대상의 우울증은 SGDS를 활용하여 측정하였고 인지기능은 MMSE-DS를 활용하여 측정하였다. 수집된 데이터는 빈도분석, 신뢰도 분석, 이원분산분석을 활용하여 분석의 목적에 따라 분석하였다. 본 연구를 통하여 나타난 결과는 다음과 같다. 첫째, 성별과 운동참여 여부에 따른 우울증 수준 차이를 분석한 결과, 성별 및 운동참여 여부에 따른 주효과 모두 통계적으로 유의한 것으로 나타났으나 상호작용 효과는 통계적으로 유의하지 않았다. 둘째, 성별과 운동참여 빈도에 따른 우울증 수준 차이를 분석한 결과, 성별에 따른 주효과는 통계적으로 유의한 것으로 나타났으나 운동참여 빈도에 따른 주효과와 상호작용 효과는 통계적으로 유의하지 않은 것으로 나타났다. 셋째, 성별과 운동참여 시간에 따른 우울증 수준 차이를 분석한 결과, 성별 및 운동참여 시간에 따른 주효과는 통계적으로 유의한 것으로 나타났으나 상호작용 효과는 통계적으로 유의하지 않은 것으로 나타났다. 넷째, 성별과 운동참여 여부에 따른 인지기능 수준 차이를 분석한 결과, 성별 및 운동참여 여부에 따른 주효과는 통계적으로 유의한 것으로 나타났으며 상호작용 효과도 통계적으로 유의한 것으로 나타났다. 다섯째, 성별과 운동참여 빈도에 따른 인지기능 수준 차이를 분석한 결과, 성별 및 운동참여 빈도에 따른 주효과는 통계적으로 유의한 것으로 나타났으나 상호작용 효과는 통계적으로 유의하지 않은 것으로 나타났다. 여섯째, 성별과 운동참여 시간에 따른 인지기능 수준 차이를 분석한 결과, 성별 및 운동참여 시간에 따른 주효과와 상호작용 효과 모두 통계적으로 유의한 것으로 나타났다. 결론적으로 본 연구를 통해 남성과 여성 모두 운동에 참여하는 것이 우울증 감소 및 인지기능 향상을 돕는다는 것을 알 수 있었고 운동 참여시 여성의 인지기능의 향상 폭이 남성에 비해 더 큰 것을 확인하였다.

성인 지적장애인들의 수영 프로그램 참여에 따른 자아효능감과 운동 몰입, 건강관련 삶의 질의 변화 (Changes in the Self-Efficacy, Exercise Commitment, and Health-related Quality of Life of the Adults with Intellectual Disability through the Participation of Swimming Program)

  • 연구목적은 성인 지적장애인들의 수영 프로그램 참여에 따른 자아효능감과 운동 몰입, 건강관련 삶의 질의 변화를 분석하는 데 있다. 연구대상은 지적장애인 거주시설에서 거주하고 있는 성인 지적장애인 12명이며, 수영 프로그램에 12주간 참여하였다. 이에 따라 대상자들의 자아효능감과 운동 몰입, 건강관련 삶의 질의 변화를 분석하였다. 자아효능감 평가는 전반적 자아효능감 척도를 활용하였으며, 일반적, 사회적 신체적 자아효능감을 평가하였다. 운동 몰입 평가는 스포츠 몰입척도를 활용하였으며, 인지몰입과 행위몰입의 수준을 평가하였다. 건강관련 삶의 질 평가는 건강관련 삶의 질 척도를 활용하여 전반적 건강관련 삶의 질의 수준을 평가하였다. 그 결과, 대상자들은 수영 프로그램 참여에 따라 자아효능감과 운동 몰입, 건강관련 삶의 질의 수준이 통계적으로 유의미하게 향상되었다. 수영 프로그램의 참여는 성인 지적장애인들의 자아효능감과 운동 몰입, 건강관련 삶의 질의 수준을 향상시키는 데 긍정적으로 작용한다. 이에 따라서, 성인 지적장애인들을 대상으로 수영 프로그램의 적용을 고려해볼 필요가 있을 것이다.

폐경 후 여성의 참여 동기 및 성공적 노화, 건강행위, 우울감에 관한 연구 (A Study on the Participation Motives, Successful Aging, Health Behavior and Depression in Women after Menopause)

  • 연구의 목적은 폐경기 이후 여성의 성공적 노화를 돕고 우울감을 낮추는 자가 관리 프로그램을 개발하고자 대상자의 참여 동기, 성공적 노화 수준, 건강행위 정도와 우울감 수준을 파악하고, 상관관계를 알아보는 것이다. 연구의 방법은 폐경기 이후 여성 252명을 대상으로 빈도, 백분율 및 일원분류 분산분석(One Way ANOVA)을 사용하여 자료를 분석하였다. 연구의 결과는 일반적 특성에 따른 운동 참여 동기와 성공적 노화의 수준 차이는 건강수준, 운동 종류, 운동 도움 여부, 거주지 등에 따라 통계적으로 유의미한 차이를 보였다. 건강행위 수준과 우울감의 수준은 연령, 건강수준, 운동 종류, 운동 수행 기간 등에 따라 유의한 차이를 보였다. 또한 우울감의 수준은 동거가족에서도 유의한 차이를 보였다. 상관관계 분석결과 운동 참여 동기와 성공적 노화, 건강행동 간은 상관계수 .001 수준에서, 우울감과 건강행동 간은 상관계수 .05 수준에서 유의한 것으로 나타났다. 결론은 폐경기 이후 여성의 자가 관리 운동 프로그램 개발은 매우 중요하며, 이는 성공적 노화를 돕고 우울감을 낮출 수 있을 것이다.

Exercise Barriers in Korean Colorectal Cancer Patients

  • Background: It has been proved that participating in exercise improves colorectal cancer patients' prognosis. This study is to identify barriers to exercise in Korean colorectal cancer patients and survivors. Materials and Methods: A total of 427 colorectal cancer patients and survivors from different stages and medical status completed a self-administered questionnaire that surveyed their barriers to exercise and exercise participation. Results: The greatest perceived exercise barriers for the sampled population as a whole were fatigue, low level of physical fitness, and poor health. Those under 60-years old reported lack of time (p=0.008), whereas those over 60 reported low level of physical fitness (p=0.014) as greater exercise barriers than their counterparts. Women reported fatigue as a greater barrier than men (p<0.001). Those who were receiving treatment rated poor health (p=0.0005) and cancer-related factors as greater exercise barriers compared to those who were not receiving treatment. A multivariate model found that other demographic and medical status were not potential factors that may affect exercise participation. Further, for those who were not participating in physical activity, tendency to be physically inactive (p<0.001) and lack of exercise skill (p<0.001) were highly significant barriers, compared to those who were participating in physical activity. Also, for those who were not meeting ACSM guidelines, cancer-related exercise barriers were additionally reported (p<0.001), compared to those who were. Conclusions: Our study suggests that fatigue, low level of physical fitness, and poor health are most reported exercise barriers for Korean colorectal cancer survivors and there are differences in exercise barriers by age, sex, treatment status, and physical activity level. Therefore, support for cancer patients should be provided considering these variables to increase exercise participation.
