허혈성 뇌졸중 환자를 위한 조기 인지훈련과 맞춤형 전화코칭 프로그램의 효과 (Effectiveness of Early Cognitive Training and Tailored Telephone Coaching Program for Ischemic Stroke)

  • 본 연구의 목적은 허혈성 뇌졸중 환자에게 적용된 조기 인지훈련과 맞춤형 전화코칭 프로그램의 효과를 확인하기 위함이다. 연구 대상 총 28명은 실험군(n=15)과 대조군(n=13)에 배정되었다. 실험군에게만 4주간의 인지 훈련과 11회기 맞춤형 전화코칭이 제공되었다. 프로그램의 효과는 우울, 간이정신상태검사, 신경심리검사, 일상생활수행능력에 관한 도구를 이용하여 총 4회(사전, 4주째, 8주째, 12개월째) 측청하였다. 반복측정 분산분석법을 통한 자료 분석 결과 실험군은 대조군에 비해 간이 정신상태검사, 언어기억력 및 집행기능의 유의미한 향상이 나타났다. 특히 일상생활수행과 관련성이 깊은 집행기능의 향상은 괄목할 만한 결과였다. 이는 뇌졸중 후 인지기능의 회복과 독립적인 일상생활을 위해 조기 개입이 매우 중요하며 중·장기적 효과를 위해 주기적인 전화코칭이 함께 이루어져야 함을 시사한다.

젊은 조현병 환자에서 공감경향과 마음이론기술: 자기보고 조현형차원 및 실행기능과의 연관성 (Empathic Tendency and Theory of Mind Skills in Young Individuals with Schizophrenia: Its' Associations with Self-Reported Schizotypy and Executive Function)

  • Objectives: Social function deficit is known as a core feature of schizophrenia. This study aimed to investigate differences in empathic tendencies and theory of mind (ToM) skills between healthy controls and young individuals with schizophrenia, and to examine the associations between empathic tendencies, ToM skills and schizotypy, and executive function in schizophrenia. Methods: Thirty patients with schizophrenia and 30 healthy controls were enrolled and assessed using the interpersonal relationship index (IRI; perspective taking, fantasy, empathic concern, and personal distress subscales), ToM-Picture Story Task (ToM-PST; sequence and cognitive questionnaire), Wisconsin schizotypy scale (revised physical anhedonia and perceptual aberration), and Stroop tests for empathic tendencies, ToM skills, schizotypy, and executive function. Results: In individuals with schizophrenia, the IRI for perspective taking and ToM-PST score for cognitive function were lower, and the IRI for personal distress was higher than those in healthy controls. The IRIs for perspective taking and fantasy were related to revised physical anhedonia, and that for empathic concern was associated with revised physical anhedonia and perceptual aberration. The ToM-PST score for sequence was associated with the Stroop test score for schizophrenia. Conclusion: These findings indicate deficits in empathic tendencies and ToM skills, which may be independently and primarily associated with schizotypy and executive function in young individuals with schizophrenia.

수면관련 호흡장애에서의 신경정신과적 증상 (Neuropsychiatric Dysfunction in Sleep-Related Breathing Disorders)

  • Sleep-related breathing disorders, especially sleep apnea syndrome are complicated by neuropsychiatric dysfunction such as excessive daytime sleepiness, cognitive dysfunction, and depression. As the determinants of daytime sleepiness, sleep fragmentation is more influential than nocturnal hypoxia. Daytime sleepiness can be improved by continuous positive airway pressure (CPAP) or surgery in up to 95% of the treated subjects. Both sleepiness and nocturnal hypoxia would cause cognitive dysfunction. While impairments in attention and verbal memory are more related with sleepiness and prominent in mild to moderate sleep apnea syndrome (SAS), impairments in general intellectual function and executive function are more related with nocturnal hypoxia and prominent in severe SAS. Several cognitive deficits related with nocturnal hypoxia may be irreversible despite CPAP or surgical treatments. So, early detection and early appropriate treatment of SAS would prevent sleepiness and cognitive deterioration.

유산소 운동이 소아 및 청소년의 인지기능에 미치는 영향 : 문헌고찰 (Effect of Aerobic Exercise on Cognitive Functions in Childen and Adolescents : A Review)

  • The purpose of this review was to investigate the relationship between aerobic exercise and cognitive function as well as synthesize the effect of aerobic exercise intervention studies centered on psychiatric symptoms associated with general cognitive deficit. Prospective studies on aerobic exercise and improvement of cognitive function were reviewed and synthesized. In addition, this literature review provides significant positive results on the relationship between aerobic exercise and general cognitive deficit associated with psychiatric symptoms. Review of this literature suggests that there is a positive relationship between participation in aerobic exercise and cognitive function. In this text, there are at least three general pathways by which aerobic exercise may facilitate executive function in children : 1) cognitive demands inherent in engaging in physical activity, 2) cognitive demands inherent in cooperation of complex motor tasks, and 3) physiological changes resulting from aerobic exercise. Another main finding of this review is that physical activity has a stronger influence on cognitive deficit, including attention-deficit hyperactivity disorder, depression, and anxiety. Development of cognitive function is the most important factor for children and youth. Therefore, future research should prove relationship between physical activity and cognitive function using a more scientific and quantitative approach design.

정상인에서 메칠페니데이트가 학습에 미치는 영향 (Effect of Methylphenidate on Learning in Normal Population)

  • Methylphenidate is a widely used stimulant for treatment of attention-deficit hyperactivity disorder (ADHD). In addition to core symptoms of attention and behavioral symptoms, methylphenidate is even effective for executive functions. However, abuse and misuse of stimulants, including methylphenidate, for the purpose of cognitive enhancement is an issue of concern worldwide. Some prejudices and misunderstandings against methylphenidate are popular ; however, little attention has been given to the neuropsychiatric evidence of methylphenidate for enhancement of cognitive function among healthy populations. In this article, our aim was to conduct a review of previous studies investigating the effect of methylphenidate in healthy populations. Findings from several recent studies have demonstrated the effectiveness of methylphenidate for enhancement of cognitive function in healthy populations. The mechanisms of cognitive enhancement are thought to be associated with motivation and the reward circuit in the brain. However, when considering the risk to benefit, an official discussion of the use of methylphenidate among healthy members of the population would be premature. Instead, investigation of epidemiological studies of the prevalence of misuse of stimulants among healthy members of the population, particularly adolescents and college students, is needed. In addition, based on achievements reported in previous studies, investigation of the effect of an approach using non-pharmacological enhancing motivation, which will in turn result in increased cognitive function would be helpful.

지적장애 청소년의 집행기능 향상을 위한 태블릿 PC 기반 음악 만들기 활동 (A Tablet PC-Based Music-Making Program for Improving Executive Function of Adolescents With Intellectual Disabilities)

  • 본 연구는 태블릿 PC를 활용한 음악 만들기 프로그램이 지적장애 청소년의 집행기능 향상에 어떠한 효과를 나타내는지 알아보고자 실시되었다. 연구 대상은 지적장애 청소년 남학생 4명으로 세션은 주 2회씩 회기별 45분으로 진행하여 총 16회기 동안 개별 중재하였다. 음악 만들기 프로그램은 노래 구상 및 태블릿 PC 조작법 습득, 음악 요소 탐색, 리듬 멜로디 가사 반주 리듬 만들기, 루프 구간 설정 및 생성, 발표하기로 구성되었고 각각의 창작 과정은 난이도에 따라 단계별로 반복 진행되었다. 집행 기능 측정을 위해서 스트룹(Stroop)검사, 아동 색 선로 검사(CCTT), 작업기억지표의 하위검사인 숫자따라 외우기(DST)와 순차연결하기(LN) 검사를 사전 중간 사후에 실시한 결과 연구 결과 참여자들 모두 세 가지 검사에서 점수가 향상되었다. 비디오 촬영을 통한 행동 분석 결과, 주의 지속 시간이 증가하고 리듬과 멜로디 수행 시 나타나는 오류를 자발적으로 개선해 나갔으며 정보 범주화를 통해 정보의 활용과 응용이 가능해졌다. 이와 같은 결과는 태블릿 PC를 활용한 음악 만들기 프로그램이 지적장애 청소년의 집행 기능을 향상시키는 중재도구로 긍정적인 효과가 있음을 증명하였다. 더 나아가 기존에 지적장애 아동기에 집중되어있던 연구를 청소년기로 확장시켜, 인지기능 저하 및 인지적 과제 참여에 대한 동기가 낮은 다른 대상군에게도 확장 적용될 수 있음을 시사한다.

Cognitive and other neuropsychological profiles in children with newly diagnosed benign rolandic epilepsy

  • Purpose: Although benign rolandic epilepsy (BRE) is a benign condition, it may be associated with a spectrum of behavioral, psychiatric, and cognitive disorders. This study aimed to assess the cognitive and other neuropsychological profiles of children with BRE. Methods: In total, 23 children with BRE were consecutively recruited. All children underwent sleep electroencephalography (EEG) and were assessed on a battery of comprehensive neuropsychological tests including the Korean versions of the Wechsler intelligence scale for children III, frontal executive neuropsychological test, rey complex figure test, Wisconsin card sorting test, attention deficit diagnostic scale, and child behavior checklist scale. Results: The study subjects included 13 boys and 10 girls aged $9.0{\pm}1.6$ years. Our subjects showed an average monthly seizure frequency of $0.9{\pm}0.7$, and a majority of them had focal seizures (70%). The spike index (frequency/min) was $4.1{\pm}5.3$ (right) and $13.1{\pm}15.9$ (left). Of the 23 subjects, 9 showed frequent spikes (>10/min) on the EEG. The subjects had normal cognitive and frontal executive functions, memory, and other neuropsychological sub-domain scores, even though 8 children (35%) showed some evidence of learning difficulties, attention deficits, and aggressive behavior. Conclusion: Our data have limited predictive value; however, these data demonstrate that although BRE appears to be benign at the onset, children with BRE might develop cognitive, behavioral, and other psychiatric disorders during the active phase of epilepsy, and these problems may even outlast the BRE. Therefore, we recommend scrupulous follow-up for children with BRE.

The Motor-cognitive Training on Cognition and Physical Performance in the Older Adults with Mild Cognition Impairment : A Literature Review

  • Objective: Older adults with mild cognitive impairment (MCI) are more likely to progress to dementia. Motor-cognitive training is applied as a dual-task to improve the cognitive and physical functions of older adults with MCI. The purpose of the study was to know the recent trends in motor training types and outcome measures used for motor-cognitive training in older adults with MCI. Design: Aliterature review Methods: This literature review was conducted in Pubmed, MEDLINE® and Google Scholar with the following key words: older adults, mild cognitive impairment, motor-cognitive training, cognition, and dual-task. The 7 studies were found with the search tool and all studies were randomized controlled trials. Results: In motor-cognitive training in older adults with MCI, 6 studies applied aerobic exercise. And 3 out of 6 studies also applied strengthening exercises. One study applied dual tasks without aerobic exercise. In the 6 studies, overall cognitive and executive function were used as outcome measures, and physical function was evaluated as gait performance. Memory and physical frailty were also used as measurement tools. As a result of all studies, when motor-cognitive training was applied, cognition and physical performance showed significant results. Conclusions: A recent five-year study applied mainly aerobic exercise and strength training to older adults with MCI and found it to improve cognitive and physical performance.

인지훈련 프로그램이 급성 허혈성 뇌졸중 환자의 인지기능과 일상생활 수행능력에 미치는 효과 (Effects of a Cognitive Training Program on Cognitive Function and Activities of Daily Living in Patients with Acute Ischemic Stroke)

  • Purpose: The purpose of this study was to examine the effects of a cognitive training program on neurocognitive task performance and activities of daily living (ADL) in patients who had a stroke. Methods: The research design for this study was a nonequivalent control group non-synchronized design. Patients were assigned to the experimental (n=21) or control group (n=21). The experimental group received a 4-week cognitive training program and usual care (i.e., rehabilitation service), while the control was received usual care only. Cognitive function was measured with a standardized neurocognitive test battery and ADL was assessed at baseline and one and two months after completion of the intervention. Repeated measures ANOVA was used to determine changes in cognitive function and ADL over 2 months. Results: The interaction of group and time was significant indicating that the experimental group showed improvement in attention, visuospatial function, verbal memory, and executive function compared to the control group which had a sustained or gradual decrease in test performance. A significant group by time interaction in instrumental ADL was also found between the experimental group with gradual improvement and the control group showing no noticeable change. Conclusion: Findings show that the cognitive training program developed in this study is beneficial in restoring cognitive function and improving ADL in patients following a stroke. Further study is needed to investigate the long-term relationship between cognitive training participation and cognitive improvement and effective functioning in daily living.

인지기능 및 인지훈련효과의 관련변인에 관한 예비연구 (A preliminary study on factors affecting cognitive function and cognitive training effects)

  • 본 연구는 노인을 대상으로 인지기능에 관한 교육과 함께 주의, 기억, 실행기능, 기억책략 등을 다루는 인지과제를 수행하는 인지훈련을 실시하여 훈련 이전의 인지기능 수준은 어떤 개인적 특성과 관련이 있는지, 주관적 인지감퇴는 실제적인 인지감퇴를 반영하는지, 인지훈련의 효과는 어떤 개인 특성과 관련이 있는지를 알아보기 위해 수행되었다. 연구 참가자는 건강한 노인 20명으로 14회기의 인지훈련을 주 1회, 회기당 2시간씩 진행하였으며 사전 및 사후로 검사를 실시하였다. 연구 결과, 인지기능은 연령과 자아존중감, 성격 특질 중 외향성과 개방성이 관련이 있는 것으로 나타났으며, 주관적인 인지감퇴는 객관적인 인지기능보다는 우울, 불안, 성격, 자기효능감, 자아존중감, 주관적 연령과의 상관이 유의하였다. 또한 인지훈련을 통해 전반적인 인지기능을 측정하는 정신상태, 기억, 전두엽기능 검사 점수가 향상되었으며 인지훈련의 효과는 연령, 주관적 인지감퇴, 불안, 자기효능감, 자아존중감, 주관적 연령, 성격과 관련이 있는 것으로 나타났다. 이러한 결과는 어떤 특성의 노인이 인지노화에 취약한지 알려주며, 인지훈련의 효과를 높이기 위해 교육 참가자의 특성을 고려해야 한다는 것을 시사한다.
