어머니의 반영기능과 유아의 공감능력, 실행기능, 배려행동 간의 구조적 관계 (The Structural Relationships among Maternal Reflective Function, Preschoolers' Empathy, Executive Functions, and Caring Behaviors)

  • The purpose of this study was to explore the relationships among mothers' reflective function, preschoolers' empathy, executive function, caring behaviors. Subjects for this study were 498 preschoolers and their mothers. Mothers' reflective function was assessed by the Parental Reflective Functioning Questionnaire(PRFQ). Preschoolers' empathy was assessed by the Questionnaire of Cognitive and Affective Empathy(QCAE). Preschoolers' executive function was assessed by the Behavioral Rating Inventory of Executive Functions-Preschool Version(BRIEF-P), and caring behaviors were assessed by the caring behaviors scale(Kim, 2015). Data were analyzed using descriptive statistics, confirmatory factor analysis(CFA), and structural equation model(SEM) with the SPSS 25.0 and AMOS 23.0 program. The major results of this study were as follows. First, mothers' reflective function showed direct effects on preschoolers' empathy, executive function, and caring behaviors. Second, the preschoolers' empathy and executive function showed direct effects on preschoolers' caring behaviors. Third, preschoolers' empathy mediated the effects of mothers' reflective function on preschoolers' caring behaviors. The findings of this study will contribute to designing intervention programs to improve preschoolers' empathy, executive function, and caring behaviors.

치매, 경도인지장애, 허혈성 뇌졸중 환자에서 인지기능, 우울 및 일상생활수행능력의 변화 비교 (A Comparative Study of Changes in Cognitive Function, Depression and Activities of Daily Living in Patients with Dementia, Mild Cognitive Impairment and Ischemic Stroke)

  • 본 연구의 목적은 치매, 경도인지장애, 허혈성 뇌졸중 환자의 인지기능, 우울과 일상생활수행능력의 변화를 비교하고 도구적 일상생활수행능력의 변화를 설명하는 요인을 확인하기 위함이다. 총 86명(치매=30명, 경도인지장애=32명, 허혈성 뇌졸중=24명)으로부터 진단 시점과 1년 뒤에 인지기능, 우울, 기본적 및 도구적 일상생활수행능력 자료를 수집하여 분석하였다. 반복측정분산분석과 다중회귀분석을 이용하였다. 집행기능(p=.037)과 도구적 일상생활수행능력(p=.023)에서 측정 시점에 따른 집단 간 교호작용이 유의한 것으로 확인되었다. 경도인지장애 집단은 진단 후 1년 동안 집행기능과 도구적 일상생활수행능력의 변화가 거의 없었으나, 시간이 지남에 따라 치매 집단에서는 감소하는 양상이 나타났고 허혈성 뇌졸중 집단에서는 향상되는 양상을 나타냈다. 또한, 집행기능이 저하될수록(p=.030), 기본적 일상생활수행능력이 저하될수록(p<.001) 도구적 일상생활수행능력도 저하되는 것으로 확인되었으며 설명력은 26.9%였다. 이러한 결과는 인지변화를 주요 증상으로 가지는 치매, 경도인지장애, 허혈성 뇌졸중 환자에서 진단 후 1년까지 질병의 초기 단계에 각기 다른 집행기능 변화 양상이 나타났으며, 이로 인해 도구적 일상생활수행능력 변화 양상이 다른 것으로 보인다. 보건의료전문가는 진단 직후부터 집행기능과 도구적 일상생활수행능력 문제를 정기적으로 평가하고 이와 관련된 기능 상태를 유지 및 증진하기 위한 중재를 제공해야 할 것이다.

집단게임놀이 프로그램이 ADHD성향 아동의 실행기능에 미치는 효과 (The Effects of Group Game Play Program on ADHD-Liked Children's Executive Functione-fla)

  • The purpose of this study is to examine the effect of group game play program on ADHD-liked children’s executive function. The game play program was implemented for children in experiment group to find out the effect of the group game play program on ADHD-liked executive function. The subjects of this study were 12 children (4th year elementary students), showing ADHD characteristics. 6 were put in experiment group and other 6 were put in control group randomly. The effectiveness of group game play on ADHD children’s executive function are as followings: The attention ability, behavior inhibition ability, cognitive flexibility of children in experiment group was improved more than in control group. It is concluded that group game play program is effective for ADHD-liked children’s executive function.

성인의 스마트폰 중독이 주관적 안녕감에 미치는 영향: 실행기능 결함과 적응적 인지적 정서조절전략의 이중매개효과 (The Influence of Smartphone Addiction on Subjective Well-being among adults : Dual Mediation Effect of Executive Function Deficit and Adaptive Cognitive Emotion Regulation Strategies)

  • 본 연구의 목적은 성인의 스마트폰 중독이 주관적 안녕감에 영향을 미치는 과정에서 실행기능 결함과 적응적 인지적 정서조절전략의 이중매개효과를 검증하고, 성인 스마트폰 중독 치료에 대한 시사점을 파악하는 것이었다. 성인 148명(남성 71명, 여성 77명)을 대상으로 자기보고식 설문을 실시하고 회귀분석을 통해 매개효과를 검증하였다. 핵심연구결과는 다음과 같다. 첫째, 스마트폰 중독과 적응적 인지적 정서조절전략 간의 관계에서 실행기능 결함은 매개역할을 했다. 둘째, 스마트폰 중독과 주관적 안녕감 간의 관계에서 적응적 인지적 정서조절전략의 매개효과는 유의미하지 않았다. 셋째, 스마트폰 중독과 주관적 안녕감 간의 관계에서 실행기능 결함과 적응적 인지적 정서조절전략은 이중매개 및 완전매개 효과를 나타냈다. 본 연구결과는 성인 스마트폰 중독군의 치료적 개입에 있어 실행기능 훈련과 적절한 인지적 정서조절전략의 함양이 도움이 될 수 있음을 시사하였다. 마지막으로 본 연구의 제한점과 전체 성인 연령층을 대상으로 한 향후연구의 필요성이 논의되었다.

Does a cognitive-exercise combined dual-task training have better clinical outcomes for the elderly people with mild cognitive impairment than a single-task training?

  • Objective: This study was to develop and verify the effects of the exercise-cognitive combined dual-task training program on cognitive function and depression of the elderly with mild cognitive impairment(MCI). Methods: The subjects were randomly assigned to the exercise-cognitive combined dual-task training group(n=32) or single-task training group(n=31). To identify the effects on cognitive function, general cognitive function, frontal lobe function, and attention/working memory were measured. Depression was evaluated using Korean version of Geriatric Depression Scale. The outcome measurements were performed before and after the 8 weeks of intervention(2 days per week). Results: After 8 weeks, general cognitive function, frontal cognitive function, attention/working memory function, depression of the dual-task training group were significantly increased than those of the single-task training group(p<0.05). Conclusion: The results indicated that an exercise-cognitive combined dual-task training for MCI was effective in improving general cognitive function, frontal /executive function, attention/working memory function and reducing depression.

지역사회에 거주하는 경증인지장애노인과 치매환자에게 적용한 인지작업치료의 효과 (The Effect of Cognitive Occupational Therapy in Community Living Elders with Mild Cognitive Impairment and Dementia)

  • 본 연구는 지역사회의 경증인지장애노인과 치매환자에게 인지작업치료를 적용한 후 인지기능과 일상생활활동 수행능력, 우울정도의 효과를 알아봄으로 인지작업치료가 인지기능회복에 미치는 효과를 알아보고자 하였다. 연구방법은 2012년 2월에서 2012년 6월까지 일개광역지역에 소재하는 노인종합복지관과 요양원에 내원한 환자 중 경증인지장애군 13명과 치매군 19명으로 전체 32명을 대상으로 8주 동안 주 1회 신체기능, 인지기능, craft를 이용한 작업치료 활동 프로그램을 실시하였다. 두군 모두에서 인지기능, 우울감에서 유의한 호전을 보였다. 특히 경증인지장애군은 치매보다 집중력과 기억력 유지에 필요한 인지기능에서 유의한 효과를 보여 향후 경증인지장애군에서 인지기능회복을 위한 프로그램의 개발과 예방적 효과에 대한 연구가 필요하다.

경증 외상성 뇌손상 환자에서 신체적 증상, 우울, 불안과 인지기능의 관계 (Relations between Somatic Symptoms, Depression, Anxiety, and Cognitive Function in Patients with Mild Traumatic Brain Injury)

  • Objectives : This study was aimed at evaluating the relationship between somatic symptoms, depression, anxiety and cognitive function in the patients with Mild Traumatic Brain Injury(MTBI). Methods : Thirty seven patients with MTBI were selected from those patients who had visited the Department of Neuropsychiatry of Wonkwang University Hospital from 2003 to 2007. To assess and quantify the somatic symptoms, depression and anxiety, Personality Assessment Inventory(PAI) was used. Assessment of cognitive function was carried out by using Korean Wechsler Adult Intelligence Scale(K-WAIS), Rey-Kim Memory Test, and Kims Executive Function Test. The effects of somatic symptoms, depression, and anxiety on the cognitive function were evaluated by Pearson correlation test. Results : Somatic symptoms, depression, and anxiety, all showed inverse correlation to cognitive function. Specifically, 1) an increase in somatic symptoms was associated with a decrease in attention, verbal short term memory, verbal recall and recognition, and visual memory. 2) An increase in anxiety was associated with a decrease in verbal recall and recognition. 3) An increase in depression was associated with a decrease in cognitive function that requires high attention and verbal memory. Conclusion : The patients with MTBI displayed diverse symptoms ranging from cognitive impairment to somatic symptoms, depression, and anxiety. Somatic and emotional symptoms were correlated with cognitive function(especially executive function). Importantly, this study raises the possibility of treating the cognitive impairment associated with MTBI by treating somatic symptoms, depression, and anxiety.

유방암 환자에서 화학요법관련 인지기능저하의 발생률과 발생양상 (Prevalence and Characteristics of Chemotherapy-related Cognitive Impairment in Patients with Breast Cancer)

  • Purpose: Evidence suggests that some patients with breast cancer experience cognitive difficulties following chemotherapy. This longitudinal study was done to examine the prevalence of cognitive impairment and trajectory of cognitive function over time in women with breast cancer, who received adjuvant chemotherapy. Methods: Participants were 137 patients with breast cancer. They completed neuropsychological tests and the Functional Assessment of Cancer Therapy-Cognitive Function before adjuvant therapy (pretest), toward the end of adjuvant therapy (posttest), and 6 months after the completion of adjuvant therapy (follow-up test). Of the patients, 91 were treated with adjuvant chemotherapy and 46 patients who did not receive chemotherapy made up the comparison group. A reliable-change index and repeated-measure ANOVA were used for statistical analyses. Results: At the posttest point, over 30% of patients showed complex cognitive impairment and reported greater difficulty in subjective cognitive function. At the follow-up test point, 22.0% of patients exhibited complex cognitive impairment and 30.8% of patients complained of subjective cognitive impairment. Repeated-measure ANOVA showed significant decreases after receiving chemotherapy followed by small improvements 6 months after the completion of chemotherapy in cognitive domains of change for attention and concentration, memory, executive function, and subjective cognitive function. Conclusion: These results suggest that chemotherapy in patients with breast cancer may be associated with objective and subjective cognitive impairments. Further studies are needed to explore the potential risk factors and predictor of chemotherapy-related cognitive changes. Also nursing interventions for prevention and intervention of cognitive impairments should be developed and tested.

주의력결핍 과잉행동장애, 불안장애 아동의 실행기능 비교 (Comparison of Executive Function in Children with ADHD and Anxiety Disorder)

  • Objectives : The purpose of this study was to investigate the deficits in executive function in children with ADHD and anxiety disorder, and further, to characterize executive function deficits among the two groups. Methods : Subjects consisted of 60 children between the ages of 5 and 14 (16 Normal, 24 ADHD, 20 Anxiety Disorder). Neuropsychological tests (KEDI-WISC, CCTT, STROOP, WCST, ROCF) for assessing cognitive and executive function were individually administered to all subjects. Results : There were no significant differences in FSIQ or PIQ among the three groups. However, the ADHD group tended to score lower on the VIQ and subtest of similarity, vocabulary, and digit span tests. The three groups did not significantly differ with respect to CCTT test results. On the STROOP test, the ADHD group showed poor performance on the word, color, and color-word subtests. The three groups did not exhibit significant differences in WCST test results ; however, the anxiety group performed poorly belonging to below 25 percentile rank on perseverative response. On the ROCF test, the ADHD group performed poorly with respect to their organization score and in particular, regarding copy and immediate recall. The anxiety group also performed poorly with regard to organization ; however, this was limited only to immediate recall. Conclusion : Children with ADHD displayed poor inhibition and organizational abilities compared to children with anxiety and normal controls. Further, children with anxiety disorder exhibited low cognitive flexibility and voluntary problem-solving abilities compared to ADHD children and normal controls. Based on these results, we suggest that the characteristics of executive dysfunction in ADHD and anxiety disorder in children are different.

Facial Emotion Recognition in Older Adults With Cognitive Complaints

  • Background and Purpose: Facial emotion recognition deficits impact the daily life, particularly of Alzheimer's disease patients. We aimed to assess these deficits in the following three groups: subjective cognitive decline (SCD), mild cognitive impairment (MCI), and mild Alzheimer's dementia (AD). Additionally, we explored the associations between facial emotion recognition and cognitive performance. Methods: We used the Korean version of the Florida Facial Affect Battery (K-FAB) in 72 SCD, 76 MCI, and 76 mild AD subjects. The comparison was conducted using the analysis of covariance (ANCOVA), with adjustments being made for age and sex. The Mini-Mental State Examination (MMSE) was utilized to gauge the overall cognitive status, while the Seoul Neuropsychological Screening Battery (SNSB) was employed to evaluate the performance in the following five cognitive domains: attention, language, visuospatial abilities, memory, and frontal executive functions. Results: The ANCOVA results showed significant differences in K-FAB subtests 3, 4, and 5 (p=0.001, p=0.003, and p=0.004, respectively), especially for anger and fearful emotions. Recognition of 'anger' in the FAB subtest 5 declined from SCD to MCI to mild AD. Correlations were observed with age and education, and after controlling for these factors, MMSE and frontal executive function were associated with FAB tests, particularly in the FAB subtest 5 (r=0.507, p<0.001 and r=-0.288, p=0.026, respectively). Conclusions: Emotion recognition deficits worsened from SCD to MCI to mild AD, especially for negative emotions. Complex tasks, such as matching, selection, and naming, showed greater deficits, with a connection to cognitive impairment, especially frontal executive dysfunction.
