Excretory urography was performed on eight healthy minipigs to evaluate radiographic anatomical status and excretive function of the urinary tracts and to get an optimal procedure. The right and left kidneys were located in $T13{\sim}L4$and $T12{\sim}L3.5$, respectively. The left kidneys were located more cra-nially than the right. The everage lengths and widths of kidneys were $8.50{\pm}0.58$$(mean{\pm}SD)$and $4.30{\pm}0.39$ cm on the ventrodorsal projection and $8.70{\pm}0.76$, and $4.10{\pm}0.40$ cm on the lateral projection, respectively. The lengths of kidneys were twice the width. When the lengths of kidneys were com-pared with lengths of the second lumbar vertebras, the ratios of kidney lengths to L2 lengths were $3.62{\pm}0.30$ on the ventrodorsal projection and $3.63{\pm}1.10$ on the lateral projection. The lengths and 쟝손 of renal pelvis including major calices were $4.01{\pm}0.46$, and 2.20{\pm}0.41$ cm, respectively. The lengths and 쟝손 of minor calices were $0.49{\pm}0.06$, and $0.10{\pm}0.01$ cm. The lengths and widths of the ureters were $12.25{\pm}2.05$, and 2.94{\pm}0.86$ cm. The nephrogram stage was reached in 3{\sim}5$ minutes, and the pyelogram phase in $5{\sim}15$ minutes individually after injection of contrast medium. The result of this study shows similar information and procedure of excretory urography for other laboratory ani-mals and leads to the potential use of this method for minipigs.
A 13-year-old, male Maltese was refered to Veterinary Teaching Hospital of Konkuk University with inappropriate urination and hematuria. CBC, serum chemistry profile, urinalysis and urine culture were performed. Malignant cells were found in the urine sediment. Thoracic and abdominal radiography, retrograde positive-contrast cystography, retrograde double-contrast cystography, excretory urography, ultrasonography were performed. On the retrograde positive-contrast cystography examination, irregular filling defects were present on the left side of the cranial part of the bladder. Partial cystectomy performed and then medical therapy was conducted. On histopathological examinations, the lesion was confirmed as a transitional cell carcinoma of the urinary bladder.
A 4-year-old female Korean short hair cat weighing 6.5 kg presented for evaluation of urinary incontinence and stranguria. On physical examination, stranguria was observed concurrently with urinary bladder distention. Abdominal radiographs revealed two small uroliths in the urinary bladder. Urinary bladder sludge was detected on abdominal ultrasound. Urine analysis indicated hematuria and bacteriuria. The cat was treated with a 4-week course of a combination of antibiotics and urinary bladder irrigation using normal saline; however, response to treatment was minimal. Excretory urography was performed to identify a congenital disorder. A small diverticulum, located to the urinary bladder apex, was identified. A tentative diagnosis of a vesicourachal diverticulum was made. Surgical exploration of the abdomen was performed and a triangular-shaped diverticulum was apparent at the urinary bladder apex. Cystotomy was performed to remove two small uroliths. Partial cystectomy was then performed for diverticulectomy. Approximately 2 cm diameter of a part of the apex was removed. Normal urination was regained 5 days postoperatively. The follow-up was completed by physical examination 2 years after surgery. There was no evidence of stranguria and urinary incontinence.
A castrated male Yorkshire Terrier dog was presented for urinary incontinence and constipation. On physical examination, the dog showed difficult urination. There were no neurological abnormalities and no bacterial detection on urinalysis. Rectal examination revealed a regular, normal-sized prostate. Urethral catheterization was performed easily. Excretory urography and retrograde positive contrast urethrocystography showed displacement of the urinary bladder to the intrapelvic region. There was no evidence of an ectopic ureter. A tentative diagnosis of urethral sphincter mechanism incompetence accompanied with a pelvic bladder was made. Cystopexy was decided to place the urinary bladder to its normal position. The neck of the urinary bladder was anchored to the body wall and prepubic tendon using mattress sutures. Additional sutures were placed to appose the lateral part of the urinary bladder and abdominal wall. A simple interrupted suture was placed to tack the apex of the urinary bladder on the abdominal incision line. A urinary catheter was placed in the urinary bladder to provide post-operative evacuation. The catheter was removed when the dog was able to urinate with minimal straining at 3 days post-operatively. The owner reported that the dogs showed normal urination without straining at 3 days after the catheter was removed. Excretory urography revealed that the urinary bladder was located on its normal position at 2 months post-operatively. Subsequent communication with the owner by telephone revealed that the dog was in good urination at 3 years 11 months post-operatively.
2 Cases of nephrotomy for removal of calculi in dog were referred to veterinary teaching hospital of Konkuk University. In case 1, a 5 year-old, castrated male Yorkshire Terrier dog was referred because of intermittent hematuria, pain in urination for one month. Hematologic and chemical examination showed mild increased BUN and CPK. Radiographic findings revealed radiopaque materials in the urinary bladder, urethra, and left kidney. Retrograde hydropropulsion was performed to move the calculi into the bladder, and cystotomy was done to remove calculi. Nephrotomy was performed to removal of the calculi from the left renal pelvis and calyx. After operation renal function were recovered and preserved. In case 2, a 5 year-old, neutral female Schnauzer dog was referred because of persistant vomiting, anorexia, and celialgia for 20 days. Hematologic and chemical examination showed stress leucogram, moderate azotemia, hypercalcemia, hyperphosphatemia, and increased ALP. Radiographic findings revealed enlargement of the left kidney and radiopaque materials in the both of the kidneys. On excretory urography, left kidney was no pyelogram. On ultrasonography, renal tissue was very thin and distended renal pelvis appeared. Nephrectomy of nonfunctional left kidney and nephrotomy for removal of calculi from the right renal pelvis and calyx were done. One week after operation, renal and hepatic functions were recovered. So, in cases of renal calculi, it is necessary that renal calculi are extracted actively as far as the patient's body condition endurable.
An 11-year-old, intact female Shihtzu showing anuria was diagnosed as a urethral leiomyoma. The tumor occupied and obstructed the urethral lumen. In this report, the urethral tumor and secondary obstruction could be diagnosed via ultrasonography and contrast study such as excretory urography and voiding urethrography. Ultrasonography revealed a hypoechoic, demarcated urethral mass in proximal urethra. Contrast study confirmed the urethral obstruction and ruled out the expansion of the tumor into the urinary bladder. The clinical signs of the dog were completely resolved after removing the urethral mass and urethral anastomosis. Leiomyoma is a benign smooth muscle tumor, and rarely reported in lower urinary tract.
A Maltese (case 1) and a Labrador Retriever (case 2) presented with urinary incontinence. General conditions were good and screening tests, including a complete blood count, serum chemistry and radiography, were performed. Excretory urography was conducted, and fluoroscopy was performed for case 1 and computed tomography was performed for case 2. The dogs were diagnosed as right extramural ectopic ureter in case 1 and bilateral intramural ectopic ureter in case 2. We performed surgical corrections, including a neoureterocystostomy for extramural ectopic ureter and neoureterostomy for intramural ectopic ureter. After surgery, the dogs were catheterized with an indwelling catheter for 3 days. Urinary incontinence improved completely and the clinical outcomes were good. The choice of adequate surgical procedure is important for correcting ectopic ureters in canines.
A Shihtzu (6-year-old, intact female) was referred to Konkuk University Teaching Animal Hospital for cystic calculus and hydronephrosis of right kiney. The dog was suspected to have ureteral rupture and peritonitis. Radiographic findings included cyctic calculi and right kidney enlargement. On ultrasonographic examination hyperechoic mass with severe acoustic shadowing was located in the right proximal ureter which was dilated. Hydronephrosis of the right kidney, right ureteral caculus and cytic calculus were confirmed by radiography and ultrasonography. Cystotomy for removal of calculus in the bladder and ureterotomy for removal of calculus in the right ureter were performed. Excretory urography (EU) performed one month after surgery revealed that the right ureter was homogeneously opacified and decreased to 3 to 6 mm in diameter. Surgical removal of ureteroliths was appropriate treatment for the ureteral and cystic calculi. The result suggests that ureterotomy is effective treament for the dilation and calculi of ureter.
Background: Among women with haematuria, defining individuals under high risk for bladder cancer based on reproductive factors prior to cystoscopy would be of great benefit in the management of this condition. The aim of this study was to compare age and reproductive factors such as menopausal status, parity, age at first delivery and age at the last delivery between women who have haematuria with or without bladder cancer. Materials and Methods: A total of 463 patients underwent diagnostic cystoscopy in D$\ddot{u}$zce University Faculty of Medicine between 1 June 2008 and 1 June 2013. Female patients who presented with persistent microscopic or macroscopic haematuria and underwent standard evaluation for haematuria including urinalysis, urine culture, urine cytology, urinary tract imaging with excretory urography or computerized tomography with contrast enhancement and endoscopic evaluation of the urethra and bladder were included in this study. Exclusion criteria were tobacco use and high risk occupations for bladder cancer such as textile, dry cleaning, painting and etc. Forteen women had hematuria due to benign conditions, and 18 due to bladder cancer. Data were retrospectively retrieved from the medical records of Duzce University Hospital. Results: Patients with haematuria due to benign reasons did not significantly differ from patients who were found to have bladder cancer in terms of age (p=0.28), menopausal status (p=0.29), mean parity (p=0.38), being nulliparous (p=0.57), parity ${\geq}3$ (p=0.22), age ${\leq}18$ years at first delivery (p=1.00), age ${\geq}30$ years at last delivery (p=0.26), age ${\geq}35$ years at last delivery (p=0.23) and percentage of the patients with advanced age (${\geq}65$ years) (p=0.18). Conclusions: It is difficult to predict a high risk for developing bladder cancer in women with haematuria based solely on reproductive factors.
A 1-year-old intact male Russian blue cat was admitted with a history of depression, partial anorexia, vomiting and dysuria. Through the blood analysis, ultrasonography and urinalysis, this cat was diagnosed as feline lower urinary tract disease with cystitis. Despite antibiotic therapy the cat showed stranguria and urinary catheterization were repeated several times. One week after, this patient appeared depression, hyperthermia and leukocytosis. Ultrasonography revealed a small amount of hypoechoic fluid around the right kidney and bacteria were found in the urine. The amount of the right subcapsular fluid was increased and bacteria were found in the fluid. On the excretory urography, leakage of contrast media were detected. The right kidney was surgically removed and suppurative fluid between the renal parenchyma and the thickened capsule was identified. After surgery, subcapsular abscess of the right kidney associated with pyelonephritis was confirmed histologically and this cat recovered without complications.
