Sinus Tract Formation with Chronic Inflammatory Cystic Mass after Beta Tricalcium Phosphate Insertion

  • Beta tricalcium phosphate (${\beta}-TCP$) is one of allogenic bone substitute which is known to have interconnected pores that draws cell and nutrients for bone generation. It has been resulted in good outcomes for bone defect coverage or augmentation. However, several studies have also reported negative outcomes and associated complications including unexpected formation of cystic mass, continuous pain and secretion. We present the case of a 36-year-old man with a right cheek cystic mass who had a history of right zygomaticomaxillary (ZM) complex fracture and surgical correction with ${\beta}-TCP$ powder insertion to ZM bone defect. Excisional biopsy under local anesthesia revealed calcified mass in a sinus tract which was found to be connected to the ZM bone defect site in postoperative computed tomography image. Further excision under general anesthesia was performed to remove the sinus tract and fine granules which filled the original defect site. Pathologic report revealed bony spicules and calcification materials with chronic foreign body reaction. Postoperative complications and recurrence were not reported.

  • Giant cell tumor of the tendon sheath (GCTTS) is a slowly growing, benign soft tissue tumor. The tumors occur predominantly on the hands and feet. Although the clinical and histopathologic features are well-defined, only a few reports have described the cytologic appearance of this entity. A 26-year-old woman presented with a gradually developing circumscribed soft tissue mass near the proximal phalanx of her left little finger for one year. Imprint and fine needle aspiration (FNA) smears were obtained from the excisional biopsy specimen. The imprint smears were composed of predominantly singly dispersed bland mononuclear cells and several giant cells. The mononuclear cells were polygonal to round, and they showed a histiocyte-like appearance. Osteoclast-type multinucleated giant cells of various sizes were randomly scattered throughout the smears, and these cells contained 3 to 50 nuclei. Nuclear atypia and pleomorphism were absent in both the single and giant cells. Loose aggregates of hemosiderin-laden macrophages and binuclear stromal cells were also seen. The cytologic features of the FNA smears were similar with those of the imprint, Additionally, the FNA smears contained several clumps of densely collagenous stromal tissue that were seldom noted in previously reported cytologic material. The cytologic features were well-correlated with the concurrent histologic findings and the diagnosis of GCTTS was made. When the clinical and radiologic datas are integrated, the diagnosis of GCTTS can be strongly suggested, based on the pre-operative cytologic specimen.

Incidentally detected odontoma within a dentigerous cyst

  • Odontoma is an asymptomatic slow-growing odontogenic tumor. It is usually found by chance in the maxilla or mandible on radiography, or when it deforms the adjacent teeth. It is commonly found in patients who are 30 years of age or younger. We report our encounter with an odontoma within a dentigerous cyst found incidentally in a 56-year-old man. He presented with abnormal fullness in the right infraorbital area of the cheek. During the evaluation of the mass, we incidentally detected the odontogenic tumor within a dentigerous cyst in the patient's maxilla. Under general anesthesia, complete surgical drainage of the infraorbital cystic mass was performed. Enucleation of the odontogenic tumor and a bone grafting from the iliac bone were also performed. The final diagnosis was odontoma. After 2 years of follow-up, there was no sign of recurrence of the tumor. In case of odontogenic tumors, even in old patients, it is important to suspect an odontoma. When odontoma accompanies a dentigerous cyst, surgical excisional biopsy should be performed to rule out malignancy. In case of a large bony defect after enucleation, autogenous bone grafting is essential for reconstruction.

Subungual Osteochondromas of the Toe: Two Case Reports for Diagnosis and Treatment

  • Benign cartilaginous tumors, known as chondrogenic tumors, show cartilage components in the microscopic diagnosis. We present two clinical cases with cartilaginous tumors of the toes showing distinctive clinical manifestations. Two juvenile patients visited our outpatient clinic due to tumors with toenail deformities. A 10-year-old girl presented with a palpable mass with a nail deformity on the left third toe. The initial pathology report was soft tissue chondroma until complete resection. Another 15-year-old male patient visited the dermatology department with a toenail deformity and underwent a punch biopsy. The pathology report was fibrosis with myxoid degeneration. Excisional biopsies were performed for both patients. In the operative field, we observed exophytic tumors connected to the distal phalangeal bones. The final pathology reports were subungual osteochondroma on both patients. The specimen exhibited mature bone trabeculae with a focal cartilaginous cap. Benign cartilaginous tumors have a slow, progressive course and do not show significant symptoms. However, tumors in subungual areas are accompanied by toenail deformities and they can cause pain. Their clinical characteristics lead to a delayed diagnosis. Surgeons can be confused between soft tissue and chondrogenic tumors. When they conduct physical examinations, these categories should be considered in the differential diagnosis.

Granuloma Formation, a Rare Complication after PDO Threads Lifting, and Adjuvant Treatment Using Dual-Frequency Ultrasound (LDM®-MED)

  • Thread-lifting is a minimally invasive procedure that shows good results and fewer complications as compared with those results and complications of standard surgery. Many procedures and techniques have been developed to create a younger appearance of facial/neck skin for aging people, and the demand for an improved aesthetic appearance is increasing. Since the incidence of side effects is much less than that of non-absorbable threads, which can lead to complications such as foreign body reactions, polydioxanone (PDO) threads are predominantly used for face lift procedures. A 66-year-old woman presented to our clinic with inflamed palpable masses. She had undergone a face lift with absorbable threads in our clinic 5 months previously. Excisional biopsy was performed with the patient under local anesthesia. During the operation, any threads were not detected and there was both fibrotic scar tissue and granulomatous tissue. For effectively promoting healing and managing the scars, treatment with LDM®-MED was performed on the day after surgery. The treatment was performed according to the author's protocol. Although foreign body granuloma as a complication after using non-absorbable thread types have been previously reported, it is relatively rare to find this type of complication after using absorbable thread. In this report, we present a case in which a 66-year-old female with foreign body granuloma after undergoing a face lift using absorbable threads was treated with the application of dual-frequency ultrasound, which promoted wound healing.

침생검 조직검사에서 진단된 비정형 관상피증식증: 수술적 절제 생검에서 악성으로 진단될 가능성을 예측할 수 있는 위험인자들 (Atypical Ductal Hyperplasia: Risk Factors for Predicting Pathologic Upgrade on Excisional Biopsy)

  • 목적 조직생검으로 진단된 비정형 관상피증식증이 수술 후 악성으로 진단되는 과소평가율과 이를 예측할 수 있는 영상 소견이나 진단 방법에 따른 위험인자를 조사하고자 한다. 대상과 방법 2년 이상의 기간 동안 시행된 9660예의 침생검을 후향적으로 분석하여 영상 소견과 조직검사 방법에 따라 과소평가 군과 비 과소평가 군의 차이점을 비교하였다. 결과 9660예의 침생검 중 169 (1.7%)예가 비정형 관상피증식증으로 진단되었다. 절제 생검을 한 112예와 2년 이상 추적검사를 한 30예를 합친 142예 중 35예에서 과소평가되었다 (24.6%, 35/142). 과소평가율의 차이는 조직생검 방법에 따라 의미 있었다; 초음파 유도 핵생검(40.7%, 22/54), 입체정위 진공 보조흡입생검(16.0%, 12/75), 초음파 유도 진공 보조흡입생검 (7.7%, 1/13) (p = 0.002). 다변량 분석에서는 초음파 유도 핵생검(교차비 5.19, 95% 신뢰구간 2.16-13.95, p < 0.001)이 독립적인 위험 인자였다. 종괴로 보이는 병변이 초음파 유도 진공 보조흡입생검으로 진단된 경우는 과소평가가 보고되지 않았다( n = 7). 결론 비정형 관상피증식증이 1.7%로 진단되었고 과소평가율은 24.6%였다. 종괴로 보이는 병변이 초음파 유도 진공 보조흡입생검으로 제거된 경우를 제외하고는 상당한 과소평가율을 보이고 있으므로 수술적 절제를 고려해야 한다.

Surgical Treatment of Gastric Gastrointestinal Stromal Tumor

  • Gastrointestinal stromal tumor is the most common mesenchymal tumor in the gastrointestinal tract and is most frequently developed in the stomach in the form of submucosal tumor. The incidence of gastric gastrointestinal stromal tumor is estimated to be as high as 25% of the population when all small and asymptomatic tumors are included. Because gastric gastrointestinal stromal tumor is not completely distinguished from other submucosal tumors, a surgical excisional biopsy is recommended for tumors >2 cm. The surgical principles of gastrointestinal stromal tumor are composed of an R0 resection with a normal mucosa margin, no systemic lymph node dissection, and avoidance of perforation, which results in peritoneal seeding even in cases with otherwise low risk profiles. Laparoscopic surgery has been indicated for gastrointestinal stromal tumors <5 cm, and the indication for laparoscopic surgery is expanded to larger tumors if the above mentioned surgical principles can be maintained. A simple exogastric resection and various transgastric resection techniques are used for gastrointestinal stromal tumors in favorable locations (the fundus, body, greater curvature side). For a lesion at the gastroesophageal junction in the posterior wall of the stomach, enucleation techniques have been tried preserve the organ's function. Those methods have a theoretical risk of seeding a ruptured tumor, but this risk has not been evaluated by well-designed clinical trials. While some clinical trials are still on-going, neoadjuvant imatinib is suggested when marginally unresectable or multiorgan resection is anticipated to reduce the extent of surgery and the chance of incomplete resection, rupture or bleeding.

치성 점액종 환자의 총의치 수복증례 (Fabrication of complete dentures for a patient with odontogenic myxoma: A case report)

  • 치성점액종은 악골에 드물게 발생하는 양성 치성 종양이다. 본 증례의 60대 무치악 남성은 전상악부에 발생한 연조직 종괴 제거를 주소로 내원하였으며 절제 생검 결과 치성 점액종으로 확진되었다. 외과적 제거 후 잔존한 가동조직의 변형을 최소화 하기 위해 고안된 투명 레진 트레이로 인상을 채득하였다. 악간관계기록 및 납의치 시적시에 중립대 영역을 설정하고 기능적 연마면 형태를 인기하여 총의치의 안정성과 유지력을 증진시킬 수 있었다. 치성 점액종 제거 후 가동조직과 중립대를 고려하여 심미적, 기능적으로 만족스러운 총의치를 제작하였기에 이를 보고하는 바이다.

Isolated Cervical Lymph Node Sarcoidosis Presenting in an Asymptomatic Neck Mass: A Case Report

  • Sarcoidosis, a systemic granulomatous disease of unknown etiology. The presentation of sarcoidal granuloma in neck nodes without typical manifestations of systemic sarcoidosis is difficult to diagnose. We describe the case of a 37-year-old woman with an increasing mass on the right side of neck. The excisional biopsy from the neck mass showed noncaseating epithelioid cell granuloma of the lymph nodes. No evidence of mycobacterial or fungal infection was noted. Thoracic evaluations did not show enlargement of mediastinal lymph nodes or parenchymal abnormalities. Immunohistochemistry showed abundant expression of tumor necrosis factor-${\alpha}$ in the granuloma. However, transforming growth factor-${\beta}$ was not expressed, although interleukin-$1{\beta}$ was focally expressed. These immunohistochemical findings supported characterization of the granuloma and the diagnosis of sarcoidosis. Sarcoidosis can present with cervical lymph node enlargement without mediastinal or lung abnormality. Immunohistochemistry may support the diagnosis of sarcoidosis and characterization of granuloma.

  • 국소적 섬유연골성 이형성증(FFCD)은 소아에서 일측성 경골 내반 소견을 보이는 드문 양성 질환이다. FFCD는 근위 경골 내측 골간단과 골간의 이행부에 오목한 방사선 투과 음영 소견을 보이는 전형적인 방사선 소견을 지니고 있다. 경골 내반이 병변 부위에서 발생한다. 내반의 교정과 골결손의 치유가 흔히 자발적으로 일어나기도 하지만 골교정술을 요하기도 한다. 저자들은 절제 조직 생검술로 진단하고 절골술 및 골이식술을 시행함으로써 치유한 FFCD에 의한 일측성 경골 내반 1예를 문헌고찰과 함께 보고하는 바이다.
