Objectives : This study aimed to evaluate the characteristics of acute stroke patients between excess syndrome and deficiency syndrome groups. Method : We recruited stroke patients from the patients admitted to the Department of Internal Medicine of Kyunghee University Oriental Medical Center, Dongguk University Ilsan Oriental Medical Center, Kyungwon University Songpa Oriental Medical Center and Kyungwon University Incheon Oriental Medical Center from April 2007 to August 2008. We diagnosed acute stroke patients as either excess syndrome or deficiency syndrome and analyzed their characteristics for type of stroke, blood test result. Sasang constitution and lifestyle. Result : We found height, weight, BMI, W/H ratio, hypertension, diabetes, migraine, silent infarction, TG, total lipid, HDL-chol, RBC, Hb, hematocrit, alcohol, smoking and Sasang constitution (Tae-eum, So-yang) were more associated with the excess syndrome group. And we found sea food and Sasang constitution(So-eum) was more associated with the deficiency syndrome group. Conclusion : According to the analysis, we found that the excess syndrome group had more risk factors than the deficiency syndrome group. These results could be utilized in the future as a basis material for Oriental medicine therapy. Further studies will be needed to better understand the differences between excess syndrome and deficiency syndrome groups among acute stroke patients.
Airborne suspended particulates were collected by an Andersen high volume air sampler in a traffic area of Seoul from September 1990 to August 1991. Origanic matter extracted from particulates, their fractions, namely acidic, basic, neutral and carcinogenic subfractions (PAHs, nitroarenes) in neutral fractions were assayed for mutagenicity on TA98, TA100 and TA98NR deficient Salmonella strains, use of the pre-incubation method. The relative contribution to total mutanenicity of organic matters was highest in neutral fraction and was lowest in basic fraction. Among subfractions, that of neutral fraction was higher nitroarenes subfraction compared to PAHs subfraction. While the carcinogenic effect of benzo[a]pyrene was calculated as 0.96 persons/million persons based on unit risk estimates by extrapolation method, life time excess cancer risk estimate of EOM, neutral, PAH fraction based on their mutagenicity was calculated as 52, 42, 3.8 persons/million persons, respectively. These findings indicate that the mutagenic hazard of the partciculate, air organic complex mixture, may be dependent upon the mutagen composition in the particulate and interactions each of them. Therfore, health risk from air organic complex mixtures based on mutagenicity might be useful indicator for evaluation of actual risk.
Background: Metalworking fluids (MWFs) are mixtures with inhalation exposures as mists, dusts, and vapors, and dermal exposure in the dispersed and bulk liquid phase. A quantitative risk assessment was performed for exposure to MWF and respiratory disease. Methods: Risks associated with MWF were derived from published studies and NIOSH Health Hazard Evaluations, and lifetime risks were calculated. The outcomes analyzed included adult onset asthma, hypersensitivity pneumonitis, pulmonary function impairment, and reported symptoms. Incidence rates were compiled or estimated, and annual proportional loss of respiratory capacity was derived from cross-sectional assessments. Results: A strong healthy worker survivor effect was present. New-onset asthma and hypersensitivity pneumonitis, at 0.1 mg/㎥ MWF under continuous outbreak conditions, had a lifetime risk of 45%; if the associated microbiological conditions occur with only 5% prevalence, then the lifetime risk would be about 3%. At 0.1 mg/㎥, the estimate of excess lifetime risk of attributable pulmonary impairment was 0.25%, which may have been underestimated by a factor of 5 or more by a strong healthy worker survivor effect. The symptom prevalence associated with respiratory impairment at 0.1 mg/㎥ MWF was estimated to be 5% (published studies) and 21% (Health Hazard Evaluations). Conclusion: Significant risks of impairment and chronic disease occurred at 0.1 mg/㎥ for MWFs in use mostly before 2000. Evolving MWFs contain new ingredients with uncharacterized long-term hazards.
Background: The beneficial effects of implantable cardioverter defibrillators (ICDs) in primary and secondary prevention patients are well established. However, data on potential differences between both groups in mortality are scarce. The aim of this study was to assess extra risk differences between primary and secondary prevention ICD recipients. Methods: Comparative mortality figures were calculated from a source article using mortality analysis methods. Results: Mortality ratio (MR) of primary and secondary prevention ICD recipients were 393% and 373%. Excess death rates (EDR) of both groups were 42 and 38 per 1,000. Discussion: MR and EDR were higher in primary prevention group. But, there was no significant difference.

In many epidemiological and medical studies, a number of cancer mortalities in categorical classification may be considered as having Poisson distribution with person-years at risk depending upon time. The cancer mortalities have been evaluated by additive or multiplicative models with regard to background and excess risks based on several covariances such as sex, age at the time of bombings, time at exposure, or ionizing radiation, cigarette smoking habits, duration of smoking habits, etc. An interest herein is to examine an additive, synergistic, or antagonistic relationship between radiation exposures and cigarette smoking habits for cancer mortalities. The results revealed a highly significant antagonistic in uence for cancer mortalities from all non-hematologic findings, lung and respiratory system with negative interaction between radiation exposures and cigarette smoking amounts.

We investigate pediatric tumor incidence data collected by the Florida Association for Pediatric Tumor program using various models commonly used in disease mapping analysis. Particularly, we consider Poisson normal models with various conditional autoregressive structure for spatial dependence, a zero-in ated component to capture excess zero counts and a spatio-temporal model to capture spatial and temporal dependence, together. We found that intrinsic conditional autoregressive model provides the smallest Deviance Information Criterion (DIC) among the models when only spatial dependence is considered. On the other hand, adding an autoregressive structure over time decreases DIC over the model without time dependence component. We adopt weighted ranks squared error loss to identify high risk regions which provides similar results with other researchers who have worked on the same data set (e.g. Zhang et al., 2014; Wang and Rodriguez, 2014). Our results, thus, provide additional statistical support on those identied high risk regions discovered by the other researchers.
Despite advances in medicine and preventive strategies, fewer than 1 in 5 people with hypertension have the problem under control. This could partly be due to gaps in fully elucidating the etiology of hypertension. Genetics and conventional lifestyle risk factors, such as the lack of exercise, unhealthy diet, excess salt intake, and alcohol consumption, do not fully explain the pathogenesis of hypertension. Thus, it is necessary to revisit other suggested risk factors that have not been paid due attention. One such factor is psychosocial stress. This paper explores the evidence for the association of psychosocial stressors with hypertension and shows that robust evidence supports the role of a chronic stressful environment at work or in marriage, low socioeconomic status, lack of social support, depression, anxiety, post-traumatic stress, childhood psychological trauma, and racial discrimination in the development or progression of hypertension. Furthermore, the potential pathophysiological mechanisms that link psychosocial stress to hypertension are explained to address the ambiguity in this area and set the stage for further research.
Objectives: The aim of this study was to investigate the exposure and risk assessment of residents near asbestos mines in Korea. Methods: To assess asbestos types and airborne concentrations, air monitoring was performed in the neighborhoods of Kwangcheon (KC) and Sinsuk (SS) mines, which were leading South Korean mines in the past. In addition, activity-based-sampling (ABS) of residents' particular activities were conducted in order to estimate the Excess Lifetime Cancer Risks (ELCRs) for the residents. Conclusions: The average concentration of airborne asbestos in KC was 0.0014 f/cc and 0.0015 f/cc by PCM and TEM, respectively. In SS it was equal at 0.0012 f/cc by PCM and TEM. No statistically significant difference was found in the average concentration of airborne asbestos between the two mines. The average asbestos concentration of ABS was 0.0048 f/cc (PCM) and 0.0042 f/cc (TEM) in KC, while it was 0.0137 f/cc (PCM) and 0.0125 f/cc (TEM) in SS. It was found that the average asbestos concentration of ABS in SS was statistically significantly higher than that of KC (p<0.01). The results of ELCRs by scenario in KC showed that the scenarios of bicycle, car, weed control, weed whacking, child playing in the dirt, and physical training fell within $1{\times}0^{-6}-1{\times}10^{-4}$, which is the acceptable range of ELCR. The scenarios of motorcycle, walker, digging, and field sweeping, however, exceeded the acceptable range. In SS, only the scenario of car fell within the acceptable range, while all of the other scenarios exceeded the acceptable range.

Background: There is little information on the dose-response relationship between exposure to occupational carcinogenic agents and mesothelioma. This study aimed to investigate this association as well as the existence of agents other than asbestos that might cause mesothelioma. Methods: The Swedish component of the Nordic Occupational Cancer (NOCCA) study consists of 6.78 million individuals with detailed information on occupation. Mesothelioma diagnoses recorded in 1961-2009 were identified through linkage to the Swedish Cancer Registry. We determined cumulative exposure, time of first exposure, and maximum exposure intensity by linking data on occupation to the Swedish NOCCA job-exposure matrix, which includes 29 carcinogenic agents and corresponding exposure for 283 occupations. To assess the risk of mesothelioma, we used conditional logistic regression models to estimate hazard ratios and 95% confidence intervals. Results: 2,757 mesothelioma cases were identified in males, including 1,416 who were exposed to asbestos. Univariate analyses showed not only a significant excess risk for maximum exposure intensity, with a hazard ratio of 4.81 at exposure levels 1.25-2.0 fb/ml but also a clear dose-response effect for cumulative exposure with a 30-, 40-, and 50-year latency time. No convincing excess risk was revealed for any of the other carcinogenic agents included in the Swedish NOCCA job-exposure matrix. Conclusion: When considering asbestos exposure, past exposure, even for short periods, might be enough to cause mesothelioma of the pleura later in life.
There are many assessment techniques used for occupational risk factors of MSDs in the workplaces. However, because all ergonomic assesment techniques or tools are based on theoretical background derived from workphysiology, biomechanics, psychophysics, industrial hygiene, work system, and etc, it is impossible to compare the assessment techniques. This study was conducted to compare the excess rates of risk factors among ergonomic assessment techniques and to make alternative methods. Site-visits to 6 automobile products and parts company provided data for process repeated work where the produced data was examined for evaluating the relationship between workplace lay-out and work posture by using ergonomic assessment techniques. We evaluated 157 jobs for simple repeated work and 37 jobs for manual materials handling (MMH). In simple repeated work, the exceeded rates of AC were 36.3% in OWAS method and 93.0% in RULA method. The exceeded rate for RULA method was significantly higher than those for OWAS method (p<0.05). In MMH, the exceeded rates of AC were 80.0% in NLE method and 76.5% in WAC method. Statistically significant differences were not identified in the exceeded rates for NLE and MAC methods (p<0.05). The analyzed results among ergonomic assessment techniques (OWAS, RULA, NLE/WAC) were applied to the same work places performing simple repeated work and manual materials handling simultaneously. The applied results showed statistically significant differences (p<0.05) among ergonomic assessment techniques (OWAS, RULA, NLE/WAC). Exceeded rates of four ergonomic assessment techniques in decreasing order was "RULA>NLE>WAC>OWAS". The RULA method was the strongest assessment technique for automobile products and parts company. We discovered that the results could easily be overestimated or underestimated when the ergonomic assessment techniques were not applied correctly during the evaluation process. Therefore, we recommend using at least 2 methods when evaluating and analysing the results.
