Identification of soil Remedial Goal due to Arsenic in Soil near Abandoned Mine- Approach to Regarding Future Land Use - (폐광산 지역의 비소오염에 대한 복원목표 설정 - 미래 토지용도를 고려한 접근방법 -)

  • Recently, It is increasing popularity to research on the soil remediation in aspect of management by reason of the hazardous impact on the contaminated soil in Korea. It was investigated high levels of arsenic salts in soil near abandoned five mines(Darak, Daduk, Jingok, Dalsung, Ilkwang) located in Youngnam area. Arsenic, classified as group A(Human Carcinogens) from IRIS, have shown statistically significant increment in skin cancer with oral exposure. This paper was conducted to predict excess cancer risk value (to the skin cancer) based on multiple pathway such as soil ingestion, dermal uptake and food(plant) ingestion contaminated by arsenic, and also, to identify the remedial goal regarded in future land use. The mine having the highest arsenic level was Daduk(mean : 1950mg/kg) and the next rank was Jingok(1690mg/kg), Ilkwang(352.37mg/kg), Dalsung(86.08mg/kg), Darak(0.83mg/kg). The chronic daily intake to the multiple exposure were calculated using Monte-Carlo simulation regarded in future land use and used q: value was $1.5(mg/kg/day)^{-1}$ to the oral proposed by IRIS(1997). The computated excess cancer risk 95th value to all the mine regarding future land use as residential and rural area were more than $10^{-4}$. If the level of acceptable risk is aimed for 1$\times$$10^{-6}$, it could be used Darak as commercial and industrial area without soil remediation due to the lowest risk value(6$\times$$10^{-8}$ and 3$\times$$10^{-8}$). Computated remedial goal based on 1$\times$$10^{-6}$ of acceptable risk to the future land use as the residential, rural, commercial and industrial area were 0.02mg/kg, 0.003mg/kg, 97.31mg/kg and 194.62mg/kg, respectively.

Economic Assessment of Coal-fired & Nuclear Power Generation in the Year 2000 -Equal Health Hazard Risk Basis- (2000년대 원자력과 유연탄 화력 발전의 경제성 평가 -동일 보건 위험도 기준-)

  • On the basis of equal health hazard risk, economic assessment of nuclear was compared with that of coal for the expansion planning of electric power generation in the year 2000. In comparing health risks, the risk of coal was roughly ten times higher than that of nuclear according to various previous risk assessments of energy system. The zero risk condition can never be achievable. Therefore, only excess relative health risk of coal over nuclear was considered as social cost. The social cost of health risk was estimated by calculation of mortality and morbidity costs. Mortality cost was $250,000 and morbidity cost was $90,000 in the year 2000.(1986US$) Through Cost/Benefit Analysis, the optimal emission standards of coal-fired power generation were predicted. These were obtained at the point of least social cost for power generation. In the year 2000, the optimal emission standard of SOx was analyzed as 165ppm for coal-fired power plants in Korea. From this assessment, economic comparison of nuclear and coal in the year 2000 showed that nuclear would be more economical than coal, whereas uncertainty of future power generation cost of nuclear would be larger than that of coal.

Apixaban versus Warfarin in Patients with Chronic Kidney Disease; A Systematic Review and Meta-analysis (만성신장병환자에서 apixaban과 warfarin의 안전성 비교: 체계적 문헌고찰 및 메타분석)

  • Background: Patients with chronic kidney disease (CKD) are at a high risk of stroke-related morbidity, mortality, and bleeding. However, the overall risk/benefit of anticoagulant therapy among patients with CKD remains unclear. Methods: The MEDLINE, EMBASE, and CENTRAL databases were comprehensively searched until July 31, 2020, to investigate the safety and efficacy of apixaban in patients with stage 4 or 5 CKD, as compared with warfarin. The primary outcome was an incidence of major bleeding. Secondary outcomes included composite bleeding (major, clinically relevant, and minor bleeding), venous thromboembolism (VTE), stroke, and death. Results: In total, seven studies consisting of 10,816 patients were included. Compared with warfarin, apixaban was associated with a reduced risk of major bleeding (OR 0.49, 95% CI 0.41-0.58). In terms of composite bleeding, apixaban tended to pose a significantly lower risk than warfarin (OR 0.51, 95% CI 0.37-0.71). There was no difference between apixaban and warfarin with respect to the risk of stroke or death (stroke: OR 1.23, 95% CI 0.49-3.12; death: OR 0.73, 95% CI 0.45-1.18). Conclusion: Among patients with stage 4 or 5 CKD, the use of apixaban was associated with a lower risk of bleeding compared to warfarin and was also found to pose no excess risk of thromboembolic events.

Metabolic Syndrome Risk by Dietary Fat Energy Ratio in Middle-aged Men - Using the 2012~2013 Korean National Health and Nutrition Examination Survey Data - (중년 남성의 지방 에너지비에 따른 대사증후군 위험도 비교 - 2012~2013년 국민건강영양조사 자료 이용 -)

  • This study aimed to compare energy nutrient intake, health related factors, physical characteristics, blood biochemical indices, prevalence of metabolic syndrome and odds ratio (OR) of metabolic syndrome based on dietary fat energy ratio. Subjects were 1,205 men aged 40~64 years. The average fat intake was 52.8 g. Subjects were divided into three groups (deficient, normal, excess) based on dietary fat energy ratio. The dietary fat energy rations of the three groups were 36.9%, 42.9% and 20.2%, respectively. Energy and protein intake were increased significantly with dietary fat energy ratio (p<0.001), whereas carbohydrate intake decreased (p<0.001). In health related factors, amount of smoking alone showed increase based on dietary fat energy ratio (p<0.001). In comparing physical characteristics, blood pressure and blood biochemical indices, excepting diastolic blood pressure, increased significantly based on dietary fat energy ratio (p<0.01~p<0.001). The rate that exceeded criteria in risk factors for metabolic syndrome was higher in the serum triglyceride (41.2%) and was lower in the waist circumference (22.2%). Prevalence of metabolic syndrome was 37.9%, and showed significant correlation to dietary fat energy ratio (p<0.05). The OR of metabolic syndrome was higher in deficient and excess group than in normal group, but it had no relationship between fat energy ratio and metabolic syndrome. The results of this study provide basic data to establish fat intake guidelines for prevention of metabolic syndrome in middle-aged men.

A Survey of Aflatoxin Contamination in Medicinal Herbs for Food and Medicine (식약공용한약재의 아플라톡신 오염실태 조사)

  • The aim of this study was to determine the incidence and contamination levels of aflatoxin in Medicinal Herbs for Food and Medicine at Yakyeang market in seoul. 191 Samples 11 items medicinal herbs for food and medicine were evaluated for the aflatoxin contamination. in result 41 samples 10 items (21.5%) were detected in the alfatoxin, a high incidence of aflatoxins items are cassiae semen (50.0%), testudinis plastrum (43.8%) and Batryticatus Bombyx (40.0%), Polygalae Radix (31.2%), Zizyphi Semen (23.5%), Dolichoris Semen, Myristicae Semen (20.0%), Nelumbinis Semen (15.8%), Glycyrrhizae Radix et Rhizoma (7.4%), Hoveniae Semen Cum Fructus (4.3%). AFB1 were detected 27 cases (14.1%), AFB2, AFG1 and AFG2 were detected 18cases (9.4%), 16cases (8.4%) and 5cases (2.6%). The excess cancer risk estimated using the cancer potency of aflatoxin B1 (7(mg/kg/day)-1 for HBsAg-and 230(mg/kg/day)-1 HBsAg+) was N.D ~ $3.79{\times}10^{-6}$ for hepatits B surface antigen negative (HBsAg-) and N.D ~ $9.68{\times}10^{-5}$ hepatits B surface antigen positive (HBsAg+) respectively.

  • In the Oriental Western Consideration of Habitual Abortion, the result of this study is summarized as follows: 1. Habitual abortion means repetition of Tatae(abortion within 3 months), Sosan(abortion after 3 months), and Bansan(abortion after 5 months), as it agrees with Hoaltae, Sutatae and Nuing-Nuta in oriental medical science. 2. Frequency in Occurrence of habitual abortion is about 0.2-0.4%> in the whole pregnancy and the Ratio of Risk increases according to frequency and age increase. 3. Generally, the cause of habitual abortion is due to the cause of the mother. Therefore, diagnosis and treatment of before-childbirth are needed, and 'measures taken to prevent abortion' is in use to improve the condition of health of before-childbirth in oriental medical science. 4. The 50-60% of early abortion is due to the heterochromosome and the ratio of risk of habirual abortion is hightest in heterochromosome. 5. The causes of habitual abortion are summarized as vital energe and blood weakness. impairment of Chong and Ren, aflection by exopathogen, fever caused by blood deficiency. weakness of the spleen and the stomach, excess of seven emotion. excess of a sexual desire and injury of a contusion and also the treatments are summarized as invigorating qi (vital energy) and enriching the blood, reducing fever and enriching the blood, reinforcing the spleen, tonifying the Chong and Ren, the practice of a sceticism, psychological peace in oriental medical science. 6. The approch of modem oriental medical science is based on diagnosis and treatment based on 'over all analysis of symptoms and signs' of traditional oriental medical science. it goes abreast with diagnosis of western medical science, and it can be expected mare inclusive effect of treatment because 'Acupuncture for prevent abortion', 'Acupuncture with the needle warmed by burning moxa', and so on being developed except medicinal therapy.

Natural radioactivity level in fly ash samples and radiological hazard at the landfill area of the coal-fired power plant complex, Vietnam

  • In this study, natural radioactivity concentrations and dosimetric values of fly ash samples were evaluated for the landfill area of the coal-fired power plant (CFPP) complex at Binh Thuan, Vietnam. The average activity concentrations of 238U, 226Ra, 232Th and 40K were 93, 77, 92 and 938 Bq kg-1, respectively. The average results for radon dose, indoor external, internal, and total effective dose equivalent (TEDE) were 5.27, 1.22, 0.16, and 6.65 mSv y-1, respectively. The average emanation fraction for fly ash were 0.028. The excess lifetime cancer risks (ELCR) were recorded as 20.30×10-3, 4.26×10-3, 0.62×10-3, and 25.61×10-3 for radon, indoor, outdoor exposures, and total ELCR, respectively. The results indicated that the cover of shielding materials above the landfill area significantly decreased the gamma radiation from the ash and slag in the ascending order: Zeolite < PVC < Soil < Concrete. Total dose of all radionuclides in the landfill site reached its peak at 19.8 years. The obtained data are useful for evaluation of radiation safety when fly ash is used for building material as well as the radiation risk and the overload of the landfill area from operation of these plants for population and workers.

Combined effect of work from home and work during nonwork time on sleep disturbance

  • Background: Owing to the coronavirus disease 2019 pandemic, being exposed to work from home and work during nonwork time simultaneously can lead to sleep disturbance; however, their combined effect is unclear. We aimed to investigate the combined effect of work from home and work during nonwork time on sleep disturbance. Methods: This study used data from the Sixth Korean Working Condition Survey and included 27,473 paid workers. Logistic regression analysis was conducted to investigate the relationship between work from home, work during nonwork time, and sleep disturbance according to sex. We re-classified participants into 4 groups based on their working from home (No/Yes) and working during nonwork time (No/Yes). The relative excess risk due to interaction was calculated to examine the effect of exposure to both telecommuting and non-regular work hours on sleep disturbance. Results: Workers exposed to work from home and work during nonwork time had significantly higher risks of sleep disturbance for all, men, and women workers (OR [95% CI]: 1.71 [1.46-2.02], 1.79 [1.43-2.23], and 1.64 [1.29-2.08] for work from home and 3.04 [2.70-3.42], 3.61 [3.09-4.22], and 2.41 [2.01-2.90] for work during nonwork time, respectively). Compared to those who were not exposed to both factors, when workers had both job factors, the ORs (95% CI) of sleep disturbance for all, men, and women were 3.93 (2.80-5.53), 5.08 (3.21-8.03), and 2.91 (1.74-4.87), respectively. The relative excess risk due to interaction of work from home and work during nonwork time was not significant for sleep disturbance. Conclusions: Work from home and work during nonwork time were each associated with sleep disturbance, but the interaction between the two factors on sleep disturbance was not observed in both men and women.

New Approaches to Assessing Nutrient Intakes Using the Dietary Reference Intakes

  • The Dietary Reference Intakes (DRI's) are new nutrient intake standards that are being set for the United States and Canada. There are currently four types of DRI's: Estimated Average Requirements (EAR), Recommended Dietary Allowances (RDA), Adequate Intakes (AI), and Tolerable Upper Intake Levels (UL). The EAR is the nutrient intake that would be adequate for about half the population, while intake at the RDA should be adequate for 97-98% of the population. When the data are insufficient to set an EAR and RDA, then an AI is set. The UL is the highest intake level that does not pose a risk of adverse effects. The EAR, AI, and UL may be used to assess intakes of both individuals and of groups of people. For individuals, the EAR is used to calculate the probability that intake is inadequate, the AI is used to decide if the probability of inadequacy is low, and the UL is used to determine if a risk of excess intake is present. For groups. the EAR is used to estimate the prevalence of inadequacy, the AI is used to decide if the prevalence of inadequacy is low, and the UL is used to estimate the prevalence of excessive intakes. Because this approach to setting and applying nutrient standards is new, research recommendations include improving estimates of risk, improving dietary data, and improving statistical methods.

The Significance of the Analytical Sciences In Environmental Assessment

  • The quality of human life is directly related to the quality of the environment. To assess environmental quality we must first determine the MCLG(Maximum Contaminant Level Goal), MCL(Maximum Contaminant Level), environmental impact and so on. The MCLG is the concentration at which no known adverse health effects occur. The MCLG is determined by risk assessment identifying which process is hazardous assessing, dose-response, human exposure, and characteristics of risk. With consideration of analytical methods, treatment technology, cost and regulatory impact, the MCL is set as close to the MCLG as possible. In this way, determination of the concentration and national distribution of contaminants is important for assessment of environmental quality The analytical sciences pose potential problems in assessing environmental quality. Continuing improvement in the performance of analytical instruments and operating technique has been lowering the limits of detectability. Contaminant concentration below the detection limit has usually been reported as ND(Not-Detected) and this has often been misunderstood as equivalent to zero. Because of this, more the contaminant concentration in the past was below the detection limit, whereas contaminants can be quantified now even though the contaminant concentration might remain the same or may even have decreased. In addition, environmental sampling has various components due to heterogeneous matrices. These samples are used to overestimate the concentration of the contaminant due to large variability, resulting in excess readings for MCL. In this paper, the significance of the analytical sciences is emphasized in both a conceptual and a technical approach to environmental assessment.
