CSP 를 이용한 정밀부착형 국부의치에 관한 임상적 연구 (A Clinical Study on CSP Attachment Partial Denture)

  • The technology of precision attachments has developed at such a pace that from a very few T-shaped attachments and bar attachments from the years 1915 to 1935 since removable bridge utilizing a T-shaped intracoronal attachment was constructed by Dr. Herman E.S. Chayes in 1906. There are now more than 120 models of the most diversified designs, ready made or laboratory fashioned. In 1971, 126 attachments were listed and classified by Mensor in his E M Attachment Selector. This selector consists of five charts giving specifications as to type, vertical dimensions, application, type of resilience, size of movement, type of retention and type of material and alloy. Thus the E M Attachment Selector is a useful guide for dentists to choose the attachment for his patients. But dentists should apply the attachment in each patient's case according to an accurate diagnosis and treatment plan. This paper is a case report of removable partial dentures utilizing CSP, PD and Bar attachment on a patient who needed full mouth reconstruction. Patient has right first, second molar and left first molar on the upper arch and also left first molar, first premolar and right canine on the lower arch. (Fig. 5)All remaining teeth are relatively healthy in their supporting tissues. On upper arch, ring shape CSP attachment was designed on left first molar and modified ring shape CSP attachment was designed on right first and second molar as the direct retainer of the removable partial denture. Full palatal coverage was used as the major connector in this case. (Fig. 23) On lower arch, author first splinted with a fixed bridge between left first molar and second premolar and a splint bar between left second premolar and right canine. (Fig. 11) A lower removable partial denture in which was designed with an Aker clasp on the left first molar and a PD attachment on .the right canine was constructed. (Fig. 17) This denture could get additional support from anterior splint bar. After both removable partial dentures were delivered to the patient (Fig. 26), author evaluated function of the dentures and supporting structures of the abutment teeth by means of clinical and X-ray examinations for eighteen months. According to the examination data author came to the conclusion that the prognosis of this case was excellent.

치아종의 임상적 특징과 치료에 대한 후향적 연구 (Retrospective Study of the Characteristics and Treatment of Odontomas)

  • 치아종은 상피성 치성 종양에 속하며 악골내 치성 종양 중에서 가장 흔한 질환으로 보통 유치의 만기 잔존이나 영구치의 매복과 연관되어 나타난다. 본 연구는 2005년 1월부터 2014년 3월까지 전북대학교 치과병원에서 치아종으로 진단을 받은 74명의 환자에서 발견된 치아종 75증례를 분석하였다. 그 결과 남자가 42명(56.8%), 여자 32명(43.2%)으로 남자에서 약 1.3배 정도 높은 발생률을 보였다. 치아종으로 진단받은 환자들의 평균 나이는 10.3세이며, 치아종의 위치는 상악 48%(36개), 하악 52%(39개)로 균등하게 분포되어 있었다. 치아종과 관련된 임상 증상으로는 인접 치아의 매복 76%, 유치의 만기 잔존 52%, 인접 치아들의 전위 또는 회전이 32%로 나타났다. 치아종은 대부분 외과적으로 제거되었으며, 제거 후 자발적으로 맹출한 경우가 17증례, 영구치 맹출을 위해 추가적 치료가 필요했던 경우가 25증례, 매복된 치아의 예후가 불량할 것으로 판단되어 발치를 한 경우가 9증례였다.

고등학교 '기술·가정' 교과 식생활 영역의 교육내용 분석: 제7차 교육과정부터 2015 개정 교육과정까지의 교과서 내용을 중심으로 (Content Analysis of Food and Nutrition Unit in High School Textbooks of Home Economics: Focus on the National Curriculums from 7th to 2015 Revised)

  • 본 연구는 제7차부터 2015개정 교육과정까지 식생활 영역의 교육과정별 변화를 살펴보기 위해 교육과정과 기술·가정 교과서 8권을 조사하여 분석하였다. 교과서 내 식생활 영역에 해당하는 그림, 그래프, 표, 사진에 있는 단어를 포함한 모든 텍스트를 파일로 전환하여 불용어를 제거하고 텍스트 마이닝 기법으로 상위 15개의 단어를 추출하였다. 이를 한눈에 파악할 수 있도록 워드클라우드(word cloud)와 언어네트워크 분석법으로 시각화하였다. 연구 결과는 다음과 같다. 제7차부터 2015개정 교육과정까지 고등학교 '기술·가정' 교과서에서 식생활 영역의 비율은 평균 10.97%였고, 교육과정이 개정을 거듭함에 따라 식생활 영역이 차지하는 비중은 대폭 감소되었다. 고등학교 '기술·가정' 교과의 교육과정에 나타난 식생활영역의 내용체계는 1개의 소영역이 독립적으로 구성되어 있고 일부 내용이 가족생활영역에 포함되어 있었다. 언어네트워크를 종합적으로 살펴보면 초대, 한식·재료, 영양·섭취·아기를 중심으로 3개의 큰 묶음이 형성되어 연결되어 있었다. 결론적으로 제7차부터 2015개정 교육과정까지 고등학교 '기술·가정' 교과서의 식생활 영역의 비율은 계속 줄어들었고, 고등학교과정에서 식생활을 통한 소통과 사회적 의미를 강조하고 있음을 알 수 있었으며, 시대적 요구에 따라 교육과정별 내용변화가 크다는 것을 확인할 수 있었다.

초등학교 6학년 과학과에서 프레어모형을 활용한 수업이 학습동기와 학업 성취도에 미치는 영향 (The Effect of Teaching Program with Frayer model on Learning Motive and Learning Achievement of 6th Grade Elementary Science Learning)

  • In this study, to raise the interest and curiosity of students and at the same time to learn science concepts meaningfully for students, the teaching and learning program was developed by applying the Frayer model. The purpose of this study was to find out the Effect of Elementary Science Teaching Program with Frayer model on Learning Motive and Learning Achievement. To this end, the 6th grade classroom of A-elementary school located in Seogwipo-city was selected the experimental group (26 patients). And the other 6th grade classroom in the same school was selected to the comparative group (27 patients). The experimental group was conducted applying the Frayer model. Comparison group has been conducted lesson program in accordance with the general science class teacher guide. Was through a pre-test of science learning motivation and academic achievement level can be assumed in the same group. After completing the experimental treatment by conducting a post-mortem examination was statistically validated. In this study, the following conclusions were obtained. First, elementary science class which applied Frayer model had the effect of to improve the scientific motivation. In particular, attention (p <.01), association (p <.01), confidence (p <.01) in the experimental group were higher than the scores of the comparative group, the difference was significant. Second, the Frayer model applied to elementary science class had a significant effect on improving science achievement. The experimental group which applied Frayer model was higher than the comparative group in science achievement post-test comparison. Between the groups showed a significant difference between the two groups (p <.01). The above findings, Elementary science class which applied Frayer model can be concluded to be effective in science and science achievement motivation. Therefore, applying the Frayer model of elementary science class could be useful in science teaching and learning methods. In addition, when it is determined through the previous study, applying the Frayer model classes will be able to derive a meaningful learning also subjected to a number of fields and areas.

치과위생사 역할중심의 문제중심학습 패키지 개발 (Development of PBL Package - focusing on dental hygienist roles -)

  • The purpose of this study was to introduce PBL to dental hygienist education in an effort to raise a question about the inauthentic and inappropriate curriculum. PBL is one of learning methods to enhance the problem-solving ability of learners, and it's attempted to develop a PBL package focusing on dental hygienist roles to lay the foundation for producing competent and expert dental hygienists with a good problem- solving ability. The literature concerned was reviewed from November 2002 through January 2003 to determine whether or not PBL was applicable to dental-hygienist course, and that turned out to be effective for dental hygienist education. And then a PBL package was developed to train students to be knowledgeable and have a knowhow and excellent problem-solving skills. The characteristics of the PBL package could be described as follows: First, that focused on dental hygienist roles to serve the purpose of this study to remedy the current unrealistic and improper curriculum and improve the problem-solving skills of learners. Second, time factor was taken into account. In this four-week course for two credits, there are four classes a week, and it's required to take six or eight weeks to apply the PBL package, which is expected to demotivate students. Therefore, it's planned to conduct more weekly classes to make a proper progress. Third, a wide variety of teaching aids were put to use, and learner would be encouraged to be more interactive and utilize teaching aids properly, and eventually, they could have an opportunity to better express themselves. Fourth, online real-time learner discussion would be attended by this researcher. Learners would have a discussion in real time in the Internet cafe chat room, and different discussion time would be allocated to each team. This researcher would take part in each team's discussion once or more. Fifth, learners would prepare one or more journal(s) about four-hour Internet cafe learning. They have to make it twice a week at least, and it would be a good opportunity for learners to look back on themselves and their teams, and their learning effect would be greater. Specific rules were presented to help them make a successful self-examination. Sixth, there are some spaces in the lower part of objective test sheets to have students describe why they make a particular answer choice. They would be asked to depict the reason of their prior evaluation and lecture assessment especially because their responses would be important for more successful discussion and feedback. Seventh, problem-solving approach was designed to attain learning objectives, stimulate the creative thinking of learners and help them share a more systematic discussion. That would serve as a secondhand guide not to make them digress when they discuss by using information they acquire from a scenario presented in class.

방사선사법 제정의 필요성 (The Necessity of Redefining the Radiological Technologist Independent Law)

  • According to the changes of the medical environment of the times, it is necessary to discuss the issues of the doctor's medical guidance and to conduct continuous research so that alternatives can be prepared systematically. Furthermore, in order to enhance the professionalism of radiological technologists and to develop the medical technician system, the new Radiological Technologist Independent Act has been established, which contains the overall contents of the scope of work, professional qualifications, and specialized education of radiological technologists, and provides quality medical services to patients through professional procedures and treatment. In order to increase the level of medical care, the purpose, definition, mission, role, and scope of work specified in the Medical Act, Medical Service Technologists, etc. Act, the Enforcement Decree, and the Enforcement Rules were variously analyzed and new directions were presented. First, the definition of a medical technician should use a generic term so that the factors of conflict and prejudice could be resolved. Second, change the doctor's guide to doctor's prescription; and then legislate the authority to sign and write medical records after examination by radiological technologists, thereby prohibiting unlicensed technicians that seriously endanger patient safety. Third, an accurate definition of radiological technologists' roles should be established; not only selection and management of radiological technologists' work but also procedures and treatment for each radiology field should be specified to suit the current medical system. Fourth, a professional radiological technologists' qualification system and a specialized education system should be established in order to secure human resources that could provide patients trust in procedures and treatment based on professional knowledge and experience in the field of radiology. Fifth, the Education and Evaluation Institute should be operated in Korea education system to educate the professional knowledge and competency for students. In addition, it is necessary to in-depth analysis of foreign cases could be applied to the medical system and education system in Korea; it could strive to nurture systematic human resources.

토압분리형 교량의 구조해석을 통한 허용 변위량 검토 (Examination of Allowable Displacement by Structural Analysis of IPM Bridge)

  • IPM Bridge는 파일벤트가 지표면으로부터 돌출되어, 교대의 과도한 변위가 유발될 수 있다. 본 연구에 사용된 교량의 형상은 IPM Bridge의 설계지침에 제시된 최대 적용 조건인 경간 120.0m, 사각 30도, 파일벤트의 돌출높이 최대 10.0m를 적용하였다. 이 교량모델을 이용하여, IPM Bridge의 최대 경간 적용조건에 따른 최대 변위를 산정하였으며, Bozozuk가 제시한 허용 변위에 근거하여 IPM Bridge의 수평변위의 안정성을 검토하였다. IPM Bridge의 최대 수평변위는 여름철의 팽창 조건보다는 겨울철의 수축 조건에서 더 크게 산정되었으며, 수평변위는 사각보다는 교량의 길이에 더 큰 영향을 받는 것으로 나타났다. 그리고 수직 변위는 사각과 연장에 큰 영향을 받지 않는 것으로 나타났다. 경간의 증가에 따라 수평변위가 크게 증가되었으며, 연장 120.0m에서의 수평변위는 Bozozuk가 제시한 허용 변위를 초과하는 것으로 나타났다. 하지만, 파일벤트에 발생되는 모멘트가 소성모멘트를 초과하지는 않았다. IPM Bridge는 설계지침에 제시된 최대 적용조건인 파일벤트의 돌출높이 10.0m, 연장 120.0m에서는 수평변위가 과도하게 발생될 수 있으므로, 설계단계에서 면밀한 검토가 필요하다.

뇌졸중으로 인한 운동 관련 후유증의 한의 치료 연구에서 뇌졸중 시기 및 평가 척도에 대한 국내 문헌 고찰 (A Review of the Stroke Stage and Outcome Measures in Stroke Motor Sequelae Clinical Studies in Korea)

  • Objectives: The aims of this study were to evaluate the fidelity of stroke stage reporting, the timeliness of the outcome measures, and the use of the core outcome set. Methods: We searched the literature using 6 domestic databases. We selected studies that used Korean medicine interventions and targeted stroke patients with motor sequelae. We examined whether the included studies reported the stroke stage and whether they used the outcome measures in the appropriate period based on the recommendations of the "Stroke Evidence Database to Guide Effectiveness". We also confirmed the use of the essential assessment tools suggested by the core outcome set. Results: Overall, 77 studies were finally selected, with 16 (21%), 55 (71%), and 6 (8%) published on the acute, subacute, and chronic phases, respectively. Only 11 of the studies directly mentioned the stroke stage. The most commonly used assessments were the National Institutes of Health Stroke Scale, Modified Barthel Index, and Manual Muscle Testing. Only 5 studies failed to apply the stage-related outcome measures at the recommended period. The outcome variables used inadequately were the National Institutes of Health Stroke Scale, Functional Ambulation Categories, 36-Item Short Form Health Survey, and Mini-Mental State Examination. Among the core outcome set items, some studies used liver and renal function tests, but no herbal medicine safety reporting was conducted. Conclusions: In future studies, we propose to ensure accurate reporting of the stroke stage with reliable outcome measures to deliver better clinical and research outcomes. Furthermore, in future clinical studies on stroke, a standard protocol that reflects the core outcome set should be developed.

텍스트마이닝과 ChatGPT 분석을 활용한 기업과 대중의 ESG 인식 비교: 지속가능경영보고서와 소셜미디어를 기반으로 (Comparing Corporate and Public ESG Perceptions Using Text Mining and ChatGPT Analysis: Based on Sustainability Reports and Social Media)

  • 최근 기업의 지속가능한 성장을 이끄는 ESG(Environmental, Social, and Governance) 관리의 중요성이 강조되고 있다. 이에, 본 연구는 기업과 일반 대중 간의 ESG에 대한 인식 차이를 실증적으로 밝히고, ESG 정책의 시행을 방해하는 부정적인 여론과 그 배경을 규명하는 것을 목표로 한다. 이를 위해, LDA(Latent Dirichlet Allocation) 토픽모델링, JST(Joint Sentiment Topic Modeling) 및 의미연결망분석 기법을 사용하여 지속가능경영보고서와 소셜미디어에서의 주요 키워드와 토픽, 그리고 그 연결관계를 분석하였다. 또한, ChatGPT를 활용하여, 텍스트마이닝 분석의 결과를 보완하였다. 분석 결과, 기업과 일반 대중 간 ESG에 대한 인식과 중요도에 상당한 차이가 있음을 확인하였다. 구체적으로, 기업들은 위기 관리, 투명한 지배구조, 윤리적 경영 등에 집중하여 신뢰를 구축하려 했으나, '그린워싱', '중대재해', '불매운동' 등과 같은 부정적 키워드가 자주 소셜네트워크에서 등장하여, 많은 대중들이 기업의 ESG 이슈 처리에 대해 의심하고 있음을 확인하였다. 본 연구는 기업, 정부 기관, 고객 및 투자자를 위한 ESG 전략수립에 도움이 될 수 있는 가이드라인을 제공한다는 점에서 의의가 있다.

췌관 내 유두상 점액종양의 치료 전략과 추적 관찰-소화기내과의 관점 (Management Strategy and Surveillance of Intraductal Papillary Mucinous Neoplasm-Gastroenterologist's Viewpoint)

  • 췌관 내 유두상 점액종양은 매우 다양한 자연경과와 예후를 가지고 있어 추적 검사방법, 간격, 기간 등이 아직도 논란이 되고 있다. 이 종양에 대한 진료지침들에서는 추적 검사를 받을 환자군과 수술적 치료가 필요한 군으로 분류하기 위하여 악성화 고위험 특징 또는 절대적 수술 적응증과 걱정스러운 특징 또는 상대적 수술 적응증으로 분류하였다. 걱정스러운 특징이나 고위험 특징이 없는 분지형 유두상 점액종양은 혼합형이나 주췌관 종양에 비하여 악성화 위험성이 매우 낮다고 보고되고 있다. 그러나 저위험 특징을 가진 환자들을 장기간 추적한 연구에서 시간이 지남에 따라서 췌장암 발생이 점점 높아지고 있다고 보고하고 있어 지속적인 장기간 추적 검사가 필요하다. 혼합형과 주췌관형 유두상 점액종양은 악성화 가능성이 매우 높아 수술적 치료가 필요하다고 알려져 있다. 무결절이고 주췌관 직경이 10 mm 미만으로 분절형 주췌관 확장을 가진 환자들은 악성화 가능성이 낮아서 즉각적인 수술보다는 주의 깊은 추적관찰을 고려해볼 수 있다. 고령이나 기저질환을 동반한 환자의 치료 방향을 결정할 때는 췌장암 발생의 위험도와 수술의 위험성을 균형있게 평가해야 한다.
