임상시험의 단계별 이해 및 실제 (Understanding of Clinical Trials and Application to the Real Practice)

  • Understanding of a clinical trial is essential in developing clinical guideline and adopting evidence based practice. In designing and executing clinical trials, following ethical requirements should be considered : social value, scientific validity, fair subject selection, informed consent, favorable risk-benefit ratio, institutional review board, and respect for human subjects. According to the stage of drug development, purpose of trials, accumulated scientific data, clinical trials for drug development are classified as phase 1, 2, 3, and 4. Phases of clinical trials can be overlapped and the judgment of entering into the next phase should be considered highly strategically. In reading, evaluating and interpreting clinical trial reports, various skills and challenges exist. Patient sample composition, trial duration, selection of endpoints, responders and non-responders, placebo effect, patient recruitment, and extrapolation to the real world are the examples of those challenges. Treatment success will come from the well balanced approach of evidence based decision making and consideration of specific single case.

Clear Aligner Therapy: Evidence, Eloquence and Reality

  • The demand for orthodontic clear aligner therapy (CAT) has increased significantly over the last decade, offering advantages over the fixed appliances (FA) including enhanced aesthetics, better hygiene and comfort, along with minimal restrictions on the patient's diet. Moreover, a marked improvement in the efficacy of tooth movement using aligners has been documented. On the contrary, there have been known limitations of CAT including the compliances issues and the apparent lack of efficacy for certain types of tooth movement such as closure of extraction space compared to FA. Thus, evidence-based evaluation of the accuracy of prediction of tooth movement with clear aligners and their ability to effectively perform major tooth and root movements compared to FA are crucial. Although several systematic reviews have investigated various aspects of the effectiveness of CAT, we are yet to obtain a rather conclusive answer to this question. The current review attempted to summarize the evidence-based findings of most systematic reviews about CAT available to date. Major issues regarding the predictability of tooth movement, the role of attachments and auxiliaries in improving the effectiveness of CAT, and the treatment outcomes in comparison to FA were investigated. Clinical recommendation have been also elaborated based on the interpretation of the findings of all systematic reviews included in this study.

뇌졸중후 요저류에 대한 침치료 임상진료지침 (Clinical Practice Guideline for acupuncture in Post-stroke urinary retention)

  • Objectives: Objectives : This study is aimed to develop a Clinical Practice Guideline (CPG) on acupuncture treatment for the patients with Post-stroke Urinary Retention; PSUR. Methods: Experts committee, consisting of stroke or methodology specialists, searched Medline, EMBASE, Cochrane Library, China National Knowledge Infrastructure, and 19 Korean medicine journals. The search terms were selected to screen the randomized controlled trials (RCTs) or systematic reviews for the effectiveness of acupuncture on PSUR compared with placebo or conventional group. Levels of evidence and grades of recommendations were appraised based on Recommendations for Development of Clinical Practice Guideline in Korean Medicine. Results & Conclusions: 4 RCT were included to build the CPG. There was a strong evidence to support the effectiveness of acupuncture treatment for PSUR. The moderate evidence was presented that over 3 times a week of the acupuncture should be performed over 4 weeks on the acupoints, such as SP6, CV3, CV6, CV4, SP9, BL28, BL23, BL22, KI3 or BL67, for 15-30 minutes. 20-140Hz frequency and 10-20 minutes of treatment is suggested if electro-acupuncture treatment is performed with. It was also suggested that the procedure should begin at the acute stage just after the vital signs of the patients are stabilized. Finally, there was a moderate evidence to support safety of acupuncture treatment for PSUR.

Evidence Based Analysis of Cisplatin for Treating Patients with Cutaneous Squamous Cell Carcinoma

  • Background: This analysis was conducted to evaluate the efficacy and safety of cisplatin based chemotherapy for treating patients with cutaneous squamous cell carcinoma. Methods: Clinical studies evaluating the efficacy and safety of cisplatin based regimens on response and safety for patients with cutaneous squamous cell carcinoma were identified using a predefined search strategy. Pooled response rates (RR) of treatment were calculated. Results: In cisplatin based regimens, 4 clinical studies which including 50 patients with advanced cutaneous squamous cell carcinoma were considered eligible for inclusion. Regimens included cisplatin, doxorubicin, or vindesine. Pooled analysis suggested that, in all patients, the pooled RR was 60% (30/50) in cisplatin based regimens. Nausea and vomiting were the main side effects. No grade III or IV renal or liver toxicity were observed. No treatment related death occurred with the cisplatin based treatments. Conclusion: Evidence based analysis suggests that cisplatin based regimens are associated with a good response rate and acceptable toxicity for treating patients with cutaneous squamous cell carcinoma.

한국 작업치료사들의 근거기반 치료(Evidence-Based Practice; EBP) : 인식도, 근거자원 사용과 방해요인 조사 (Evidence-Based Practice(EBP) Among Korean Occupational Therapists : Use of Resources, Perceptions, and Barriers)

  • 목적 : 본 연구는 한국 작업치료사들의 근거기반 치료(Evidence-Based Practice; EBP)에 대한 인식도, 임상적 의사결정에 사용하는 근거자원, EBP 실행을 방해하는 요인을 조사하고자 하였다. 연구방법 : 2009년 12월부터 3월까지 임상에 있는 작업치료사들에게 총 500부를 배부하였고, 그 중 회수된 160부(32%)를 기술통계를 통해 분석하였다. 결과 : 설문의 응답자는 남성보다 여성이 많았고, 치료경력이 5년 미만인 치료사가 전체의 90% 이상을 차지하였다. 응답자의 대부분은 EBP와 관련된 교육 경험이 없고(76%), 현재 실시하지 않는 것(55%)으로 나타났다. 또한 치료 시 활용하는 근거자원도 주관적인 판단이었다. 그러나 인식도 조사에서 EBP를 통해 작업치료사의 전문성이 향상될 수 있고(96%), 임상적인 판단에 도움을 준다고(88%) 응답하였다. 치료사의 대부분은 EBP 적용을 위해 도움을 줄 수 있는 개선책으로 근거자원 탐색을 위한 시간적인 여유 확보(90%)와 교육과정 참여가 필요(92%)였다. 이와 같은 결과는 임상 현장에서 EBP에 대한 적용 정도는 낮으나 인식도는 높은 것을 보여주어 향후 EBP에 대한 교육프로그램 활성화의 필요성을 간접적으로 제시하였다. 결론 : 한국 작업치료가 질 좋은 서비스를 제공하기 위해 적극적인 교육과정 개설과 강사 개발이 필요하고, 전국 대학의 작업치료전공 학생들이 근거기반의 교육을 받을 수 있도록 힘써야 할 것이다. 또한 작업치료사의 전문성을 향상시키기 위한 EBP가 조속히 이루어져 하고, 그에 교육프로그램과 그 효과성에 대한 연구가 끊임없이 이루어져야 할 것이다.

Pharmacologic Considerations for Youth with Posttraumatic Stress Disorder

  • Children exposed to potentially traumatic events are at risk of developing posttraumatic stress disorder (PTSD). However, the subsequent developmental course of posttraumatic stress symptoms appears to vary considerably. In this regard, some PTSD symptoms resolve without significant interventions, while for many children and adolescents, they persist until the patient receives appropriate treatment specifically designed to address PTSD and other trauma related symptoms. Evidence-based psychotherapies represent the standard of care for children with PTSD and, while psychopharmacologic interventions are utilized for many youth with posttraumatic stress symptoms and PTSD, there is little data available to guide the use of these medications in this population. However, given the structural challenges involved in disseminating and delivering evidence-based psychotherapies in all settings, prescribing clinicians should be aware of the medications whose use in children with pediatric PTSD has been studied. Herein, we review the PTSD assessment modalities, as well as the use of pharmacologic interventions in PTSD, including antiadrenergic agents, selective serotonin reuptake inhibitors and other medications.

Rib Fractures: To Fix or Not to Fix? An Evidence-Based Algorithm

  • Rib fractures are a common injury resulting from blunt chest trauma. The most important complications associated with rib fractures include death, pneumonia, and the need for mechanical ventilation. The development of new osteosynthesis materials has stimulated increased interest in the surgical treatment of rib fractures. Surgical stabilisation, however, is not needed for every patient with rib fractures or for every patient with flail chest. This paper presents an easy-to-use evidence-based algorithm, developed by the authors, for the treatment of patients with flail chest and isolated rib fractures.

  • Pulmonary arterial hypertension (PAH) is often difficult to diagnose and challenging to treat. Untreated, it is characterized by a progressive increase in pulmonary vascular resistance leading to right ventricular failure and death. The past decade has seen remarkable improvements in therapy, driven largely by the conduct of randomized controlled trials. Still, the selection of most appropriate therapy is complex, and requires familiarity with the disease process, evidence from treatment trials, complicated drug delivery systems, dosing regimens, side effects, and complications. We tried to provide evidence-based treatment recommendations for physicians involved in the care of these complex patients. Due to the complexity of the diagnostic evaluation required, and the treatment options available, it is strongly recommended that consideration be given to referral of patients with PAH to a specialized center.

Does Aging Matter? The Efficacy of Carpal Tunnel Release in the Elderly

  • Open release remains the gold standard in the treatment of carpal tunnel syndrome in cases where conservative treatment fails. However, the efficacy of carpal tunnel release in the elderly has been debated in the literature throughout the years. This review aims to review the current evidence pertaining to the efficacy of carpal tunnel release in the elderly. Based on the current evidence, the outcome of carpal tunnel release is unpredictable in the elderly. Elderly patients are also less satisfied with the operation compared to younger patients. The authors recommend that these messages be conveyed to elderly patients before surgery. Moreover, open carpal tunnel release should be offered in the early stages of treatment whenever operative management is indicated.
