3 Case Reports on Tension-type Headache Treated by Chuna manipulative Therapy (긴장성 두통에 대한 추나요법 치험 3례)

  • Objectives: The Purpose of this study is to report three cases on tension-type headache treated by Chuna manipulative therapy. Methods: Three patients diagnosed tension-type headache were treated with Chuna manipulative therapy only. To evaluate effect of treatment, visual analogue scale(VAS) and headache impact test(HIT) were used. We checked VAS and HIT score three times(pretreatment, post-treatment, follow-up). An interval among each evaluation time is 4 weeks. Results: At post-treatment evaluation, VAS and HIT score declined significantly compared with pre-treatment. At follow-up evaluation, the pain did not increased. Conclusions: These results suggests that Chuna manipulative therapy may be effective for tension-type headache.

Acupuncture Treatment for Ankylosing Spondylitis Using China National Knowledge Infrastructure: A Literature Review (강직성 척추염의 침 치료에 대한 최근 연구동향 분석: China National Knowledge Infrastructure 검색을 중심으로)

  • Objectives This study was performed to research the trends of Chinese acupuncture treatment for ankylosing spondylitis (AS). Methods We searched clinical studies about Chinese acupuncture treatment for AS through China National Knowledge Infrastructure. We analyzed the characteristics of selected studies according to research design, sample size, publication year, treatment methods, evaluation criteria and adverse events. Results 16 studies published from 2015 to 2020 were selected. They consisted of 11 randomized controlled trials and 5 case reports. In these studies, several types of acupuncture treatments were performed and especially there were many treatments that applied thermal stimulation of needles. The most commonly used evaluation criteria was effective rate. Conclusions This study showed that acupuncture treatment could be effective for AS. It suggested that various studies should be conducted to provide reliable evidence about acupuncture treatment for AS in Korea.

Improvement of the Cognitive Perceptual Assessment for Driving (CPAD) based on Usability Test

  • Objective: The aim of this study was to perform a usability test for CPAD (Cognitive Perceptual Assessment for Driving) and improve it based on the test results. Background: The cognitive perceptual assessment for driving is a computer-based assessment tool to assess the driving capacity of people with brain-damages. It may be a good tool for evaluating the brain-damaged drivers' safe driving capabilities and screening cognitive and perceptual deficits related to driving. We performed a usability test to improve the CPAD based on the result. Method: Both the software consisting of 8 sub-tests (depth perception, sustained attention, divided attention, stroop test, field dependency, digit span, trail making A, trail making B) and the hardware including the input and output devices ( joystick, mouse, keyboard, touch screen) were evaluated through user interviews. Also we identified the problems and issues in using them by observing the participants performing the CPAD tasks. Results: Based on the results, the task instructions were visually and auditorily improved and more practice examples were added to help the users understand the instructions better and increase the input accuracy, a response time window was added and the joystick and touch screen were simplified, which made it easier for the user to perform the CAPD tasks. Conclusion: User discomforts were minimized by improving the task environment, unless it had affected the evaluation criteria. Application: We plan to distribute the improved version of the CPAD to the regional rehabilitation hospitals, and the driving support centers for people with disabilities throughout the country, so it could be used as an evaluation tool for disabled drivers' cognitive and perceptual functions.

Evaluation of a Community-Based Rehabilitation Program in Wonju City (원주시 지역사회중심 재활사업 평가)

  • This study evaluated the community-based rehabilitation services provided by the Wonju Public Health Center from Jan. 2000 to Dec. 2002. Ninety-four persons with disabilities dwelling in the community participated and the surveys were completed in an interview during home visits. The respondents' demographic, socio-economic, and medical characteristics, rehabilitation service received, willingness to receive home-visit rehabilitation services, and satisfaction with the rehabilitation services were analyzed by frequency and percentage. A Likert scoring system consisting of five agreement-disagreement categories was applied to each item, consisting of Very Satisfied, Satisfied, So-So, Poorly Satisfied, and Very Poorly Satisfied. The major findings were as follows: 1) The rehabilitation services used included medical rehabilitation (26.9%), followed by social assistance (23.5%), diagnosis by a physician at home (17.3%), medical examination (12.3%), housekeeping services (6.2%), and vocational and educational rehabilitation (3.5%). 2) Of the medical services, the respondents desired physical therapy at home and free rental of rehabilitation equipment, such as wheelchairs, canes, walkers, the most, followed by home visit occupational therapy, nursing services, and oriental medicine service in descending order. 3) Some of the respondents expressed so-so satisfaction (50.0%) or dissatisfaction (16.9%) with the rehabilitation services provided by the Wonju Public Health Center. These findings should prove useful when planning or extending community-based rehabilitation programs for the homebound disabled in the community.

Technical Trends in Intelligent Rehabilitation Services (지능형 재활 서비스 기술 동향)

  • Physical rehabilitation consists of exercises that help individuals who have suffered injury, disease, or nervous system disorders to recover lost or impaired functions. The exercises need to be tailored to the degree and type of the patient's condition. To this end, a comprehensive rehabilitation data delivery system that connects medical institutions and local communities should be established, and an intelligent rehabilitation service technology should be developed. An intelligent rehabilitation service integrates and manages the patient's health information provided by medical institutions and the overall function evaluation information measured in the community. Based on the collected data, personalized rehabilitation services can be provided. We examine supportive policies and technological trends for rehabilitation exercise and sports. Additionally, we investigate development trends and key technologies of intelligent rehabilitation services being developed at ETRI.

Factors Influencing Caregiver Burden During Rehabilitation of Stroke Patients (뇌졸중 환자 재활 시 간병인 부담에 영향을 미치는 요인)

  • In this study, we aimed to determine which medical conditions of the stroke patients admitted for rehabilitation more than four weeks after onset affect caregiver burden. Participants diagnosed with stroke and their respective primary caregivers were enrolled and assessed after an 8-week follow-up period. The areas of evaluation for stroke patients included neurologic state, cognition, performance in daily life movements, gait, and balance, and caregivers were evaluated in the area of burden. The evaluation was conducted at the start of the rehabilitation course and eight weeks later. Patient caregivers were found to be under mild to moderate burden while providing care throughout the hospitalization period. The patient's neurologic state and cognition were correlated with caregiver burden. In the all patient and the subacute stroke patient group, multiple regression analysis confirmed that the neurologic state and balance stability were factors that influenced caregiver burden. Hence, we suggest that improvement in the patient's balance stability be emphasized during the course of rehabilitation to mitigate caregiver burden.

Assessment of Acupuncture Needle Safety and Stability on Applying Electroacupuncture (전기자극의 시술에 따른 일회용 호침의 안전성 및 안정성 연구)

  • Objectives : The electroacupuncture was generally has been used in oriental medicine doctors. In recent years, there have been a few studies about safety and stability of acupuncture needle in itself, but then again research of acupuncture needle safety and stability on applying electroacupuncture have been insignificant. Therefore we investigated the safety and change in mechanical characteristic of acupuncture needle in electroacupuncture therapy. Methods : We observed mechanical characteristic change by SEM(Scanning Electron Microscope) and EDX(Energy Dispersive X-ray Spectroscopy), evaluated the hardness by vickers hardness tester. We used MTT assay and cell stain to study about biocompatibility of electroacupuncture. Results : In this study, any corrosion of material, alternation of elements, and change of hardness were not observed in surface analysis using SEM and EDX. In cytotoxity evaluation using MTT assay and cell stain, cell survival rate was low when practicing the electroacupuncture for more than 3 hours. Conclusions : Change of mechanical property was not observed based on the test results using surface analysis and hardness estimation by the electroacupuncture. And considering the biocompatibility, electroacupuncture was thought to be safe in an hour based on cytotoxity evaluation using MTT assay and cell stain.

Application of AHP-PROMETHEE to determine areal and linear priorities for sewer rehabilitation projects (하수관로의 면단위 및 선단위 정비 우선순위 결정을 위한 AHP-PROMETHEE기법의 적용)

  • Since sewer rehabilitation program requires long construction period and enormous capital investment, determination of rehabilitation priorities is important with systematic planning considering appropriate evaluation parameters. In this research, we applied PROMETHEE(Preference Ranking Organization METHod for Evaluations) known as very objective and scientific multi-criteria decision-making analysis, using the weights determined by AHP(Analytic Hierarchy Process) for the selected sewer evaluation items to calculate the rehabilitation priorities for each sewer sub-catchment in basin Gusan 1 of Seoul. Preference functions and preference thresholds were estimated for each criterion of ratio of lack of hydraulic capacity of sewers, defect ratio, ratio of sewers with velocity less than its minimum criteria, and density of sewers in the sub-catchment. As a result, it was found that region d had the first priority among four sub-catchments. For each and every sewer located in region d, we could also rank sewers to be rehabilitated urgently.

Clinical Importance of Peak Cough Flow in Dysphagia Evaluation of Patients Diagnosed With Ischemic Stroke

  • Objective To investigate the relationship between peak cough flow (PCF), pulmonary function tests (PFT), and severity of dysphagia in patients with ischemic stroke. Methods This study included patients diagnosed with ischemic stroke, who underwent videofluoroscopic swallowing study (VFSS), PCF and PFT from March 2016 to February 2017. The dysphagia severity was assessed using the videofluoroscopic dysphagia scale (VDS). Correlation analysis of VDS, PFT and PCF was performed. Patients were divided into three groups based on VDS score. One-way ANOVA of VDS was performed to analyze PCF, forced vital capacity (FVC), forced expiratory volume in one second (FEV1), and age among the different groups. Results The correlation coefficients of VDS and PCF, VDS and FVC, and VDS and FEV1 were -0.836, -0.508, and -0.430, respectively, all of which were statistically significant at the level of p<0.001. The one-way ANOVA indicated statistically significant differences in PCF, FVC, FEV1, and age among the VDS groups. Statistically significant differences in VDS and age were observed between aspiration pneumoia and non-aspiration pneumonia groups. Conclusion Coughing is a useful factor in evaluating the risk of aspiration in dysphagia patients. Evaluation of respiratory and coughing function should be conducted during the swallowing assessment of patients with ischemic stroke.

Development of a Curriculum for Rehabilitation Nurse Specialist Program (재활 전문간호사 교육과정안 개발연구)

  • The purpose of this study is to develop a systematic and efficient curriculum for the rehabilitation nurse specialist program. This research was carried out as a group work of 15 experts in order to share various opinions about the curriculum, and also through literature review. Articles, curriculums of other Clinical Nurse Specialist Programs, medical laws guidelines, as well as Clinical Nurse Specialist Program from the Korean Nurses Association were reviewed, and the issue was discussed throughly via group meetings. The developed curriculum is as follows: 1. Educational philosophy lies in the fact that the rehabilitation nurses support the patients to maximize their potential and functional level, so that they could maintain healthy state and re-adapt to changed environment. Furthermore the rehabilitation nurses are disposed of arbitrary decision power under their own responsibility, thus they take charge of welfare and healthy environment of the local society through the patients(subjects) and local resources. 2. Educational goals are to train rehabilitation nurse specialist, who correspond to the social needs, so to say, those who have the knowledge and skills for nursing practice, education and research. 3. The curriculum consists of 37 credits, of which 24 credits are based on lectures and 13 credits based on clinical practice. General courses are 3 subjects (5 credits) ; nursing theory, nursing research, and laws/ethics. Mandatry courses are 8 subjects with 19 credits; advanced physical assessment, pharmacology, pathophysiology, issues in rehabilitation nursing, advanced rehabilitation nursing intervention I, advanced rehabilitation nursing intervention II, sports physiology, special rehabilitation nursing intervention. As for the clinical practice courses, assessment and evaluation for rehabilitation(64 hours), community and home based rehabilitation nursing(128 hours), hospital based rehabilitation nursing(128 hours), institution based rehabilitation nursing(96 hours) would be treated. 4. Contents of the curses were developed to correspond with the courses' objectives and specific items. 5. Evaluation would be carried out both in the lecture and in the clinical practice. The knowledge and skills of the students would be measured to ensure full validity and credibility. However this developed curriculum should be continuously modified and updated in more desirable direction.
