Risk factors of chronic subscapularis tendon tear

  • Background: Chronic subscapularis tendon tear (SBT) is a degenerative disease and a common pathologic cause of shoulder pain. Several potential risk factors for chronic SBT have been reported. Although metabolic abnormalities are common risk factors for degenerative disease, their potential etiological roles in chronic SBT remains unclear. The purpose of this study was to investigate potential risk factors for chronic SBT, with particular attention to metabolic factors. Methods: This study evaluated single shoulders of 939 rural residents. Each subject undertook a questionnaire, physical examinations, blood tests, and simple radiographs and magnetic resonance imaging (MRI) evaluations of bilateral shoulders. Subscapularis tendon integrity was determined by MRI findings based on the thickness of the involved tendons. The association strengths of demographic, physical, social, and radiologic factors, comorbidities, severity of rotator cuff tear (RCT), and serologic parameters for SBT were evaluated using logistic regression analyses. The significance of those analyses was set at p<0.05. Results: The prevalence of SBT was 32.2% (302/939). The prevalence of partial- and full-thickness tears was 23.5% (221/939) and 8.6% (81/939), respectively. The prevalence of isolated SBT was 20.2% (190/939), SBT combined with supraspinatus or infraspinatus tendon tear was 11.9% (112/939). In multivariable logistic regression analysis, dominant side involvement (p<0.001), manual labor (p=0.002), diabetes (p<0.001), metabolic syndrome (p<0.001), retraction degree of Patte tendon (p<0.001), posterosuperior RCT (p=0.010), and biceps tendon injury (p<0.001) were significantly associated with SBT. Conclusions: Metabolic syndrome is a potential risk factor for SBT, as are these factors: overuse activity, diabetes, posterosuperior RCT, increased retraction of posterosuperior rotator cuff tendon, and biceps tendon injury.

  • East Asian traditional medicine(Asian medicine) should be understood correctly on the basis of Asian medical network. For the correct understanding we analyzed Asian medical history which could be divided into three categories based on Dong-Eui-Bo-Gam. There are three mechanisms of human diseases, which are the pathogenesis of six external etiological factors(wind, cold, summer-heat, dampness, dryness and fire) due to affection by exopathogen [外感六氣], the pathogenesis of five viscera due to internal injury [內傷五藏], and the pathogenesis of physical constitution due to vital activity [生命體質]. While Chinese medicine has mainly developed the pathogenesis of the former two, Korean traditional medicine has mainly developed the pathogenesis of physical constitution.

A Clinical Review of Lung Abscess - 27 cases - (폐농양의 임상적 고찰)

  • During the preantibiotic era, lung abscess carried a high mortality, however, with the introduction of penicillin and subsequently other antibiotics, significant improvement was resulted. Clinical review was performed on 27 cases of lung abscess, those were admitted and treated at the Department. of Thoracic and Cardiovascular Surgery, Chonbuk University Hospital, from 1979 to August, 1986. The following results were obtained. The most common age of occurrence was between the age of 40 and 60 [14 cases] and 22 cases were male patients. The common symptoms were cough, putrid sputum production, fever and chilliness. Numerous etiological factors may play a role in the formation of lung abscess; pneumonia [7 cases], aspiration [5 cases], bronchiectasis [2 cases], liver abscess [1 case] and broncholith [1 case]. The bacteriologic study revealed increased incidence of Enterobacteriaceae [15 cases]. The treatment methods were postural drainage [12 cases]. pneumonostomy [5 cases], lobectomy [8 cases], thoracostomy [2 cases].

  • Avascular necrosis of bone has become a well-recognized complication of renal transplantation. While preexisting metabolic bone disease, especially hyperparathyroidism, and metabolic disturbances induced by steroids have been implicated as etiological factors, the pathogenesis is controversial. The diagnosis of avascular necrosis of bone had been based on a history of joint pain and radiographic demonstration of bone necrosis. Recently the bone scan using 99mTc-methylene diphosphonate is helpful in determining the early stage of bone necrosis. We report two cases of avascular necrosis of femur head, of which diagnosis was made by the bone scan using 99mTc-methylene diphosphonate.

  • Amelogenesis imperfecta patients suffered common clinical problems of poor esthetics, teeth sensitivity, and loss of occlusal vertical dimension. Amelogenesis imperfecta is a group of inherited disorders primarily affecting dental enamel. Variants of amelogenesis imperfecta generally classified hypoplastic, hypocalcified, or hypomaturation types based on the primary enamel defects. The mildest problems were found in the pitted hypoplastic type whereas the most severe problems were encountered in the hypocalcified type amelogenesis imperfecta. Management stragies include composite resin veneer and jacket crowns for anterior teeth as well as steel crowns for posterior teeth. Knowledge of the clinical features and dental complications of each variants if amelogenesis impecta helps in the diagnosis of the condition and allows institution of early preventive measures. The objective of this paper is to provide a review of the current concepts of the wide spectrum of etiological factors involved in the pathogenesis of this significance clinical entity in the primary dentition.

  • Obesity tracks from childhood into adulthood, and the persistence of obesity rises with age among obese children. Obesity are independent risk factors for increased morbidity and mortality throughout the lifecycle. Obese individuals develop resistance to the cellular actions of insulin, characterized by an impaired ability of insulin to inhibit glucose output from the liver and to promote glucose uptake in fat and muscle. Insulin resistance is a key etiological factor for type 2 diabetes mellitus, dyslipidemia, hypertension, nonalcoholic steatohepatitis, polycystic ovarian syndrome.

  • Uterine bleeding is a physiologic monthly event occuring during the reproductive period of the nonpregnant woman. As with any other physiologic event, normal women have different menstrual patterns and at times, it is difficult to define when normality ends and abnormality begins. Dysfunctional uterine bleeding is an all-inclusive term: its diagnosis usually bepends on the exclusion of local or systemic organic causes. The term dysfunctional refers to alterations in the physiologic mechanism controlling a multitude of cyclic events in the women's reproductive function. These mechanism are primarily neuroendocrine in nature. According to oriental medicine, the Metrorrhagia(崩漏) means an abnormal bleeding of femail genetalia, and is devided to bungjung and bungha. The etiological factors of metrorrhagia are heat of blood, asthenic coldness, asthenia of chong and ren channels, deficiency of blood, deficiency of vital energy, stagnated blood, and asthenia of spleen & stomach.

  • A traumatic Braun-Sequard syndrome of the cervical spine is reported in a 53-year-old male. The patient recovered spontaneously over several days with surgical intervention. The diagnosis was made on magnetic resonance imaging with physical examination, which also demonstrated subsequent resolution of bone marrow intensity. The etiological factors of spinal Braun-Sequard syndrome are reviewed.

  • The Authors report a case of sleep paralysis patient who is 25-years old man. He complained that he has been experiencing episode "being suffocated" during sleep and could not move by himself while semiconsciousness state. He was a only son of divorced parents and could not attach anybody from yong childhood. His symptoms was began intermittently after his early teens. Medical history and physical examination revealed no significant finding. A nocturnal polysomnogram showed loss of muscle tone during terminal REM period and sudden hyperventilation attack followed by wake-up. Psychosocial stress and insufficient sleep were presumed to be an etiological factors. Imipramine 50mg per day almost completely relieved symptoms.

  • Bibliographic study on the qi of Chong Channel ascending adversely(衝氣上衝) through the oriental medical books was carried out. And the following results were obtained; 1. Etiological factors of the qi of Chong Channel ascending adversely were failure of the kidney in receiving air(腎虛不納), transverse invasion of the hyperactive liver-qi (肝氣橫恣), insufficiency of the stomach-qi (胃氣虛弱), deficiency of the Front Midline Channelyin(任陰不足), etc. 2. Main symptom of the qi of Chong Channel ascending adversely were upward adverseness of qi to the chest(氣上衝胸). 3. Therapeutic measures of the qi of Chong Channel ascending adversely were relieving Chong Channel(鎭衝), astringing Chong Channel(斂衝), lowering the adverse flow of qi(降逆), tranquilizing Chong Channel(安衝). 4. Main durgs of the qi of Chong Channel ascending adversely were Fluoritum, Haematitum, TuberPinelliae, Fossilia Ossis Masto야, Concha Ostreae, Flos Inulae, etc. 5. Main prescriptions of the qi of Chong Channel ascending adversely were Kangwijinchongtang(降胃鎭衝湯), Younggyeogamtang(?桂五甘湯), etc.
