Recent Advances in Surgical Treatments for Growing Patients with Cleft (성장기 구순구개열 환자의 수술치료의 최신지견)

  • Cleft lip and palate is the most common teratologic condition of oromaxillofacial units, probably associated with genetic and environmental causes. The goal of cleft surgery is to optimize facial esthetics and stomatognathic function while minimizing growth disturbances from surgical intervention. In this article, the author suggests the recent surgical strategies that minimize cleft nasal deformity and midfacial skeletal constriction. From the author's surgical experiences and literature reviews, only considerate surgeries would achieve functional improvement and facial esthetics in patients with cleft lip and palate.

  • To be successful in aesthetic prosthetic treatment, the crown part aesthetic (White Esthetic) and the tissue part aesthetic (Pink Esthetic) must be well controlled. Clinically, white esthetics are the area of dental technicians and pink esthetics are the area of dentists, but a deeper understanding of counterparts can produce more successful results. In this paper, I will briefly describe this process.

  • An 18 year old female with oligodontia and maxillary hypoplasia was treated using an interdisciplinary team approach involving orthodontists, maxillofacial surgeons and prosthodontists. Full mouth one-piece fixed partial dentures were the final restoration. The fixed partial dentures fabricated for the maxilla and mandible using the concept of a shortened dental arch resulted in improved esthetics and the masticatory function. This paper describes the treatment procedures for an oligodontia patient with alveolar bone hypoplasia.

  • The aim of this clinical report is to describe the complex treatment of an adult Class III malocclusion patient who was disappointed with the outcome of a previous oral rehabilitation. Interdisciplinary treatment planning was performed with a primary indication for implant removal because of marginal bone loss and gingival recession, followed by orthodontic and surgical procedures to correct the esthetics and skeletal malocclusion. The comprehensive treatment approach included: (1) implant removal in the area of the central incisors; (2) combined orthodontic decompensation with mesial displacement and forced extrusion of the lateral incisors; (3) extraction of the lateral incisors and placement of new implants corresponding to the central incisors, which received provisional crowns; (4) orthognathic surgery for maxillary advancement to improve occlusal and facial relationships; and finally, (5) orthodontic refinement followed by definitive prosthetic rehabilitation of the maxillary central incisors and reshaping of the adjacent teeth. At the three-year follow-up, clinical and radiographic examinations showed successful replacement of the central incisors and improved skeletal and esthetic appearances. Moreover, a Class II molar relationship was obtained with an ideal overbite, overjet, and intercuspation. In conclusion, we report the successful esthetic anterior rehabilitation of a complex case in which interdisciplinary treatment planning improved facial harmony, provided gingival architecture with sufficient width and thickness, and improved smile esthetics, resulting in enhanced patient comfort and satisfaction. This clinical case report might be useful to improve facial esthetics and occlusion in patients with dentoalveolar and skeletal defects.

Implant Supported Overdenture using Milled Titanium Bar with $Locator^{(R)}$ Attachment on Fully Edentulous Maxillae : A Case Report (상악 완전 무치악에서 $Locator^{(R)}$ attachment가 장착된 milled titanium bar를 이용한 임플란트 지지 피개의치: 증례 보고)

  • The purpose of this case report is to introduce new attachment system(milled titanium bar with $Locator^{(R)}$ attachment) for implant supported overdenture in maxillary edentulous patients. A 56-years-old male patient visited the hospital due to the mobility of his maxillary fixed partial dentures(10-unit bridge). Including temporomandibular joint(TMJ), there was no specific PMHs to influence dental treatment. In radiographic and clinical evaluation, there was a severe bone resorption and mobility in maxillary teeth. Accordingly all the remaining maxillary teeth was extracted and fabrication of implant supported overdenture was planned. The milled titanium bar with $Locator^{(R)}$ was designed as an attachment system, considering the stability and retention of denture, masticatory efficiency, oral hygiene care, esthetics, pronunciation, and patient's financial state. The milled titanium bar was manufactured using CAD/CAM technology, and $Locator^{(R)}$ attachment connected to the bar by tap & drill method. For over 1-year, in terms of function and esthetics, satisfactory result was obtained.

  • In cases with maxillary lateral incisor missing, the problems were usually treated by the choice between space opening for prosthetic lateral incisor replacement and space closure with cuspid sub stitution for the alteral insicors. The decision of the choice could be perplexing in evaluation of the individual case. Therefore, various diagnostic criteria, such as occlusion, dental esthetics, canine position and inclination, tooth size relationship, maxillary lip length, and skeletal relation had to be evaluated. On the basis of this diagnostic information, the treatment planning could be established. A case was shown to illustrate the treatment of patient with congenitally maxillary lateral incisor missing.

I-shaped incisions for papilla reconstruction in second stage implant surgery

  • Purpose: Pink gingival esthetic especially on the anterior teeth has been an important success criterion in implant-supported restoration. Inter-implant papillae are a critical factor for implant esthetics, and various techniques for inter-implant papilla reconstruction have been introduced. The aim of this study is to suggest and evaluate a surgical technique for reconstructing inter-implant papillae. Methods: A 28-year-old man had an implant placed on the #13 and #14 area. Four months after implant placement, a second stage surgery was planned for inter-implant papilla reconstruction. At the time of the abutment connection, I-type incisions were performed on the #13i & #14i area followed by full-thickness flap elevation and connection of a healing abutment on underlying fixtures without suture. Results: Two weeks after the second stage implant surgery, soft tissue augmentation between the two implants was achieved. Conclusions: I-shaped incisions for papilla reconstruction performed during the second stage implant surgery were useful for inter-implant papilla reconstruction and showed a good esthetic result.

  • Dental porcelain should have adequate strength and similar esthetics as natural teeth. Recently esthetics has become a high priority in clinical dentistry. Thus the evaluation of color stability of dental porcelain has become an important part in dental research. The purpose of this study was to evaluate the difference in color stability of 3 types of porcelain(Vintage, VMK 95, CERAMCO II). The porcelain were divided into groups by 2 types of treatment(glazing or non-glazing and thermocycling or non-thermocycling). The porcelain were dyed with methylene blue and distilled water, and then the color stability was evaluated with a spectrophotometer The results were as follows; 1. The color change of porcelain increased in the order of VMK 95, Vintage, CERAMCO II. But there was no significant difference. 2. The color change decreased in the glazing group compared to the non-glazing group(p<0.05). 3. The color change increased after thermocycling But there was no significant difference. 4. The color change according to dye method was significantly larger with methylene blue copared to distilled water(p<0.05).

  • Dental ceramics is well known to have excellent esthetics, biocompatibility as well as high compressive strength. However, the fragility of ceramics against tensile and shear loads leading to the delayed fracture of micro crack on ceramic surface and the backwardness of ceramic fabrication technique limit the usage of ceramic materials in dentistry. Among all ceramic materials, zirconia has been introduced to overcome the drawback of conventional dental ceramics in the field of dentistry due to the nature of zirconia featuring proper opalescence and high fracture toughness. Also, novel manufacturing techniques enable ceramic materials to prepare high esthetic anterior and posterior all ceramic system. In this paper, it is introduced and discussed that novel techniques characterizing the bond strength between zirconia core and veneering ceramics and analyzing the fluorescence of dental ceramics in order to overcome the gap between the results of basic research and the feasibility of the results in the field of dental clinics.

Clinical evaluation of root coverage in Miller class III/RT2 labial gingival recession treated with interdisciplinary periodontal-orthodontic therapy: a randomized controlled clinical trial

  • Purpose: The aim of current study was to evaluate percentage root coverage (RC%) in isolated Miller class III/RT2 labial gingival recession (GR) associated with malaligned mandibular anteriors, using interdisciplinary periodontal-orthodontic treatment as compared to mucogingival surgery alone. Methods: Thirty-six systemically healthy patients having isolated Miller class III/RT2 GR with respect to malaligned mandibular anteriors, were randomly divided into test group: mucogingival surgery using subepithelial connective tissue graft followed by orthodontic treatment and control group: mucogingival surgery alone. Primary clinical parameters included (RC%), recession depth, keratinized tissue width, mid-labial clinical attachment level, interdental clinical attachment level (iCAL), periodontal phenotype (PP), gingival thickness (GT), root coverage esthetics score (RES) and hypersensitivity. Total duration of follow up was 12 months. Results: Mean RC% was significantly more achieved in test group (66.67%±40.82%) in comparison to control group (39.93%±31.41%) at the end of study (P=0.049). Further, complete root coverage was attained in 5/8 cases of test group versus 1/2 cases of control group after 3/12 months respectively. RES and hypersensitivity, showed statistically significant improvement after complete follow up period in both the groups. An ideal RES score of 10 was achieved in 4/7 cases of test group while in 1/2 cases of control group after 3/12 months respectively. Correlation analysis revealed significant negative correlation between RC% and iCAL. Correlation of RC% with GT and PP was non-significant. Conclusions: Interdisciplinary periodontal-orthodontic approach may be more beneficial in terms of achieving improved RC%, esthetic and resolution of hypersensitivity in the management of Miller class III/RT2 GR in malaligned mandibular anteriors.
