An innovative prostheses design for rehabilitation of severely mutilated dentition: a case report

  • Partial edentulism has multiple implications in relation to function, esthetics and future rehabilitative treatment. This case report illustrates the management of a patient with extreme consequences of partial edentulism. The main clinical findings were unopposed remaining teeth, overeruption of the remaining teeth, loss of vertical dimension of occlusion, and significant disfigurement of the occlusal plane. Following the diagnostic procedure, a well-coordinated prosthodontic treatment involving liaison with other dental disciplines was indicated. The management involved an innovative combination of fixed and removable prostheses in conjunction with crown lengthening surgery and strategic implant placement. Series of provisional prostheses were applied to facilitate the transition to the final treatment.

- Esthetic Retreatment of Old Restorations with Cervical Discolorations - (구 보철물 치경부 변색의 심미 재수복)

  • As patients have concerned the esthetics of dental restorations, the porcelain fused to metal restorations have gain their popularity due to their strength and esthetic appearance for many years. However, metal collar in subgingival level of porcelain fused to metal restoration often causes black shadows. Inadequately positioned metal collar causes plaque depositions, gingival inflammations, black shadows, and discolorations of margin area. Those problems can be avoided by a clear finishing line, well-fabricated provisional restorations with precise margin and fine polishing, collarless porcelain fused to metal restoration, and all ceramic restoration. In this case report, collarless porcelain fused to metal restoration was used to treat cervical discoloration from old restorations.

  • 아름다움이라는 것은 완전히 객관적이기도 하지만 극히 주관적인 표현의 결과일 수 있다. 심미치의학은 얼굴에서 표현되는 이러한 주관적인 면과 객관적인 면을 조화시킬 수 있는 학문이기에 21세기에도 여전히 치과 치료에 있어서 중요한 화두 중의 하나임에 틀림이 없다. 과거 심미적인 치료로 행하였던 많은 치료 술식들(open faced crown, resin faced crown,3/4 crown,7/8crown등)이 치과 치료 술식의 발달과 함께 재료와 치아 보철물 제작술 등의 발달로 인해 점차 사라지거나 변화하고 있는 것이 사실이다. 지금 소개하는 증례들은 현재 치과계에서 심미적인 치료 술식으로 인정하고 있기에 이러한 치료 술식에 대한 이해가 필요할 것이다. 우리 임상가들의 최종목표는 환자들에게 자연스러운 얼굴과 웃음을 회복시켜주는 것이다. 각 증례에 대한 한계에 대해 미리 알 수 있는 방법을 모색해 보고, 예측 가능한 치료를 시행하기위한 과정들을 살펴보고자 한다.

CROUZON'S DISEASE: A Case Report (Crouzon's disease 의 증례)

  • This is a case report of Crouzon's disease as a kind of craniofacial dysostosis by premature closure of unilateral coronal suture, showed plagiocephalic skull. 5-year-old boy was visited for the treatment of dental caries and oral examination. Physical examination showed hypertelorism, internal strabismus, and saddle nose. Intraoral radiographs showed congenital missing of upper right and left deciduous and permanent lateral incisors. Cephalometric analysis showed shortening the posterior cranial base length, clockwise growth pattern and class III and open bite tendency. Posterior-anterior and submentovertex view showed multiple radiolucencies-digital impression on inner surface of cranial vault. Maxillo-facial and neuro-surgical treatment was required to improvement of facial esthetics and optic complications. Continuous examination was needed to the growth and development.

  • Treatments for restoring the function and esthetics of missing teeth include fixed bridge, partial denture, orthodontic movement, implantation and autotransplantation. However, there is no absolute indications for each techniques. Due to undevelopment of root and root resorption after autotransplantation, the success rate of autotransplantation over the past decades have been low. Recently. with the study on biological principles of the healing of periodontal ligament, the success rate of autotransplantation began to improve. We report the cases of successful autotransplantation which resulted in ideal healing of periodontal ligament, gingiva and alveolar bone.

  • Maxillary and mandibular anterior dental arches often have the problems of occlusal relation and esthetics by malformations of teeth, congenital missing, et at. Though the clinician usually use the anterior ratio to overcome this problems, he has the limitation of a direct application this ratio to the prediction of anterior occlusal relationship by the change of anterior ratio as dental arch form, intercanine width, segment depth and arch perimeter. So this study examine maxillary and mandibular anterior dental arch forms by least square method using Korean normal occlusion models(man : 20 casts, woman : 20 casts). Maxillary and mandibular anterior dental arches of Korean normal occlusion models are curve fitted to polynomial function, beta function, hyperbolic cosine function in order. And this accuracy of curve fitting is constant regardless of man/woman and maxilla/mandible. The relationships between intercanine width, segment depth, and arch perimeter based on this owe fitted dental arch form are acquired. This relationships will give the prediction of anterior dental arch form and the information of more accurate anterior ratio according to intercanine width.

  • Mental retardation is defined by the American Psychiatric Association as" subnormal general intellectual functioning which originates during the developmental period and is associated with impairment of either learning and social adjustment or maturation, or both." A patient with mild to moderate mental retardation can be managed adequately using restraints and medications. However, in case of severe or profound mental disability, dental treatment cannot be accomplished even with the use of behavior modification, physical restraints and sedation techniques. When treatment in the dental office has much difficulty, hospitalization for dental treatment under general anesthesia can and should be considered. This case presents the treatment of a patient with mental retardation who was referred to our department for comprehensive dental care. Dental examination revealed widespread dental caries and a severe anterior open bite with crowding problems. Under general anesthesia, generalized caries treatment was performed by our department, and the anterior dental esthetics was achieved in collaboration with the department of prosthodontic dentistry.

Level of perception related to changes in lower facial height (하안면 고경 변화의 인지도에 관한 연구)

  • Understanding the level of a person's perception of changes that have occurred on the face after orthodontic treatment is critical to the process of orthodontic diagnosis and treatment planning. The purpose of this study was to determine the level of perception of profile and frontal changes in lower facial height. Forty students attending art school participated in a study evaluating the level of a participant's perception of changes in the lower facial height. Participants compared computer-graphic frontal and profile photographs with balanced proportions and photograph simulations of 1, 2, 3, and 4mm changes in lower facial height from stomion to the chin. At least a 2 mm change in lower facial height for the profile view and 3mm in the frontal view was needed to be perceived after orthodontic treatment. The level of a person's perception of the change in lower facial height was more sensitive in the profile view than in the frontal view, and information about facial changes given prior to evaluation enhanced the level of perception.

The effect of different fiber reinforcements on flexural strength of provisional restorative resins: an in-vitro study

  • PURPOSE. The aim of this study was to compare the flexural strength of polymethyl methacrylate (PMMA) and bis-acryl composite resin reinforced with polyethylene and glass fibers. MATERIALS AND METHODS. Three groups of rectangular test specimens (n = 15) of each of the two resin/fiber reinforcement were prepared for flexural strength test and unreinforced group served as the control. Specimens were loaded in a universal testing machine until fracture. The mean flexural strengths (MPa) was compared by one way ANOVA test, followed by Scheffe analysis, using a significance level of 0.05. Flexural strength between fiber-reinforced resin groups were compared by independent samples t-test. RESULTS. For control groups, the flexural strength for PMMA (215.53 MPa) was significantly lower than for bis-acryl composite resin (240.09 MPa). Glass fiber reinforcement produced significantly higher flexural strength for both PMMA (267.01 MPa) and bis-acryl composite resin (305.65 MPa), but the polyethylene fibers showed no significant difference (PMMA resin-218.55 MPa and bis-acryl composite resin-241.66 MPa). Among the reinforced groups, silane impregnated glass fibers showed highest flexural strength for bis-acryl composite resin (305.65 MPa). CONCLUSION. Of two fiber reinforcement methods evaluated, glass fiber reinforcement for the PMMA resin and bis-acryl composite resin materials produced highest flexural strength. Clinical implications. On the basis of this in-vitro study, the use of glass and polyethylene fibers may be an effective way to reinforce provisional restorative resins. When esthetics and space are of concern, glass fiber seems to be the most appropriate method for reinforcing provisional restorative resins.

Comparative finite element analysis of mandibular posterior single zirconia and titanium implants: a 3-dimensional finite element analysis

  • PURPOSE. Zirconia has exceptional biocompatibility and good mechanical properties in clinical situations. However, finite element analysis (FEA) studies on the biomechanical stability of two-piece zirconia implant systems are limited. Therefore, the aim of this study was to compare the biomechanical properties of the two-piece zirconia and titanium implants using FEA. MATERIALS AND METHODS. Two groups of finite element (FE) models, the zirconia (Zircon) and titanium (Titan) models, were generated for the exam. Oblique (175 N) and vertical (175 N) loads were applied to the FE model generated for FEA simulation, and the stress levels and distributions were investigated. RESULTS. In oblique loading, von Mises stress values were the highest in the abutment of the Zircon model. The von Mises stress values of the Titan model for the abutment screw and implant fixture were slightly higher than those of the Zircon model. Minimum principal stress in the cortical bone was higher in the Titan model than Zircon model under oblique and vertical loading. Under both vertical and oblique loads, stress concentrations in the implant components and bone occurred in the same area. Because the material itself has high stiffness and elastic modulus, the Zircon model exhibited a higher von Mises stress value in the abutments than the Titan model, but at a level lower than the fracture strength of the material. CONCLUSION. Owing to the good esthetics and stress controllability of the Zircon model, it can be considered for clinical use.
