A CLINICAL CONSIDERATION ON THE TEETH TRANSPOSITIONS (치아전위의 임상적 고찰)

  • Tooth transposition is the phenomenon in which two teeth in the dental arch are exchanged. The tooth most frequently involved in transposition is the permanent maxillary canine, especially with the first premolar. The etiology of transposition is still an enigma. Interchange of tooth germs, migration during eruption, genetic factor, local pathologic factors may be suggested as possible etiologic factors. There are three main options for treatment, which are the extraction of one of the transposed teeth, the alignment in the transposed position, the orthodontic movement of the teeth to their correct positions. If complete transposition occurred especially in mandible, alignment of teeth in the transposed position followed by occlusal reduction and esthetic restoration is usually recommended. The presented two-case report will describe the maxillary canine to the first premolar and to lateral incisor transposition with associated peg lateralis and retained primary canines. Although alignment the transposed teeth in original position is ideal treatment, it companies many complication as root resorption, alveolar bone destruction, gingival deheisence, etc. Therefore the treatment procedures in these case was relative cost-benefit effective method to both clinician and patients.

Immediate implant placement in areas of aesthetic priority (상악 전치부에서 단일 치아 발거 후 즉시 임플란트 식립)

  • Implants placed immediately after tooth extraction have been shown to be a successfully predictable treatment modality. Several clinical papers suggest that placing implants immediately after tooth extraction may provide some advantages: reduction of the number of surgical procedures or patient visits, preservation of the dimensions of alveolar ridge, and shortening of the interval between the removal of the tooth and the insertion of the implant supported restoration. In this case report, three patients received single immediate implant placements to replace a maxillary anterior tooth at the time of extraction. As the three cases were somewhat different, treatment protocols had to be modified as follows: Case 1. Immediate implant placement with healing abutment connection. Case 2. Immediate implant placement with immediate provisionalization. Case 3. Immediate implant placement with Guided Bone Regeneration(GBR). Every implant of these cases was placed in proper position buccolingually, mesiodistally and apicocoronally, The procedures following implantation such as immediate provisionalization and GBR were free of problem. Healing of each case was uneventful. In all cases, treatment outcomes were mostly satisfactory and the results maintained during follow-up periods. However, one case (Case 3) showed some papilla loss due to failure in delicate soft tissue handling during surgery. This papilla loss was compromised by prosthetic means. In conclusion, immediate implant placement in the fresh extraction socket can be a valid and successful option of treatment in aesthetic area. Moreover, this treatment protocol seems to maintain the preexisting architecture of soft and hard tissues in most cases.

The effect of tooth brushing and thermal cycling on a luster change of ceromers finished with different methods

  • Statement of problem. Luster loss in esthetic anterior ceromer restoration can occur and can be related with rough surface texture. Understanding durability of surface finishing methods like polishing and surface coating have critical importance. Purpose. This study evaluated the effect of tooth brushing and thermal cycling on surface luster of 3 ceromer systems (Artglass, Targis, Sculpture) treated with different surface finishing methods. Material and methods. Seventy-two samples were prepared: 12 for control group Z100, 12 for Artglass, 24 for Targis, and 24 for Sculpture. Half of the Targis and Sculpture were polished according to the manufacturer's recommendation. The rest of the samples were coated with staining and glazing solution for Targis and Sculpture, respectively. All specimens were subjected to 10,000 cycles between $5^{\circ}C\;and\;55^{\circ}C$ with 30 seconds dwell time. Tooth brushing abrasion tests were performed in a customized tooth brushing machine with 500g back and forth for 20,000 cycle. Luster comparisons were based on grading after direct observation, and light reflection area was measured with Image analysis software. Results. All materials showed an decrease in luster grade after thermal cycling and tooth brushing. The post-tooth brushing results revealed that the glazed Sculpture had greater mean luster grade than did any other groups. While, the stained Targis group showed greatest changes after tooth brushing (p < 0.05), polished Targis and Sculpture did not show significant changes. However, glazed Sculpture showed discretely fallen out glaze resin. Conclusion. From the results of this study, all of the ceromer specimens were much glossy than control composite group after tooth brushing. coatings used for Targis and Sculpture had not durability for long term use.

COLOR COMPARISON OF VARIOUS POST SYSTEMS WITH EMPRESS 2 CROWNS (포스트의 종류에 따른 Empress 2 도재관의 색상에 관한 비교연구)

  • Development of new ceramics and esthetic needs of patients increase the use of all ceramic restorations. Fractured teeth often need metal post and core as foundation for final restoration. When all ceramic restorations are planned, metal post and core may lead to compromised esthetics because of opacity and gray color of metal post and core. Many techniques have been proposed to solve this problem such as application of an opaque porcelain to the metal core or all ceramic post and core. This study was performed to evaluate effect on color of Empress 2 crown according to various post systems. Color was evaluated by the CIE $L^*a^*b^*$ systems and measured by spectrophotometer (Model CM-3500, Minolta, Japan). Specimens were divided into 4 groups as follows Group 1 : gold cast post specimen + Empress 2 crown specimen Group 2 : application of an opaque porcelain to gold cast post specimen + Empress 2 crown specimen Group 3 : cosmopost specimen + Empress 2 crown specimen Group 4 : In-ceram post specimen + Empress 2 crown specimen. The results obtained as follows, 1. $L^*$ Values showed that group 2, 3, 4 were higher than group 1 with significant difference and group 2, 3, 4 were not significantly different. 2. $a^*$ Values and $b^*$ values also showed that group 2. 3, 4 were higher than group 1 with significant difference and group 2, 3, 4 were not significantly different. 3. ${\Delta}E^* ab$ Values showed that ${\Delta}E^* ab$ 3 was the highest value and ${\Delta}E^* ab$ 5 was the lowest value.

Correction of anterior ridge defect for conventional prosthesis (보철 치료시 전치부 치조제 결손부의 처치)

  • Purpose: Anterior ridge defect after tooth extraction results in unfavorable appearance. Ridge augmentation procedures should be preceded by careful surgical-prosthetic treatment planning, and various techniques can be used in anterior ridge augmentation. Materials and Methods: Three patients showed deformed ridges after tooth extraction. Three different techniques ; onlay-interpositional connective tissue graft; bovine hydroxyapatite graft with free connective tissue graft; bovine hydroxyapatite graft with resorbable collagen membrane following free connective tissue graft; were used for anterior ridge augmentation. Result: Soft tissue graft can be used in small amount of ridge defect, hard tissue graft combined with soft tissue graft can be used in large amount of ridge defect. After ridge augmentation, about three months of healing period, augmented tissue was stabilized. The final restoration was initiated after this healing period, and the tissue form was maintained stable. Conclusion: Careful diagnosis and surgical-prosthetic treatment planning with joint consultation prior to surgery should be performed in order to attain an optimal esthetic results.

Full mouth rehabilitation of severely worn dentition with implants and removable partial dentures (심한 마모를 가진 환자에서 임플란트와 가철성 국소의치를 이용한 전악수복 증례)

  • Excessive tooth wear can lead to decrease in occlusal vertical dimension and can cause pathological changes in the oral environment and masticatory system. When recovering occlusal vertical dimension and occlusion, accurate diagnosis and analysis are essential. This clinical case describes a 75-year-old woman with severely worn dentition due to loss of the posterior support. Full mouth rehabilitation with occlusal vertical dimension increment was planned. Clinical and radiographic examinations, occlusal vertical dimension evaluation, and diagnostic wax-up were performed and patient adaptability was evaluated using provisional restorations. As for definitive restoration, considering economic condition of the patient, removable partial denture was fabricated and solitary implants were placed in the mandibular left and right posterior region to increase support and retention of the removable partial denture. During one year of follow-up, functional and esthetic outcomes were observed satisfactory.

RESTORATION OF MISSED MAXILLARY CENTRAL INCISOR USING AUTOTRANSPLANTATION OF MESIODENS : A CASE REPORT (정중과잉치의 자가이식을 통한 상실된 상악 중절치의 수복 : 증례보고)

  • Supernumerary tooth is developed from overgrowth of dental laminar and is mainly observed in the maxillary anterior area, named 'mesiodens. Mesiodens cause many problems such as malposition of permanent anterior tooth, diastema, formation of cyst, and eruption to oral or nasal cavity. Extraction, therefore, is generally recommended choice of treatment. This case report describes that the patient is a 9 years and 8 months old boy, whose unilateral maxillary incisor was missing due to trauma, and he had a mesiodens. We practiced autotransplantation of the mesiodens to the missing maxillary central incisor area and then restored with prosthetics, substantially kept out alveolar bony resorption and provided esthetic appearance. Mesiodens was too small to be used as an abutment because its length is totally 14mm, so that poor prognosis was anticipated due to bad crown/root ratio. However, we observed clinically acceptable result during the observation period of 8 months after operation.

Immediate implant placement for schizophrenic patient with outpatient general anesthesia

  • The difficult oral healthcare in intellectually disabled patients with poor behavioral control has led to debate over the cost-effectiveness and validity of implant treatment in these patients. The patient in the present report had schizophrenia that had led to poor oral care and severe dental caries in the full mouth. Tooth extraction and a removable prosthesis were planned, but the guardian wanted an implant procedure. Since the guardian showed strong will and cooperation with regard to the patient's oral healthcare, extraction followed by immediate implant placement was performed across two rounds of general anesthesia. Since the outcome appears successful, we present this case report. Immediate implant placement after tooth extraction requires fewer surgeries and rounds of general anesthesia, reduces horizontal bone resorption, and can achieve better esthetic results. Therefore, as long as a certain degree of oral care is possible, this can be a positive option for restoration of a partially edentulous mouth, even in intellectually disabled patients.

Repair of the Cleft Lip using Triangular Cheiloplasty-A Case Report (삼각피판법을 이용한 편측 불완전 구순열 환자의 구순 교정 수술-증례 보고)

  • After Tennison introduced a triangular flap method which, for the first time, preserved the Cupid's bow, Randall gave this method a sound mathematical basis. This method is also called as an inferior triangular cheiloplasty which is characterized by making a small triangular flap from the lateral border of cleft destined to be fitted into an incision on the medial side of cleft. He postulated that the height obtained was equal to the sum of the median of the two triangles used in the cheiloplasty. Using this technique, a 22 month-old male patient with incomplete unilateral cleft lip was corrected primarily. The deviation of the columella and flattening of the nostril on the cleft side were minimal. The operation was done under general anesthesia and patient was healed uneventfully. We tried to improve the symmetry and esthetic feature of philtrum, nostril sill, alar-facial groove, preventing the notch formation on the nostril floor, and to reconstruct the muscle sling in the upper part of lip. The shape of Cupid's bow was restored, and the symmetry of columella was regained as a result. In summary, the inferior triangular cheiloplasty is effective to correct the primary unilateral cleft lip, results in the restoration of favorable anatomy and function.

Effects of staining liquids and finishing methods on translucency of a hybrid ceramic material having two different translucency levels

  • PURPOSE. Beverages may affect the translucency of esthetic dental restorative materials.The aim of the present study was to investigate the effects of coffee and red wine on the translucency of a PICN material with two translucency levels, and finished with different methods. MATERIALS AND METHODS. 2M2 high translucent and translucent VITA Enamic hybrid ceramic blocks were investigated. Rectangular specimens with the dimensions of $12mm{\times}14mm{\times}2mm$ were prepared. The specimens were finished and polished with different methods as suggested by the manufacturer. The translucency parameters of the specimens were evaluated before and after 24 hours, 7 days, and 28 days immersion in distilled water, coffee and red wine. Translucency parameters were measured using a portable spectrophotometer. RESULTS. At the end of 28 days, there was no statistically significant difference between the groups of specimens kept in coffee (P>.05). In the red wine groups, there was a statistically significant difference between the control group and all other groups (P<.05) at the end of 28 days. CONCLUSION. The translucency of hybrid ceramic for a restoration may not be important regarding the effects of coffee on translucency change because the specimens with different translucencies and finishing methods that were immersed to coffee had similar translucency parameters at the end of 28 days. The translucency of hybrid ceramic may be important in the case of red wine, however, since the results showed that highly translucent specimens exposed to red wine demonstrated better translucency parameters than specimens made from translucent blocks at the end of 28 days.
