Response to Bee Venom Acupuncture and Polymorphism of Interleukin 4 Receptor and Interleukin 10 in Rheumatoid Arthritis

  • Objectives : We investigated to find the relationship between single-nucleotide polymorphism (SNP) of IL4R, IL-10 and bee venom therapy efficacy in patients with RA treated with bee venom for 8 weeks. Methods : Korean RA patients (n=114) and healthy subjects (n=109) were included in this prospective study. Korean bee venom was dissolved in saline (diluted 1:3000) and administrated into acupuncture points. Bee venom therapy was applied twice a week and continued for 8 weeks. The clinical response was evaluated using various assessments before and after treatment. Disease severity was measured by determining the number of tender joints and swollen joints. Laboratory studies included ESR, CRP, and rheumatoid factor. Genotyping for IL-4R and IL-10 polymorphism was done by pyrosequencing analysis. Results : 1. In IL4R genotypes, there was significant difference between RA ptitients tind controls group. 2. In IL4R genotypes, there was significant difference among Good, Mild and Bad responders to in RA patients, but in the frequency of alleles and carriers, there were no significant difference. 3. There was no significant difference between RA patients and controls group in IL-10 gene genotypes. 4. In IL-10 genotypes, there was no significant difference among Good, Mild and Bad responders to in RA patients. 5. There was no significant difference in the improvement of ESR, CRP and KHAQ scores after bee venom therapy in RA patients among the IL4R or IL-10 genotypes. Conclusions : In IL-4R genotypes, there was significant difference between RA patients and control group, and among Good, Mild and Bad responders in RA patients. However, in IL-10 genotypes, there was no significant difference between RA patients and controls group and among Good, Mild and Bad responders in RA patients.

Effect of Irradiation Dose and Storage Time on the Free Radical Concentrations in Gamma-reradiated Dried Seasoning Powder

  • Electron spin resonance (ESR) spectroscopy was used to investigate the effect of irradiation dose and storage time on the free radical concentrations in gamma-irradiated dried seasoning powder. Seasoning powders of dried squid flavor, shrimp flavors, kimchi flavor, spicy beef soup flavor and soy sauce flavor were irradiated with doses of 0, 1, 3, 5, 7 and 9 kGy at room temperature using a Co-60 irradiator. Characteristic signals of free radicals were observed in all the irradiated samples of this experimental while these signals were not detected in non-irradiated samples. Since the free radical concentrations linearly increased with the applied doses (1~9 kGy), highly positive correlation coefficients ($R^2$ = 0.9285~0.9965) were obtained between irradiation doses and free radical concentrations during all the storage times. Free radical concentrations of the irradiated flavored seasoning powder did not change even at 16 weeks of storage at room temperature, while those of dried squid, shrimp and spicy beef soup flavors decreased until 2 weeks of storage after irradiation with 5 kGy or over, and these of soy cause flavors slowly decreased until 8 weeks of storage after irradiation with 3 kGy or over. Although the free radical concentrations decreased with storage times, the characteristic signals of the irradiated seasoning powders of dried squid, shrimp, spicy beef and soy sauce flavors were observed even after 16 weeks of storage at room temperature.

  • Objectives: Rheumatoid arthritis (RA) is a chronic autoimmune disease, principally characterized by synovial inflammation of the joints. We previously reported the effect of acupuncture for RA, but the mechanism is still unclear. Various factors such as oxidative stress and angiogenesis were involved in the pathogenesis of RA, and recently, it has also been reported that cytokines also play a major role in RA. Thus, we investigated whether acupuncture could induce any changes in the levels of cytokines including vascular endothelial growth factor (VEGF), angiogenin and epidermal growth factor (EGF) as well as erythrocyte sedimentation rate (ESR), c-reactive protein (CRP), and rheumatoid factor (RF) in the sera of RA patients. Methods: The forty three patients who met the American College of Rheumatology (ACR) criteria for RA recruited. The acupuncture group (n=21) underwent 14 sessions of partially individualized acupuncture treatment for 6 weeks, and the control group had no treatment (n=13) over the same periods. We evaluated ESR, CRP and RF. In addition, to find out the mechanism of acupuncture, we assessed the changes of the cytokine activities using protein cytokine array in the sera of the patients. Results: Acupuncture significantly decreased the levels of ESR and CRP, but RF were not changed after 6-week acupuncture treatments. Moreover, acupuncture reduced the levels of VEGF, angiogenin and EGF in the sera of the patients. Interestingly, they were related with angiogenesis, which is an important process in the pathogenesis of RA. The levels of oncostatin, interleukin(IL)-$1{\alpha}$, IL-8, leptin, monocyte chemotactic protein-1, macrophage-derived chemokine, macrophage inflammatory proteins-1, platelet-derived growth factor BB and RANTES were not changed significantly. Conclusions: The effect of acupuncture for reliving RA symptoms can be partially explained by inhibition of angiogenesis factors in the sera of the RA patients.

  • Two synthetic routes for the 1,4-diboracyclohexene-2 ring 8 have been developed. Method i) starts with 1,2-bis(dichloroaluminyl)ethane, in which the AlCl$_2$ group is replaced by BCl$_2$. Exchange of the chlorine with BI$_3$ in 1,2-bis-(dichloroboryl)ethane yields the corresponding iodo compound, which reacts with the alkynes to heterocycles 8a, b in good yield. In method ii) B$_2$Cl$_4$ is added to alkenes, replacement of chlorine with BI$_3$ yields the bis(diiodoboryl)ethane derivatives which undergo redox reactions with alkynes to give 8c, d. The diiodo derivative 8a forms the pyridine adduct 9a, and reacts with ether to give the ethoxy derivative 8f. 8a-d react with AlMe$_3$ to yield the corresponding dimethyl derivatives 8g-j, which give unstable radical anions when treated with potassium in THF. The ESR parameters are reported. In electrochemical experiments irreversible reductions of 8g-j are observed. 8g-j react with (C$_5$H$_5$)Co(C$_2$H$_4$)$_2$ to give the intermediate 16 VE complexes (C$_5$H$_5$)Co(8), in which C-H activation occurs with formation of the corresponding red 1,4-diboracyclohexadiene complexes 10. The X-ray structure analyses of 10h and 10j are reported.

  • The optical and the electrical properties of octadecylviologen-(TCNQ$\^$-/)$_2$LB(Langmuir-Blodgett) films have been studied. The films were made at 25mN/m with Y-type formation using kuhn type apparatus. According to a study of UV/visible absorption spectra, DS C, ESR, and I-V characteristics depending on temperatures, there is an internal change in OV-(TCNQ$\^$-/)$_2$ LB films near 350K. We have observed a status of Langmuir films using Brewster angle microscope at room temperature.

Effects of Body Weight Control Program Contains Exercise and Auricular Acupuncture on Body Composition, Blood Composition and Physical Ability in Obese Children (운동과 이침요범을 병행한 체중조절 프로그램이 비만아동의 체성분, 혈액구성 및 체력에 미치는 영향)

  • Objectives: The purpose of this study was to investigate the changes of obesity, blood composition and physical ability in obese children on body weight control program. Methods : The body weight control program included exercise, nutrition education and auricular acupuncture was performed 4 times a week during 12 weeks. 32 obese children whose body fat ratio were over 30% participated in this study. Measuring materials were body composition(weight, muscle mass, fat mass, fat ratio, BMI), physical measurement(height, waist circumference, chest circumference, hip circumference), physical ability(grip strength, sit-ups, sitting trunk flexion, sit-down stand-up test, standing long jump, opened eyes foot balance, side step), blood composition(WBC, Hb, ESR, Total Cholesterol, TG, GOT, GPT, BUN, Creatinine, Glucose) and survey(changes of eating habits, changes of living habits, knowledge of obesity, self-confidence, self-esteem). Results : From the results, positive changes were made in BMI, height, waist circumferences, chest circumferences, hip circumferences, sit-ups, sitting trunk flexion, sit-down stand-up test, standing long jump, opened eyes foot balance, side step, Hb, ESR, GOT, GPT and changes of eating habits through the body weight control program. Conclusions : This results suggest that body weight control program including exercise, nutrition education and auricular acupuncture may be effective for helping obese children.

  • Objectives : The Purpose of this study is to investigate the clinical application of conservative Korean treatment including hip joint MST for Bilateral Sacroillitis with Synovitis in Rt. Hip Joint Caused by Ankylosing Spondylitis. Methods : Patient is hospitalized at Dept. of Korean Internal Medicine, Jaseng Korean Medicine Hospital, diagnosed as Bilateral Sacroilitis with Synovitis in Rt. Hip Joint Caused by Ankylosing Spondylitis, and treated by herbal medicine, acupuncture, moxibustion, and hip joint MST. This study was measured by NRS(Numeric Rating Scale), SLRT(Straight leg raising test), Patrick test, and Blood test(ESR, CRP). Result : After conservative treatment, the patient's pain was controlled and NRS score was decreased. SLRT score also improved. Patrick test unchanged. ESR, CRP score was decreased. Conclusion : As seen in this one case, conservative Korean treatment including hip joint MST has a positive effect to control pain with Bilateral Sacroillitis with Synovitis in Rt. Hip Joint Caused by Ankylosing Spondylitis.

  • For the first time we have synthesized thiabenzene radical by the reaction of thiopyrylium cation with alkali metals. As might be expected for a free radical, ESR-spectrum of 2,4,6-triphenylthiabenzene radical shows the single signal with g-value of 2.0045. The proton signal of 2,4,6-triphenylthiabenzene radical in nmr spectrum shifts to the higher field than that of 2,4,6-triphenylthiopyrylium cation by ca. 0.5 ppm.   • We synthesized the binuclear Tetradentate Schiff base nickel(II) and copper(II) complexes ; [Ni(II)$_2$(SMPO)$_2$(L)$_2$], [Ni(II)$_2$(SPPD)$_2$(L)$_2$] and [Cu(II)$_2$(SMPD)$_2$] and [Cu(II)$_2$(SPPD)$_2$] (where, L : Py, DMSO and DMF). We identified the structure of these complexes by elemental analysis, IR-spectrum, T.G.A, D.S.C and ESR measurements. According to the results of cyclic voltammetry and DPP measurements in aprotic solvent included 0.1M TEAP as supporting electrolyte, we knew that diffusional controlled redox process of one step with one electron was irreversible process in 0.1M TEAP-Py solution. Also it was reversible or quasi reversible process in 0.1M TEAP-DMSO solution and reversible or E.C reaction mechanism in 0.1M TEAP-DMF solution at mononuclear complexes ; [Cu(II)(SOPD)] and [Ni(II)(SOPD)(L)$_2$]. But, we knew that diffusional controlled redox process of two step for one electron of binuclear complexes was as follows. The values of redox potential for dimeric complexes in 0.1M TEAP-L solution (where, L ; Py, DMSO and DMF) with scan rate 100mV/sec.

Serum levels of interleukin-6 and interleukin-8 as diagnostic markers of acute pyelonephritis in children

  • Purpose: Early diagnosis and treatment of acute pyelonephritis in children is of special importance in order to prevent serious complications. This study was conducted to determine the diagnostic value of serum interleukin (IL)-6 and IL-8 in children with acute pyelonephritis. Methods: Eighty-seven patients between 1 month to 12 years old with urinary tract infection (UTI) were divided into 2 groups based on the result of 99m-technetium dimercapto-succinic acid renal scan: acute pyelonephritis (n=37) and lower UTI (n=50) groups. White blood cell (WBC) count, neutrophil (Neutl) count, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) concentration, platelet count, and serum IL-6 and IL-8 concentrations of both groups were measured and compared. Results: There was a significant difference between two groups regarding WBC count, Neutl count, ESR, and CRP concentration (P<0.05). In addition, the difference between the two groups regarding serum IL-6 and IL-8 concentrations was not significant (IL-6, 60 and 35.4 pg/mL and IL-8, 404 and 617 pg/mL, respectively). The sensitivity and specificity of serum IL-6 and IL-8 for diagnosis of acute pyelonephritis were 73%, 42% and 78%, 32%, respectively. Sensitivity, specificity, negative and positive predictive values of serum IL-6 and IL-8 were less than those of acute phase serum reactants such as CRP. Conclusion: This study showed that there was no significant difference between acute pyelonephritis and lower UTI groups regarding serum IL-6 and IL-8 levels. Therefore, despite confirming results of previous studies, it seems that IL-6 and IL-8 are not suitable markers for differentiating between acute pyelonephritis and lower UTI.
