Killian-Jamieson 게실의 수술적 치료 - 1예 보고 - (Surgical Experience with Killian-Jamieson Diverticulum - A case report -)

  • Killian-Jamieson 게실은 경부 식도에 발생하는 드문 질환이며 Zenker 게실과는 발생 기전과 부위가 다른 것으로 알려져 있다. Killian-Jamieson 게실의 치료원칙에 대해서는 정립된 바가 없다. Killian- Jamieson 게실 환자에서 경부 절개 하에 수술적 치료를 시행하여 이를 보고하고자 한다.

Risk factors predicting development of complications in 72 dogs with esophageal foreign bodies

  • The aim of this study was to determine predictive risk factors implicated in complications in dogs with esophageal foreign bodies. Medical records of 72 dogs diagnosed with esophageal foreign bodies by endoscopy were reviewed retrospectively. Factors analyzed included age; breed; gender; body weight, location, dimension, and type of foreign body; and duration of impaction. To identify risk factors associated with complications after foreign body ingestion, categorical variables were analyzed using the chisquare or Fisher's exact tests and multivariate analysis, as appropriate. Complications secondary to esophageal foreign body ingestion included megaesophagus, esophagitis, perforation, laceration, diverticulum, and pleuritis. Univariate analysis revealed that the location and duration of impaction after foreign body ingestion were associated with an increased risk of esophageal laceration and perforation. Multivariate analysis showed that age, duration of impaction, and foreign body dimension were significant independent risk factors associated with the development of complications in dogs with esophageal foreign bodies. In conclusion, these results showed that longer duration of impaction and larger foreign body dimensions may increase the risks of esophageal laceration, perforation, and plueritis in dogs.

성인의 선천성 기도-기관지루: 1례 보고 (Congenital Bronchoesophageal Fistula in Adult - Report of a case -)

  • We have experienced a case of 42-year-old woman with congenital broncho-esophageal fistula. The patient had productive coughing since childhood. A barium-swallowing examination showed a lower esophageal diverticulum communicating via a fistula with posterior basal segment of right lower lobe. Bronchography showed bronchiectasis in right middle and lower lobes. At thoracotomy resection of the diverticulum, bronchoesophageal fistula, and right middle and lower lobe of lung were performed. The postoperative course was uneventful.

식도기관지루를 합병한 식도게실 1례 보고 (Esophageal diverticulum complicated with esophagobronchial fistula-Report of one case-)

  • Acquired communicated diverticula between the esophagus and respiratory system are infrequent, and they are caused by carcinoma, trauma, infection, and traction. This report reviews the feature of benign esophagobronchial fistula due to midesophageal diverticulum. Patient is twenty year old man with excellent result by surgical intervention . The surgical procedures consist of divertuculectomy and superior segmentectomy of lower lobe of right lung. Clinically and radiologically, the patient is free from substernal distress, regurgitation, esophagorespiratory fistula, and esophageal stricture after surgical treatment.

식도게실의 외과적 치료 (Surgical Treatment of Esophageal Diverticulum)

  • Sixteen patients with esophageal diverticulum operated on between July 1979 and September 1988 at the Department of Thoracic Surgery of National University Hospital have been studied. There were 2 cases of the pharyngoesophageal diverticula, 12 cases of the midesophageal diverticula, and 2 cases of the epiphrenic diverticula. Twelve cases of midesophageal diverticula consisted of 9 cases of pulsion type and 3 cases of traction type. There were 13 women and three men, whose ages ranged from 25 to 65 years with an average age of 45.5 years. Diverticulectomy alone in three cases, diverticulopexy with myotomy in two cases, and diverticulectomy with myotomy in 11 cases were performed. There were no deaths or morbidity and all patients have achieved marked improvement of their symptoms except four patients who had a concomitant hypopharyngeal carcinoma, had a postoperative recurrence in epiphrenic diverticulum, and had two cases of postoperative transient regurgitation which subsided spontaneously one and one and half year later.

  • Killian-Jamieson diverticulum is a rare diverticular disease. This disease differs from Zenker's diverticulum in its location and mechanism. Various treatment modality have been attempted, but traditional surgical treatment has been recommended for a symptomatic Killian-Jamieson diverticulum due to the concern of possible nerve injury. We performed surgical treatment by cervical incision. We report here on a case of Killian-Jamieson diverticulum and we briefly review the relevant literature.

  • The first case was a 20 year old female who has been suffered from epigastric pain, and anorexia for 2 years. A thumb tip sized pulsion diverticulum 4cm above the esophagocardial junction was elicited by esophagogram and on exploration. A diverticulectomy with long esophagocardiomyotomy was performed. The second case was a 30 year old house wife who has had postprandial epigastric pain for 2 months accompained with frequent vomiting. Fiberscopy and esophagogram showed epiphrenic diverticulum of the esophagus. Same operative procedures were carried out and obtained a good result as first case. The third case was a 55 year old house wife who was admitted to this Chest Surgery Department because of regurgitation and intermittent vomiting for approximately 3 months. Esophagogram showed a large epiphrenic diverticulum of the esophagus. On exploration, a tennis ball sized pulsion diverticulum was found on the anterolateral wall of the esophagus. A partial esophagectomy including the diverticulum and esophagoesophagostomy was performed. The specimenshowed some erosive changes of the mucosal surface of the diverticulum and also the esophagus suggestive of diverticulitis and esophagitis. She has been satisfactory result until 4 months postoperatively, when she developed regurgitation and epigastric pain. Esophagogram showed stenosis of the operative site. Readmission and esophageal dilatations were done and improved without any problem. Epiphrenic or supradiaphragmatic diverticulum of the esophagus is a rare condition. Pathophysiologically, the conditions accompanied the spasm of the esophagus, many authors prefered the procedures of a diverticulectomy plus long esophagocardiomyotomy rather than simple diverticulectomy or esophagectomy and esophagoesophagostomy. Here we report the cases and reviewed the literatures.

부식성 식도협착 환자의 비개흉적 식도 적출술 - 2례 보고 - (Esophagectomy without thoracotomy in corrosive esophageal stricture - case report -)

  • The strictured esophagus is not removed usually in corrosive injury. But corrosion carcinoma and the late complications such as esophago-bronchial fistula, chronic mediastinal abscess, diverticulum can occur in case that strictured & inflamed esophagus be remained. Recently transhiatal esophagectomy is being done with low mortality and morbidity. So we are reporting 2 cases of esophageal stricture in young patients which were treated with transhiatal esophagectomy and esophagocologastrostomy. All of 2 cases were successfully treated and recovered. Postoperative esophageal function tests showed the mild reflux but it did not count clinically.

  • Killian-Jamieson and Zenker diverticula are both rare pharyngoesophageal diverticula. Both are outpouching of the mucosal and submucosal layers of the esophageal wall, which protrude through a mucosal gap at the level of the pharyngoesophageal esophagus. When these diverticula are large enough, they can be in proximity to the thyroid gland and may mimic a thyroid nodule. We report a case in which a diverticulum was filled with dietary residue and thus simulated a thyroid cyst on CT scan. And it was finally diagnosed as a Killian-Jamieson diverticulum by the surgery.

폐절제후 농흉에 합병한 견인성 중부식도게실의 치험례 (A Traction Diverticulum of the Esophagus Complicated with Empyema After Pneumonectomy)

  • This is a case report of surgically treated esophageal traction diverticulum which was resulted from postpneumonectomy empyema. In March, 1976, left lower lobectomy and thoracoplasty were performed at a hospital to treat long standing lung abscess, after operation it developed into empyema. One year later [April, 1977], We did decortication and left upper lobectomy[ultimate pneumonectomy], which was followed by empyema again, 3 months later it developed esophagopleurocutaneous fistula. Esophagograms bowed an adult thumb tip sized traction diverticulum in the midportion of the esophagus. Finally in January, 1978, after 6 months of gastrostomy feeding, fistulectomy and diverticulectomy were performed The funnel shaped diverticulum was in midesophagus and retracted by surrounding inflammatory scar tissue. Now the postoperative course was uneventful.
