개에서 식도 이물에 의한 식도 천공의 비침습적 치료를 통한 성공적 관리 증례 (A Successful Management of an Esophageal Perforation Caused by Esophageal Foreign Body with a Non-operative Treatment in a Dog)

  • 5년령 중성화하지 않은 암컷 포메라니언이 닭뼈를 삼킨 후 구토를 주증으로 내원하였다. 내시경을 통한 식도이물 제거 후 식도에 작은 천공을 확인하였다. 수술적 치료 대신 비수술적 관리를 통하여 치료하였고 3주 후 내시경을 통한 재검사 결과 천공부위의 완전한 폐쇄를 확인하였다. 본 연구는 식도 이물에 의한 식도 천공을 비수술적 관리를 통해 성공적으로 치료한 증례에 대한 보고이다.

식도이물;12년간 235례의 경험 (Esophageal Foreign Bodies : Experiences of 235 cases for 12 years)

  • We have clinico-statically analysed the 235 cases of esophageal foreign body during the period the past 12 years from March InS to December 1996 at the Department of Otolaryngology-Head and Neck Surgery, Korea University College of Medicine, Guro Hospital. The results were as follow : 1) Coin was the most frequent foreign body in the esophagus(172 cases : 73.2%). Bone was the next frequent foreign body (21 cases 8.8%). 2) In sex distribution, 124 cases(52.8%) of patients were the female and 111 cases(47.2%) were the male, so the sex ratio was 1:1.12. 3) In the age distribution, 134 cases(56.9%) were under 5 years of age, coin was the most frequent materials in this age group(111 cases 47.2%). 4) The most common symptom was vomiting(136 cases : 57.9%). The next common symptom was dysphagia and odynophagia was 41 cases(17.4%) 5) The most prevalent site of lodgement was the first narrowing(181 cases : 77.0%), the third narrowing(31 cases : 13.2%) and the second narrowing of esophagus(23 cases 9.8%) in order. 6) In duration of lodgement, 207 cases(88.1%) were lodged for one day. 7) The foreign bodies of esophagus were removed successfully by esophagoscopic procedure under local or general anesthesia. There were 3 cases of complication, one case of esophageal bleeding and two cases of esophageal rupture.

  • 이비인후과영역의 이물중 특히 소아에서 많이 보는 동전에 의한 식도이물은 임상경험을 통해 수 없이 많은 예를 접해 온 것이 사실이다. 시대의 변천, 생활의 다양화에 따라 이물의 종류도 다양해 지는 것을 경험할 수 있다. 저자들은 최근 15개월된 여아에서 식도의 제 1협착 부위에 저류된 뻐스토큰을 제거하였기에 주의를 환기시키는 의미에서 이를 보고하는 바이다.

PTP(Press-Through-Pack)에 의한 식도이물 3례 (Three Cases of A Press-Through-Pack in the Esophagus)

  • The PTP (Press-Through-Pack) has been widely used as a packing material for tablets and capsules. But esophageal foreign bodies attributed to the PTP may cause injury to esophageal mucous membrane, potentially inducing severe complications such as hemorrhage, perforation, etc. We report three cases of PTP foreign bodies in esophagus with reference to recent literature.

수술적 처치가 필요했던 식도이물에 관한 임상적 고찰 (A Surgical Treatment of the Esophageal Foreign Body (10 cases report))

  • 충남대학교병원에서 1980년7월부터 1995년 8월까지 경험한 식도이물 10례를 수술 치료하였다. 나이는 25 세에서 71세로, 평균 나이는 45.3세였고 남녀의 비는 6:4였다 가장 흔한 증상은 연하곤란, 발열, 이물감, 경부 동통이였다. 이물질은 생선뼈 3례였고, 거품 약포장지가 2례였고, 맥주병 마개,욕조 물마개, 깨진 사발 조각, 닭고기, 콩이 각각 1례였다. 진단은 수용성 식도 조영제와 식도 내시경을 이용하였다. 10례중 2례에서는 과거에 가성소다 섭취로 인해 식도 협착이 있었다. 1례에서는 정신과적 문제가 있었다. 식도이물은 모두 수술적 처치로 제거하였다 5례에서는 경부식도절게술을 시행하였고 1례는 우측 개흉술 을, 1례는 개복하여 위를 통해 위관을 역류성으로 통과 제거하였으며, 2례는 경부농양이 있어 절개후 배농하 였다. 3례는 수술 후 누출이 있었으나 수술치료없이 보존적 요법으로 호전되었으며,1례는 일시적인 애성이 있 었다. 1례는 외상성 기흉이 발생하여 폐쇄적 홉관 삽관술로 치료하였다. 수술 사망자는 없었다.

삼차원 재건 기술을 이용한 모의 이물 탐색 (Detection of Simulative Foreign Body Using three Dimensional Reconstruction Technique, Introduction and Application)

  • Background and Objectives Detailed information about the impacted esophageal foreign body is essential for safe extraction. Three dimensional reconstruction technique was applied to know shape, size and location of the simulative foreign bodies of stone, hyoid bone and endotracheal tube. Materials and Methods Submandibular gland stone, hyoid bone and endotracheal tube were used to simulate impacted foreign bodies. Axial CT, multi-planar reconstruction, volume of interest and virtual camera of Rapidia software were used to get information about the simulative foreign bodies from CT data. Shape and size were compared with the real materials. Exact locations were measured in appropriate modes of Rapidia. Results Shapes of the simulative foreign bodies matched well with the real materials. Size and location could be measured in various modes with some variable results. Conclusion 3D technique can be applied to get information about the simulative foreign bodies. This technique could be applied to the impacted esophageal foreign body.

위 절개술 접근과 긴 겸자를 이용한 식도 내 이물 제거 5례 (Gastrotomy Approach Retrieval of Esophageal Foreign Body using Long Forceps Technique in Five Dogs)

  • 이물 섭취 병력을 보이는 다섯 마리 개가 건국대학교 부속 동물병원과 한강 동물병원에 내원하였다. 신체 검사에서 무기력, 식욕 감퇴, 구토 증상을 보였으며 일반 방사선 사진에서 심장 앞쪽 또는 뒤쪽 흉부 식도에 이물이 걸려 있는 것을 확인하였다. 식도의 조영 촬영에서 큰 이물에 의한 식도 팽창을 확인 할 수 있었으며 흉강 내로의 조영제 누수는 관찰 되지 않았다. 위 절개술을 통한 식도 내 이물 제거를 긴 겸자를 이용하여 실시 하였다. 다섯 마리 모두에서 식도 내 이물을 성공적으로 제거하였다. 수술 후 10일 또는 2년에 정기 검진이 마무리 되었으며 수술과 관련한 구토, 연하 곤란, 식욕 저하, 과다 유연과 같은 합병 증상이 없음을 확인하였다.

Application of Three-dimensional Reconstruction in Esophageal Foreign Bodies

  • This study was conducted to investigate the clinical application of three-dimensional (3D) reconstructed computed tomography (CT) images in detecting and gaining information on esophageal foreign bodies (FBs). Two patients with esophageal FBs were enrolled for analysis. In both cases, 3D reconstructed images were compared with the FB that was removed according to the object shape, size, location, and orientation in the esophagus. The results indicate the usefulness of conversion of CT data to 3D images to help in diagnosis and treatment. Use of 3D images prior to treatment allows for rapid prototyping and surgery simulation.

개에서 발생한 소화기계 이물에 대한 회고분석 - 143 증례 (Retrospective Study of 143 Dogs with Alimentary Foreign Bodies)

  • This study was retrospectively investigated the clinical features, diagnostic imaging, treatment, and prognosis of foreign bodies according to alimentary site and type of foreign body, in 143 dogs. Among 28 breeds, Maltese, Shih Tzu, and terriers including the Yorkshire terrier were presented with high prevalence. Bone was the most commonly identified (33%) foreign body regardless of alimentary site. Approximately 76% of foreign bodies were in the stomach and small intestine. All esophageal foreign bodies were diagnosed on survey radiography, with the most common location being the caudal thoracic esophageal segment. Further diagnostic examinations including ultrasonography or contrast studies were additionally performed to detect radiolucent foreign bodies and perforation or leakage in the stomach and intestine. However, most seeds could be identified based on the characteristic features such as hyperdense thin double lines and inner gas on survey radiography. In conclusion, complications such as peritonitis and intestinal perforation were mainly observed in cases with seeds and linear foreign bodies. Especially, fabric foreign bodies could be induce peritonitis and re-perforation with high prevalence after surgical correction.

양성 식도질환의 외과적 요법 (Surgical treatment of benign esophageal disease)

  • A clinical analysis was performed on 49 cases of the benign esophageal diseases experienced at Department of Thoracic and Cardiovascular Surgery, Seoul National University Hospital during 7 year period from 1977 to 1983. Of 49 cases Of the benign esophageal diseases, there were 19 patients of esophageal stricture, 11 of achalasia, 6 of perforation, 3 of bronchoesophageal fistula, 3 of esophageal perforation, 3 of esophageal leiomyoma and one of esophageal foreign body. Twenty three patients were male and 26 were female. Ages ranged from 4 years to 74 years with the average age of 34.7 years. Of 19 patients of esophageal strictures, 7 patients were male and 12 were female and ages ranged from 6 years to 74 years with the average being 33.8 years. Causes of esophageal strictures were corrosive of esophageal strictures were dysphagia, vomiting, general weakness, weight loss and pain that order and developed on several different parts of esophagus. Operations were performed in 18 cases, of whom 7 patients were performed by esophagocologastrostomy, 4 gastrostomy, 4 esophagogastrostomy, 1 esophageal resection and esophagoesophagostomy, 1 esophagotomy and dilatation and 1 scar revision. Five patients had one or two complications; 2 anastomotic leakage, 1 wound infection, 1 localized empyema, 1 bilateral pneumothorax and 1 respiratory failure. One patient expired due to respiratory failure arising from aspiration pneumonia. The average age of achalasia patients was 33.1 years and symptom durations were from 2 months to 10 years with the average of 3.3 years. Main symptoms were dysphagia, vomiting, weight loss, pain and cough in that order. Modified Hellers myotomy was performed in 11 patients with one complication of restenosis. One patient was operated on by using longitudinal incision and transverse sutures with good result. Of 6 patient of esophageal diverticulum, 2 patients were traction diverticulum on the midesophagus, 2 were pulsion diverticulum on the midesophagus and 2 were pulsion diverticulum on the lower esophagus. Diverticulectomy was performed on 2 cases of traction diverticulum and esophagocardiomyotomy with or without diverticulectomy was erformed on 4 cases of pulsion diverticulum with good results. Of 5 patients of congenital bronchoesophageal fistula, the chief complaints were productive cough in 4 patients and hematemesis without respiratory symptoms in one patient. Two patients were operated on by using fistulectomy only and 3 by fistulectomy with pulmonary lobectomy. Of 3 patients of esophageal perforation, causes were foreign body ingestion, esophageal stricture after ECG and corrosive esophagitis. Two patient were operated on by using drainage and gastrostomy with symptomatic improvement but one patient died due to septic shock after thoracotomy. Three patients of esophageal leiomyoma were all male and 2 patients were operated on by using enucleation and one by distal esophagectomy with esophagogastrostomy. In one patient of esophageal foreign body, it was removed by esophagotomy through the right thoracotomy.
