Eruption Guidance of Horizontally Impacted Permanent First Molar with Primary Retention of Primary Second Molars: Case Reports (제2유구치의 일차성 만기잔존이 동반된 제1대구치 수평매복의 맹출유도 : 증례보고)

  • Tooth eruption involves a complex developmental process of tooth migration from the dental follicular origin to the final occlusion position in the oral cavity via the alveolar process. Disturbance of tooth eruption can occur at any point in a series of eruption stages; however, horizontal impaction of the mandibular first molar and primary retention of the primary mandibular second molar are rarely observed simultaneously. This study describes the treatment for two cases of horizontally impacted first molar with primary retention of primary molar. The primary retention of the primary mandibular second molar was extracted, and orthodontic traction was applied to the horizontally impacted primary mandibular first molar. Subsequently, displacement of the premolar tooth bud was improved and space regaining for eruption was achieved, guiding to normal eruption of the first molar.

The Development of Head Circumferences, Changes of the Eyes and the Eruption time of Deciduous Teeth in Shih-tzu Pups (Shih-tzu자견에서 머리둘레, 눈의 변화 및 유치의 맹출 시기의 변화상)

  • The purpose of this study was to document; the development of head circumferences, changes of the eyes, and the time of eruption of deciduous teeth in Shih-tzu dogs. In 12 pups of 4 Shih-tzu bitches, the development of head circumferences were estimated from whelping day (Day 0) to 41 days after birth, and the changes of the eyes and the time of eruption of deciduous teeth were estimated from whelping day to 45 days. The head circumferences were $11.24{\pm}0.67\;(mean{\pm}S.D)$ cm in Day 0 and $19.87{\pm}1.42cm$ in Day 41 and the daily increased length was $0.22{\pm}0.08cm$, respectively. The changes of the eyes wert: determined the first and the completely opening time of eyes. The first and completely opening time of the eyes in pups Day $10.75{\pm}0.97$ (range: $9.50{\sim}13.00$) and Day $13.17{\pm}1.27\;(11.50{\sim}15.33)$, respectively. In the time of deciduous teeth eruption in Shih-tzu, the first day of eruption was started in Day 35.90. The rank of deciduous teeth eruption was $C_1,\;C^1,\;P_3,\;I^3,\;I^2,\;P_4,\;I^1\;and\;P_2$, respectively.

Effect of slow forced eruption on the vertical levels of the interproximal bone and papilla and the width of the alveolar ridge

  • Objective: Forced eruption has been proposed for the reconstruction of deficient bone and soft tissue. The aim of this study was to examine the changes in the alveolar ridge width and the vertical levels of the interproximal bone and papilla following forced eruption. Methods: Patients whose hopeless maxillary anterior teeth were expected to undergo severe bone resorption and soft tissue recession upon extraction were recruited. In addition, patients whose maxillary anterior teeth required forced eruption for restoration due to tooth fracture or dental caries were included. Before and after forced eruption, the interproximal bone height was measured by radiographic analysis, and changes in the alveolar ridge width and the interproximal papilla height were measured with an acrylic stent. Results: This prospective study demonstrated that the levels of the interproximal alveolar bone and papilla were significantly increased by 1.36 mm and 1.09 mm, respectively, in the vertical direction. However, the alveolar ridge width was significantly reduced by an average of 0.67 mm in the buccolingual direction. The changes in the level of the interproximal alveolar bone and papilla were positively correlated. Conclusions: Although the levels of the interproximal bone and papilla were significantly increased, the alveolar ridge width was significantly decreased following forced eruption. There was a modest positive and significant correlation between the changes in the height of the interproximal alveolar bone and the papilla. Based on our findings, modification of vertical forced eruption should be considered when augmentation of the alveolar ridge width is required.

  • A cyst that develops in children's jaw occasionally disturbs the eruption of the succedaneous teeth. These teeth, however, usually have the potential of eruption. So, if the obstacles to eruption are eliminated, it is possible that the teeth erupt spontaneously to their normal position. In those cases, it usually requires the management of the cyst and the eruption guidance of the displaced teeth. Many surgical procedures have been described for the elimination of cysts. When the cyst is large and displaces the permanent teeth, marsupializaion is a surgical technique that may be preferred to enucleation in treatment of cysts. In marsupializaion, if the opening is maintained properly, it may be possible to manage the cyst and guide the displaced teeth into the normal position. In these cases, the cysts were managed with marsupialization in concomittent application of acrylic obturators, and as a result the displaced permanent teeth were guided into normal position. Even though the etiologic factors of the two cases are different, the treatment was the same. And both cases show that the potential for heal ing is remarkable with spontaneous relocation of displaced tooth, provided the opening is maintained during the eruption of the permanent tooth.

A Study on the Classification of the Stage of Root Development and Crown Eruption for Permanent Teeth (영구치의 치근발육과 맹출시기의 분류에 관한 연구)

  • This study was performed to investigate the age distribution with tooth calcification and degree of eruption of permanent teeth. For the study, healthy 184 patients from 5 to 19 years old without any previous serious dental treatment were randomly selected, and intraoral standard films and dental casts were taken for evaluation of stage of calcification and degree of eruption, respectively. Tooth calcification of 13 stages, designed by the author based on the Nolla's classification and eruption level of 4 or 5 degree was used. Data were processed by SAS/Stat program and the obtained results were as follows; 1. The age of root completed with open apex in lower posterior teeth were 13.8 years for first premolar, 14.0 years for second premolar, 10.5 years for first molar, and 14.2 years for second molar. There were no significant difference between right and left side. 2. As for the sequence of eruption, first molar was the first teeth erupted in upper arch, while central incisor was the first teeth in lower arch. In general, eruption of lower teeth were slightly earlier than the corresponding teeth of upper arch. 3. There were no difference of age of the same stage of development between Nolla's and the author's classification. From the results, the author's classification can be used for estimation of age with more finely in age of 8 to 15 years old. 4. Multiple regression equations for age with Nolla's(Ns) and the author's(Ks) classification of tooth calcification, and degree of eruption(DE) were as follow; Age(by #34) = 7.55 + 0.76Ks34 + 0.80DE34 - 0.72Ns34 Age(by #35) = 7.10 + 0.81Ks35 + 0.6IDE35 Age(by #37) = 6.61 + 0.82Ks37 + 0.5IDE37. Age(by #44) = 7.02 + 0.62Ks44 + 0.82DE44 Age(by #45) = 8.04 + 0.93Ks45 + 0.64DE45 - 0.89Ns45 Age(by #47) = 6.40 + 0.86Ks47 + 0.56DE47.

  • Modern volcanoes, Laoheishan and Huoshaoshan, have erupted during $1720\~1721$ in the Wudalianchi volcanic group, NE china. They comprise scoria and spatter cones that consist of potassium-rich phono-tephritic pyroclastic deposits and lavas, and include wide lava flow fields. The Laoheishan scoria cone is a polygenetic multiple volcano that overlaps earlier and later edifices with more complicated internal structures produced in greater scale and in earlier time than the Huoshaoshan. There is a funnel-shaped crater in the center of the later edifice of the Laoheishan scoria cone. The Huoshaoshan spatter cone is a monogenetic simple volcano with a central pit crater. The volcanic sequences indicate eruption processes that followed a repeated pattern that progressed through 5 stages of explosive and effusive eruption including lava fountains and Strombolian eruptions in the Laoheishan, and a recognizable pattern of 2 stages that started with Strombolian eruption and progressed through lava effusion in the Huoshaoshan.

  • Impaction of permanent incisor occurs rare than the canine & third molar. But it's often observed in school age child. The causes of impaction are trauma, space deficiency, mesiodens, infections of root apex, etc. In spite of elimination of cause, normal eruption of impacted tooth is rare. Though eruption is normal, the position of tooth will be incorrect. Because the impacted tooth results in malocclusion, root resorption of adjacent tooth, pathologic cystic change, it should be confirmed the precise position by clinical and radiographic exam and found the correct location by appropriate treatment plan. In case of pathologic change of impacted tooth and injury to adjacent tooth, it will be extracted. But through orthodontic retraction, the function and esthetics of tooth can be restored. It is important that impacted tooth should be detected early and diagnosed correctly, and appropriate treatment plan should be made. Before impacted tooth is retracted, the considerations of space for alignment and anchorage should be preceded and through appropriate force and mechanics, the side effects, for example, a root resorption should be minimized. In this study, we guided impacted tooth to normal position by using a forced eruption.

Cantilever-Type Traction Appliance for Mandibular First Permanent Molars with Eruption Disturbances (하악 제1대구치 맹출 장애에서 Cantilever-Type의 견인 장치를 활용한 맹출 유도)

  • Eruption disturbances of the mandibular first molars can cause multiple problems in occlusal development although they are extremely rare. Early diagnosis and treatment are very important to prevent complications associated with eruption disturbances. The present case report describes the treatment of two cases of eruption disturbances of the mandibular first permanent molar. A fixed appliance composed of a cantilever spring with mandibular second deciduous molar as an anchor tooth was used to tract the impacted mandibular first permanent molar. The success of both cases shows that this novel traction appliance can be used to induce the eruption of an impacted mandibular first permanent molar within a short time in patients with intact mandibular second deciduous molar as an anchor tooth.

Association between Developmental Anomalies of Permanent Lateral Incisors and Tooth Eruption Disturbances (영구 측절치 발육 이상과 치아 맹출 장애의 연관성)

  • The purpose of this study was to examine the prevalence of developmental anomalies of permanent lateral incisor and eruption disturbances and analyze the association between two components. Panoramic radiographs of 3984 patients (aged 5 to 13 years) who visited the Department of Pediatric Dentistry of Seoul National University Dental Hospital between November 2016 and October 2017 were screened. The prevalence of developmental anomalies of permanent lateral incisors was 10.2%. The most common developmental anomalies were congenitally missing teeth(66.1%), followed by peg lateralis(33.5%). The prevalence of eruption disturbances was 16.5%. Among the patient with developmental anomalies of permanent lateral incisors, associated eruption disturbances were appeared on 31.1% of patients. Peg lateralis(p < 0.001), underdeveloped lateral incisors(p < 0.001) and dens invaginatus(p = 0.004) were associated significantly with eruption disturbance of permanent teeth. Eruption disturbance in patients with peg lateralis and dens invaginatus was most prevalent in maxillary canine. For patients with underdeveloped lateral incisor, eruption disturbance of maxillary central incisor was most frequent. It is important for patients with developmental anomalies of lateral incisors to detect associated eruption disturbance early through regular checkup. Diagnosis and treatment plan in view of such relationships is important in order to treat appropriately at the optimal time.

Predictive Factors of Ectopic Eruption of the Maxillary First Permanent Molar (상악 제1대구치 이소 맹출의 예측 인자)

  • In order to provide a diagnostic basis for predicting the possibility of the self-correction of ectopic first permanent molars, differences among normal eruption, reversible and irreversible ectopic eruption of maxillary first permanent molars were retrospectively analyzed. The angles of the long axes and the occlusal lines between the maxillary first permanent molar and the adjacent tooth were measured by panoramic radiographs. The occlusal relationship of second primary molars was also investigated. There is a statistically significant difference between the ectopic eruption group and normal group (p < 0.05), but not between the reversible and irreversible ectopic eruption groups (p > 0.05). The angles between the second primary molar and the first permanent molar, the second primary molar and the second permanent molar in ectopic groups showed a smaller degree than those of the control group. Mesial step was found more frequently in the ectopic eruption group than the normal group. In conclusion, the angulation of the first permanent molar and tooth germ of the maxillary second permanent molar showed close relation with ectopic eruption of the maxillary first permanent molar and ectopic first permanent molar is likely to occur in class III patients with maxillary deficiency.
