This study was intended as an investigation of characteristics of background site atmospheric respirable particulate matters(RPM), and fine particles(<2.5 ${\mu}{\textrm}{m}$). The particle size distributions during the phenomenon of Yellow Sand(YS) occurs from April, 2001. Atmospheric aerosol particulate matter was directly collected on the Jeju island between 1 to 30, April, 2001 using an eight-stage cascade impacter(particle size range: 0.43-11 ${\mu}{\textrm}{m}$), and cyclone separator(cut size: 2.5, 10 ${\mu}{\textrm}{m}$). The episode of YS observed in background monitoring site, Kosan and appeared 2 times at sampling period. The mass concentrations of fine and coarse particles for YS episode were 34.2 and 59.6 $\mu\textrm{g}$/㎥, respectively, which were significantly increased amounts compared to 13.3 and 13.0 $\mu\textrm{g}$/㎥ for NonYS(NYS). Most size distributions had two peaks, one at 0.43∼.65 ${\mu}{\textrm}{m}$ and the other at 3.3${\mu}{\textrm}{m}$4.7 ${\mu}{\textrm}{m}$. The result of analysis of water-soluble ion component indicated that sulfate was mainly ion component, but nitrate and calcium ion was significantly increased at the YS episode.
Human Powered Vessel Festival (HPVF) hosted by the College of Engineering of Chungnam National University, the Society of Naval Architects of Korea and the Korea Ocean Research & Development Institute has been held annually in Daejon since 1999. Solar boat powered by solar energy appeared for the first time in 2006, and it is expected that more new teams will enter this competition in the coming years. In this paper it is described that design procedures and manufacture processes of the solar boat named EPISODE. Firstly, It is aimed to understand compositions of the electric system and their performance properties and to show the design procedures using commercial design packages line WAVIS, CATIA, HCAD etc. Through the trial test, we confirmed ship speed, maneuverability, durability and operation condition of the electric system and found some problems. We plans to make reported problems go away and install hydrofoils for more improved speed performance.
The accuracy of ozone sensitivity coefficients estimated with HDDM (High-order Decoupled Direct Method) can vary depending on the $NO_x$ (Nitrogen Oxides) and VOC (Volatile Organic Compound) conditions. In order to evaluate the applicability of HDDM over the Seoul Metropolitan Area (SMA) during a high ozone episode in 2007 June, we compare BFM (Brute Force Method) and HDDM in terms of the $1^{st}$-order ozone sensitivity coefficient to explain ozone change in response to changes in NOx and VOC emissions, and the $2^{nd}$-order ozone sensitivity coefficient to represent nonlinear response of ozone to the emission changes. BFM and HDDM estimate comparable ozone sensitivity coefficients, exhibiting similar spatial and temporal variations over the SMAduring the episode. NME (Normalized Mean Error) between BFM and HDDM for the episode average $1^{st}$- and $2^{nd}$-order ozone sensitivity coefficients to NOx and VOC emissions are less than 3% and 9%, respectively. For the daily comparison, NME for the $1^{st}$- and $2^{nd}$-order ozone sensitivity coefficients are less than 4% ($R^2$ > 0.96) and 15% ($R^2$ > 0.90), respectively. Under the emission conditions used in this study, two methods show negative episode average $1^{st}$-order ozone sensitivity coefficient to $NO_x$ emissions over the core SMA. The $2^{nd}$-order ozone sensitivity coefficient to $NO_x$ emissions leads ozone to respond muchnonlinear to the reduction in $NO_x$ emissions over Seoul. Nonlinear ozone response to reduction in VOC emissions is mitigated due to the $2^{nd}$-order ozone sensitivity coefficient which is much smaller than the $1^{st}$-order ozone sensitivity coefficient to the emissions in the magnitude.

Background: Acute pyelonephritis (APN) is one of the most common community-acquired bacterial infections. Recent increases of antimicrobial resistance in urinary pathogens might have changed the other epidemiologic characteristics of APN. The objective of this study was to describe the current epidemiology of APN in Korea, using the entire population. Methods: From the claims database of the Health Insurance Review and Assessment Service in Korea, the patients with International Classification of Diseases, 10th Revision codes N10 (acute tubulo-interstitial nephritis) or N12 (tubulo-interstitial nephritis, neither acute nor chronic) as the primary discharge diagnosis during 2010-2014 were analyzed, with two or more claims during a 14-day period considered as a single episode. Results: The annual incidence rate of APN per 10,000 persons was 39.1 and was on the increase year to year (35.6 in 2010; 36.7 in 2011; 38.9 in 2012; 40.1 in 2013; 43.8 in 2014, P = 0.004). The increasing trend was observed in both inpatients (P = 0.014) and outpatients (P = 0.004); in both men (P = 0.042) and women (P = 0.003); and those aged under 55 years (P = 0.014) and 55 years or higher (P = 0.003). Eleven times more women were diagnosed and treated with APN than men (men vs. women, 6.5 vs. 71.3), and one of every 4.1 patients was hospitalized (inpatients vs. outpatients, 9.6 vs. 29.4). The recurrence rate was 15.8%, and the median duration from a sporadic episode (i.e., no episode in the preceding 12 months) to the first recurrence was 44 days. The recurrence probability increased with the number of previous recurrences. The average medical cost per inpatient episode was USD 1,144, which was 12.9 times higher than that per outpatient episode (USD 89). Conclusion: The epidemiology of APN in Korea has been changing with an increasing incidence rate.

Background: Variation exists in the initial treatment for the first episode of primary spontaneous pneumothorax (PSP), and no definitive consensus exists due to a lack of high-quality evidence. This study examined the outcomes of needle aspiration and closed thoracostomy in first episodes of PSP requiring intervention. Methods: This study was a randomized, prospective, single-center trial conducted between December 2015 and August 2016. Patients of all ages with a documented first episode of PSP who were unilaterally affected, hemodynamically stable, and had a pneumothorax measuring over 25% in size were included. Patients with underlying lung disease, severe comorbidities, bilateral pneumothorax, tension pneumothorax, recurrent pneumothorax, traumatic pneumothorax, and pregnancy were excluded. Patients were randomly assigned to the needle aspiration or closed thoracostomy group using a random number table. Results: Forty patients with a first episode of PSP were recruited, and 21 and 19 patients were included in the needle aspiration group and the closed thoracostomy group, respectively. The hospital stay of each group was $2.1{\pm}1.8days$ and $5.4{\pm}3.6days$, respectively (p<0.01). However, no significant differences were found in the success rate of initial treatment or the 1-month and 1-year recurrence rates. Conclusion: Needle aspiration is a favorable initial treatment in patients experiencing a first episode of PSP.
Objectives: This study provides insights on the impact of a targeted intervention (TI) programme on behaviour change among injecting drug users (IDUs) in India. Methods: This paper examined the data from the Integrated Biological and Behavioural Surveillance 2014-2015 for IDUs in India. Logistic regression was performed to understand the factors (TI programme services) that affected injecting risk behaviours by adjusting for covariates. Propensity score matching was conducted to understand the impact of the TI programme on using new needles/syringes and sharing needles/syringes in the most recent injecting episode by accounting for the covariates that predicted receiving the intervention. Results: Participants who received new needles and syringes from peer educators or outreach workers were 1.3 times (adjusted odds ratio, 1.29; 95% confidence interval [CI], 1.09 to 1.53) more likely to use new needles/syringes during most recent injecting episode than participants who did not receive needles/syringes. The matched-samples estimate (i.e., average treatment effect on treated) of using new needles in the most recent injecting episode showed a 2.8% (95% CI, 0.0 to 5.6) increase in the use of new needles and a 6.5% (95% CI, -9.7 to -3.3) decrease in needle sharing in the most recent injecting episode in participants who received new needles/syringes. There was a 2.2% (95% CI, -3.8 to -0.6) decrease in needle sharing in the most recent injecting episode among participants who were referred to other services (integrated counselling and testing centre, detox centres, etc.). Conclusions: The TI programme proved to be effective for behaviour change among IDUs, as substantiated by the use of new needles/syringes and sharing of needles/syringes.
The costliness index (CI) is an index that is used in various ways to improve the quality of medical care and the management of appropriate treatment in medical institutions. However, the current calculation method for CI has a limitation in reflecting the actual medical cost of the patient unit because the outpatient and inpatient costs are evaluated separately. It is desirable to calculate the CI by integrating the medical cost into the episode unit. We developed an episode-based CI method using the episode classification system of the Centers for Medicare and Medicaid Services to the National Inpatient Sample data in Korea, which can integrate the admission and ambulatory care cost to episode unit. Additionally, we compared our new method with the previous method. In some episodes, the correlation between previous and episode-based CI was low, and the proportion of outpatient treatment costs in total cost and readmission rates are high. As a result of regression analysis, it is possible that the level of total medical costs of the patient unit in low volume medical institute and rural area has been underestimated. High proportion of outpatient treatment cost in total medical cost means that some medical institutions may have provided medical services in the ambulatory care that are ancillary to inpatient treatment. In addition, a high readmission rate indicates insufficient treatment service for inpatients, which means that previous CI may not accurately reflect actual patient-based treatment costs. Therefore, an integrated patient-unit classification system which can be used as a more effective CI indicator is needed.
This study was carried out to survey the high ozone episode of summertime in Busan. The selected day was July 18, 1999 and August 24, 2001 which recorded exceed to 12ppb/hr at 3 station in Busan simultaneously. In case July 18, 1999, thick cloud and variable wind made weak ozone concentration during morning hour. And increase of ozone concentration by revolution of mixed layer for morning hour did not occur in this case study day. Photochemical reaction by strong radiation after 1100LST made sharp increase rate of ozone concentration(50ppb/hr). In case August 24, 2001, the meteorological condition of this day was not general wind with gradient force, very clear day with less cloud amount, high insolation and sunshine. Dongsamdong, Beomcheondong, Daeyeondong, and Sinpyeongdong had double peak which twice maximum concentration in the early afternoon and late afternoon. Ozone concentration of this day was in inverse proportion to Nitrogen oxide strongly. Ozone concentration exceed to 60ppb/hr occurred at 1400LST, continued to 2300LST.
Hourly data of PM10 concentration collected from nine automatic air quality monitoring stations in Busan from 1999 to 2002 were analyzed to evaluate the spatio-temporal variation and meteorological characteristics of PM10 episodes in Busan metropolitan area. Mean concentrations ranged from 47$\mu\textrm{g}$/㎥ to 77$\mu\textrm{g}$/㎥. For most stations, mean seasonal hourly concentrations are lowest in summer and highest in spring. PM10 episode above daily mean standard(150$\mu\textrm{g}$/㎥) exhibited a maximum frequency at Gamjeondong and a minimum at Dongsamdong, and a maximum in March and a minimum in July and August. The diurnal variation of PM10 episode days is strongly influenced by traffic loads and meteorological conditions.
In this study, we analyzed the relationship between the time-variation trend of air pollution concentration index and the meteorological conditions with CO(carbon monoxide) concentration and meteorological observation data in high-CO episode days. CO is a representative automobile air pollutant. The results are as follows; 1. Most of the high-CO episode days within 30 classes appeared in winter season. 2. Most of them appeared under the surface weather conditions with east-west high-pressure system. The surface winds in this high-pressure area were very light. 3. The high-CO episode days were due to unusual accumulation within urban atmosphere in the morning. 4. The Atmospheric stabilities were more stable, and then the wind-ventilation conditions were worse than yearly mean atmospheric condition in Daegu.
