  • Two strains of genetically epilepsy-prone rats (GEPRs) have been derived from Sprague Dawley stock. One strain, known by the acronym GEPR-9, has a more pronounced epileptic condition than the other strain, known by the acronym GEPR-3. Only a small fraction of commercially available Sprague Dawley rats exhibits evidence of epilepsy. GEPRS are similar to most humans with epilepsy in that their general behaviors appear normal . GEPRS also share other traits with their non-epileptic counterparts. They are susceptible to forebrain and brainstem seizures produced by convulsant drugs and electrical currents. Because GEPRs and normal rats share these seizure non-epileptic brain rather than to an understanding of epilepsy. However, humans wi th epilepsy, the GEPR and other mammal inn models of genetic epilepsy are distinctive because they are characterized by seizure predisposition.

뇌파의 중첩 분할에 기반한 CNN 앙상블 모델을 이용한 뇌전증 발작 검출 (Epileptic Seizure Detection Using CNN Ensemble Models Based on Overlapping Segments of EEG Signals)

  • 뇌파(electroencephalogram, EEG)를 이용한 진단이 확대되면서 EEG 신호를 자동으로 분류하기 위한 다양한 연구가 활발히 이루어지고 있다. 본 논문은 일반인과 뇌전증 환자에게서 추출한 EEG 신호를 효과적으로 식별할 수 있는 CNN 모델을 제안한다. CNN의 학습에 필요한 데이터를 확장하기 위하여 EEG 신호를 낮은 차원의 신호로 분할하고, 이것을 다시 여러 개의 세그먼트로 중첩 분할하여 CNN 학습에 이용한다. 이와 더불어 CNN의 성능을 개선하기 위하여 CNN 앙상블 전략을 제안한다. 공개된 Bonn 데이터세트로 실험을 수행한 결과 뇌전증 발작을 99.0% 이상의 정확도로 검출하였고, 앙상블 방식에 의해 3-클래스와 5-클래스의 EEG 분류에서 정확도가 향상되었다.

식간 (食癎)으로 진단된 영아기 환아 1례에 대한 증례 보고 (A Case Report of Infant Diagnosed as Sik-Gan (食癎))

  • Objectives The purpose of this study is to report a case of one infant patient diagnosed as Sik-Gan (食癎) who was treated by Korean medical treatment. Methods We diagnosed an infant patient as Sik-Gan (食癎) and treated him with herbal medicine, acupuncture, moxa and chuna therapy while correcting his eating habit. To measure the degree of the patient's progress, the frequency and exact symptoms of seizure events, and gastrointestinal symptoms were recorded. Results Korean medical treatment reduced the patient's the frequency of seizure, and improved gastrointestinal symptoms. Conclusions A patient who has past medical history of epileptic seizures, unhealthy eating habits and gastrointestinal malfunctions is prone to have a specific form of seizure called the Sik-Gan (食癎). In this report, we have proven that variety of Korean medical treatment can considerably be effective in preventing recurring seizure events and improving the patients' gastrointestinal symptoms.

Potassium bromide를 적용한 개의 epilepsy 치료에 관한 증례 (Case Report of Potassium Bromide Therapy for Canine Epilepsy Patient)

  • Six year-old female shihtzu was presented as primary epileptic seizure. We had been treated with high dose phenobarbital sodium (6.5 mg/kg), but could not achieved adequate control seizures. In an add-on therapy, the dog was given potassium bromide at a dose of 20 to 40 mg/kg daily for a period of 24 months. This dog has showed a greater than 50% reduction in seizure frequency. The therapeutic range of serum concentration of potassium bromide was from 0.7 to 1.3 mg/ml at 6weeks after treatment. To begin an add-on therapy with potassium bromide it can be recommended at a daily dose of 20 to 40 mg/kg.

간질 치료에서 뇌파의 임상적 유용성에 관한 논란: 긍정과 부정적 관점에서 (Controversies in Usefulness of EEG for Clinical Decision in Epilepsy: Pros. and Cons.)

  • Electroencephalogram (EEG) is an indispensable tool for diagnosis of epilepsy and is the only assisting barometer of complete remission of epilepsy, which means prolonged, persistent suppression of cortical excitement in epileptic focus in addition to the clinical control of epileptic seizure. The specific morphologies or distribution of epileptic form discharges give us good information for the classification of seizure or epilepsy and epileptic syndromes, which consists of "Pros." in terms of diagnostic approach. In contrast, the EEG as a tool for long-term follow up might be limited due to the various clinical situation of each patient, which consists of "Cons." in terms of the usefulness of EEG for clinical decision. "Cons." aspect of EEG, which clinicians are more frequently coped with than those of "Pros", is an obstacle of utilization of follow up EEG in clinical practice. This is an overview about controversies in usefulness of EEG and the detailed aspects of "Pros." and "Cons." of EEG for clinical decision will be discussed following two articles. We tried to make consensus for the usefulness of EEG especially in the situation of "Cons." with plausible guideline.

Seizures in Patients with Brain Tumors

  • Objective : To determine the presentation, incidence, and risk factors of seizures in patients treated for brain tumors. Methods : One hundred patients who consecutively underwent a craniotomy for the treatment of supratentorial brain tumors were assessed. The pathologies of the patients enrolled in the study included glioma [n=56], meningioma [n=31], metastatic brain tumor [n=7], primary central nervous system lymphoma [n=4], and central neurocytoma [n=2]. Anti-epileptic drugs [AEDs] were administered to all patients for up to six months after the surgery. Pre-defined variables for outcome analysis included tumor grade and location, extent of tumor resection, number of seizures, age at tumor diagnosis, adjuvant therapy, medication and radiological abnormalities. Results : Thirty patients [30%] presented at least a single episode of seizure at the time of admission. Five of these patients [16.7%] developed the seizure during the follow-up period. Newly developed seizure was noticed in six out of seventy patients [8.6%] without prior seizure. Histopathology was malignant gliomas in 10 and supratentorial meningioma in one. Early seizure developed only in two patients. Conclusion : Compared with patients without seizure, patients with seizure at the time of admission showed younger age [p=0.003], a higher portion of low-grade glioma [p=0.001], tumor location in the frontal and temporal lobes [p=0.003] and cortical involvement [p=0.017]. Our study suggestes that tumor progression is considered a significant risk factor for seizure development in glioma patients.

Alteration in NCX-3 immunoreactivity within the gerbil hippocampus following spontaneous seizures

  • Although NCX-3 is highly expressed in the brain, the distribution of NCX-3 in the epileptic hippocampus is still controversial. Therefore, to assess the distribution and pattern of NCX-3 expression in epileptic hippocampus, we performed a comparative analysis of NCX-3 immunoreactivities in the hippocampus of seizure-resistant (SR) and seizure-sensitive (SS) gerbils. In SR gerbils, NCX-3 immunoreactivity was higher than pre-seizure SS gerbils, particularly in the pavalbumin (PV)-positive interneurons. Three h post-ictal, NCX-3 immunoreactivity in the SS gerbil hippocampus was markedly elevated to the level of SR gerbils. Six h post-ictal, the expression of NCX-3 was reduced to the level of pre-seizure SS gerbils. Therefore, the results of the present study suggest that down-regulation of NCX-3 expression in the SS gerbil hippo-campus may be involved in the hyperexcitability of SS gerbils due to an imbalance of intracellular $Na^+/Ca^{2+}$ homeostasis and $Ca^{2+}$ concentration.

동면돌입과 전간발작의 상호관련성에 관한 연구 (A Study on Correlation of Entry into Hibernation and Epileptic Fit in Hibernators)

  • 냉동생물학 연구분야 중 동면의 기전은 미래의 우주과학과 저온생물학이 풀어야 될 중요한 과제로서, 박쥐를 비롯한 동면동물을 사용하는 응용과학에서 가장 주목을 받아 오고 있다. 동면동물의 동면돌입 현상이 체온하강과 함께 뇌 전도상 전간발작 (epileptic seizure)을 유도시킨다는 가설을 검증하기 위하여 여러 가지 박쥐를 포함하는 동면동물을 대상으로 박쥐의 정상 동면돌입과정과 전간발작유발인자인 tetrahydrocannabinol (THC), pentylenetetrazol (metrazol), intermittent light stimulation (ILS)이 박쥐의 체온 (Tb), 심박률 (HR), 뇌전도 (EEG), 뇌영상 (brain mapping) 등에 미치는 영향을 관찰하여 다음과 같은 결과를 얻었다. 정상 동면돌입과정에 있어서는 안주애기박쥐는 1∼2개의 고평부를 형성하지만 대체로 2시 간 후에 동면에 안정 (심박률 15회/분)되었고 작은갈색박쥐는 2시간 30분에 동면온도에서 안정되었다. THC는 햄스터의 체온을 감소시켰고, pentylenetetrazol (metrazol)의 유의한 체온하강효과는 볼 수 없었으나, ILS는 긴가락박쥐, 쇠박쥐의 체온과 심박률을 하강시켰으며, 문둥이박쥐는 정상적으로 동면돌입시 뇌전도상 epileptic seizure시 나타나는 뇌파 (3 spikes and wave/sec)를 나타내었으며, 사람의 경우 ILS 자극 (Intensity 4∼8, 20 flashes/sec)을 받은 9명 중 1명에서 epileptic seizure시의 EEG상의 뇌영상 (brain map) (우측 후두엽의 일부만이 활성)으로 추측되는 소견을 볼 수 있었다. 이상의 실험결과는, 북부 온대지방의 동면동물인 박쥐와 햄스터의 경우 동면돌입과 체온하강 및 전간발작과 상당한 관련성이 있음을 암시한다고 사료된다.

  • Objectives The purpose of this study is to report two cases of epileptic children who were diagnosed as Sik-Gan(食癎) and were treated by oriental medical treatment. Methods We diagnosed the patients as Sik-Gan(食癎) and treated them with herbal medicine, acupuncture, moxa and oriental physical therapy. We observed the improvement of patient's disease by checking seizure frequency and clinical progression of gastrointestinal symptoms. Results Oriental medical treatment reduced patients' the number of seizures, and improved gastrointestinal symptoms. Conclusions This study showed that the oriental medicine can be a meaningful treatment for epileptic children who were diagnosed as Sik-Gan(食癎), but more relevant studies on epileptic children diagnosed as Sik-Gan(食癎) are needed.
