Endovascular Treatment with Intravenous Thrombolysis versus Endovascular Treatment Alone for Acute Anterior Circulation Stroke : A Meta-Analysis of Observational Studies

  • Objective : The aim of this study was to determine outcome of ischemic stroke patients in the anterior circulation treated with endovascular treatment (EVT) with intravenous thrombolysis (IVT) versus EVT alone group. Methods : A systemic literature review was performed using online database from January 2004 to January 2017. Primary outcomes were successful recanalization seen on finial angiography and good outcome at three months. Secondary outcomes were mortality and the development of symptomatic intracranial hemorrhage (S-ICH) after the procedure. A fixed effect model was used when heterogeneity was less than 50%. Egger's regression test was used to assess publication bias. Results : Five studies were included for final analysis. Between EVT with IVT and EVT alone group, successful recanalization (odds ratio [OR] 1.467, p=0.216), good clinical outcome at three months (OR 1.199, p=0.385), mortality (OR 0.776, p=0.371), and S-ICH (OR 1.820, p=0.280) did not differ significantly. Egger's regression intercept with 95% confidence interval (CI) was 1.99 (95% CI -2.91 to 6.89) in successful recanalization and -0.27 (95% CI -6.35 to 5.80) in good clinical outcome, respectively. Conclusion : The two treatment modalities, EVT with IVT and EVT alone, could be comparable in treating acute anterior circulation stroke. Studies to find specific beneficiary group for EVT alone, without primary IVT, are needed further.

Detachable Coil Embolization for Saccular Posterior Inferior Cerebellar Artery Aneurysms

  • Objective : Surgical treatment of posterior inferior cerebellar artery (PICA) aneurysms is challenging due to limited surgical accessibility. Endovascular approach has a benefit of avoiding direct injury to the brainstem or lower cranial nerves. Therefore, it has recently been considered an alternative or primary modality for PICA aneurysms. We retrospectively assessed outcomes following detachable coil embolization of saccular PICA aneurysms. Methods : From February 1997 to December 2007, we performed endovascular procedures to treat 15 patients with 15 PICA aneurysms. Fourteen patients with 14 PICA aneurysms morphology of which was saccular were reviewed retrospectively. Twelve patients had ruptured aneurysms. The aneurysms arose from the PICA origin site (n=12), the PICA lateral medullary segment (n=1), or the PICA tonsilomedullary segment (n=1). Results : Complete aneurysm occlusion was achieved in 10 patients, residual neck in 3, and residual sac in one. Radiological follow-up was performed in 7 patients with mean duration of 34.7 months (range, 1-97 months) and showed stable or complete occlusion in 6 patients. There were no rebleeding or retreatment after endovascular treatment. Thromboembolism was the only procedure-related complication (n=4 ; 28.6%). Asymptomatic PICA infarction occurred in two patients and symptomatic PICA infarction in two elderly patients with poor clinical grade. Of these procedural PICA infarction cases, 1 symptomatic PICA infarction patient developed ventriculitis and septic shock leading to death. The clinical outcome was good in 10 patients (71.4%). Conclusions : In the present study, detachable coil embolization has shown as an efficient modality for PICA saccular aneurysms challenging indications of microsurgery. However, thromboembolic complications should be considered, especially in poor clinical elderly patients with ruptured aneurysms.

The Fate of Partially Thrombosed Intracranial Aneurysms Treated with Endovascular Intervention

  • Objective : The fate of partially thrombosed intracranial aneurysms (PTIAs) is not well known after endovascular treatment. The authors aimed to analyze the treatment outcomes of PTIAs. Methods : We retrospectively reviewed the medical records of 27 PTIAs treated with endovascular intervention between January 1999 and March 2018. Twenty-one aneurysms were treated with intraluminal embolization (ILE), and six were treated with parent artery occlusion (PAO) with or without bypass surgery. Radiological results, clinical outcomes and risk factors for major recurrence were assessed. Results : The initial clinical status was similar in both groups; however, the last status was better in the ILE group than in the PAO group (p=0.049). Neurological deterioration resulted from mass effect in one case and rupture in one after ILE, and mass effect in two and perforator infarction in one after PAO. Twenty cases (94.2%) in the ILE group initially achieved complete occlusion or residual neck status. However, 13 cases (61.9%) showed major recurrence, the major causes of which included coil migration or compaction. Seven cases (33.3%) ultimately achieved residual sac status after repeat treatment. In the PAO group, all initially showed complete occlusion or a residual neck, and just one case ultimately had a residual sac. Two cases showed major recurrence, the cause of which was incomplete PAO. Aneurysm wall calcification was the only significantly protective factor against major recurrence (odds ratio, 36.12; 95% confidence interval, 1.85 to 705.18; p=0.018). Conclusion : Complete PAO of PTIAs is the best option if treatment-related complications can be minimized. Simple fluoroscopy is a useful imaging modality because of the recurrence pattern.

  • 저자들은 복부 대동맥에 발생한 동맥-요관루를 혈관 내 접근을 통해 성공적으로 치료할 수 있었던 드문 증례를 보고하는 바이다. 동맥-요관루는 극히 드물지만, 사망률이 7~23%에 이르는 치명적인 질환이다. 저혈량쇼크와 같은 치명적 합병증을 예방하기 위해서는 조기 진단과 함께 즉각적인 치료가 필수적이다. 하지만 질환 자체가 희귀하고 민감도가 높은 검사 방법이 없기 때문에, 진단을 위해서는 높은 수준의 임상적 의심이 반드시 필요하다. 복강 내 수술, 방사선 치료 및 요관 스텐트의 장기 설치 등의 특징적인 과거력을 가진 환자에서 외상적 사건이 동맥-요관루 발생의 촉발 요인이 될 수 있다. 복부 대동맥에 발생한 동맥-요관루 환자에서도 혈관 내 인조혈관 스텐트의 삽입은 효과적인 치료 방법이다.

하지 심부정맥 혈전증에서 중재적 치료의 유용성 (The Efficacy of Endovascular Treatment for Deep Vein Thrombosis)

  • 배경: 심부정맥 혈전증은 폐색전증이나 만성 심부정맥 부전증을 일으킬 수 있는 질환으로 기본적인 치료로 항응고요법이 사용되어 왔다. 그러나 고식적인 항응고요법은 치료 이후 재발과 혈전후 증후군의 빈도가 높은 것으로 드러나 최근에는 중재적 시술을 이용한 치료가 적극적으로 이루어지고 있다. 이에 저자들은 하지 심부정맥 혈전증을 가진 환자들을 대상으로 중재적 치료의 유용성을 조사하였다. 대상 및 방법: 2006년 12월부터 2008년 7월까지 심부정맥 혈전증으로 중재적 시술을 시행한 29명의 환자를 대상으로 임상양상 및 치료 방법, 치료 경과를 후향적으로 조사하고 이전에 항응고제만으로 치료한 45명의 환자들을 대조군으로 하여 결과를 비교 하였다. 결과: 환자의 평균연령은 중재적 치료의 경우 55.4세, 대조군의 경우 53.7세로 두 군 모두 여자에서 더 많이 발생하였다. 도관을 이용한 혈전 용해제를 투여한 환자는 총 22명(78.5%)이었고 흡입 혈전제거술을 시행한 환자가 18명(62.0%), 스텐트를 삽입한 환자가 25명(86.2%)이었다. 폐색전증의 위험이 있어 하대정맥 내 필터를 삽입한 환자가 15명(51.7%)있었다. 대조군의 경우 저분자량 헤파린으로 치료한 경우가 39명(86.7%), 와파린을 사용할 수 없어 아스피린만으로 치료한 경우가 7명(15.6%), 헤파린으로 치료한 경우가 6명(17.8%) 있었다. 혈전용해제 또는 항응고제 사용 후 출혈로 인한 합병증은 발생하지 않았다. 추적 관찰 기간 동안 중재적 치료의 경우에 대조군과 비교하여 혈전증의 재발에는 유의한 차이가 없었으나 혈전후 증후군은 통계학적으로 유의하게 적게 나타났고(p-value=0.008) 사망한 경우가 없어 임상적으로 더 양호한 결과를 보였다. 결론: 중재적 치료는 고식적 항응고요법과 비교하여 안전하며 빠른 증상 호전과 낮은 합병증 발생률을 보이지만 아직까지 장기적인 유용성과 그 효과에 대한 연구는 이루어지지 않았으므로 향후 이 부분에 대한 연구가 필요할 것으로 생각된다.

The Utility and Benefits of External Lumbar CSF Drainage after Endovascular Coiling on Aneurysmal Subarachnoid Hemorrhage

  • Objective : Cerebral vasospasm still remains a major cause of the morbidity and mortality, despite the developments in treatment of aneurysmal subarachnoid hemorrhage. The authors measured the utility and benefits of external lumbar cerebrospinal fluid (CSF) drainage to prevent the clinical vasospasm and its sequelae after endovascular coiling on aneurysmal subarachnoid hemorrhage in this randomized study. Methods : Between January 2004 and March 2006, 280 patients with aneurysmal subarachnoid hemorrhage were treated at our institution. Among them, 107 patients met our study criteria. The treatment group consisted of 47 patients who underwent lumbar CSF drainage during vasospasm risk period (about for 14 days after SAH), whereas the control group consisted of 60 patients who received the management according to conventional protocol without lumbar CSF drainage. We created our new modified Fisher grade on the basis of initial brain computed tomography (CT) scan at admission. The authors established five outcome criteria as follows : 1) clinical vasospasm; 2) GOS score at 1-month to 6-month follow-up; 3) shunt procedures for hydrocephalus; 4) the duration of stay in the ICU and total hospital stay; 5) mortality rate. Results : The incidence of clinical vasospasm in the lumbar drain group showed 23.4% compared with 63.3% of individuals in the control group. Moreover, the risk of death in the lumbar drain group showed 2.1 % compared with 15% of individuals in the control group. Within individual modified Fisher grade, there were similar favorable results. Also, lumbar drain group had twice more patients than the control group in good GOS score of 5. However, there were no statistical significances in mean hospital stay and shunt procedures between the two groups. IVH was an important factor for delayed hydrocephalus regardless of lumbar drain. Conclusion : Lumbar CSF drainage remains to playa prominent role to prevent clinical vasospasm and its sequelae after endovascular coiling on aneurysmal subarachnoid hemorrhage. Also, this technique shows favorable effects on numerous neurological outcomes and prognosis. The results of this study warrant clinical trials after endovascular treatment in patients with aneurysmal SAH.

Endovascular Treatment of Dural Arteriovenous Fistulas : Single Center Experience

  • Objective : Treatment of intracranial dural arteriovenous fistulas (dAVFs) remains a challenge. However, after introduction of Onyx, transarterial approach is the preferred treatment option in many centers. We report our experience of dAVFs embolization with special emphasis on transarterial approach. Methods : Seventeen embolization procedures were performed in 13 patients with dAVFs between Jan 2009 and Oct 2014. Clinical symptoms, location and type of fistulas, embolization methods, complications, radiological and clinical outcomes were evaluated using charts and PACS images. Results : All 13 patients had symptomatic lesions. The locations of fistulas were transverse-sigmoid sinus in 6, middle fossa dura in 4, cavernous sinus in 2, and superior sagittal sinus in 1 patient. Cognard types were as follows : I in 4, IIa in 2, IIa+IIb in 5, and IV in 2. Embolization procedures were performed ${\geq}2$ times in 3 patients. Nine patients were treated with transarterial Onyx embolization alone. One of these required direct surgical puncture of middle meningeal artery. Complete obliteration of fistulas was achieved in 11/13 (85%) patients. There were no complications except for 1 case of Onyx migration in cavernous dAVF. Modified Rankin scale score at post-operative 3 months were 0 in 11, and 3 in 2 patients. Conclusion : Transarterial Onyx embolization can be a first line therapeutic option in patients with dAVFs. However, transvenous approach should be tried first in cavernous sinus dAVF because of the risk of intracranial migration of liquid embolic materials. Furthermore, combined surgical endovascular approach can be considered as a useful option in inaccessible route.

May-Thurner 증후군과 동반된 장골동맥-장골정맥루: 증례 보고 (Ilio-Iliac Arteriovenous Fistula with May-Thurner Syndrome: A Case Report)

  • 장골동맥-장골정맥루는 드문 질환으로 일으키는 주요 요인으로는 선천성 이상 발생, 의인성, 외상 등이 있다. 그중에서도 May-Thurner 증후군과 동반된 장골동맥-장골정맥루에 대한 문헌은 매우 드물다. 이에 따라, 저자들은 80세 남성에서 발생한 May-Thurner 증후군과 동반된 장골동맥-장골정맥루의 증례를 보고하고자 한다. 컴퓨터단층촬영 및 혈관조영술을 통해 매우 넓은 범위의 장골동맥-장골정맥루를 확인하였고 이에 대해 다양한 크기의 색전용 코일 및 1400-2000 ㎛ 크기의 젤라틴 입자를 사용하여 색전술을 시행하였다. 색전술 이후, 추적을 위해 시행한 복부-골반 컴퓨터단층촬영에서 왼쪽 다리의 부종이 개선됨을 확인하였다.

$Outback^{(R)}$ $LTD^{TM}$ Catheter를 이용한 표재성 대퇴동맥의 만성동맥폐색증의 성공적인 재개통술: 3예 보고 (The $Outback^{(R)}$ $LTD^{TM}$ Catheter: The Novel Re-Entry Technique in Recanalization of Chronic Inflow Occlusion of the Superficial Femoral Arteries in 3 Cases)

  • 만성동맥폐색증 환자의 혈관내 시술 시 발생되는 기술적 실패의 가장 많은 이유 중의 하나는 폐색부위를 내막하 진행 후 진강내 재진입의 실패에 기인한다. 진강내 재진입 카테터는 유도철사의 진강내 재진입을 용이하게 하여, 대부분 수술로 전환될 만성폐색질환에서 성공적인 혈관내 시술을 가능하게 한다. 본원에서는 표재성 대퇴동맥의 만성동맥폐색증의 혈관내 시술 시 기존의 장비로 진강내 재진입이 실패한 3예에서 $Outback^{(R)}$ $LTD^{TM}$ catheter를 이용하여 성공적으로 치료하였기에 문헌 고찰과 함께 보고하는 바이다.

A Comparative Study of Abdominal Aortic Aneurysm: Endovascular Aneurysm Repair versus Open Repair

  • Background: Endovascular aneurysm repair (EVAR) has dramatically changed the management of abdominal aortic aneurysms (AAAs) as the number of open aneurysm repairs have declined over time. This report compares AAA-related demographics, operative data, complications, and mortality after treatment by open aneurysm repair or EVAR. Methods: We retrospectively reviewed 136 patients with AAAs who were treated over an 8-year time period with open aneurysm repair or EVAR. Results: The mean age of the EVAR group was higher than that of the open repair group (p=0.001), and hospital mortality did not differ significantly between groups (p=0.360). However, overall survival was significantly lower in the EVAR group (p=0.033). Conclusion: Although EVAR is the primary treatment modality for elderly patients, it would be ideal to set slightly more stringent criteria within the anatomical guidelines contained in the instructions for use of the EVAR device when treating younger patients.
