High Prevalence of Haplorchis taichui, Phaneropsolus molenkampi, and Other Helminth Infections among People in Khammouane Province, Lao PDR

  • The prevalence of liver and intestinal helminth infections, including Opisthorchis, Haplorchis, Phaneropsolus, hookworms, Enterobius, and Taenia, was surveyed in Khammouane province, Lao PDR. Fecal specimens were collected from 1,242 people (590 men and 652 women) in 3 Mekong riverside villages and were examined by the Kate-Katz thick smear technique. The overall helminth egg positive rate was 81.1%. The positive rate for small trematode eggs, including Opisthorchis viverrini, heterophyids, and lecithodendriids, was 81.1% and the positive rate for hookworms was 6.7%. To obtain adult worms, 35 people who were positive for small trematode eggs were treated with 20-30 mg/kg praziquantel and 10-15 mg/kg pyrantel pamoate, and then purged. Diarrheic stools were collected from 33 of these people and searched for helminth parasites using a stereomicroscope. Mixed infections with various helminths (Haplorchis taichui, Haplorchis yokogawai, Prosthodendrium molenkampi, Phaneropsolus bonnei, echinostomes, hookworms, Trichostrongylus spp., Trichuris trichiura, Enterobius vermicularis, and/or Taenia saginata) were found. The total number of helminth specimens collected was 20,907 (approximately 634 per person). The most common species was H. taichui, followed by P. molenkampi, 0. viverrini, P. bonnei, E. vermicularis, hookworms, and Trichostrongylus spp. These results show that diverse species of intestinal nematodes, trematodes, and cestodes are infecting humans in Khammouane province, Lao PDR.

Survey on the Status of C. sinensis Infection in Rural Inhabitants (Yeoju Eup, Kyunggi Do) (농촌지역 주민(여주읍)에 있어서 간흡충 감염현황 조사 - 학생을 통한 대변수집 방안 및 결과 -)

  • In order to observe the infestation rate and intensity of C. sinensis in Yeoju Eup, Kyung-gi Do, 9,512 stool specimens were collected from the inhabitants through student's helps. The specimens were examined by cellophane thick smear technique and Stoll's egg dilution technique. The epidemiological status was analysed statistically by the regession equation and catalytic curve with the results obtained from this area. The results are as follows: 1) Collection rate oft he stool samples was 66.5%, 9.512 out of 14,300 inhabintants complied with our survey. 2) The infestation rate of C.sinensis was 5.0%, and for male 7.8%, for female 2.3%. 3) Average E.P.G. by Stoll's egg dilution technique in this area was 1,572, and for male 1,853, for female 676. 4) The degree of the intensity of C. sinensis infection by E.P.G. count was distributed as 65.2% in light infection, 32.0 % in moderate infection, 2.8% in heavy infection and none in very heavy infection. 5) The intensity of endemicity in this area was represented with the regression equation calculated with cumulative percentages of E.P.G. counts. Regression equation was y=3.887+1.695 log x and Cs. $D_{50}$ was 4.54. 6) The two-stage catalytic model was applied and the calculation lead to the equation $y=0.267(e^{-0.004t}-e^{-0.019t})$; a=0.004 < b=0.019. 7) Other helminthic infection rate in this area was 1.4% in A.lumbricoides 2.7% in T. trichiura, 1.6% in M. yokogawai md 0.2% in Taenia sp. respectively.

Therapeutic evaluation of sustained-releasing praziquantel (SRP) for clonorchiasis: Phase 1 and 2 clinical studies

  • Sustained-releasing praziquantel (SRP) tablet was designed for single dose treatment regimen of clonorchiasis. A previous pre-clinical study confirmed its sustained-releasing characteristics and a better cure rate than conventional praziquantel (PZQ). In this clinical study, the pharmacokinetics of this SRP tablet were investigated in human volunteers (phase 1; 12 volunteers), and its curative efficacy was examined in clonorchiasis patients (phase 2; 20 volunteers). In the phase 1 clinical study, blood concentrations of both tablets showed wide individual variation. The $AUC_{last}$ of SRP was $497.9{\pm}519.0ng{\cdot}hr/ml\;(mean{\pm}SD)$ and PZQ of $628.6{\pm}695.5\;ng{\cdot}hr/ml$, and the $AUC_{inf}$ of SRP was $776.0{\pm}538.5\;ng{\cdot}hr/ml$ and of PZQ $658.6{\pm}709.9\;ng{\cdot}hr/ml$. $C_{max}$ values of SRP and PZQ were $90.7{\pm}82.2ng/ml\;and\;214.9{\pm}251.9\;ng/ml$, and $T_{max}$ values were $3.42{\pm}1.43\;hr\;and\;1.96{\pm}1.23\;hr$, respectively. SRP tablets showed similar AUC values, but lower $C_{max}$ and longer $T_{max}$ values than PZQ. In the phase 2 study, SRP at 30 mg/kg (single dose) achieved a 60% cure rate and a 95.5% egg reduction rate. The cure rate of a single dose SRP was unsatisfactory compared with that of the conventional PZQ dose, but much better than that achieved by a single dose PZQ.

  • The therapeutic effects of praziquantel on Fibricola seoulensis infection were studied by experimental infection of albino rats with the metacercariae obtained from the snakes. Total 51 albino rats were infected each with 500 or 1, 000 meta cercariae in number through intragastric tubes. One or four weeks la er the rats were treated with 1, 5, 10 or 20 mg/kg single dose of prazipuantel and sacrificed after 3 days or 10-120 minutes to search for the worms in their small intestine. The worm recovery rate at 3 days after the treatment was 10.7 % in 1 mg/kg dose group and o. 03~0.2% in 5, 10 and 20 mg/kg groups, while that of untreated control was 51. 3%. The minimum effective dose to treat F. seoulensis infection in the rats is considered to be 5 mg/kg in single dose. By observing the distribution pattern of worms in the small intestine after the treatment, dislodgement of the worms from the duodenum, their normal habitat, to the lower portions was recognizable within as early as 10 minutes. The majority of worms was found to have descended to the ileo-caecal portion after 60-120 minutes.

The Prevalence of Intestinal Parasites in Inhabitants and Swine Pen Human Latrine System in Cheju Do (제주도(濟州道) 일부지역(一部地域)에서의 장내기생충감염상황(腸內寄生虫感染狀況) 및 분식사양편소(糞食飼養便所))

  • This study was undertaken to evaluate the present status of parasitic infection and swine pen human latrine system in Cheju Do, from July to September 1982. The 663 stool specimens (male 323 and female 340) and 579 scotch tape anal swabs collected from 161 households of 2 areas in Cheju Do were examined. The methods employed were formalin-ether technique for the prevalence rate of various helminthic and protozoan infection, and scotch tape anal swab technique for the prevalence rates of Enterobius vermicularis. In addition to these, questionaire was used to evaluate the present status of swine pen human latrine system and prevalence rates of taeniasis in these areas. The results are as follows ; 1) Prevalence rates of parasitic infections of any kind was 33.9%. It was 35.7% in Cheju City in contrast to 32.1% in North Cheju Gun. The infection rates of Trichuris trichiura was 10.0% and it was the highest prevalence rate in this survey. The prevalence rates of the other parasites were as follows ; Ascaris lumbricoides 2,3%, Hookworm 0.2%, Clonorchis sinensis 0.5%, Hymenolepis nana 1.5%, Entamoeba coli 3.2%, and Giardia lamblia 0.5%. 2) The infection rates of Enterobius vermicularis in 579 peoples (male 285, female 294) by applying scotch tape anal swab technique was 13.1% through the survey. It was 16.9 in Cheju City and 8.5% in North Cheju Gun. 3) The Infection rate of Taenia species by applying the stool examination and making up a question was 19.2%(21.4% in Cheju City and 16.7% in North Cheju Gun). 4) Sexual distribution of the parasitic infections showed slightly higher rate in female than that of male. 5) The positive rates of parasitic infection by the stool examination and questionaire positive cases of taeniasis were higher in 0-9 and over 60 year old than any other age group. 7) The swine pen human latrine systems were used in 46 households (28.6%). 7) Relationship between swine pen human latrine system and taeniasis was not noted.

Description of a Male Gnathostoma spinigerum Recovered from a Thai Woman with Meningoencephalitis (수막뇌염을 동반한 진국인 유극악구충증 1예)

  • A coiled nematode, which was removed surgically from a Thai woman, was consulted to the authors in July, 1987. She was known to surfer from meningoencephalitis since she was in Thailand. Numerous eosinophils were detected from her CSF. The worm was 12.3mm long and 0.9mm wide. It had a head bulb beset with eight rows of spines, a cervical constriction, esophagus, cervical sacs, dark intestine and testis, Cuticle of anterior half of the worm was covered with numerous spines. The spines at anterior part was stout and had 3∼4 tips, but they became slender, shorter, single tipped and sparser and finally they disappeared posteriorly. Cuticular spines reappeared at tail which had 4 pairs of pedunculated papillae. By above morphological characteristics, the worm was identified as an adult male of Gnathostcma spinigerum. The Present case is the first authentic case of imported intracranial gnathostomiasis in Korea, although clinical informations of the case were obtained limitedly.

  • The prevalence of liver and intestinal fluke infections was surveyed on residents of Savannakhet Province, Laos. Fecal specimens were collected from a total of 981 residents in 4 Mekong riverside villages and examined by the Kato-Katz thick smear technique. The results revealed that the overall helminth egg positive rate was 84.2%, and the positive rate for small trematode eggs, including Opisthorchis viverrini, heterophyids, or lecithodendriids, was 67.1 %. To obtain adult flukes, 38 small trematode egg positive cases were treated with a 20-30 mg/kg single dose of praziquantel and purged. Diarrheic stools were then collected from 29 people and searched for helminth parasites using stereomicroscopes. Mixed infections with O. viverrini and 6 kinds of intestinal flukes were found, namely, Haplorchis taichui, Haplorchis pumilio, Haplorchis yokogawai, Prosthodendrium molenkampi, Phaneropsolus bonnei, and echinostomes. The total number of flukes collected was 7,693 specimens (av. no. per treated person; 265.3). The most common species was O. viverrini, followed by H. taichui, P. molenkampi, echinostomes, H. pumilio, P. bonnei, and H. yokogawai. The results indicate that foodborne liver and intestinal fluke infections are prevalent among residents of Savannakhet Province, Laos.

In vitro cultivation of Gymnophalloides seoi metacercariae (Digenea: Gymnophallidae) (Gymnophaiioides seoi (Digenea: Gymnophallidae) 피낭유충의 시험관내 배양)

  • Gymnophclloides seoi is a human intestinal trematode prevalent on southwestern islands in Korea. In the present study, we investigated whether G. seoi metacercariae can grow and develop into adults by in vitro cultivation. The metacercariae were obtained from naturally infected oysters, and cultured in uitro for 5 days under three conditions; $37^{\circ}C/5%{\;}CO_2,{\;}41^{\circ}C/8%{\;}CO_2,{\;}or{\;}41^{\circ}C/15%{\;}CO_2$, in NCTC 109 complete media containing 20% FBS and 1% antibiotics-antimycotics. The degree of worm growth and development was compared with that grown in uiuo of C3H mice. The length of the worms cultivated in uitro was $200-300{\;}\mu\textrm{m}$, not significantly different from metacercariae, whereas the length of the worms recovered from C3H mice was significantly larger, $300-400{\;}\mu\textrm{m}$. The worms produced eggs when grown in C3H mice or cultured in vitro for 2 days under $41^{\circ}C/8%{\;}CO_2{\;}or{\;}41^{\circ}C/5%{\;}CO_2$, but not when cultured under 37$^{\circ}C/5%{\;}CO_2$. Among the in vitro conditions, $41^{\circ}C/15%{\;}CO_2$ was best for egg Production, although the number of eggs was about half of worms obtained from C3H mice. In conclusion, in vitro cultivation of G. semi metacercariae into egg-pioducing adults was partially successful under culture conditions of $41^{\circ}C/5%{\;}CO_2{\;}or{\;}41^{\circ}C/8%{\;}CO_2$.

Scanning Electron Microscopical Findings of Echinochasmus japonicus Tegument (Echinochusmus juponicus 표피 미세구조의 주사전자현미경적 관찰)

  • Tegumental ultrastructures of Echinochasmus japonicus were observed by scanning electron microscopy. The worms were recovered from albino rats which were experimentally infected with the metacercariae obtained from Pseudorasbora parva. Followings are summarized findings. 1. The worms were minute and plumpy gourdshaped with attenuated anterior and round posterior end. The tegument of whole body was wrinkled transversely and covered with cobblestone-like cytoplasmic processes. 2. Head crown was armed with 24 collar spines which were embedded in cytoplasmic pockets. The spines were arranged in a row with an interruption at dorsomedian line, however, the 2nd and the 4th spines were outstretched more than others. Oral and ventral suckers were muscular with numerous type II sensory papillae, and genital pore opened between the two suckers. 3. Tegumental spines were spade-shaped with broad base and pointed tip. They were compact in ventra-lateral tegument or dorsal surface of anterior body. They were not found between the two suckers and dorsal surface of posterior body. 4. Two types of sensory papillae, uni-ciliated (Type I) and roundly swollen sensory papillae (Type II), were observed. The type I papillae were chiefly distributed on ventral surface of tegument and type II were on the lips of suckers. Arrangement of collar spines, shape and distribution of tegumental spines or sensory papillae are regarded as characteristic features of E. japonicus.

  • A transmission electron microscopic study was performed on the ultrastructure of the tegumental layer of GymophoLloines seoi (Digenea: Gymnophallidae) metacercarlae and adults. The metacercariae were obtained from naturally infected oysters, Crcssosoea gigas, and the adults from experimentally infected C3H mice. The tegumental layer generally revealed a small number of foldings, numerous small vacuoles, sines, and muscle bundles. Beneath the muscle layer, nuclei of the tegumental cells were located. There was little difference in the structure of the tegument between the metacercariae and adults. The oral sucker, having well-developed muscle layers, showed a similar structure to the ventral sucker except numerous foldlngs in the ventral sucker. The ventral pit was surrounded by a thin spcpiu layer, where a number of microtubules and mitochondria were seen. Around the ventral pit located well-developed circular and longitudinal muscles. The results showed that the ultrastructure of the tegumental layer of G. seoi metacercariae and adults revealed little difference from other trematodes in general. The ventral pit, a peculiar structure of this trematode, seems to function as a sphincter or an accessory adhesive organ.
