세대별 QLF 기기의 평활면 비와동형 법랑질 우식 병소 탐지에 관한 진단정확도 비교 (Comparison of fluorescence loss measurements among various generations of QLF devices)

  • Purpose: The aim of in vitro study was to compare the diagnostic accuracy to detect non-cavitated enamel caries on smooth surface by using four kinds of the QLF devices. Materials and Methods: A total of 52 human permanent premolars and molars were used. Fluorescence images were captured by the QLF devices (Inspektor Pro, QLF-D, Qraycam, and Qraypen). Fluorescence loss of the QLF was calculated. The severity of lesions was categorized into the following 3 scores using polarized light microscopy: normal (S), enamel demineralization to outer half of enamel (D1), and inner half of the enamel up to the dentin-enamel junction (D2). The Kruskal-Wallis test was used to compare the fluorescence loss among the QLF devices. Spearman rank correlation coefficient between histological scores and fluorescence loss of the devices was calculated. The sensitivity, specificity, and area under the receiver operating curve (AUROC) were calculated to compare their diagnostic accuracies. Results: The correlation coefficients between histological scores and the fluorescence loss of the devices showed 0.77 to 0.81 (P < 0.001). All histological scores, the fluorescence loss among the devices showed no statistical difference. Among the devices, sensitivity, specificity, and AUC values of the fluorescence loss showed 0.84 to 0.94, 0.76 to 0.90, and 0.90 to 0.92, respectively. Conclusions: All QLF devices had no difference with excellent diagnostic accuracies to detect non-cavitated enamel caries on smooth surface.

치과 환자로부터 발거된 치아에서의 법랑돌기의 발현빈도 (The Prevalence of Enamel Projection on Molar Teeth Extracted from Dental Patients)

  • 많은 연구에서 대구치의 백악법랑 경계부에 나타나는 법랑돌기의 발현이 치주질환과 밀접한 연관성을 보인다고 하였다. 이 연구는 서울대학교 치과병원에 내원한 환자에서 발거된 치아 660개 중 법랑돌기의 관찰이 가능하고, 제 1, 2대구치의 구별이 가능한 530개의 치아를 대상으로 치면별, 치아별 법랑돌기의 발현빈도를 알아보고자 시행하였다. 법랑돌기의 관찰이 용이하도록 깨끗이 세정한 후, 8배 전자확대경을 이용하여 상, 하악 제 1, 2대구치의 협면, 근심면, 원심면, 설면(구개면)에 나타나는 법랑돌기를 조사하였다. 법랑돌기의 발현빈도는 상악에서 45.49%, 하악에서 39.62%로, 하악에서보다 상악에서 높았으며, 제1대구치는 45.22%, 제2대구치는 39.89%로, 제2대구치보다 제1대구치에서 더 높게 나타났다. 또한 치면별 발현빈도는 전체 대상치아의 협면에서 가장 높게 나타났으며, 상악 제1대구치 협면에서 78.27%, 상악 제2대구치 협면에서 81.23%, 하악 제1대구치 협면에서 76.86%, 하악 제2대구치 협면에서 70.3%의 발현율을 보였다. 이 연구의 결과는 이전의 보고에 비해 높은 법랑돌기의 발현빈도를 보여주고 있으며, 이는 8배 전자확대경을 이용한 관찰에 의한 결과로 생각된다.

유기산 완충용액의 포화도가 법랑질 및 상아질의 재광화에 미치는 영향과 수산화인회석의 AFM 관찰 (THE EFFECTS OF THE DEGREE OF SATURATION OF ACIDULATED BUFFER SOLUTIONS IN ENAMEL AND DENTIN REMINERALIZATION AND AFM OBSERVATION OF HYDROXYAPATITE CRYSTALS)

  • Dental caries is the most common disease in the maxillofacial area. There are many factors contributing to its development, but complete understanding and prevention is not fully known. Since the structure of the coronal and root portion of the tooth is different, the remineralization and demineralization process is also known to be different. In this study, by using a partially saturated buffer solution, we created artificial enamel and dentin caries and evaluated mineral loss. A remineralization solution with four different degrees of saturation (degree of saturation ; group 1, 0.268, group 2, 0.309, group 3, 0.339, group 4, 0.390, PH 4.3, F-2ppm) was used on a demineralized specimen. The mineral precipitating quantity and depth was evaluated by using microradiography. Using an atomic force microscope (AFM), hydroxyapatite crystals of normal, demineralized, and remineralized enamel and dentin were evaluated. The results were as follows: 1. As the degree of saturation of the remineralizing solution increased, the mineral precipitation in the enamel was increased. In group 4, mineral precipitation was limited near the surface. 2. As the degree of saturation of the remineralizing solution increased, the mineral precipitation in the dentin was decreased and it occurred in a deeper portion. In group 4, however, mineral precipitation occurred on the surface and its quantity increased. 3. There was a statistically significant interaction between enamel and dentin mineral content changes on specimens treated with remineralization and demineralization solution (demineralization r=0.44, remineralization r=0.44, p<0.05). 4. Demineralized hydroxyapatite crystals showed central and peripheral dissolving and widening of intercrystal spaces under the AFM. 5. In dentin remineralization small crystal precipitation occurred between the large crystals. We conclude that by adjusting acidulated buffer solution's degree of saturation, we can control enamel and dentin remineralization. In addition, the AFM is highly useful in evaluating changes in remineralized and demineralized hydroxyapatite crystals.

Debonding에 의한 법랑질 Crack의 증가 (THE INCREASE OF ENAMEL CRACK IN DEBONDING TEETH)

  • The aim of this study was to clarify whether bonding/debonding procedure will affect the occurance of enamel crack. The frequency of enamel crack was compared between before-bonding and after-debonding on 200 human extracted teeth. Each facial surface of the tooth was divided in 9 fragments. A presence of crack, which was classified by its direction as vertical, horizontal and oblique crack, was surveyed in each fragment. Number of all cracks in facial surface was 1355 at before-bonding, and 1605 at after-debonding, so it revealed significant increase rate of $18.5\%$, but compared by fragment, cracks were significantly Increased in OC, OD, CC and GC fragments. All kinds of cracks were significantly increased, especially increase rate of oblique crack reached $54.9\%$. The increase rate of cracks was not superior at any fragment or region, but some evidence was seen in CC fragment. Judging from the above, increase of crack is unavoidable with bonding/debonding procedures.

수종시판 청량음료가 법랑질 표면에 미치는 영향 (The Effect of Acid Beverages Purchased on the Retail Market upon the Surface of Tooth Enamel)

  • 본 연구는 건전한 상악 제 3대구치 법랑질을 1군:콜라(pH 2.39), 2군:박카스(pH 2.69), 3군:요구르트(pH 3.49), 4군:오렌지쥬스(pH 3.85), 5군:우유(pH 6.64)에 30분, 60분, 120분간 노출시킨 후 주사전자현미경으로 관찰하여 다음과 같은 결론을 얻었다. 1. 대조군과 우유를 제외한 4종 음료의 pH는 평균 3.105로서 치아 표면의 법랑질 탈회를 유발시키기 충분한 수준이었고, 대부분의 시료에서 탈회 현상을 관찰할 수 있었다. 2. 산성음료에 노출되는 시간에 따른 법랑질 탈회 정도가 다르게 나타났다. 30분 동안 노출된 치아보다 60분, 120분 동안 노출된 치아에서 법랑질 탈회가 심하게 나타났고, 60분과 120분 시료의 차이는 크지 않았다. 3. 칼슘과 인의 함유량이 많은 제품에서는 유사한 pH를 갖는 음료와 비교해 볼 때, 법랑질표면의 탈회 정도가 덜 하였다.

Regeneration of a Micro-Scratched Tooth Enamel Layer by Nanoscale Hydroxyapatite Solution

  • Hydroxyapatite (HAp)-based materials have attracted considerable attention on account of their excellent stability and recrystallization. Nanoscale HAp powders with a mean particle size of 200 nm were used to regenerate the enamel layers of damaged teeth. An artificially scratched tooth was immersed in a nanoscale HAp powder suspension in d.i. water (HAp of 70 wt%) at 37 ${^{\circ}C}$ for a period of 1~3 months. SEM and AFM showed that the scratched surface was ultimately inlaid with HAp after three months and the roughness increased from 2.80 to 5.51. Moreover, the hardness of the neo-generated HAp layer on the crown was similar to that of the innate layer. $Ca^{2+}$ and ${PO_4}^{3-}$ ions from the HAp powders dissolved in d.i. water were precipitated on the tooth to produce cemented pasteson the enamel surface due to its high recrystallizing characteristics, resulting in a hard neo-regenerated HAp layer on the enamel layer. This nanoscale HAp powder solution might be used to heal decayed teeth as well as to develop tooth whitening appliances.

미세 레이저 가공의 표면코팅 후 전해 에칭 (Laser Micro Machining and Electrochemical Etching After Surface Coating)

  • Laser beam machining (LBM) is fast, contactless and able to machine various materials. So it is used to cut metal, drill holes, weld or pattern the imprinted surface. However, after LBM, there still leave burrs and recast layers around the machined area. In order to remove these unwanted parts, LBM process often uses electrochemical etching (ECE). But, the total thickness of workpiece is reduced because the etching process removes not only burrs and recast layers, but also the entire surface. In this paper, surface coating was performed using enamel after LBM on metal. The recast layer can be selectively removed without decreasing total thickness. Comparing with LBM process only, the surface quality of enamel coating process was better than that. And edge shape was also maintained after ECE.

불소농도가 Seeded Enamel Mineral과 합성 Hyproxyapatite에 Crystal 성장에 미치는 영향 (Seeded Crystal Growth onto Enamel Mineral and Synthetic Hydroxyapatite in Dilute Supersaturated Solutions Containing Low Concentrations of Fluoride)

  • The present study was undertaken to investigate the crystal growth onto enamel mineral and synthetic hydroxyapatite seeds in media resembling the enamel fluid composition. Effects of fluoride at low concentrations on the precipitation were also examined in a benchtop crystal growth model adopting a miniaturized reaction column. X-ray diffraction and Fourier transform infrared spectroscopy(FTIR), as well as chemical analyses, were employed for characterization of both seed materials before and after experimentation. Remarkable findings were that (1) both biological and synthetic seeds at the same total surface areas yielded rather similar precipitation rates at all levels of fluoride concentration in solution and (2) the precipitation rate was accelerated in a manner depending on fluoride concentrations in media. FTIR differential analysis disclosed that the precipitating phase was characterized as poorly crystallized apatite, which incorporated subtle carbonate. Most of the fluoride ions in soution were readily incorporated into crystals. The overall results support the view that the seeded crystal growth model is of value to gain insight into the mechanism of enamel crystal growth under fluoride regimens.

Characteristics of the molar surface after removal of cervical enamel projections: comparison of three different rotating instruments

  • Purpose: The aim of this study was to evaluate and compare tooth surface characteristics in extracted human molars after cervical enamel projections (CEPs) were removed with the use of three rotating instruments. Methods: We classified 60 extracted molars due to periodontal lesion with CEPs into grade I, II, or III, according to the Masters and Hoskins' criteria. Each group contained 20 specimens. Three rotating instruments were used to remove the CEPs: a piezoelectric ultrasonic scaler, a periodontal bur, and a diamond bur. Tooth surface characteristics before and after removal of the projections were then evaluated with scanning electron microscopy (SEM). We analyzed the characteristics of the tooth surfaces with respect to roughness and whether the enamel projections had been completely removed. Results: In SEM images, surfaces treated with the diamond bur were smoothest, but this instrument caused considerable harm to tooth structures near the CEPs. The piezoelectric ultrasonic scaler group produced the roughest surface but caused less harm to the tooth structure near the furcation. In general, the surfaces treated with the periodontal bur were smoother than those treated with the ultrasonic scaler, and the periodontal bur did not invade adjacent tooth structures. Conclusions: For removal of grade II CEPs, the most effective instrument was the diamond bur. However, in removing grade III projections, the diamond bur can destroy both adjacent tooth structures and the periodontal apparatus. In such cases, careful use of the periodontal bur may be an appropriate substitute.

  • In one group that tested for the effects of grinding and etching on the deciduous teeth, S.E.M. examination on the ground or unground labial surface of deciduous maxillary central incisors were made after etching procedure with 40% phosphoric acid for 60 secs., 120 secs., 180 sees. each. In another group that tested for the degree of resin penetration to the ground and etched deciduous teeth, composite resin application was done to the ground deciduous maxillary central incisors that had been acid-etched for 30 secs., 60 secs., 90 sees., 120 sees., 180 secs. each. The tooth-resin specimens were cut at the middle 1/3 of the crown by 2mm thickness, and the adjacent tooth materials were demineralized by 10% hydrochloric acid, the author observed the tags of the resin replica with S.E.M.. Following results were obtained. 1. After 40% phosphoric acid etching, the unground deciduous enamel surface showed various types of etching pattern. 2. For the formation of regular micropores on deciduous enamel surface by acid etching with 40% phosphoric acid, the time over 120 secs. should be requested. 3. After 40% phosphoric acid etching, the ground deciduous enamel surface showed the same etching pattern that has been a preferential removal of prism peripheries despite different etching time. 4. On the ground group that etched over 60 secs. to 180 secs., the length of tags was $5{\mu}m$ to $8{\mu}m$, with a mean of $7{\mu}m$.
