치과병원 근무 치과위생사의 직급체계와 직무유형 조사 (Survey on Grade System and Job Types of Dental Hygienists in Dental Hospital)

  • Purpose: This study is to provide basic data for establishing successful organization management strategies of organizations by understanding rank systems and duty types of dental hygienists who work in dental hospitals. Methodology/Approach: The study conducted a survey and an interview survey two times targeting the entire dental hospitals. The first survey secured the response results of 113 hospitals in the result that conducted the survey targeting 190 dental hospitals in the whole country except 24 ones including dental college hospitals, dental hospitals affiliated with medical colleges or general hospitals, military dental hospitals, and dental hospitals for the disabled among the 214 ones which were registered in the Health Insurance Review & Assessment Service as of December 2015. The second survey conducted the interview survey targeting persons in charge of personnel management by selecting 34 dental hospitals with rank systems of 4 rank systems and above. Finding: The dental hospital has found that dental hygienists-centered human resources were composed. The number of ranks has found that 1 to 6 levels are shown and level 3 is highest. Titles of staff levels have found that 32 places are highest in order of 'employees

치과위생사 취업 역량, 업무, 직무만족에 관한 연구 - 공무원·공기업 취업자·연구원 중심으로 (A study on career competency, task and job satisfaction of dental hygienists - Focusing on public officials, public institution workers, and researchers)

  • Objectives: The purpose of this study was to investigate career competency, tasks, and job satisfaction of public servants, public institutions, and researchers. Methods: The survey was conducted about career competency, job satisfaction, and satisfaction on work life. Next, they interviewed on the characteristics of each job by two or three dimensions. The following conclusions were obtained from July to August 2017. Results: Career competencies were GPA with 3.87, 818 points of TOEIC score, and ITQ certification. Public servants required the information on literacy skills for employment and job performance, while civil servants need more than one year of clinical experience in the dental hospital. The non-commissioned officer needed a written test and fitness training. The health insurance review and assessment center required more than one year of experience from general hospital or medical institutions. Researchers required a research career, language skill, and professors required research and teaching experiences with clinical experience more than three years. The main job tasks were as follows; for public servants, they were official document processing and community projects. For the civilian workers and military/noncommissioned officers, they were medical assistant and administrative works. The employees of the health insurance review and assessment service are examining the medical expenses and the medical examination, the researchers are experimenting, researching and writing articles, and the teaching staff are lecturing and conducting individual research. Conclusions: The results of job satisfaction survey showed that occupational satisfaction was the highest in civil servants, researchers, and teaching professions. Job security was the highest in health workers and health inspectors' evaluation centers, and time vacancy was the highest in civilian workers and military/noncommissioned officers. If you want to work in such an institution, you should prepare elements that match your basic literacy and job specific characteristics. And we should try to increase the satisfaction of work even after work.

대구시민의 의료기관 이용률과 연관요인 (Utilization Rate of Medical Facility and Its Related Factors in Taegu)

  • 도시지역 주민의 의료기관 이용률과 그 관련요인을 조사분석 하기 위하여 1988년 7월 3일부터 7월15일까지 1단계 단순집락 표본추출을 통하여 선정한 대구시 남구 지역의 450가구를 대상으로 면접조사를 실시하여 431가구 1,723명에 대한 자료를 분석하여 다음과 같은 결과를 얻었다. 대상자 1,000명당 조사기간 2주간의 급성이환율은 101이었고 9세이하군에서 202로 가장 높았고, 지난 1년간 만성이환율은 77이었으며 고연령층, 저소득층 그리고 의료보호 적용군 등에서 유의하게 높았다(p<0.01). 대상자 1,000명당 2주간의 의료기관 이용률은 689이었고, 이중 병의원이용률이 294로 가장 높았고, 약국 보건소 그리고 한의원 및 한약방 순이었다. 성별로는 여자가 785회로 남자 591보다 높았으며, 연령별로는 70세이상 군이 2,022회로 가장 높았다(p<0.01). 그외 특성에서는 의료보호 적용군 2,057(p<0.01), 월30만원미만 소득군 346, 자영업이 907로 가장 높았다. 2주간의 의료기관 이용자 1인당 평균 방문회수는 3.25회이었고, 이중 병의원이 3.26회로 가장 많았으며, 한의원 및 한약방, 약국 그리고 보건소 순이었다. 성별로는 여자가 3.47회로 남자보다 많았다. 그외 50대연령군이 5.02회, 의료보호적용군 6.41회, 월30만원미만 소득군 3.78회, 그리고 생산직이 3.64회로 가장 많았다. 대상자 1,000명당 연간입원율은 27.6이었고, 여자 38.9 남자 16.3으로 여자가 높았고 연령별로는 70세이상군이 133.3으로 가장 높았다. 의료보장상태별로는 의료보험적용군이 비적용군보다 2배이상 높았으며, 의료보호적용군에서는 한건도 없었다. 월 30만원미만 소득군이 20.8로 가장 낮았으며 소득이 증가할수록 입원율이 높아졌다. 직업별로는 무직 및 기타직이 35.9로 가장 높았고, 전문, 관리 및 사무직이 가장 낮았다. 입원의료 이용자 1인당 연평균 입원일수는 총 22.5일이었으며, 성별로는 남자가 28.1일로 여자보다 많았다. 연령별로는 40대가 72.3일로 가장많았다. 직장, 직종 및 지역의료보험적용군이 28.8일, 월 30만원미만 소득군이 44.5일 그리고 무직 및 기타직이 21.9일로 가장 많았다. 대상자 1,000명당 연간 총 입원일수는 560일이었으며, 여자가 661일로 남자보다 많았으며, 연령별로는 70세이상군이 2,800일로 가장많았다. 의료보험 적용군이 비적용군보다 3배이상 많았으며, 직업별로는 무직 및 기타직이 789일로 가장 많았다. 2주간 병의원 이용여부를 종속변수로 한 지수형회귀분석에서 유의한 계수는 9세이하군(+), 70세이상군(+), 급성질병(+), 만성질병(+), 공 교의료보험적용(+), 직장, 직종 및 지역의료보험적용(+) 그리고 전문, 관리 및 사무직(-) 등이었다. 약국이용여부 분석에서는 9세이하군(+), 50-69세군(+), 70세이상군(+), 금성질병(+), 만상질병(+) 그리고 공.교의료보험적용(-) 등이 유의하였다. 2주간의 병의원 외래이용회수에 대한 중회귀분석에서는 급성질병(+), 만성질병(+) 직장, 직종 및 지역의료보험적용(+), 전문, 관리 및 사무직(-) 등이 유의한 변수였으며, 약국이용회수 분석에서는 급성질병(+), 만성질병(+), 공.교의료보험적용(-) 그리고 직장, 직종 및 지역의료보험적용(-)등이 유의하다. 연간 입원의료 이용여부에 대한 지수형 회귀분석에서 통계적으로 유의한 변수로는 9세이하군(+), 70세이상군(+), 만성질병(+), 공.교의료보험적용(+) 그리고 직장, 직종 및 지역의료보험적용(+) 등이었다.

치과의료기관 방문시 불만처리서비스에 대한 의료소비자의 평가 (Client Services Evaluation in Dentistry Sector)

  • The purpose of this study was to examine how clients who visited dental institutions perceived client services, what sorts of client services were provided to them and to what extent they were satisfied with them. It's ultimately meant to seek ways to enhance and enlarge client services. A survey was conducted on 379 residents who used dental institutions in Seoul and Gyeonggi provinces, and the findings of this study were as follows: 1. 64.6 percent of the people investigated weren't aware of client services, and 33.0 percent viewed them as part of medical services. 77.5 percent had grievances about dental institutions they'd ever used. 2. The biggest complaint was that the dental treatments they received weren't covered by the dental insurance. As for how to solve their grievances, the largest group of them told the employees of the dental institutions about their complaints or didn't use them again. When asked whether the dental institutions took any steps to get rid of their grievances, the greatest group replied they had no idea. and the second largest group answered they took no measure. 3. By age and educational level, those who were in their 60s and up and stopped at elementary school were best cognizant of client services. Among client service variables, they were most satisfied with how the employees handled their complaints, and as to overall satisfaction, their willingness to revisit ranked highest. 4. Concerning connections among client service awareness, service variables and overall satisfaction level, their awareness of client services had a positive correlational relationship with every service variable and satisfaction level. Among the service variables, prompt client services and employee attitude were positively correlated to overall satisfaction level, but service procedure, facilities and information services exercised little impact on that. 5. As for what factors affected their content with client services, their client service awareness was identified as one of the important factors to influence their use of dental institutions, the outcome of their visit and their willingness to revisit. The above-mentioned findings suggested that dental institutions should strengthen publicity activities to inform people of client services, and encourage them to express their grievances. In addition, they should take an immediate action to remove their complaints, and try to get a successful feedback to offer higher-quality medical services and customer-oriented services.

중년층의 구강건강지식, 실천 및 보철치료 만족실태 조사 (A study for middle-aged on oral health knowledge, oral health care and satisfaction with prosthetic treatment)

  • Objectives : The purpose of this study was to examine the oral health knowledge and actual oral health care of the selected subjects, their decision making about prosthetic treatment, the state and characteristics of their prosthetic treatment and their satisfaction with prosthetic treatment in an attempt to provide some information on the improvement of the quality of life related to oral health and the promotion of oral health. Methods : The subjects in this study were 250 people who received education in two different lifelong education institutions in the city of Busan. After a survey was conducted from May 23 to June 15, 2011, the answer sheets from 217 respondents were analyzed. Results : 1. As for the general characteristics of the respondents, the men(52.1%) outnumbered the women. Those who were in their 60 and up(47.5%) made up the largest age group, and the married people(65.4%) outnumbered the unmarried ones. By occupation, the company employees(20.3%) made up the biggest group. By education, the greatest group was high-school graduates(36.1%). By monthly mean income, the biggest group gained an income of 2.01 million won or more(36.9%). As to medical security, community-based insurance was most prevailing (36.9%). In terms of health promotion, the largest group worked out to stay fit(52.4%). 2. Regarding oral health knowledge, they had a good knowledge about the cause of dental caries(56.7%), but they weren't well aware of the right time for regular dental checkup(47.9%). In relation to oral health care, regular scaling wasn't prevailing(16.9%). 3. Regarding connections between the general characteristics and satisfaction with prosthesis, the less-educated respondents expressed significantly more dissatisfaction(p=0.015). By monthly mean income, those who had a smaller income were dissatisfied in general(p=0.028). Conclusions : The findings of the study illustrated that it's required to spread awareness of the importance of oral health among people in general, and that differentiated incremental oral health care should be provided for different age groups. In order to raise the satisfaction of patients with prosthesis, how to relieve their pain and anxiety and how to adjust prosthetic treatment cost properly should carefully be considered. In addition, the government should take measures to offer assistance for the low-income classes in preparation for an increase in the elderly population.

구조방정식모형을 이용한 사무직 공부원의 직업관련 특성과 직무스트레스가 사회심리적 건강상태에 미치는 영향 (Effects on the Psychosocial Health Status of Job Stress and Job-related Characteristics among Clerical Public Officers using the Structural Equation Model)

  • 본 연구는 공무원들의 스트레스나 피로수준과 같은 사회심리적 건강상태가 그들의 직급, 근무경력 등의 직업관련 특성이나 업무요구도, 업무자율성, 직장 내에서의 사회적 지지와 같은 직무스트레스 내용에 의해 영향을 받고 있는지를 구조방정식모형을 통하여 검토하고자 시도하였다. 연구대상은 대전광역시의 5개 정부 공공기관에 근무하는 일반 사무직 행정공무원 805명이었으며, 자료 수집은 2009년 10월 1일부터 11월 30일까지의 기간 동안에 표준화된 자기기입식 설문지(self-administered questionnaire)를 사용하여 실시하였다. 연구결과, 공무원들의 스트레스 및 피로수준은 직급, 근무경력, 업무만족도, 업무자율성, 상사의 지지도 및 동료의 지지도와는 유의한 음의 상관관계를 나타낸 반면, 년 간 결근일수, 업무요구도와는 유의한 양의 상관관계를 나타내었다. 구조방정식모형 분석 결과 연구대상자들의 직무스트레스 요인은 직업관련 특성 요인보다 사회심리적 건강상태에 더 큰 영향을 미쳤으며, 직무스트레스가 높을수록 사회심리적 건강상태를 나타내는 스트레스와 피로수준을 높여주는 것으로 나타났다. 한편, 직업관련 특성의 인지정도가 높을수록 직무스트레스 요인을 감소시키는 효과가 있는 것으로 나타났다. 따라서 공무원들의 스트레스나 피로를 감소시키기기 위해서는 직업관련 특성에 따른 직무내용의 개선을 위한 프로그램의 개발 및 시행이 필요할 것으로 생각된다.

보건진료소 관할 농촌지역 주민의 고혈압 유병률, 건강행태 및 고혈압 관리 양상 (The Prevalence, Health behaviors, and Control of Hypertension in Rural Areas in Korea)

  • Purpose: The purpose of this study was to identify the prevalence, health behaviors, and control of hypertension in rural areas in Korea. Method: A total of 927 subjects above age 20 were selected from the areas which fell under the jurisdiction of the 24 Community Health Center located in Chungcheongbuk-do. The employees in the Community Health Center visited and interviewed patients. Data were collected using a questionnaire from July to October 2002. Result: The result showed that women had higher hypertension prevalence rates than men and the increment of its rate leveled up according to age. The hypertension prevalence rate was significantly high when the monthly income was less than 1 million won, their type of the Medical Insurance was the Guardian, they were only able to read Korean characters, and they were bereaved of spouses. In the Health behavior related to hypertension, the hypertension group showed a significantly lower level than the non- hypertension group in terms of drinking rate, smoking rate, intake of salt and intake of meat. There was no significant difference in the exercise rate and coffee intake rate between these groups. In the degree of the obesity the hypertension group was significantly higher than the non-hypertension group. The factors related to hypertension were that the duration for the incidence of hypertension was 12 to 60 months and took up 41.2%. The places where the medical check-ups occurred were the Community Health Center at 46.6%, the medical institution was mostly hospitals recording 46.5%. There were 66.1% of the targets who knew well about their blood pressure and there were 64.7% people who received education about it. As for the education place, the rate of Community Health Center was mostly high and it stood at 77.0%. In the aspect of the management of hypertension, the targets who took medicine on a regular basis were up to 76.1% and the targets who measured blood pressure once a month happened to be about 46.1 %. The targets who always recorded their blood pressure were 3.8%, chest X-ray as a related examination of hypertension reached 32.6%, electrocardiogram examination was 36.2%, cholesterol and serum lipid examination took up 33.6%, and the eye ground examination took 7.3%, which showed the lowest level of all.

대전지역 일부 직장인의 혈압, 혈액성분 및 체위의 변화 (Changes in Blood Pressures, Blood Profiles and Physical Conditions among Adults in the Daejeon Area)

  • The body mass index, blood pressures and blood profiles of adults, who worked at three colleges in the Daejeon area, were evaluated for use in developing well-balanced menus at their workplace cafeterias and/or in creating nutrition education programs tailored to their health statuses. The data analyzed were from physical examinations performed in 2002 and in 2004 by the National Health Insurance Corporation. Overall, levels of systolic and diastolic blood pressure and serum cholesterol were lower in the 2004 samples than in those of 2002, but most samples in 2004 showed higher levels of SGOT. In 2002, statistical differences occured in the levels of blood pressure and serum cholesterol among age groups, but not in 2004. And in 2004, the older the age group, the higher the BMI. Among subjects in their twenties, three variables (systolic and diastolic blood pressure, and serum cholesterol) showed statistical differences; this increased to four variables (systolic and diastolic blood pressure, blood glucose and serum cholesterol) among subjects in their thirties and to six variables (Hb, SGOT, systolic and diastolic blood pressure, blood glucose and serum cholesterol) among those in their forties, followed by a decline to four variables (systolic and diastolic blood pressure, Hb and serum cholesterol) among subjects in their fifties. In general, the results of physical examinations showed improvements in blood pressure and blood profiles in 2004 in comparison with results in 2002. The level of SGOT was the exception. Despite these improvements, of those employees who had physical examinations in 2004, $5.2\%$ had high blood glucose and $18.1\%$ hypertension, $27.9\%$ hypercholesterolemia, $8.3\%$ registered abnormal liver functions, and $31.3\%$ were classified as overweight and obese. Moreover, because some vulnerable blood profiles were found in the older age groups, various programs, including nutrition education, and/or development of healthful diets as well as excercise activities, may be needed within their workplaces.

한 전자제품 연구소 남자 종사자들의 생활습관실천과 대사증후군의 관련성 (Lifestyle and Metabolic Syndrome among Male Workers in an Electronics Research and Development Company)

  • Objectives : The objectives of this study were to determine the relationship between lifestyle-implementation and metabolic syndrome in an electronics research and development company, and to provide a foundation for health providers of health management programs for setting priorities. Methods : From July 1 to July 16, 2008 we carried out a descriptive cross-sectional survey. Consecutive workers of one R & D company in Seoul, Korea (N=2,079) were enrolled in study. A checklist for lifestyle (from the National Health Insurance Corporation) consisted of questions regarding diet, drinking, smoking and exercise. After the survey, researchers obtained data from health profiles for metabolic syndrome(waist-circumference, triglycerides, HDL cholesterol, blood pressure and fasting blood sugar level). Lifestyle was recorded as good or not good. Statistical analysis of metabolic syndrome and the lifestyle of subjects was done using multiple logistic regression analysis. Results : The prevalence of metabolic syndrome in our study gropu was 13.3% (N=277). After adjustment for age, the adjusted odds ratios (odds ratio, 95% confidence intervals) for metabolic syndrome increased in proportion to the number of bad habits: two (1.72, 1.23-2.44), three (2.47, 1.73-3.56), and four (3.63, 2.03-6.34). Relative to subjects eating both vegetables and meat', the OR for 'meat' eaters was 1.66 (1.18-2.31). Compared with 'nonsmokers and ever-smoker', the OR for 'current-smoker' was 1.62 (1.25-2.10). Compared with 'Healthy drinker', the OR for 'unhealthy drinker' was 1.38 (1.05-1.83). Conclusions : Poor lifestyle was associated with an increased likelihood of metabolic syndrome. These findings suggest that lifestyle-based occupational health interventions for young employees should include a specific diet, smoking cessation, and healthy-drinking programs.

국회 청원제도를 통한 보건의료 입법사례 연구: 13-20대 보건복지위원회 청원을 중심으로 (Healthcare Legislation Cases in the National Assembly Petition System: Focused on Petitions to the Health and Welfare Committee of the 13th National Assembly through the 20th Assembly)

  • The right to petition is a classical right of the people in constitutional states, and in Korea, it is a statutory right in the Constitution, the National Assembly Law, the Petition Law, and the Local Autonomy Act. The healthcare community first made a successful petition to the National Assembly when it achieved the amendment of the Government Organization Act through a petition to the National Assembly for the independence of the Ministry of Health, and this achievement served as the basis for further petitions. Since then, the healthcare community has successfully achieved the enactment and amendment of related occupational laws through National Assembly petitions, such as the amendment of Article 41, Paragraph 7 of the former Medical Insurance Act (Korean Medical Association, 14th Assembly), enactment of the Dental Health Act (Korean Dental Association, 15th Assembly), and amendment of the Health Functional Foods Act (Korea Pharmaceutical Association, 16th Assembly). Its petition accomplishment rate is higher than the total petition accomplishment rate of the Health and Welfare Committee of the National Assembly. However, along with the overall decrease in the number of National Assembly petitions, the Korean Medical Association and Korea Pharmaceutical Association have not achieved any results through petitioning since the 16th Assembly (June 2000), and the Korean Hospital Association and Korean Nurses Association have not achieved any results through petitioning since the 17th Assembly (April 2004). Furthermore, no National Assembly petitions have been made at all for 5 years (2014-2018). The Korean Medical Association and Korea Pharmaceutical Association previously showed a high petition accomplishment rate through their accumulated experience with National Assembly petitions and vigorous policy assistance from doctors/pharmacists/nurses turned lawmakers. More specifically, healthcare organizations have achieved results by actively conducting organized activities with the National Assembly, as implemented by a national assembly director and employees, and in case of petitions for legislation, each group has established infrastructure for reviewing the relevant laws by appointing a legislative director, as well as a legal advisor and advisory counsel. Although the organization that has submitted the most petitions to the National Assembly is the Korean Hospital Association, the group with the highest petition success rate is the Korean Medical Association, which may be linked to the relatively high proportion of doctors who have become lawmakers. Furthermore, the fact that other healthcare organizations were highly interested in petitioning the National Assembly has had major implications for the petition activities of healthcare organizations.
