신종감염병의 선진적 관리를 위한 제언 (Suggestions for Advancing the Control of Emerging Infectious Diseases)

  • Coronavirus disease 2019 (COVID-19), which originated in Wuhan, China, is pandemic. It has occurred in more than 170 countries on six continents. In Korea, COVID-19's cases are more than 9,000. The reasons of pandemic COVID-19 are that COVID-19 can spread asymptomatic or early in symptoms although similar reproductive number to severe acute respiratory syndrome coronavirus (SARS), that there are more travelers in China and world than the SARS of 2003, and that the control of COVID-19 was contaminated to political considerations in China, World Health Organization (WHO), and Korea. Emerging infectious diseases such as COVID-19 will occur in the future. To prepare for control of emerging infectious disease, first, the Ministry of Health should be independent from Ministry of Health and Welfare and the department of disease management should be established. Second, experts for making evidence about the emerging infectious diseases should be trained and the plan that is the mobilization of manpower and facilities in large cases will be established. Third, the WHO should enhance its capacity to manage emerging infectious diseases and Korea will support the country of occurred emerging diseases through experts in the analysis of emerging infectious diseases.

  • Antimicrobial resistance and emerging infectious diseases, including avian influenza, Ebola virus disease, and Zika virus disease have significantly affected humankind in recent years. In the premodern era, no distinction was made between animal and human medicine. However, as medical science developed, the gap between human and animal science grew deeper. Cooperation among human, animal, and environmental sciences to combat emerging public health threats has become an important issue under the One Health Initiative. Herein, we presented the history of One Health, reviewed current public health threats, and suggested opportunities for the field of public health through better understanding of the One Health paradigm.

  • The emergence of viral diseases transmitted is nowadays a central problem in the world. Problem, which is becoming very critical in developing countries, where the health systems are not yet enough developed to face the bursting of such diseases. Emerging viral diseases constitute one of the major threats to human being that are arising in the modern world. Besides bio-chemical and medical researches, new orientations are developed to understand the environmental dimensions of such emergence. Questions concerning the inter-plays between the environmental and disease dynamics are building up new investigations, both in remote sensing and GIS, for the elaboration of levels of organization of space and environment in relation to incidences, to gain understandings in these issues. Environmental attributes attached to land cover types: area, spatial heterogeneity and physical state, are derived from remote sensing and applied to uncover related dimensions of the Dengue disease.

심혈관계 질환 진단을 위한 복합 진단 지표와 출현 패턴 기반의 분류 기법 (Multi-parametric Diagnosis Indexes and Emerging Pattern based Classification Technique for Diagnosing Cardiovascular Disease)

  • 심혈관계 질환의 진단 위해서 복합 진단 지표를 이용한 출현 패턴 기반의 분류 기법을 제안하였다. 복합 진단 지표 적용을 위해서 심박동변이도의 선형/비선형적 특징들을 세 가지 누운 자세에 대해 분석하였고 ST-segments로부터 4개의 진단 지표를 추출하였다. 이 논문에서는 질환진단을 위해서 필수 출현 패턴을 이용한 분류 모델을 제안하였다. 이 분류 기법은 환자 그룹의 질환 패턴들을 발견하며, 이러한 출현 패턴은 심혈관계 질환 환자들에서는 빈발하지만 정상인 그룹에서는 빈발하지 않는 패턴들이다. 제안된 분류 알고리즘의 평가를 위해서 120명의 협심증(AP: angina pectrois) 환자, 13명의 급성관상동맥증후군(ACS: acute coronary syndrome) 환자 그리고 128명의 정상인 데이터를 사용하였다. 실험 결과 복합 지표를 사용하였을 때, 세 그룹의 분류에 대한 정확도는 약 88.3%였다.

대학생의 건강증진 생활양식, 신종전염병 감염에 대한 건강신념 및 위생행위 간의 관계 (Relationships between Health Promoting Lifestyle, Health Belief about Emerging Infectious Disease and Hygiene Behavior of College Students)

  • 본 연구의 목적은 대학생의 건강증진 생활양식, 신종전염병에 대한 건강신념 및 위생행위 간의 관계를 확인하고자 수행하였다. 서울에 소재한 일개 대학에서 252명 대학생이 건강증진 생활양식, 신종전염병에 대한 건강신념 및 위생행위에 관한 질문을 포함하는 온라인 설문조사에 참여하였다. 연구결과, 첫째, 대학생의 성별과 지각된 건강상태에 따라 건강증진 생활양식에 유의한 차이를 보였고, 신종전염병 예방교육이 필요하다고 응답한 대학생과 그렇지 않은 대학생에서 위생행위에 유의한 차이를 보였다. 둘째, 대학생의 위생행위에 영향을 미치는 요인은 전공, 건강증진 생활양식 및 신종전염병에 대한 건강신념으로 나타났다. 이와 같은 결과를 토대로 대학생을 대상으로 하는 건강관련 교육을 적용함에 있어서 신종전염병에 관한 건강신념을 확립할 수 있는 교육내용을 포함할 필요가 있으며, 신종전염병을 예방할 수 있는 위생행위를 포괄적으로 다루는 프로그램을 개발할 필요가 있다.

신종 인플루엔자 대유행 개관 (Overview of Pandemic Influenza)

  • Influenza virus has a unique characteristics of annual epidemics of acute respiratory disease with attack rate of 10%-30% of the population. It is also the classical emerging infectious disease causing global pandemics when new antigenic shift occur. This antigenic shift is the key to its ability to evoke periodic pandemics, and it has caused at least 3 pandemics in 20th century. I reviewed these 3 pandemics in their natural courses and the epidemiology of the recent emerging influenza A viruses, especially the H5 and H7 subtypes. I descr ibed the epidemics of these vi ruses in human population and why we should be prepared to these viruses.

Factors influencing nursing students' care intentions toward emerging infectious diseases patients: A descriptive-predictive study

  • Purpose: As coronavirus disease 2019 (COVID-19) and other emerging infectious diseases continue to threaten health security, a strategy is required to increase nursing students' care intention for patients with such diseases. This study aimed to identify factors influencing Korean nursing students' care intentions in regard to patients with emerging infectious diseases. Methods: The care intention, COVID-19 knowledge level, ethical sensitivity, beliefs, attitudes toward the care intention, subjective norms, and perceived behavioral control were investigated using the theory of planned behavior. An online survey was completed from December 2020 to January 2021 by 227 nursing students who had complete a clinical practicum. Descriptive statistics, correlation, and hierarchical regression analyses were conducted on the data. Results: The care intention was higher in men than in women nursing students and in junior than in senior students. Behavioral and normative beliefs, attitude toward the behavior, and perceived behavioral control significantly predicted care intention. Ethical sensitivity increased the predictability of nursing students' care intentions for emerging infectious disease patients. Conclusion: The theory of planned behavior predicted nursing students' care intentions for emerging infectious diseases. Therefore, an experience-based response program on emerging infectious diseases is required for nursing students.

A Case-Study of Implementation of Improved Strategies for Prevention of Laboratory-acquired Brucellosis

  • Background: In 2012, the Alaska Section of Epidemiology investigated personnel potentially exposed to a Brucella suis isolate as it transited through three laboratories. Methods: We summarize the first implementation of the United States Centers for Disease Control and Prevention 2013 revised recommendations for monitoring such exposures: (1) risk classification; (2) antimicrobial postexposure prophylaxis; (3) serologic monitoring; and (4) symptom surveillance. Results: Over 30 people were assessed for exposure and subsequently monitored for development of illness. No cases of laboratory-associated brucellosis occurred. Changes were made to gaps in laboratory biosafety practices that had been identified in the investigation. Conclusion: Achieving full compliance for the precise schedule of serologic monitoring was challenging and resource intensive for the laboratory performing testing. More refined exposure assessments could inform decision making for follow-up to maximize likelihood of detecting persons at risk while not overtaxing resources.

  • Coronavirus disease 2019 (COVID-19), which causes serious respiratory illness such as pneumonia and lung failure, was first reported in Wuhan, the capital of Hubei, China. The etiological agent of COVID-19 has been confirmed as a novel coronavirus, now known as severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which is most likely originated from zoonotic coronaviruses, like SARS-CoV, which emerged in 2002. Within a few months of the first report, SARS-CoV-2 had spread across China and worldwide, reaching a pandemic level. As COVID-19 has triggered enormous human casualties and serious economic loss posing global threat, an understanding of the ongoing situation and the development of strategies to contain the virus's spread are urgently needed. Currently, various diagnostic kits to test for COVID-19 are available and several repurposing therapeutics for COVID-19 have shown to be clinically effective. In addition, global institutions and companies have begun to develop vaccines for the prevention of COVID-19. Here, we review the current status of epidemiology, diagnosis, treatment, and vaccine development for COVID-19.

코로나19의 증거생성 질병관리 (Evidence-Developing Disease Control of Coronavirus Disease 2019)

  • Coronavirus disease 2019 (COVID-19) has been going on for nearly a year. As of December 1, 2020, more than 60 million confirmed cases and 1.4 million deaths have occurred in 220 countries and territories. There is no effective treatment for COVID-19 so far, and vaccines are being developed. However, the vaccine of COVID-19 can be freed from COVID-19 only if there are positive answers to some questions: "Aren't there any major side effects from the vaccine?"; "Is the vaccine effective for enough time?"; "Does the vaccine sufficiently reduce the mortality rate?"; and "Does the vaccine sufficiently reduce the reproduction number?" The fight against COVID-19 will continue for a long time. Therefore, we need to respond by developing the evidence for control of COVID-19, the emerging infectious disease. Extensive quarantine measures in the early stages of COVID-19 need to be elaborated through developing the evidence. Korea Disease Control and Prevention Agency should establish a database for evidence developing COVID-19 control. In this database, based on more than 3 million tested individuals, epidemiological investigation results, health insurance claims data, and government data such as the Ministry of Public Administration and Security, etc. should be linked. It is necessary to disclose this database to academia, etc. to create the evidences of COVID-19 and manage COVID-19 through these. And through the developed evidences, guidelines for physical health and mental health in the era of Corona 19 should be developed.
