Study on Business Model of e-Call System and Feasibility Analysis (긴급구난체계(e-Call) 비즈니스 모델 개발 및 타당성 연구)

  • The number of deaths in Korea is higher than the OECD average. Therefore, an e-Call system is being developed as a vehicle ICT-based emergency rescue system that automatically detects an accident in the event of a vehicle accident and transmits related information to the center. In order to overcome the limitations of social acceptability and function of e-Call system, we propose a model that allows users to be aware of the necessity of service voluntarily. We predicted the market share of e-call services according to the proposed business model and analyzed it through B/C analysis. Benefits are calculated on a penetration basis, and device purchase and communications costs are calculated for each period. B/C analysis shows that pessimistic scenarios are 0.98 in 2025 and 1.01 in 2030. In an optimistic scenario, it is 1.05 in 2025 and 1.20 in 2030, which is more economical.

  • 엘리베이터와 화재경보기의 경우에는 비상 경보 시스템의 벨을 누르면 경비실과 관리실의 인터폰으로 연결되어 있으므로 컨트롤러를 설치하고 인터폰선에 직렬통신방식인 485통신을 이용해 송신하고 MMI에서 수신하고 인터넷으로 서버가 있는 곳으로 보내기 위해서 이더넷을 이용하는데 MMI와 이더넷 사이에 MUX를 설치해 경보 신호를 받아들이도록 한다. 이더넷을 통해 서버에 접속되어 서버에 있는 관리시스템에 의해 등록된 관계자의 핸드폰으로 SMS를 전송하고 인터넷이 설치하기 어려운 곳에서는 무선 모뎀을 설치하여 무선으로 서버에 접속하여 유선과 같은 방식으로 SMS를 전송하는 2중화로 시스템을 구성하였다.

  • On the internet phone or PSTN(Public Switched Telephone Network), the automatic callback is an useful service in the case of busy state when one user calls the other. By using this service, automatic redial is possible when the other party hangs up. However, in the basic automatic callback service, the user who wants callback should wait until the other party hangs up even in the case of emergency. Therefore in this paper, to solve this problem we have extended CPL(Call Processing Language) and, within user system we have included and linked this extended CPL processing module and dialog event package which processes SIP INVITE initiated dialog state informations. We have implemented this system for being used in SIP(Session Initiation Protocol)-based VoIP(Voice over IP) system.

  • In this paper, the u-Service system that is based on location-aware technology is designed and implemented for a silver town. On the real silver town the developed system has been employed and tested to provide convenient and secure living for residents. It provides services such as emergency call, intelligent elevator operation, and hands-free door access based on the location of the residents with personal device as called smart tag. It can also be applied to other service areas such as the location-aware u-Service for hospital, high-rising complex building, APT, etc.

Prehospital care after return of spontaneous circulation in out-of-hospital cardiac arrest patients: Based on Heart Saver laureate (병원 전 심정지 환자의 자발순환 회복에 관한 병원전 처치 - 하트세이버 수상자를 중심으로 -)

  • Purpose: We aimed to improve the survival rates of out-of-hospital cardiac arrest patients. Methods: We analyzed data regarding cardiopulmonary resuscitation (CPR) outcomes and clinical characteristics of out-of-hospital cardiac arrest patients. The data included prehospital emergency medical service reports of 207 patients, 135 patients of Heart Saver, who survived over 72 hours after return of spontaneous circulation (ROSC) in Gyeonggi-do from January, 2012 to December, 2013. Data were analyzed using SPSS 18.0 descriptive statistics. Results: Among patients who achieved ROSC, 87.6% were men and 73.6% were aged 41-70 years; 86.7% were cases of witnessed cardiac arrest, and cardiopulmonary resuscitation was performed by bystanders in 65.9% of cases. The initial electrocardiogram showed ventricular fibrillation or pulseless ventricular tachycardia in 96.3% of patients. The call time was 1.0 minutes, arrival time was 6.3 minutes, time spent at the scene was 8.0 minutes, hospital arrival time was 10.0 minutes, and total CPR duration was 9.6 minutes. The certificate of them was paramedics in 89.6%. Conclusion: To improve the survival rates of out-of-hospital cardiac arrest patients, standard prehospital care for these patients and educational programs regarding CPR for lay rescues should be developed.

The Chronic Health Effects of Work-Related Stressors Experienced by Police Communications Workers

  • Background: Law enforcement communications (i.e., 911 dispatch and call takers) is a challenging and stressful occupation. The purpose of this study is to identify the main stressors associated with employment in law enforcement communications, and to identify and provide context to how these stressors affect workers' health and wellbeing. Methods: This research study included focus groups with 23 call takers and 911 dispatchers employed by a large, urban law enforcement agency in 2018. Thematic analyses were conducted to identify trends. Results: Four themes of stressors emerged (i.e., the high stakes nature of some 911 calls for service, understaffing, supervisor-related stress, and recruiting practice). Two health-related themes emerged as being occupation-related: weight gain and poor sleep patterns/insufficient sleep). Specifically, participants reported negative eating habits resulting in weight gain and obesity, lack of sleep and irregular sleep schedules, and development of hypertension and/or diabetes since beginning their jobs. Conclusion: Law enforcement communications professionals experience a number of the same stressors facing law enforcement officers in patrol. These stressors, combined with the sedentary nature of the job, could result in long-term, chronic health problems.

Knowledge and Current Status about AED in the Public Facilities - Focused on the Gwangju City - (다중이용시설에서의 AED에 관한 지식 및 운영실태에 관한 연구 - 광주광역시 중심으로 -)

  • Purpose: In this study, we investigated the better application of the law which is about the AED installation and more effective ways of emergency medical care system, to understand the law and to research the current condition of public facilities which belong to local governments, and to seize the aspect of safety guards who currently work in order to provide the installation of AED in the public facilities and to provide more efficient emergency medical service with the effectuation of the immunity law of the good intention of first-aid treatment. Methods: In Gwang-ju, 234 public facilities have been identified by 31 December, 2008. With the exception of the duplication, we researched 158 facilities and received the answers from 95 of them. Results: In the research, 53% of them have had internal emergency first-aid education, and 55% of them didn't have this education and a CPR education manual, and 30% of the facilities even didn't know how to connect with the manager of the company for the first-aid department. On the other hand, most of them were highly interested in CPR and AED education on the ratio of 91% and 93%. 88% of them have been trained about first-aid, 51% of them haven't been retrained, 17% have never been trained. so, the reality of emergency system at public facilities is serious. 78% of them knew they are working at public facilities, though 49% of them didn't know about AED installation. 57% of them didn't know the fact there is the immunity law related with good intentions for first-aid treatment. 63% of the facilities have security guards, and 30% of them didn't answer the questions. Also, many of them agreed to the opinion that all employees should have first-aid training. At representative survey report of participator of public-facility, emergency treatment is 61%, 16% of patients calling. Accordingly they importantly think better doing an on-site first-aid than evacuating the patient. And the rates show that 57% of them answerers tend to call Fire-Office(119) for evacuating the patients, and 28% of them EMIC(1339) for the first-aid. Conclusions: In this study, we are suggest to improve the details of the efficient operations and management after the grasp of the uninstallation, indifference, and unreliable conditions of AED. 1) Need a publicity of AED install cognition which is an emergency medical instrument at public facilities. 2) Arrangement of safety agents at facilities and concerns about them for good management from the parties concerned. 3) Need a designation of legal details according to the decision of the AED installation and the standard of the AED installation. 4) Training about first-aid of safety guards and the persons concerned in the facilities should be practiced participation with the positive and through this, first-aid treatment could be done by anyone who knows the immunity law related to medical emergency. 5) The brochures for the potential users and the results form practicing the instructions need to be improved in many ways through recording the emergency cases that have happened.

  • This paper proposes a system that utilizes USN(Ubiquitous Sensor Network), Bluetooth and smart phone to improve the function of senior houses. In typical approach, a system in a senior house either directly accesses the status of elderly people by its sensor or is alerted by elderly people who trigger an emergency bell, derive a decision and take an appropriate action. In addition, it is possible for a designated social worker to check the status of senior patients through monitoring system connected by UTP(Unshielded Uwisted Pair) cables, but the responsible person has to be present to monitor patients' status. However, the new system, suggested in this paper, embed Bluetooth function in a blood pressure gauge, thus the smart phone receives patients' health information such as blood pressure through Bluebooth, if any abnormal event occurs. Consequently, the smart phone sends SMS(Short Message Service) to a responsible social worker or a designated hospital. When this program in the paper becomes a reality, an unmanned system that is able to determine suitable actions for certain events will be established, even if a social worker were absence.

  • The author Nakajima was involved in the field of disaster communications and emergency medical care as guest research scientist at the Fukushima Nuclear Accident Independent Investigation Commission established by the National Diet of Japan and reviewer of the Commission's report, and Kurokawa was the chairman of this Commission. Looking back over a decade, we are on the liability issue of bureaucrats and telecom operators, so it's becoming clear what was hidden at the time. The battery of NTT DoCoMo's mobile phone repeaters had a capacity of only about 24 hours, and communication failures increased after one day. The Government also failed to issue an announcement of "Vent from reactor" under the Telecommunications Act Article No. 129. This mistake lost the opportunity to use the third-party telecommunications (e.g. taxi radios). Furthermore, as a result of LASCOM (telecommunications satellite network for local governments via GEO) and a variety of unexpected communication failures, the evacuation order "Escape!" could not be notified to the general public well. As a result, the general public was exposed to unnecessary radiation exposure. Such bureaucratic slow action in emergencies is common in the response to the 2020 coronavirus.

A Case Report of ROSC for Out-of Hospital Cardiopulmonary Resuscitation: Based on one Area Heart Saver (병원 전 심폐소생술에 의한 자발순환 회복 8례: 일 지역 하트세이버 수여자를 기준으로)

  • This study was to investigate the cardiopulmonary resuscitation (CPR) outcomes and the clinical characteristics of outof- hospital cardiac arrest (OHCA) by analyzing date in one region. A total 8 patients, survived over 72 hours after ROSC. The cardiac arrest were caused by unknown (5 cases), cardiac disease (3 cases). The most common place of cardiac arrest was in the home (6 cases), witnessed cardiac arrest (8 cases), cardiopulmonary resuscitation by bystander was performed (4 cases), agonal respiration (3 cases). The initial electrocardiogram (ECG) showed ventricular fibrillation (VF) in 7 cases, pulseless electrical activity (PEA) other rhythms in 1 cases. Average time from call to scene arrival 6.1 minutes, call to hospital arrival 23.0 minutes, duration of total CPR was 8.7 minutes. The certificate of them were level 1 emergency medical technician (EMT) 6 cases, level 2 EMT 3 cases, total 3 members 7 cases. To improve the rate of survival for out-ofhospital cardiac arrest victims, development of continuous CPR educational program for lay rescue.
