Improvement of Damage Range Calculation for First Response to Chemical Accidents (화학사고의 일차 대응을 위한 피해영향범위 산정 개선 방안)

  • Calculation of the damage impact of chemical accidents is an important element in site, and the initial isolation distance and the protective action distances are significant factors in coping the chemical accident. In this study, three major cities that represent each Province were selected, and the safety distances were calculated considering regional climate conditions. The results were compared with the prescribed values in Emergency Response Guidebook. It is concluded that the regional meteorological conditions such as temperature, vapour pressure, relative humidity, wind speed, and cloud cover should be reflected in estimating the initial isolation distance and the protective action distance.

  • The natural hydrology and geological conditions of Xintian County was investigated, the development law of regional karst fissures was studied, the groundwater was collected and tested through a large-scale collection of groundwater to obtain the change law of chemical characteristics and water quality characteristics of groundwater, and the water quality evaluation was carried out for the regional karst groundwater in this paper. The results show that, the whole area is dominated by carbonate rock distribution areas, and the distribution of water systems is relatively developed. The strata are distributed from the Lower Paleozoic Cambrian to the Cenozoic Quaternary, and contain multiple first-order folds. The regional karst dynamic action is strong, and many tunnels or caves of different scales were shown, which are conducive to the enrichment of groundwater. Karst groundwater is neutral and alkaline water, the water is clear and transparent with good taste, and meets the national drinking water hygiene standards. The content of toxic trace elements and fluoride in the water source is generally lower than the limit value specified by the national standard and the accumulated toxic heavy metals is never found. The overall water quality is of good quality and suitable for the development and utilization of various purposes.

Long-Term Survival Benefit of the Bronchial Arterial Embolization for Patients Presenting with Non-Traumatic Hemoptysis in a District Emergency Center (권역 응급의료센터에 내원한 비외상성 객혈 환자에서 기관지 동맥 색전술의 장기 생존 효과)

  • Background : This study was conducted to evaluate the survival benefit of the bronchial arterial embolization (BAE) for patients presenting with non-traumatic hemoptysis. Methods : The clinical data were retrospectively collected from the medical records and the Order Communicating Systems (OCS). The information dealing with death was collected from national death certificates. After enrolled patients were divided with two group such as BAE group (patients who were managed with BAE) and non-BAE group (patients who were managed with conservative modality), the survival benefit of BAE was estimated during the observational period of 24 months through using the Kaplan-Meier survival graph and the Cox-proportional hazard regression analysis. Results : The number of total cases was 272. Of these, BAE group involved 63 and non-BAE group involved 209. 69 cases had the malignant pulmonary lesions, 149 cases had non-malignant chronic lung lesion such as the mycobacteria infection, fungus ball, or bronchiectasis (BE), and 54 cases had the other pathologic conditions. For each sub-groups such as 'malignant lung lesion' group, 'non-malignant chronic lung lesion' group as well as about all cases, the adjusted hazard ratios (HRs) of BAE for death was not significantly different compared to the conservative management. But the adjusted HRs as to underlying causes such as 'malignant lung lesion' group and 'the other conditions' group increased significantly compared to 'non-malignant chronic lung lesion' group. Conclusion : There was no significant survival benefit by BAE procedure on survival in patients presenting with non-traumatic hemoptysis.

A Real Situation Experimental Study on The Thermal Protection Performance of Firefighter Clothes and Gloves (소방방화복 및 소방장갑의 열 보호 성능에 대한 실제 화재 실험 연구)

  • Purpose: This study aimed to evaluate the thermal protective function of firefighter clothes and gloves through real scale fire simulations. Methods: Firstly, the fire simulation by real scale flame was performed for firefighter clothes. A manikin equipped with firefighter clothes was directly exposed to flames which energy average is 84 Kw/m2. for 22 seconds. Heat flux gauges attached on the body measured surface temperature elevation. Secondly, we also performed the other fire simulation by hot plate exposure to firefighter gloves. Firefighter gloves with heat flux gauges exposed hot plate which temperature is 300℃ in both dry and moist conditions. Primary outcome was surface temperature change of manikin body (first simulation) and hand (second simulation) over times. Results: In the first flame simulation, the surface temperature of face and shoulders elevated more rapidly comparing with the other body surface area when initial period of flame shutter open. After 18sec of shutter open, the surface temperature of upper trunk elevated rapildy. After shutter closure, high surface temperature kept continuously on right side of face and left shoulder. In the second hot plate simulation, fingers and palms showed higher surface temperature than the other areas of hands in the both dry and wet conditions. Conclusion: This study suggests that the real scale flame enables firefighter clothes to lose their heat protective function suddenly after 18 seconds. Additionally, the protective function of firefighter gloves were relatively weaker in the palmar side of fingers than the other parts of hand. There should be additional study for evaluate thermal protection performance of firefighter clothes. And, further effort for reinforce palmar side of fingers of firefighter gloves should be done.

Which Laypersons Want Cardiopulmonary Resuscitation Education? (일반인의 심폐소생술 교육 희망에 대한 영향 요인 분석)

  • Purpose: Few data exist regarding the cardiopulmonary resuscitation(CPR) education in relationship to characteristics of socio-economic status and health-medical conditions in Korea. The purpose of this study is to describe and analyze the characteristics of which laypersons want the CPR education. Methods: Based on a health survey of Incheon Metropolitan City adults(n=5,114), tests of the differences between the group that wants the CPR (n=1318) and the group that doesn't (n=3576), and a logistic regression analysis of two groups was performed on socio-economic status-gender, age, marital stats, education level, numbers of family members, and monthly household income-and health-medical conditions-diseases, accident experience, EMS(emergency medical system) experience, and health status. Results: Even the participation rate of the CPR education in Korea is only 4.3%, which is extremely lower than other developed countries, there are statistically significant differences between the group that wants the CPR and the group that doesn't on gender(p=0.005), age(p=0.000), education level(p=0.000), numbers of family members(p=0.000), monthly income(p=0.000). diseases(p=0.000). health status(p=0.042). Furthermore, age(OR: 1.025, p=0.000), education level(OR : 0.721, p=0.000), numbers of family members(OR: 0.809, p=0.000), and health status(OR: 1.077, p=0.000) are statistically significant factors on the wish for the CPR education. Conclusion: Accident and EMS experiences are not so much influential factors on that laypersons want the CPR education in Korea. Therefore, certain subgroups of laypersons such as high-risk patients and family members need targeted outreach programs in CPR education.

Comparison between Self-esteem and Health Promotion Behavior of Health Department and Non-Health Department College Students - Focused on comparison between Emergency Medical Technology Department Students and Engineering College Students - (보건계열과 비보건계열 대학생의 자아존중감과 건강증진행위에 관한 비교 연구 - 응급구조과와 공과계열 학생 간 비교 -)

  • Purpose: This study examines and compares the differences between self-esteem and health promotion behavior of health department and non-health department college students. Method: The research instruments were as follows : The first one is RSES developed by Rosenberg and translated into Korean by Jeon. The second one is HPLP developed by Walker et al, and including six subareas by Lee and modified to be suitable to Korean culture by Suh. Data were statistically analyzed with SPSS PC/12.0. Results : 1. Comparison of self-esteem had no significant difference(p=.190) and health promotion behaviors had significant differences(p=.000) between the two groups. 2. According to general characteristics and health-related characteristics, the differences of self-esteem in health department showed significant differences in grade(p=.020), sex(p=.000), age(p=.000), parents' economic conditions(p=.048), and smoking(p=.030) while non-health departments revealed significant differences in grade(p=.003), sex(p=.000), age(p=.000) and smoking(p=.001). 3. According to general characteristics and health-related characteristics, the differences of health promotion behaviors in health department showed significant differences in grade(p=.008), sex(p=.021), age(p=.000) and parents' economic conditions(p=.017) while non-health department revealed significant differences in sex(p=.000) and age(p=.000). 4. There were positive correlations between self-esteem, health promotion behaviors and subareas in health department (r=.422) and non-health department (r=.383). Conclusion: There were significant positive correlations between self-esteem and health promotion behaviors in health and non-health department college students. Therefore, health related programs should be developed to enhance self-esteem and health management by college students can influence themselves on self-esteem and health promotion behaviors.

Clinical characteristics of hereditary neuropathy with liability to pressure palsy presenting with monoparesis in the emergency department

  • Hereditary neuropathy with liability to pressure palsy (HNPP) is a rare neurological genetic disease caused by deletion of the peripheral myelin protein 22 gene and presents in childhood or young adulthood. We report four cases of HNPP with typical and rare presentations, reflecting the broad clinical spectrum of this disease. Two patients presented with mononeuropathies that are frequently observed in HNPP; the remaining two presented with bilateral neuropathy or mononeuropathy anatomically present in the deep layer. This reflects the broad clinical presentation of HNPP, and clinicians should differentiate these conditions in young patients with monoparesis or bilateral paresis. Although HNPP is currently untreatable, early diagnosis in the emergency department can lead to early detection, eventually resulting in less provocation and recurrence which may cause early motor nerve degeneration.

  • This paper deals with the electromagnetic clutch which stop the generator at emergency by using the engine power of generator. The electromagnetic field was analyzed using Flux-2d program with different conditions : voltage, air gap, coil locations. As a result, the maximum magnetic flux density of electromagnetic clutch occurred between the coil and wheel : 0.27[T], 0.41[T] at 12[V], 24[V]. The maximum flux density was at the center location of the coil.

  • In this research, we determine the proper number of entrances in the evacuation facilities for civil and military personnel. The current standards on the emergency evacuation facility only sets the number of possible evacuees and the according architectural floor area since the evacuation have been completed. However, other than static standards such as the floor area, there is needs to consider the dynamic standards along with allowable evacuation time such as the number of entrance, staircases and so on. Therefore, we need to consider the number of entrances and their location in order to account for the allowed evacuation time of particular number of people in a set area. The outcome could be contributed to the standards which should contain the dynamic conditions associated with evacuation.

Study on the Safety Evacuation of University Dormitory Occupants Based on Pathfinder

  • This study uses Pathfinder software to simulate and analyze the safe evacuation of university dormitory occupants. Fire safety issues in densely populated dormitory buildings are gaining increasing attention, and improving emergency evacuation efficiency is crucial for reducing the harm caused by fire incidents. The study focuses on a student dormitory building in a university, simulating different evacuation scenarios and analyzing the impact of factors such as evacuation routes, personnel distribution, exit width, and stair width on evacuation time. Based on the actual dormitory conditions, parameters such as gender ratio, height range, shoulder width, and walking speed of the occupants were set, and evacuation times in various scenarios were compared.The simulation results show that proper planning of evacuation routes and increasing stair width significantly reduced evacuation times. The study recommends that universities establish systematic emergency response plans, conduct regular evacuation drills, optimize student dormitory layouts, and consider increasing stair width in dormitory building designs to improve evacuation efficiency and safety.
