Design of Low-cost Automated Ventilator Using AMBU-bag (암부백을 이용한 저가형 자동 인공호흡기 설계 및 제작)

  • This study proposes the design and implementation of a low-cost emergency ventilator which can be helpful during the COVID-19 pandemic where the supply of automatic ventilators is not smooth compared with the urgent demand worldwide. Easy implementation and lower price were made possible by using AMBU-bag and off-the-shelf embedded micro-controller board. Moreover, while 3D printing is used by companies and experts around the world to build prototype hardware, materials which are readily available from surrounding environments so that people in countries where it is difficult to access many advanced technologies could manufacture the system. The design features AMBU-bag automation, not use 3D printing, and it can contrl speed. By allowing speed control, ventilation can be performed according to the conditions of the patient being used. A complementary point in the study is that it is difficult to fix the start point of the wiper motor used first. A method for complementing this is a method for replacing the brush DC motor with a position feedback function. Secondly, the AMBU-bag may wear out in the long-term process of compressing the AMBU-bag because the arm and the fixing frame are made of wood. To complement this, the part of fixing frame and arm parts that the AMBU-bag touches need to be wrapped in a material such as silicon to minimize friction.

Transcholecystic Duodenal Drainage as an Alternative Decompression Method for Afferent Loop Syndrome: Two Case Reports (들장관증후군의 대체 감압 치료로서 경담낭 십이지장 배액술: 두 건의 증례 보고)

  • Afferent loop syndrome (ALS) is a rare complication of gastrectomies and gastrointestinal reconstruction. This can predispose patients to fatal conditions, such as cholangitis, pancreatitis, and duodenal perforation with peritonitis. Therefore, emergency decompression is necessary to prevent these complications. Herein, we report two cases in which transcholecystic duodenal drainage, an alternative decompression treatment, was performed in ALS patients without bile duct dilatation. Two patients who underwent distal gastrectomy with Billroth II anastomosis sought consultation in an emergency department for epigastric pain and vomiting. On CT, ALS with acute pancreatitis was diagnosed. However, biliary access could not be achieved because of the absence of bile duct dilatation. To overcome this problem, a duodenal drainage catheter was placed to decompress the afferent loop after traversing the cystic duct via a transcholecystic approach. The patients were discharged without additional surgical treatment 2 weeks and 1 month after drainage.

A Study to Improve the Vessel Navigation Suspension Order System (항행정지명령 제도의 개선방안에 관한 연구)

  • With the introduction of the maritime safety supervisor system in Korea following the passenger vessel Sewol accident, the safety control system for coastal vessels has been reinforced. A maritime safety supervisor performs marine accident prevention activities by offering periodical or occasional guidance and supervision. When a deficiency is found during guidance or supervision sessions, an order for improvement is issued to rectify the problem by such measures as supplementation or replacement of vessel facilities, improvement of work hours and working conditions for employees, educational and training programs for executives and employees, or the improvement of other affairs relating to maritime safety control. However the present order for navigation suspension by a maritime safety supervisor is limited solely to supplementation or replacement. For this reason, orders for navigation suspension against other facilities is impossible, even if a serious deficiency that could lead to a marine accident such as poor cargo loading conditions or unqualified crew is identified. Therefore, the order for vessel navigation suspension to secure seaworthiness should be expanded to include cargo loading/unloading operations, certificates and documentation, maritime safety management systems, and human elements including emergency drills and working conditions, in addition to the supplementation or replacement of vessel facilities that exists.

Treatment of Complete Scalp Avulsion with the Conditions of Unstable Vital Signs: A Case Report (불안정한 생체징후 하의 두피 결출상 환자의 치험례)

  • Purpose: Scalp avulsion is a life-threatening injury that may cause trauma to the forehead, eyebrows, and periauricular tissue. It is difficult to treat scalp avulsion as it may lead to severe bleeding. Therefore, emergency scalp replantation surgery is necessary, and we must consider the function, aesthetics, and psychology of the patients. A case of scalp avulsion leading to massive bleeding was encountered by these authors, which led to a failure to achieve the proper operation conditions in an adequate time period. Methods: A 49-year-old female was hospitalized due to having had her head caught in a rotatory machine, causing complete scalp avulsion which included the dorsum of the nose, both eyebrows, and ears. Emergent microsurgical replantation was performed, where a superficial temporal artery and a vein were anastomosed, but the patient's vital signs were too unstable for further operation due to excessive blood loss. Three days after the microanastomosis, venous congestion developed at the replanted scalp, and a medicinal leech was used. Leech therapy resolved the venous congestion. A demarcation then developed between the vitalized scalp tissue and the necrotized area. Debridement was performed 2 times on the necrotized scalp area. Finally, split-thickness skin graft with a dermal acellular matrix ($Matriderm^{(R)}$) was performed on the defective areas, which included the left temporal area, the occipital area, and both eyebrows. Results: The forehead, vertex, right temporal area, and half of the occipital area were successfully replanted, and the hair at the replanted scalp was preserved. As stated above, two-thirds of the scalp survived; the patient could cover the skin graft area with her hair, and could wear a wig. Conclusion: Complete scalp avulsion needs emergent replantation with microsurgical revascularization, but it often leads to serious vital conditions. We report a case with acceptable results, although the microanastomosed vessel was minimal due to the patient's unstable vital signs.

P-y Curves from Large Displacement Borehole Testmeter for Railway Bridge Foundation (장변위공내재하시험기를 이용한 철도교 기초의 P-y곡선에 관한 연구)

  • The lateral stability of bridge foundations against train moving load, emergency stopping load, earthquakes, and so on is very important for a railway bridge foundation. A borehole test is much more accurate than laboratory tests since it is possible to minimize the disturbance of ground conditions on the test site. The representative borehole test methods are Dilatometer, Pressuremeter and Lateral Load Tester, which usually provide force-resistance characteristics in elastic range. In order to estimate P-y curves using those methods, the non-linear characteristics of soil which is one of the most important characteristics of the soil cannot be obtained. Therefore, P-y curves are estimated usually using elastic modulus ($E_O$, $E_R$) of lateral pressure-deformation ratio obtained within the range of elastic behavior. Even though the pile foundation is designed using borehole tests in field to increase design accuracy, it is necessary to use a higher safety factor to improve the reliability of the design. A Large Displacement Borehole Testmeter(LDBT) is developed to measure nonlinear characteristics of the soil in this study. P-y curves can be directly achieved from the developed equipment. Comparisons between measured P-y curves the LDBT developed equipment, theoretical methods based on geotechnical investigations, and back-calculated P-y curves from field tests are shown in this paper. The research result shows that the measured P-y curves using LDBT can be properly matched with back-calculated P-y curves from filed tests by applying scale effects for sand and clay, respectively.

  • Resilience is defined as the intrinsic ability of a system to adjust its functioning prior to, during, or following changes and disturbances, so that it can sustain required operations or functions with the related systems under both expected and unexpected conditions. Resilience engineering is a relatively new paradigm for safety management that focuses on how to cope with complexity under pressure or disturbance to achieve successful functioning. This study aims to develop a quantitative resilience model for severe accident response organizations of nuclear power plants using the Analytic Hierarchy Process (AHP) method. First, we investigated severe accident response organizations based on a radiation emergency plan in the Korean case and developed a qualitative resilience model for the organizations with resilience-influencing factors, which have been identified in the author's previous studies. Then, a quantitative model for entire severe accident response organizations was developed by using the Analytic Hierarchy Process (AHP) method with a tool for System Dynamics. For applying the AHP method, several experts who are working on implementing, regulating or researching the severe accident response participated in collecting their expertise on the relative importance between all the possible relations in the model. Finally, a sensitivity analysis was carried out to discuss which factors have the most influenceable on resilience.

Static performance of a new GFRP-metal string truss bridge subjected to unsymmetrical loads

  • A unique lightweight string truss deployable bridge assembled by thin-walled fiber reinforced polymer (FRP) and metal profiles was designed for emergency applications. As a new structure, investigations into the static structural performance under the serviceability limit state are desired for examining the structural integrity of the developed bridge when subjected to unsymmetrical loadings characterized by combined torsion and bending. In this study, a full-scale experimental inspection was conducted on a fabricated bridge, and the combined flexural-torsional behavior was examined in terms of displacement and strains. The experimental structure showed favorable strength and rigidity performances to function as deployable bridge under unsymmetrical loading conditions and should be designed in accordance with the stiffness criterion, the same as that under symmetrical loads. In addition, a finite element model (FEM) with a simple modeling process, which considered the multi segments of the FRP members and realistic nodal stiffness of the complex unique hybrid nodal joints, was constructed and compared against experiments, demonstrating good agreement. A FEM-based numerical analysis was thereafter performed to explore the effect of the change in elastic modulus of different FRP elements on the static deformation of the bridge. The results confirmed that the change in elastic modulus of different types of FRP element members caused remarkable differences on the bending and torsional stiffness of the hybrid bridge. The global stiffness of such a unique bridge can be significantly enhanced by redesigning the critical lower string pull bars using designable FRP profiles with high elastic modulus.

  • Crohn' disease is an auto-immune disease characterized by intermittent chronic diarrhea, high fever, weight loss, abdominal spastic pain or abdominal discomfort which is followed by granulomatous necrosis and cicatrical inflammation. It is also called segmental enteritis or granulomatous enteritis. In western medicine the exact cause is undefined, however it is presumed as an immunological unbalance in alimentary tract commoonly occured in ileum portion of small intestitine or ascending colon and therefore immuno suppressive agents(usually steroids) and anti-inflammatory drugs are prescribed. In case of emergency such as ileus, perforation of intestinal wall surgical methods are considered. In oriental medicine this falls under the category of diarrhea(泄瀉), dysentery(痢疾), splenic diarrhea(脾泄). As to the pathological mechanism the abnormal ascending and descending circulation of stomach and splenic energy(脾不升淸, 胃不下降) the hepatic stagnation(肝鬱氣滯) and dysfunction of small intestine in expelling urine and feces(小陽淸獨不利) all together causes such condition. Main treatments are inducing diuresis(利小便), warming kindey to reinforce yang(溫賢助陽), nourishing the middle energy to invigorate spleen(補中健脾), elimination of the dampness by cooling(淸熱燥濕). In this case the patient was diagnosed as soyangyin(少陽人) constitution and herb medicine soyangyin Hyongbangjihwan-tang(少陽人 荊防地黃湯), Sa-am acupuncture Sojangjeonggyeok(小腸政格) was applied. There was an significant improve in chief complaints and general conditions.

A Study to Assess the Need of Customized Visiting Health Care Services for Children according to Socioeconomic Status in a Province (사회계층에 따른 영유아 맞춤형 방문건강관리사업 요구도)

  • Purpose: This study was conducted to survey children's health status and need of customized visiting health care services in one province. Methods: The participants in this study were 237 caregivers of infants and preschoolers. Data were collected at the participant's home or public health center. Results: Many of the children did not receive developmental screening tests or dental examinations. In the beneficiary group, the prenatal checkup rate and children's vaccination rate were lower, and caregivers had more health problems than the other groups. On the assessment of home safety, unsafe conditions were more frequently found in the beneficiary group. The caregivers in the beneficiary group showed lower child rearing confidence than the other groups, and wanted customized visiting health care services most in the areas of developmental screening, regular health check-up and counseling, nutritional supplementation, and oral health care. Conclusion: These results indicate that it is necessary for children and parents in poverty to be provided with professional home visiting interventions for the promotion of child health and prevention of developmental problems and diseases. These findings can be used for developing future customized visiting health care service programs for infants and children in this community area.

Which Emphasizing Factors Are Most Predictive of Hematoma Expansion in Spot Sign Positive Intracerebral Hemorrhage?

  • Objective : The spot sign is related with the risk of hematoma expansion in spontaneous intracerebral hemorrhage (ICH). However, not all spot sign positive patients undergo hematoma expansion. Thus, the present study investigates the specific factors enhancing the spot sign positivity in predicting hematoma expansion. Methods : We retrospectively studied 316 consecutive patients who presented between March 2009 to March 2011 with primary ICH and whose initial computed tomography brain angiography (CTA) was performed at our Emergency Department. Of these patients, 47 primary ICH patients presented spot signs in their CTA. We classified these 47 patients into two groups based on the presence of hematoma expansion then analyzed them with the following factors : gender, age, initial systolic blood pressure, history of anti-platelet therapy, volume and location of hematoma, time interval from symptom onset to initial CTA, spot sign number, axial dimension, and Hounsfield Unit (HU) of spot signs. Results : Of the 47 spot sign positive patients, hematoma expansion occurred in 26 patients (55.3%) while the remaining 21 (44.7%) showed no expansion. The time intervals from symptom onset to initial CTA were $2.42{\pm}1.24$ hours and $3.69{\pm}2.57$ hours for expansion and no expansion, respectively (p=0.031). The HU of spot signs were $192.12{\pm}45.97$ and $151.10{\pm}25.14$ for expansion and no expansion, respectively (p=0.001). Conclusions : The conditions of shorter time from symptom onset to initial CTA and higher HU of spot signs are the emphasizing factors for predicting hematoma expansion in spot sign positive patients.
