Recently, there has been increasing concern regarding the collapse of hydraulic structures due to abnormal climate conditions. Therefore, numerous studies of the collapse of hydraulic structures have been carried out. In particular, the velocity of the propagation of a flood wave-front is important for predicting the inundation safety and establishing an EAP (Emergency Action Plan). Although many hydraulic tests have been conducted for precise predictions of a flood wave-front, the scale effect from downsizing has not considered. In this study, the relationships between surface tension and the concentration of surfactant, between surface tension and the velocity of flood wave propagation, and between surface tension and the Weber Number were derived through hydraulic tests using a surfactant and image analysis equipment. Based on these relations, the modification factor for the scale effect of the front of flood wave propagation was suggested. The results highlight the necessity of a modification factor when the Weber Number is lower than 12.2, but the scale effect can be ignored when the Weber Number over 12.2.

Both metabolic syndrome (MetS) and elevated LDL cholesterol (LDL-C) increase the risk for cardiovascular disease (CVD). We hypothesized that low HDL cholesterol (HDL-C) would further increase CVD risk in women having both conditions. To assess this, we recruited 89 women with MetS (25-72 y) and LDL-C ${\geq}$ 2.6 mmol/L. To determine whether plasma HDL-C concentrations were associated with dietary components, circulating atherogenic particles, and other risk factors for CVD, we divided the subjects into two groups: high HDL-C (H-HDL) (${\geq}$ 1.3 mmol/L, n=32) and low HDL-C (L-HDL) (< 1.3 mmol/L, n=57). Plasma lipids, insulin, adiponectin, apolipoproteins, oxidized LDL, Lipoprotein(a), and lipoprotein size and subfractions were measured, and 3-d dietary records were used to assess macronutrient intake. Women with L-HDL had higher sugar intake and glycemic load (P< 0.05), higher plasma insulin (P< 0.01), lower adiponectin (P< 0.05), and higher numbers of atherogenic lipoproteins such as large VLDL (P < 0.01) and small LDL (P<0.001) than the H-HDL group. Women with L-HDL also had larger VLDL and both smaller LDL and HDL particle diameters (P<0.001). HDL-C was positively correlated with LDL size (r=0.691, P<0.0001) and HDL size (r=0.606, P<0.001), and inversely correlated with VLDL size (r=-0.327, P<0.01). We concluded that L-HDL could be used as a marker for increased numbers of circulating atherogenic lipoproteins as well as increased insulin resistance in women who are already at risk for CVD.

Seafarers are often placed in circumstances that require emergency evacuations due to various causes, including ship collisions, sinking, stranding, and fires. Achieving shorter evacuation time is an important factor in increasing the survival rate during these circumstances, but the narrow and complicated structure of ships is an obstacle when it comes to executing a quick evacuation. Also, unpredictable restrictions may be imposed by bad sea or weather. In this study, various experiments were conducted with sailors currently on board ships in order to examine factors that increase evacuation time. The data was then and analyzed. Evacuation time was measured by dividing crews into groups: sailors that were given an explanation of the ship's structure and those that were not. Furthermore, the visibility range was divided into 0 m, 3 m, and 5 m. The results indicated that, having an explanation of the ship structure did not have much of an effect on evacuation time but visibility conditions led to an increase in evacuation time with a maximum of 2.5 to 2.6 times longer when the visible distance was 5 m, 0 m and 3 m. Therefore, ensuring a visible distance of over 5 m was determined to be the most important factor for reducing evacuation time. In the future, effort should be made to ensure a greater visible distance to improve the survival rate of seafarers and passengers on board ships that encounter incidents.

Recently, the need for disaster-preventing broadcast has increased gradually to cope with natural disaster like earthquake and tsunami causing enormous losses of both life and property. In disaster-preventing broadcast system, the wake-up signal is used to alert user terminal and switch the current state of channel to the emergency channel, which is for the fast and efficient delivery of emergency information. In this paper, we propose the detection method of wake-up signal for disaster-preventing broadcast systems. The wake-up signals for disaster-preventing broadcast should have a good auto-correlation property in low power and narrow-band conditions that does not affect the existing digital television (DTV) system. The suitability of the m-sequence and complementary code (CC) is analyzed for wake-up signals according to signal to noise ratio. A wake-up signal is proposed by combining the direct sequence spread spectrum (DSSS) technique and pseudo noise (PN) sequences such as Barker and Walsh-Hadamard codes. By using the proposed method, a higher detecting performance can be achieved by the spreading gain compared to the single long m-sequence and the Golay code.

High-rise buildings and complex facilities are a representative urban system for the masses, and it requires an increasing role of commodity and safety. Smoke and toxic gasses can cause accidents due to fire in these systems. The purpose of this study is to develop a fiber screen material for emergency evacuation passages that can be avoided quickly and safely in cases of disasters. The fiber screen material is applicable to folding devices for emergency evacuation passages. The material is different from general steel material in that it is lightweight with less burden during storage for a long time in a roll form in a folding device. It also has an excellent secondary function in that it is less affected by radiant heat. Three kinds of fiber screen materials were selected that have good flame retardancy and post-processing characteristics. A performance evaluation was performed by a heat shrinkage test, contact heat test, combustibility test, flame retardancy test, tensile strength test, and tear strength test. As a result, the lightweight fabric shows excellent performance through post-processing, and silicone resin coating can secure safety of the pizza by the fiber screen material performance and radiant heat. The optimum post-treatment conditions were evaluated by performing a burning test after coating two kinds of glass fibers and four types of flame-retardant silicone resins with different weight and thickness.

Most embankment of the reservoirs (99.1 %) have been constructed in the earth filled type in Korea because the construction of this type is less expensive and simpler than others such as concrete one. However, it has to be reinforced the slope to prevent the breach due to overtopping or piping under unexpected flood conditions. This study has been analyzed the retardation effect using three types (L, T, $L^*$ shape) of stiffener in order to reinforce embankment when they are collapsed by overtopping flow. Experimental results showed that L-type stiffener is the most effective in delaying the breaching of embankment and reducing the soil erosion when compared with others. The reinforced embankment breaching showed that time delay was occurred about 1.73 to 2.29 times and the peak flowrate was reduced compared to non-stiffener embankments due to energy dissipation by collision and less soil erosion. The embankment breaching mostly leads to major damages because of the lack of repair time. Thus, since these stiffeners can resist the rapid breach, it would be possible to earn the time to emergency repair and lifesaving, as well as reduction of damages of embankment in downward region with decreasing peak flowrate. Results from this study would be used for the basis when establishing the emergency action plan for the reservoirs on the verge of hazard.
Background: Generalization of standardized surgical techniques to treat aortic valve (AV) and aortic root diseases has benefited large numbers of patients. As a consequence of the proliferation of patients receiving aortic root surgeries, surgeons are more frequently challenged by reoperative aortic root procedures. The aim of this study was to evaluate the outcomes of redo-aortic root replacement (ARR). Methods: We retrospectively reviewed 66 patients (36 male; mean age, $44.5{\pm}9.5years$) who underwent redo-ARR following AV or aortic root procedures between April 1995 and June 2015. Results: Emergency surgeries comprised 43.9% (n=29). Indications for the redo-ARR were aneurysm (n=12), pseudoaneurysm (n=1), or dissection (n=6) of the residual native aortic sinus in 19 patients (28.8%), native AV dysfunction in 8 patients (12.1%), structural dysfunction of an implanted bioprosthetic AV in 19 patients (28.8%), and infection of previously replaced AV or proximal aortic grafts in 30 patients (45.5%). There were 3 early deaths (4.5%). During follow-up (median, 54.65 months; quartile 1-3, 17.93 to 95.71 months), there were 14 late deaths (21.2%), and 9 valve-related complications including reoperation of the aortic root in 1 patient, infective endocarditis in 3 patients, and hemorrhagic events in 5 patients. Overall survival and event-free survival rates at 5 years were $81.5%{\pm}5.1%$ and $76.4%{\pm}5.4%$, respectively. Conclusion: Despite technical challenges and a high rate of emergency conditions in patients requiring redo-ARR, early and late outcomes were acceptable in these patients.
Background: Bronchiectasis and asthma are different in many respects, but some patients have both conditions. Studies assessing the effect of bronchiectasis on asthma exacerbation are rare. The aim of this study is to investigate the effect of bronchiectasis on asthma exacerbation. Methods: We enrolled 2,270 asthma patients who were followed up in our hospital. Fifty patients had bronchiectasis and asthma. We selected fifty age- and sex-matched controls from the 2,220 asthma patients without bronchiectasis, and assessed asthma exacerbation and its severity based on the annual incidence of total asthma exacerbation, annual prevalence of steroid use, and frequency of emergency room visits and hospitalizations due to asthma exacerbation in each group. Results: Fifty patients (2.2%) had bronchiectasis and asthma. The annual incidence of asthma exacerbation was higher in patients with asthma and bronchiectasis than in patients with asthma alone ($1.08{\pm}1.68$ vs. $0.35{\pm}0.42$, p=0.004). The annual prevalence of steroid use ($0.9{\pm}1.54$ vs. $0.26{\pm}0.36$, p=0.006) and the frequency of emergency room visits ($0.46{\pm}0.84$ vs. $0.02{\pm}0.13$, p=0.001) due to asthma exacerbation were also higher in patients with asthma and bronchiectasis than in patients with asthma alone. Conclusion: Bronchiectasis is associated with difficult asthma control.
Changing social conditions have resulted in a situation where elderly patients are no longer cared for by family and where medical care hospitals play a more prominent role. In this study, the unique elements of the medical service required from a long term care hospital were identified using conventional and exploratory analysis, and the causal relationship between medical service quality, relationship quality, and Revist intent was confirmed. The intermediary role and the quantitative importance of relationship quality (including trust and commitment) were also characterized. This study identifies the key points and indicators that the administrators of a long term care hospital can use to effectively plan their medical service offering in order to secure the commitment of customers through relationship quality. The theoretical indications of this study are set out below. First, four factors are selected as being the key elements determining service quality: medics, administrative service, healthcare environment, and subsidiary facilities. Second, it seems that medics, administration service, and the healthcare environment have some effect on the evaluations made in relation to trust and satisfaction (subsidiary facilities are not considered to be a key element). Third, patient satisfaction has a positive impact on trust and commitment and can be regarded as a key element for establishing connections. Fourth, commitment is likely to be strengthened when trust is significant. Fifth, as trust and commitment increase, revist intent strengthens. Lastly, this study illustrates how the levels of trust and commitment play a modulating role between patient satisfaction and revist intention. There are many practical indications from the findings of this study. First, the influences of medics, the administrative service, and the healthcare environment on trust and satisfaction vary. Especially, the healthcare environment is likely to be more important than medics. Accordingly, it is essential to establish an elderly-friendly environment, to improve a hospital's structure, and to maintain a clean environment. Second, medics must show compassion to their patients and be patient when providing explanations to elderly patients who often lack powers of concentration. Third, in order to establish patient trust, it is essential that medics provide an excellent medical service. Ultimately, these elements of relationship quality may strengthen the revist intention of elderly patients.

A series of tests dedicated to station blackout (SBO) accident scenarios have been recently performed at the $Prim{\ddot{a}}rkreislauf-Versuchsanlage$ (primary coolant loop test facility; PKL) facility in the framework of the OECD/NEA PKL-3 project. These investigations address current safety issues related to beyond design basis accident transients with significant core heat up. This work presents a detailed analysis using the best estimate thermal-hydraulic code TRACE (v5.0 Patch4) of different SBO scenarios conducted at the PKL facility; failures of high- and low-pressure safety injection systems together with steam generator (SG) feedwater supply are considered, thus calling for adequate accident management actions and timely implementation of alternative emergency cooling procedures to prevent core meltdown. The presented analysis evaluates the capability of the applied TRACE model of the PKL facility to correctly capture the sequences of events in the different SBO scenarios, namely the SBO tests H2.1, H2.2 run 1 and H2.2 run 2, including symmetric or asymmetric secondary side depressurization, primary side depressurization, accumulator (ACC) injection in the cold legs and secondary side feeding with mobile pump and/or primary side emergency core coolant injection from the fuel pool cooling pump. This study is focused specifically on the prediction of the core exit temperature, which drives the execution of the most relevant accident management actions. This work presents, in particular, the key improvements made to the TRACE model that helped to improve the code predictions, including the modeling of dynamical heat losses, the nodalization of SGs' heat exchanger tubes and the ACCs. Another relevant aspect of this work is to evaluate how well the model simulations of the three different scenarios qualitatively and quantitatively capture the trends and results exhibited by the actual experiments. For instance, how the number of SGs considered for secondary side depressurization affects the heat transfer from primary side; how the discharge capacity of the pressurizer relief valve affects the dynamics of the transient; how ACC initial pressure and nitrogen release affect the grace time between ACC injection and subsequent core heat up; and how well the alternative feeding modes of the secondary and/or primary side with mobile injection pumps affect core quenching and ensure stable long-term core cooling under controlled boiling conditions.
