Three Mile Island: Medical and Public Health Aspects of a Radiation Accident

  • The March 1979 accident at Three Mile Island provided physicians specializing in radiation medicine an opportunity to observe the field under conditions never seen before. Since no, injuries occurred at the site or within the community, medical personnel were immediately involved in efforts to allay fear, provide accurate information, and replace labortory resources rendered ineffective by the release in the reactor building. Valuable insights concerning medical emergency planning are derived from the accident; suggestions are made for handling any future mishaps.

  • When emergency lighting using a D.C. source is needed, we select the fluorescent lamp that has higher efficiency than that of an incandescene lamp. This paper deals with a series inverter for a fluorescent lamp load, which has high frequency output characteristics. First, the appropriate operating frequency is determined and second, the optimum value of circuit elements is determined in consideration of several important conditions. The results obtained by experimentations are represented.

  • This paper presents a methodology to evaluate the restoration capability of the feeder and its applications to the system planning and operation in the normal and emergency states.   • In a design of LNG storage tank, one of the major loading conditions that significantly affects liquid tightness of the outer concrete wall is the cryogenic temperature of LNG under the emergency condition of LNG leakage. Proposed in this study are the more consistent procedures to ensure the liquid tightness of LNG tank focusing on the design of prestressing tendon. It is expected that the proposed schemes lead to a more effective serviceability design of LNG tank that satisfies various requirements for the liquid tightness in an efficient manner.

  • Sometimes, there are conditions when rapid control of the hypertension needed such as malignant hypertension, and the preoperative pereparation of the hypertensive patient requiring emergency operation, etc. Using the adaptive control algorithm, the mean arterial pressure reduction is achived rapidly and safely in hypertensive rabbit. Among the adaptive control algorithms the pole assignment self tuning control algorithm is superial to the one step ahead minium variance control algorithm. The convergence time is less than 600 sec, and the standard deviation of experimental data are less than 4 mmHg.

  • The performance analysis of water systems is very important to urban disaster mitigation. It will benefit the task of preparedness and emergency response through a more practical and more quantitative approach. In this research work, an overview of hydraulics of water system has been provided. A methodology for such implementation based on scenario simulation and hydraulic analysis has been developed. The water system of Taipei Water Department was selected as a test bed for case study. Its serviceability following a major earthquake around Taipei metropolitan area has been quantified.

  • In a CANDU (CANada Deuterium Uranium) reactor, fuel channel integrity depends on the coolability of the moderator as an ultimate heat sink under transient conditions such as a loss of coolant accident (LOCA) with coincident loss of emergency core cooling (LOECC), as well as normal operating conditions. This study presents assessments of moderator thermal-hydraulic characteristics in the normal operating conditions and one transient condition for CANDU-6 reactors, using a general purpose three-dimensional computational fluid dynamics code. First, an optimized calculation scheme is obtained by many-sided comparisons of the predicted results with the related experimental data, and by evaluating the fluid flow and temperature distributions. Then, using the optimized scheme, analyses of real CANDU-6 in normal operating conditions and the transition condition have been performed. The present model successfully predicted the experimental results and also reasonably assessed the thermal-hydraulic characteristics of a real CANDU-6 with 380 fuel channels. A flow regime map with major parameters representing the flow pattern inside a calandria vessel has also proposed to be used as operational and/or regulatory guidelines.

  • Carboxytherapy is very similar to mesotherapy in terms of methods, conditions it treats, and outcomes. An important difference consider, however, is that carboxytherapy administers carbon dioxide gas into the subcutaneous layer, just underneath the skin, whereas mesotherapy administers a cocktail of vitamins, minerals, and drugs and into the mesoderm of the skin. There have been many previous case reports of complications of mesotherapy such as hematoma, granulomatous panniculitis, cellulitis, and abscesses. However, complications of carboxytherapy have rarely been reported because the carbon dioxide gas is absorbed within a week and is infused into the subcutaneous and superficial layers. A 27-year-old woman who had twice undergone carboxytherapy to reduce abdominal fat (3 days and 2 weeks ago) at an oriental medical clinic visited the emergency department due to high fever, myalgia, severe back pain, and subcutaneous emphysema from the buttocks to the lower chest wall area. A computed tomography (CT) scan was performed for diagnosis and treatment. We immediately started broad spectrum antibiotics and consulted with the department of radiology, which could not rule out acute peritonitis due to needle injury. A radiologist confirmed abdominal myositis and needle puncture induced acute pyelonephritis. Pyelonephritis can even lead to septicemia, which can have fatal consequences. Therefore, if the patient has costovertebral or back pain after undergoing needle puncture or acupuncture therapy, the emergency physcians need careful initial evaluation for diagnosis and treatment.

De Lege Frenda for Improvement of Marine Telemedicine Service System (해양원격의료 지원제도 개선을 위한 관련 법령정비 방안)

  • Expansion and spreading of marine telemedicine is rather restricted due to the conflict of laws relating to medical service and lack of provisions in the Seafarers' Act, Medical Service Act, etc. Thus, this study is intended to reveal the current status and problems of marine emergency medical advice system for the furtherance of health care of seafarers and emergency medical assistance conditions and deduce relevant proposals for legislative improvements thereof in order to resolve underlying problems and issues. The results of this study can be summated as follows. First, in respect of directions to provide marine emergency service based on marine telemedicine system, emergency radio medical advice system needs to be strengthened to meet domestic and international instrument, marine telemedicine system needs to be provided through integrating u-Health technology and special marine medical center needs to be established. Second, regarding directions to provide health promotion service based on the marine telemedicine system, a new process of health care service for seafarers needs to be devised and provided involving seafarers' life cycle covering from prior to boarding to after leaving a ship. The conclusions of this study can be given as follows. First, the following new provisions need to be introduced in the Seafarers' Act. (1) The Minister of Oceans and Fisheries and a shipowner shall conduct matters pertaining to preventive health promotion and care for seafarers; (2) a provisions regarding establishment of seafarers' health promotion center by the Minister; (3) a special exemption permitting marine telemedicine service and qualification requirements for marine telemedicine assistant; (4) shipowner's obligation of carrying seafarers' health measuring equipment on board. Second, the relevant provisions regarding medical care persons needs to be revised in such a way that master or chief officer shall be appointed to be in charge of medical care on board. Last but not least, it is also essential to amend and update the minimum standards on drug and medicines to be carried on board and medicine chest and equipment on board.

Genoprotective Effect of Melatonin Against to the Genotoxicity of Glyphosate on Human Blood Lymphocytes (글라이포세이트의 유전자 독성에 대한 멜라토닌의 유전자 보호 효과)

  • Purpose: Glyphosate is a widely used non-selective herbicide. Previous studies have shown that glyphosate has genotoxicity, and that even low-doses of glyphosate can cause DNA damage. Melatonin is a hormone produced and secreted by the pineal gland that is known to be a potent anti-carcinogen, anti-oxidant, and genetic protector. This study was conducted to investigate the genoprotective effect of melatonin against glyphosate in human blood lymphocytes. Methods: Human peripheral blood was obtained from 15 young, healthy volunteers and cultured under four different toxicologic conditions. The four groups consisted of a control group, glyphosate only group (300 ng/mL), glyphosate with low level of melatonin group ($50{\mu}M$), and glyphosate with high level of melatonin group ($200{\mu}M$). The mean Sister Chromatid Exchange (SCE) frequency of each group was then analyzed. Results: Glyphosate exposed groups had a higher mean SCE frequency ($10.33{\pm}2.50$) than the control group ($6.78{\pm}2.31$, p<0.001). Interestingly, the group that received a low-level of melatonin had a lower mean SCE frequency ($8.67{\pm}2.58$) than the glyphosate-only group, while the group that received a high level of melatonin had a much lower mean SCE frequency ($8.06{\pm}2.50$) than the glyphosate-only group. There was statistical significance. Conclusion: Melatonin exerted a potent gene protective effect against the genotoxicity of glyphosate on human blood lymphocytes in a dose-dependent fashion.
