의인성 신경손상을 모방한 손뒤침근내 결정종으로 인한 뒤뼈사이신경병증 증례 (A Case of Posterior Interosseus Neuroapathy due to Intrasupinator Ganglion Mimicking Iatrogenic Injection Nerve Injury)

  • Posterior interosseus (PI) nerve compression is a rare form of compressive neuropathy. It can cause pain along the radial nerve course and weakness of radial nerve innervating muscles without sensory symptoms. A 65-year-old man visited our institution complaining weakness of finger extension and thumb abduction after 2 times of injections at the right elbow in local clinic. The patient's clinical history and physical examination implied an iatrogenic radial nerve injury caused by the injection. The electrophysiologic study revealed of posterior interosseus neuropathy (PIN) with incomplete conduction block. However, the ultrasound study showed that the PI nerve was compressed by an anechoic cyst. The magnetic resonance imaging also confirmed of a ganglion cyst, not a hematoma. After repeated aspirations and a steroid injection, the electrophysiologic study showed recovery of motor weakness. Despite of the clue which implying an iatrogenic injury, clinician should consider other possibilities such as ganglion cysts and ultrasound guided aspiration and steroid injection could be an effective option for conservative management.

얼굴마비에서 얼굴근육의 전기생리학적 양상 (Electrophysiologic Pattern of Facial Muscles in Bell's Palsy)

  • Backgrounds: Electrodiagnostic tests have been developed to estimate the degree of facial nerve injury during the acute phase. Side-to-side amplitude comparison with the affected side expressed as a percentage of the nonaffected side has been one of the most valuable electrophysiologic methods of assessing facial nerve functioning. This study was designed to know whether there is any difference in the side-to-side comparison of amplitudes and terminal latencies of the compound muscle action potentials (CMAP) of the facial muscles in the patients with Bell's palsy. Methods: Electroneurographic recordings with surface electrodes on the frontalis, orbicularis oculi, nasalis, and orbicularis oris muscles were made within 2 weeks post-onset (mean, day 7) in 39 patients. Results: Of the 39 Bell's palsy patients, 38 patients (97.4%) recovered satisfactorily within 6 months. The amplitude of CMAP in all patients was not reduced to 10% or less of that of the contralateral healthy muscle. The correlation of amplitude change between four facial muscles was relatively strong, but the correlation of latency change was weak. When the electroneurographic values were compared in the four muscle groups, the general linear models procedure did not show any significant difference for CMAP amplitude and latency changes (p=0.62-0.63). Conclusions: This study did not show any significant clinical advantage of electroneurographic recordings in more than one facial muscle at the early stage of Bell's palsy.

중증근무력증 환자에서 발생한 길랑-바레 증후군 1례 (Guillain-Barré Syndrome in a Patient with Myasthenia Gravis)

  • Both myasthenia gravis and Guillain-$Barr{\acute{e}}$ syndrome are autoimmune disorder, but it is very rare that both of them occur together. A 53-year old woman with history of myathenia gravis complained of weakness of extremities, worsening progressively for several days. Electrophysiologic study showed findings of motor axonal neuropathy without sensory involvement. She became improved in aspect of clinical symtoms and electrophysiologic findings after infusion of immunoglobulin. This is a case of Guillain-$Barr{\acute{e}}$ syndrome in a patient with myasthenia gravis.

수근관 증후군에서 임상양상척도 및 신경전기진단 결과와 정중신경 초음파 소견의 상관관계 (The Correlation between Ultrasonographic Findings of Median Nerve and Clinical Scale and Electrodiagnotic Data in Carpal Tunnel Syndrome)

  • Background: The aim of this study is to identify the correlation between ultrasonographic findings of median nerve and clinical scale and electrophysiologic data in carpal tunnel syndrome. Methods: Forty three patients (79 hands) with electrophysiologically confirmed carpal tunnel syndrome were evaluated. Clinical symptoms were examined by Historical-Objective (Hi-Ob) scale. Electrophysiologic data and Padua scale were used for severity of electrophysiology. In ultrasonographic study, cross sectional area and flattening ratio of median nerve were measured at distal wrist crease level (DWC), 1cm proximal to distal wrist crease level, and 1cm distal to distal wrist crease level. The correlation between Hi-Ob scale, electrophysiologic data and ultrasonography was measured with Spearman rank test. Results: The mean Hi-Ob scale was 2.4. Mean Padua scale was 4.0. In ultrasnonographic study, cross sectional area and flattening ratio were $0.112\;cm^2{\pm}0.025$ and $3.0{\pm}0.6$ at 1cm proximal to DWC level, $0.118{\pm}0.026\;cm^2$ and $2.9{\pm}0.4$ at DWC level, and $0.107{\pm}0.032\;cm^2$ and $3.0{\pm}0.4$ at 1 cm distal to DWC level. Hi-Ob scale was not correlated with cross sectional area and flattening ratio of median nerve. Hi-Ob scale was correlated with Padua scale positively (r=0.44) and correlated with amplitudes of CMAP and SNAP, negatively (r=-0.33; r=-0.30). Cross sectional area of median nerve was significantly correlated with Padua scale, amplitudes and latencies of CMAP and amplitudes of SNAP. Conclusions: Ultrasonographic findings of median nerve and electrodiagnostic data had statistically significant correlation. Consequently, ultrasonography could be an adjunctive method in diagnosis of carpal tunnel syndrome.

동반이환된 특발성 수근관증후군과 족근관증후군의 전기생리학적 특징 (Electrophysiologic Characteristics of Combined Idiopathic Carpal Tunnel Syndrome and Tarsal Tunnel Syndrome)

  • Background: Carpal tunnel syndrome (CTS) and tarsal tunnel syndrome (TTS) are thought to share a similar pathophysiology, compression of the median and plantar nerve by the carpal tunnel and flexor retinaculum. A few reports introduced the relationship between idiopathic CTS and TTS without definite evidence of coexistence. The current study was designed to analyze the electrophysiologic characteristics of combined idiopathic CTS and TTS by comparing with each idiopathic CTS or TTS. Methods: We retrospectively collected patients with combined idiopathic CTS and TTS (CTS-TTS group) from June 2001 to February 2009. Patients with each idiopathic CTS or TTS were collected as controls. Electrophysiologic data of median and plantar nerves were compared between CTS-TTS group and controls. Results: CTS-TTS group was composed of 31 patients. Control group of each CTS or TTS were 50 CTS and 49 TTS patients. In comparison of median nerve conduction study between CTS-TTS group and CTS control group, decreased compound muscle action potential amplitude (p<0.001), decreased median sensory nerve action potential amplitude (p<0.001) and sensory nerve conduction velocity at finger stimulation (p=0.013) were prominent in CTS-TTS group. Decreased medial plantar sensory nerve action potential amplitude (p=0.034) was indicated when CTS-TTS groups and TTS control group were compared. Conclusions: If the electrophysiology study of patients with CTS or TTS was suggestive of severe degree of nerve injury, concerns about the possibility of combined CTS and TTS would be helpful.

  • Purpose: Complete release of the transverse carpal ligament (TCL) is accepted as the standard treatment for carpal tunnel syndrome (CTS). However, loss of grip and pinch power are reported in some patients after complete release of the TCL. This study was designed to evaluate the effectiveness of complete versus partial carpal tunnel release by using the inching technique. Methods: Nineteen patients (a total of 27 hands) who each had a confirmed diagnosis of CTS were selected from September 2002 to February 2003. The cases were divided into three groups(mild, moderate and severe) based on preoperative electrodiagnostic studies. The patients with partial carpal tunnel syndrome were classified into the mild or moderate groups, while patients with complete carpal tunnel syndrome were classified into the moderate or severe groups. Patient oriented data (functional and symptomatic) were collected and electrophysiologic studies were undertaken preoperatively and postoperatively(on the 2nd week, 1st month, 3rd month and 6th month after surgery).Results: In this study, the mild and moderate groups showed both good functional and symptomatic results and improvements in electrophysiologic studies.Conclusion: Carpal tunnel syndrome patients classified into mild or moderate groups based on nerve conduction studies, and whose precise compression sites were pinpointed using the inching technique, can be treated by partial carpal tunnel release.

염색체 17p11.2 유전자 결손을 동반한 유전성 압박마비 편향 신경병증의 임상적, 전기생리학적 특성 (Clinical and Electrophysiological Features of HNPP Patients with 17p11.2 Deletion)

  • Objectives : Although the diagnosis of hereditary neuropathy with liability to pressure palsies (HNPP) is important for correct prognostic evaluation and genetic counseling, the diagnosis is frequently missed or delayed. Our main aim on undertaking this study was to characterize the electrodiagnostic features of HNPP. Material and Methods : Clinical, electrophysiologic and molecular studies were performed on Korean HNPP patients with 17p11.2 deletion. The results of electrophysiologic studies were compared with those of Charcot-Marie-Tooth disease type 1A (CMT1A) patients carrying 17p11.2 duplication. Results : Eight HNPP (50 motor, 39 sensory nerves) and six CMT1A (28 motor, 16 sensory nerves) patients were included. The slowing of sensory conduction in nearly all nerves and the distal accentuation of motor conduction abnormalities are the main features of background polyneuropathy in HNPP. In contrast to CMT1A, where severity of nerve conduction slowing was not different among nerve groups, HNPP sensory nerve conduction was more slowed in the median and ulnar nerves than in the sural nerve (p<0.01), and DML was more prolonged in the median nerve than in the other motor nerves (p<0.01). TLIs were significantly lower in HNPP than in the normal control and CMT1A patients for the median and ulnar nerves (p<0.01), and were also significantly reduced for the peroneal nerve (p<0.05) compared with those of the normal controls. Conclusion : The distribution and severity of the background electrophysiologic abnormalities are closely related to the topography of common entrapment or compression sites, which suggests the possible pathogenetic role of subclinical pressure injury at these sites in the development of the distinct background polyneuropathy in HNPP.

양측 반회후두신경사이의 신경문합궁에 대한 실체 (Cervicomediastinal Anastomotic Loop between Recurrent Laryngeal Nerves)

  • The anatomic course of recurrent laryngeal nerve, its branch and its function were most significant for laryngeal, thyroid and tracheoesophageal surgery. Furthermore, the vocal cord palsy resulting from multi-etiology was serious complication and resulted in affecting the life quality. So, for the prevention of this complication, the concepts and knowledge about anatomic course and variants are very important. At now, most of anatomic courses and it's function has been identified precisely. But recently, the report about the anastomotic loops of both recurrent laryngeal nerve was published. In this study, we explored three cadevors for identifying the reality of the anastomotic loops between recurrent laryngeal nerves. Finally, we identified the cervicomediastinal anastomosis at tracheoesophageal groove in 2 of 3 cadevors that was confirmed by pathologic finding. This anatomic reliefs related to it's branch are extremely interest, although research is still in its initial phase. Our study will be extended toward histomorphometrical study and progressive electrophysiologic study, and we will be able io gather the largest amount of useful data regarding any possible use of this anatomic entity in future.

감각성 길랑바레 증후군 1예 (A Case of Sensory Guillain-Barre syndrome)

  • The sixty two-year-old woman was admitted with facial diplegia and ataxic gait. Neurological examination revealed areflexia and sensory ataxia with decreased sensation of position and vibration in both lower extremities. Electrophysiologic study suggest motor dominant demyelinating polyneuropathy and bilateral facial neuropathy. CSF study revealed no cells and increased proteins. After intravenous immunoglobulin therapy, sensory ataxia and electrophysiological study had markedly improved for 3 months.

Ebstein씨 심기형에 동반된 Wolff-Parkinson-White 증후군 및 방실결절 회귀성 빈맥에 대한 수술치험 1례 보고 (Surgical Treatment of Wolff-Parkinson-White Syndrome Combined with AV Nodal Reentrant achycardia in a Patient with Ebstein`s Anomaly - A report of one case -)

  • A 17 year old female patient with Ebstein`s anomaly received surgical treatment for WPW syndrome and AV nodal reentrant supraventricular tachycardia[SVT] Electrophysiologic study revealed that an anomalous pathway was located in the right posterolateral portion and antegrade dual AV nodal pathway responsible for AV nodal reentrant tachycardia. The patient was underwent surgery on February 18, 1987. Intraoperative mapping was used to define the location of accessory pathway. The accessory pathway was cryoablated through the epicardium. Simultaneously discrete cryoablation around the perinodal area was performed to prevent AV nodal reentrant SVT. The atrialized right ventricle of Ebstein`s anomaly was plicated with 11 pledget mattress sutures under the cardiopulmonary bypass. Two and half years after surgery, the patient has no evidence of WPW syndrome or supraventricular tachycardia.
