간호사의 전자의무기록(EMR) 인식도와 만족도에 따른 자율성 및 간호업무성과 (The Autonomy, Nursing Performance based on the Awareness and satisfaction of EMR System for Nurses)

  • 본 연구는 간호사를 대상으로 전자의무기록(EMR) 인식도, 만족도에 따른 간호사 자율성과 간호업무성과와의 관계를 파악하기 위한 서술적 상관관계연구이다. 구조화된 설문지를 이용하여 2013년 8월 종합병원 간호사 194명을 임의표집하여 자료를 수집하였으며, 수집된 자료는 SPSS 20.0 프로그램을 이용하여 빈도와 백분율, 평균과 표준편차, t-test, ANOVA, Pearson's correlation coefficiency로 분석하였다. 연구 결과는 다음과 같다. 대상자의 EMR에 대한 인식도는 3.68, 만족도는 3.47로 중간이상이었고, 대상자의 자율성은 3.25로 중간수준, 간호업무성과는 3.55로 비교적 낮은 수준이었다. EMR 인식도에서는 특수부서간호사가 일반병동간호사에 비해 유의하게 높았고, 컴퓨터자격증 소지자가 미소자에 비해 유의하게 높았다. EMR 만족도는 컴퓨터자격증 소지자가 미소지자에 비해 높았고, 간호업무성과는 수간호사이상이 일반간호사나 주임간호사에 비해 높았다. 또한 대상자의 EMR에 대한 인식도가 높을수록 만족도가 높았고, 만족도가 높을수록 간호사자율성 및 간호업무성과가 높았다. 따라서 간호사의 자율성과 간호업무성과를 높이기 위해 간호사의 EMR 인식도 및 만족도를 높이고 이를 위한 컴퓨터 자격증에 대한 활용 등의 방법에 대한 고려가 필요하다.

새로운 항암제 처방 감사 시스템 도입을 통한 의료의 질 향상 (Efficacy of new inspection system of Anticancer Drug Prescription)

  • Background : The number of outpatient injected anticancer drug is increasing. and the pathway of prescribing, compounding, and injecting anticancer drug is processed very rapidly in out-patient department. Moreover, Dose of anticancer drug is often changed depending on side effect of patients. So we need more effective inspection of anticancer drug prescriptions. The purpose of this study was to analyze the prescription errors for anticancer drugs in Out-Patient Department and to suggest system to prevent them. Method : The study took place at Asan Medical Center from July to September 2007. The pharmacists performed inspection of anticancer drug prescriptions before compounding and injecting. We used protocol-based anticancer drug order program and Electronic Medical Record (EMR). Result : During the study period, we analyzed 4683 prescriptions for out-patient. And we detected 55 medication errors (1.2%). Most common errors included dosage above or below the correct ones (56.3%), followed by incorrect treatment duration. Because most of dosing errors were in the range of usual dosage, it was hard to detect them. So when inspecting the prescription, we considered the medical records of individual patients. As a result, we could raise the efficiency of intervention. Therefore inspection using EMR could possibly reduce the number of anticancer drug errors. Conclusion : we are preventing the medication errors on stability and dosage above or below the maximum therapeutic dose according to the previous inspection system. However most of dosing errors were in the range of usual dosage according to the result of this study. Because of there was interpatient variability of dosage depending adverse effect. For improvement of quality assurance, we suggest inspection system based on patient's medical history.

개인건강서비스를 위한 보안 요구사항 (Security Requirements of Personal Health Service)

  • 본 논문에서는 다양한 형태의 개인건강서비스들이 ICBM(사물인터넷, 클라우드, 빅데이터, 및 모바일) 환경에서 제공될 때, 프라이버시 이슈를 포함하여 개인건강서비스에 대한 보안 요구사항이 제안된다. 개인건강과 연관된 서비스들은 클라우드 환경에서 제공될 것이 예상되므로, 우선적으로 클라우드 환경의 보안 요구사항에 대해 조사한 후, 클라우드 환경에서의 직접적인 위협과 간접적인 위협을 포함한 보안 위협을 개인건강서비스의 보안 관점에서 분석한다. 그리고 본 논문에서 의료서비스를 위한 전자의료기록(EMR)에 대한 보안 요구사항에 기반을 두고 개인건강서비스를 위한 보안 요구사항을 도출한 뒤, 클라우드 환경의 보안요구사항이 개인건강서비스의 보안요구사항에 의해 충족될 수 있음을 나타내는 관계를 보임으로서 제안된 개인건강서비스에 대한 보안 요구사항의 타당성을 제시한다.

의료기관 전문 의료용 CRM 프레임 설계 (Medical CRM Frame Design for Medical Institution)

  • 현재 병원에서는 병원관리시스템(HIS), 의료영상시스템(PACS), 처방전달시스템(OCS), 전자의무기록(EMR), 전사적 자원관리(ERP) 등 각 과별 업무별로 독립적인 시스템을 사용하고 있으며, 각 시스템에 종속적인 각각의 DB를 운용하고 있다. 이에 따라 정보의 원내 통합이 불가능하며, 데이터의 투명성과 일관성을 유지하기 어려운 상황이다. 본 연구는 기존의 타 시스템과의 유연한 연계처리를 통한 데이터 통합환경을 제공하고, 이를 통해 고객이 원하는 최적의 서비스를 최적의 타이밍에 제공할 수 있는 의료 CRM 프레임을 설계하였다. 고객관리 프로세스 표준화에 의해 고객관리, 홍보/마케팅, 서비스관리, 통계/분석의 4가지 의료 CRM 프레임을 설계하였고 개인화된 의료정보 서비스 제공을 위한 모바일 의료 콘텐츠를 확보하여 고객 데이터를 바탕으로 고객의 특성과 건강 상황에 따라 맞춤형 모바일 콘텐츠를 제공할 수 있도록 설계하였다.

1개 대학 한방병원에서 EMR을 통해 보고된 한약에 의한 약물유해반응의 현황 (Status of Herbal-drug-associated Adverse Drug Reactions Voluntarily Reported by EMR)

  • Objectives : The aim of this study was to systematically investigate herbal-drug-associated adverse drug reactions (herbal ADRs) reports submitted by a single oriental hospital and to analyze the general characteristics, causative agents, clinical manifestations, severity and types of herbal medicines which caused herbal ADRs. Methods : This study proceeded with IRB approval. The data on herbal ADR were collected prospectively from January 2008 to February 2012 by EMR of Dongguk University Ilsan Oriental Hospital. The World Health Organization (WHO)-Uppsala Monitoring Center (UMC) criteria was used to determinate causality for each herbal ADR. WHO-Adverse Reaction Terminology (WHO-ART) System Organ Class (SOC) code and WHO severity category were also used in this study. Results : A total of twenty eight cases were reported. Twenty two cases were assessed to have over possible relations with herbal medication. The gender ratio of these cases were 64.6 percent female and 36.4 percent male, demonstrating no statistical significance. Patients aged over 60 were 59.1%. Gastro-intestinal system was reported to be the most frequently affected organ (38.8%), and followed by psychiatric system (22.4%), and integumentary system (22.4%). The most common clinical symptom was headache (12.2%), followed by diarrhea (10.2%), and pruritus (10.2%). The severity of most cases was assessed to be mild (89.8%). The percentage of moderate ones was 10.2%, and there were no severe cases. Conclusions : Progressive study and further analysis on herbal ADRs are warranted for safety in the clinical use of herbal medicines.

웹 마이닝을 활용한 의학 용어 추출 시스템 설계 및 구현 (Design and Implementation of Extracting Medical Terms Using Web Mining)

  • 최근 방대해진 의료 정보 관리와 통합을 위해 다양한 전자기록 시스템이 개발되어왔다. 이 중에 EMR(Electronic Medical Record)은 병원 내의 의료 정보를 전산으로 처리하는 것이다. 이때 많은 의학 용어가 사용되는데 이것을 체계적으로 지원하기 위해 SNOMED(Systematized Nomenclature of Medicine)와 같은 용어 체계가 필요하다. 본 논문에서는 이러한 용어 체계를 위해 의학 용어를 자동으로 구축하는 시스템을 제안하고자 한다. 제안한 시스템은 웹 마이닝을 통해 자동으로 웹에서 데이터를 수집하고 분류해서 의학 용어 데이터를 구축하게 된다.

Clinical Information Interchange System using HL7-CDA

  • In highly developed society, information and communication technologies are widely used for better medical services. These information and communication technologies should be more and more acceptable in all hospitals for exchange medical records. EMR becomes more convenient than the previously used paper charts. It will be able to record medical institutions every time and dual treatment. Each is different specifications for each medical institution to use the program or document to exchange it. The personal clinic records still does not exchange well. To solve this gap between medical alienation, this paper describes the concepts of HL7-CDA and proposes types of telemedicine system. To resolve time and space constraints, new form of treatment methods presents in future directions after described about related systems. CDA enables electronic medical records to the each medical center and gradually expanded by exchanging the patient's medical records. This paper is using XML-based CDA documents as a hierarchical for medical information exchange standards compliant HL7-CDA documents. It could be possible currently used structural variety of multimedia data. Thus It is able to send and receive HL7-CDA-based medical information and clinical information to identify the medical institutions of medical information with interchange system design and building standards, and through mutual exchange of clinical information.

DICOM 표준 영샹을 위한 이미지 기반의 주석 (An Image-Based Annotation for DICOM Standard Image)

  • 본 논문에서는 DICOM 영상 내에 이미지 기반의 주석을 생성/삽입할 수 있는 새로운 DICOM 객체를 제안한다. 본 논문에서 제안한 주석 처리 방법은 이미지 자체를 주석으로 사용하기 때문에 사용자가 입력한 다양한 형태의 정보(문자, 스케치, 그림)를 주석 내에 쉽게 저장할 수 있다. 주석 영상은 원 영상과는 독립적으로 저장/관리 되기 때문에 원 영상에는 전혀 영향을 주지 않고 삽입/추가/변경 등이 가능하다. 또한, 제안된 주석은DICOM영상 내에 직접 저장되기 때문에 PACS/EMR이 설치되어 있지 않은 중소 병원에 매우 유용할 것으로 기대된다.

임상간호사의 영상표시단말기 증후군 및 영향요인 (Factors influencing Video Display Terminal Syndrome in Clinical Nurses)

  • Purpose: The present research is a descriptive study aimed at understanding clinical nurses' Video display terminal (VDT) syndrome and identifying the factors that affect their VDT syndrome. Methods: Data were collected from 239 clinical nurses working in two metropolitan cities. Research tools included subject's VDT syndrome assessing musculoskeletal, ophthalmic, dermal, psychiatric, and whole body syndromes. The data were analyzed using frequency analysis, average and standard deviation, t-test, One-way ANOVA, and multiple regression analysis with the SPSS/WIN 20.0 program. Results: The subjects' VDT syndrome score was 1.34 out of 5. There were significant differences in participating subjects' VDT syndrome, hospital's size, working unit, health status, diagnosis of illness, having an Order Communication System (OCS), having an Electronic Medical Record (EMR) System, continuous VDT working for more than one hour, break time during VDT use, VDT use time, comfort of the chair, adjustability of the height of the chair, size of the VDT's desk, distance between the monitor and the user's eyes, resolution of the monitor, and frequency of eye exercise during VDT use. According to the research, influencing factors on VDT syndrome in clinical nurses included size of the VDT's desk, frequency of eye exercise during VDT use, having an EMR system, break time during VDT use, diagnosis illness, and having an OCS' system. Conclusion: The findings from this study can be used as a basis for future VDT syndrome prevention education and programs for clinical nurses.

Image-Centric Integrated Data Model of Medical Information by Diseases: Two Case Studies for AMI and Ischemic Stroke

  • In the medical fields, many efforts have been made to develop and improve Hospital Information System (HIS) including Electronic Medical Record (EMR), Order Communication System (OCS), and Picture Archiving and Communication System (PACS). However, materials generated and used in medical fields have various types and forms. The current HISs separately store and manage them by different systems, even though they relate to each other and contain redundant data. These systems are not helpful particularly in emergency where medical experts cannot check all of clinical materials in the golden time. Therefore, in this paper, we propose a process to build an integrated data model for medical information currently stored in various HISs. The proposed data model integrates vast information by focusing on medical images since they are most important materials for the diagnosis and treatment. Moreover, the model is disease-specific to consider that medical information and clinical materials including images are different by diseases. Two case studies show the feasibility and the usefulness of our proposed data model by building models about two diseases, acute myocardial infarction (AMI) and ischemic stroke.
