중완, 신궐 및 관원 경혈의 뜸 자극과 무 자극 대상군의 16채널 뇌파 변화에 관한 연구 (EEG 16 channel variations between the non-stimulation and the moxibustion stimulated subjects for CV4, CV8, and CV12 acupuncture points)

  • 본 연구에서는 건강한 남성을 대상으로 뜸 자극을 가한 대상군과 자극을 가하지 않은 대상군의 뇌파를 분석하여 어떠한 영향을 미치는지에 대해 실험하였다. 뜸 자극은 뜸 링을 사용하는 간접 뜸 자극 방식을 적용하였고, 뇌파는 국제10-20전극법을 적용하여 16채널 쌍전극 측정법으로 측정하였다. 측정된 데이터는 ${\alpha}$, ${\beta}$, ${\delta}$, ${\Theta}$파의 주파수 스펙트럼을 구하고, 16채널 전체를 평균한 뇌파변화추이와 16채널 각각의 뇌파변화추이에 대해 분석하였다. 그 결과, 중완, 관원, 신궐의 뜸 자극은 체표감각신경의 반응에 따라 두정엽(ch 2, 3, 6, 7)의 뇌파신호가 낮았고, 무 자극 대상군과 뜸 자극 대상군의 큰 차이는 후두엽(ch 4, 8)의 변화로 뜸 자극 시 온화한 온열 ($42{\sim}44^{\circ}C$)자극에서 ${\alpha}$파 증가와 ${\beta}$파 감소, 약간의 뜨거움($45{\sim}48^{\circ}C$)의 반응 시 ${\alpha}$파 감소와 ${\beta}$파 증가의 반응이 있었다. 그리고 자극 후 1시간에서 ${\beta}$, ${\delta}$, ${\theta}$파 감소와 ${\alpha}$파의 증가로 정신생리 향상효과가 있음을 알 수 있었다.

Appraisal of Guidelines for Research & Evaluation (AGREE) II를 이용한 교통사고 상해증후군의 국내·외 기개발 임상진료지침의 평가 (Appraisal of Guidelines for Research & Evaluation II Appraisal of Clinical Practice Guidelines for Traffic Injuries)

  • Objectives This study was aimed to evaluate clinical practice guidelines (CPGs) of traffic injuries, which has already been developed at domestic or outside of country, and to explore the Korean medical treatments included in the CPGs. Methods Twelve electronic databases (PubMed, Cochrane library, China National Knowledge Infrastructure [CNKI {Chinese Academic Journals, CAJ}], Research Information Sharing Service [RISS], Oriental Medicine Advanced Searching Integrated System [OASIS], KoreaMed, Korean Medical Guideline Information [KoMGI), National Guideline Clearinghouse [AHRQ], Core Outcome Measures in Effectiveness Trials Initiative Website [COMET], Turning Research into Practice [TRIP], The National Institute for Health and Care Excellence [NICE], and Medical Research Information Center [MedRIC]) up to July 2021 were searched. Only systematically developed CPGs for traffic injuries were selected and appraised. The appraisal was conducted by using Appraisal of Guidelines for Research & Evaluation (AGREE) II tool. Results Six CPGs were included and evaluated. All CPGs were appraised as highly recommended because they exceeded 60% in more than 4 among 6 domains of AGREE II, including domain of 'rigor of development', and 30% in the rest. Recommendations related to Korean medicine treatments such as on manual therapy related to Chuna were given in 6 CPGs, 4 for acupuncture, 1 for Qigong and 1 for Taping. Conclusions The 6 CPGs were developed up to now through a systematic development methodology and there were many recommendations for Korean medical treatments related to manual (Chuna) treatment, acupuncture and Qigong. However, the evidence for the side effects and risk factors of Korean medical treatments was scantly reflected in CPGs. Therefore, it is considered that balanced CPG with benefits and risks should be developed, covering Korean medical diagnosis, treatment and prognosis.

한의과대학 부속 한방병원 응급실 내원환자에 대한 고찰 : 후향적 차트리뷰 (A Study of Patients Who Visited the Emergency Department at a Korean Medical Hospital: A Retrospective Chart Review)

  • Objectives: By analyzing the characteristics and the distribution of diseases of patients who visited the emergency room of a Korean medical hospital, we sought to determine the methods of activating the Korean emergency medical system and to use this analysis as a basis for future research. Methods: We conducted a retrospective review of 959 patients who visited the emergency department of the Dong-Eui University Korean Medical Hospital from January 2016 to December 2016. The review was conducted using electronic medical records created during the emergency department visit. Results & Conclusions: In distribution of sex, the rate of males was 47.9%, and that of females was 52.1%. In distribution of age, the greatest number of patients were in their 50s (27.5%), followed by those in their 60s (19.9%) and 40s (14.8%). In distribution of residence, most patients were from Busan (84.9%). In distribution of week, more patients visited the emergency room on Sundays and holidays (44.3%). September was the busiest month (12.5%). Visits usually occurred during daytime and nighttime, and there were few visits at dawn. In the analysis of the time interval between onset of symptoms and the emergency room visit, most patients visited within 24 hours (46.5%). In the distribution of diseases, facial palsy was the most common (34.6%). In the systematic distribution of diseases, circulatory diseases were the most common (56.7%), followed by diseases of the musculoskeletal system (28.6%). The rate of first emergency room visit was 52.9%, and the rate of revisit was 47.1%. The rate of hospitalization after emergency room treatment was 27.5%, and the rate of discharge was 72.4%. The rate of revisit was 63.4%. In the distribution of treatment before arrival at the emergency room, direct was the most common (51.0%), and the outpatient department of western medicine was next. In the distribution of treatment in the emergency room, acupuncture was the highest (91.4%), followed by herb-med (43.0%).

한양방 협진치료를 받은 치매와 경도인지장애 환자에 대한 후향적 의무기록 분석 (Retrospective Analysis of Patients Suffering from Dementia or Mild Cognitive Impairment Treated by Collaboration between Western and Korean Medicine)

  • Objectives: To investigate the characteristics of patients diagnosed with dementia or mild cognitive impairment who are treated by means of a blend between Western and Korean medicine. Methods: We searched for outpatients with dementia or mild cognitive impairment by means of a collaboration between Western and Korean medicine from August 1, 2015, to July 31, 2017, through electronic medical records in Wonkwang Hospital. The records were retrospectively analyzed according to the patients' demographic and clinical characteristics, pathway of medical care, diagnostic tests, treatment, and medical expenses. Results: Thirteen patients were included in the analysis. Among them, six patients were diagnosed with mild cognitive impairment, five with dementia, Alzheimer's type, one patient with frontotemporal dementia, and one patient with unspecified dementia. Twelve of the thirteen patients were over 60 years of age. The number of pathways from the Dept. of Neurology to the Dept. of Neuropsychiatry of Korean Medicine was almost the same as the opposite pathway. The most used diagnostic test in Korean medicine was a neuropsychological test such as SNSB, MMSE and GDS. In Western medicine, hematology and neuroimaging were frequently used for patients. Acupuncture in Korean medicine and medication in Western medicine were the most frequently used. In Korean medicine, uncovered service costs were much higher than covered service costs,. whereas, in Western medicine, covered service costs were higher than uncovered service costs. Conclusions: This study describes the basic characteristics of dementia and mild cognitive impairment patients treated by a collaboration between Western and Korean medicine. Based on these results, a clinical pathway of the collaborative practice system between Western and Korean medicine for dementia patients needs to be developed.

부인암 및 유방암의 입원 중 한양방 협진 분석 및 고찰 (An Analysis of Korean-Western Medicine Integrative Care for Patients on Hospitalized with Gynecologic and Breast Cancer)

  • Objectives: The purpose of this study is to analyze the effect of Korean-Western medicine integrative care for patients on hospitalized with gynecologic and breast cancer. Methods: The records were searched for integrative medical treatment the Department of Obstetrics and Gynecology in a Korean medical hospital between January 1st, 2021 and April 30, 2024, based on the Electronic Medical Record system (EMR). The records were reviewed and analyzed based on the patient's characteristics, the chief symptom, Korean medical treatment which patients received, and follow-up observation Results: 61 cases were screened, the patient's age is the highest in their 50s with 16 cases (26.23%). The average hospitalization period for patients with integrative care is 7.64 days. Among the patients with integrative care, 60 patients (98.36%) underwent surgery. Gastro-intestinal symptoms (nausea/vomiting, constipation /diarrhea etc.) showed the largest proportion in total symptoms (40.72%). After Korean-medicine integrative treatment, 62.30% of patients improved their symptoms, and 19.67% of cases were not improved because follow-up was not performed. Acupuncture is the most frequently performed Korean-medicine treatment (90.16%), followed by moxibustion (55.74%) and herbal medicine (37.70%). Among the herbal medicines, Gamisipjeon-tang and Geonchil-dan were the two most used herbal medicines. Conclusions: Included cases showed that treatments used in Korean medicine hospitals for integrative care with gynecologic and breast cancer are safe and effective based on several evidence. In the future, further high-quality large-scale study is needed to prove effectiveness of Korean-Western integrative care for patients with gynecologic and breast cancer.
