전기 자극을 이용한 피드백의 형태가 무릎성형 수술 환자의 넙다리 네갈래근 등척성 운동 학습에 미치는 영향 (Effects of Electrical Stimulation Biofeedback on Motor Learning of Quadriceps Isometric Exercise of Total Knee Replacement)

  • The purpose of this study was to determine the effect of electrical stimulation biofeedback on motor learning of quadriceps muscle isometric exercise in 3 patients who have undergone total knee replacement surgery. A multiple baseline design across subjects was used. The electrical stimulation biofeedback was provided with each patient during quadriceps isometric exercise, which last 10 to 14 sessions with 10 repetitions each sessions. After training patients received 4 retention tests. Maximum muscle activity was measured pre- and post- electrical stimulation biofeedback training and retention test to evaluate the effect of biofeedback training. Maximum isometric muscle activity of quadriceps was increased after electrical stimulation biofeedback training in all subjects. The results indicate that a electrical stimulation biofeedback training is a useful method to improve motor learning of quadriceps isometric exercise in total knee replacement.

Pelvic Floor Muscle Exercise by Biofeedback and Electrical Stimulation to Reinforce the Pelvic Floor Muscle after Normal Delivery

  • Purpose. This study was conducted to investigate the effectiveness of pelvic floor muscle exercise using biofeedback and electrical stimulation after normal delivery. Methods. The subjects of this study were 49 (experimental group: 25, control group: 24) postpartum women who passed 6 weeks after normal delivery without complication of pregnancy, delivery and postpartum. The experimental group was applied to the pelvic muscle enforcement program by biofeedback and electrical stimulation for 30 minutes per session, twice a week for 6 weeks, after then self-exercise of pelvic floor muscle was done 50-60 repetition per session, 3 times a day for 6 weeks. Maximum pressure of pelvic floor muscle contraction (MPPFMC), average pressure of pelvic floor muscle contraction (APPFMC), duration time of pelvic floor muscle contraction (DTPFMC) and the subjective lower urinary symptoms were measured by digital perineometer and Bristol Female Urinary Symptom Questionnaire and compared between two groups prior to trial, at the end of treatment and 6 weeks after treatment. Results. The results of this study indicated that MPPFMC, APPFMC, DTPFMC were significantly increased and subjective lower urinary symptoms were significantly decreased after treatment in the experimental group than in the control group. Conclusions. This study suggested that the pelvic floor muscle exercise using biofeedback and electrical stimulation might be a safer and more effective program for reinforcing pelvic floor muscle after normal delivery.

Functional Electric Stimulation-assisted Biofeedback Therapy System for Chronic Hemiplegic Upper Extremity Function

  • Purpose: Rehabilitative devices are used to enhance sensorimotor training protocols, for improvement of motor function in the hemiplegic limb of patients who have suffered a stroke. Sensorimotor integration feedback systems, included with these devices, are very good therapeutic frameworks. We applied this approach using electrical stimulation in stroke patients and examined whether a functional electric stimulation-assisted biofeedback therapy system could improve function of the upper extremity in chronic hemiplegia. Methods: A prototype biofeedback system was used by six subjects to perform a set of tasks with their affected upper extremity during a 30-minute session for 20 consecutive working days. When needed for a grasping or releasing movement of objects, the functional electrical stimulation (FES) stimulated the wrist and finger flexor or extensor and assisted the patients in grasping or releasing the objects. Kinematic data provided by the biofeedback system were acquired. In addition, clinical performance scales and activity of daily living skills were evaluated before and after application of a prototype biofeedback system. Results: Our findings revealed statistically significant gradual improvement in patients with stroke, in terms of kinematic and clinical performance during the treatment sessions, in terms of manual function test and the Purdue pegboard. However, no significant difference of the motor activity log was found. Conclusion: Hemiplegic upper extremity function of a small group of patients with chronic hemiparesis was improved through two weeks of training using the FES-assisted biofeedback system. Further research into the use of biofeedback systems for long-term clinical improvement will be needed.

복압성요실금 환자에서 Kontinence HMT2000을 이용한 바이오피드백과 전기자극치료의 효과 (The Efficacy of Biofeedback and Electrical Stimulation by Kontinence HMT2000 in the Treatment of Stress Urinary Incontinence Patients)

  • Background: To evaluate the efficacy of EMG biofeedback and pelvic floor electrical stimulation in the stress urinary incontinence patients by Kontinence HMT2000. Materials and Methods: A group of 14 patients with stress urinary incontinence were treated with combined biofeedback and intravaginal electrical stimulation during 12 sessions from 2 weeks to 6 weeks. Results: At immediate post treatment, subjective cure rate was 28% and improvement rate was 57% and failure rate was 15%. Thus the overall success rate for this treatment was 85%. The result of 3 months after treatment showed cure rate 14% and improvement rate was 43%. Intravaginal pressure increased by an average of 11.9 cmH2O. Increased vaginal pressure was found in 93% of the patients and more than 50% increment of intravaginal pressure was 71%. Conclusions: Combined biofeedback and pelvic floor electrical stimulation by use of Kontinence HMT2000 is effective for the patients who have good compliance, relative low degree stress urinary incontinence. In order to attain good results, a well structured program that teaches specific muscle exercise and the patients should be followed by regular interval reinforcement treatment.

기능적 전기 자극을 이용한 생체되먹임 융합 자세조절 훈련이 뇌졸중 환자의 근활성도와 균형 능력에 미치는 영향 (The Effects of Biofeedback Fusion Postural Control Training using Functional Electrical Stimulation on the Muscle Activity and Balance Ability of the Stroke Patient)

  • 본 연구에서는 기능적 전기 자극을 이용한 생체되먹임 융합 자세조절 훈련을 중재하여 급성기 뇌졸중 환자들의 다리 근활성도와 균형 능력에 미치는 효과에 대하여 알아보고자 하였다. 뇌졸중 환자는 기능적 전기 자극을 이용한 생체 되먹임 융합 자세조절 실험군 15명과 일반적인 생체되먹임 융합 자세조절 대조군 15명으로 나누어 8주간 주 5회, 30분간 시행하였고, 다리 근활성도를 평가하기 위해 근전도를 이용하여 가쪽넓은근, 안쪽넓은근, 넙다리곧은근, 넙다리두갈래근를 측정하였다. 균형 능력을 측정하기 위해 Biorecue를 이용하여 균형 능력을 측정하였다. 다리 근활성도 비교에서는 가쪽넓은근, 안쪽넓은근, 넙다리곧은근, 넙다리두갈래근에서 통계적으로 유의한 차이를 보였고, 균형 능력 비교에서는 신체 중심 이동면적, 총 궤적 길이, 안정선한계에서 통계적으로 유의한 차이를 보였다. 이에 따라 기능적 전지 자극을 이용한 생체되먹임 자세조절 훈련이 일반적인 생체되먹임 자세조절 훈련 보다 다리 근활성도와 균형 능력을 향상시키는데 효과적임을 알 수 있었다.

근전도 바이오피드백을 이용한 훈련이 안면신경마비 환자의 운동학습에 미치는 영향 (Effect of Electromyographic Biofeedback Training on the Muscle Activities of Unilateral Facial Palsy)

  • The purpose of this study was to determine EMG biofeedback training effect on the muscle activities in 3 unilateral facial palsy patients along with multiple baseline design across subjects. The auditory feedback about facial muscles (orbicularis oris, orbicularis oculi, frontalis) was provided with each patient during facial exercise training. Electromyographic (EMG) activity during maximal voluntary contraction and maximal compound muscle action potential (CMAP) amplitude elected by supramaximal electrical stimulation on facial nerve of facial muscles were measured pre- and post- EMG biofeedback training to evaluate motor learning. EMG activity during maximal voluntary contraction was increased after EMG biofeedback training and CMAP amplitude elected by supramaximal electrical stimulation was not changed in all subjects. The results indicate that EMG biofeedback training is useful method to improve motor learning of facial excercise training in unilateral facial palsy patients.

Effectiveness of Intra-anal Biofeedback and Electrical Stimulation in the Treatment of Children With Refractory Monosymptomatic Nocturnal Enuresis: A Comparative Randomized Controlled Trial

  • Purpose: To compare the effects of intra-anal biofeedback (BF) and intra-anal electrical stimulation (ES) on pelvic floor muscles (PFMs) activity, nocturnal bladder capacity, and frequency of wet night episodes in children with refractory primary monosymptomatic nocturnal enuresis (PMNE). Methods: Ninety children of both sexes aged 8-12 years with refractory PMNE participated in this study. They were randomly assigned to 3 groups of equal number: control group (CON) that underwent behavioral therapy and PFM training, and 2 study groups (BF and ES) that underwent the same program in addition to intra-anal BF training and intra-anal ES, respectively. PFMs activity was assessed using electromyography, nocturnal bladder capacity was evaluated by measuring the first morning voided volume, and a nocturnal enuresis diary was used for documenting wet night episodes before treatment and after 3 months of treatment. Results: After training, all groups showed statistically significant improvements in all measured outcomes compared to their pretreatment findings. The ES group showed significantly greater improvements in all measured outcomes than the CON and BF groups. Conclusions: Both intra-anal BF training and ES combined with behavioral therapy and PFMs training were effective in the treatment of PMNE, with intra-anal ES being superior to BF training.

바이오피드백 시스템을 활용한 하지근력 강화운동이 지역사회기반 노인의 하지기능과 보행에 미치는 효과 (The Effect of Lower Extremity Muscle Strengthening Exercise using Biofeedback System on Lower Extremity Function and Gait in Community-based Elderly)

  • 본 연구는 바이오피드백 시스템을 활용한 하지 근력 강화 운동이 지역사회기반 노인에게 어떠한 영향이 있는지 알아본 연구이다. 지역사회기반 노인 10명을 대상으로 주 3회, 6주 동안 바이오피드백 시스템(기능적 전기 자극 동반)을 활용하여 운동을 실시하였다. 데이터 분석은 Wilcoxon signed rank test를 사용하였다. 본 연구 결과 신체 기능은 유의하게 좋아졌으나 근력 및 근육량에서는 유의한 차이가 없었다. 노인들도 지역사회기반 ICT 활용 가정 바이오피드백 훈련이 가능하며, 하지 기능 개선을 유도할 수 있음을 확인할 수 있었지만, 대상자수 확대, 성별 구분, 개인 맞춤형 훈련 방식 제공 등으로 보다 명확한 효과를 입증할 수 있도록 추가적인 연구가 필요하다.

Implementation of Multi-adaptive Filter for EOG Removal and Biofeedback Output Controller

  • In this paper, a multi-adaptive filter is proposed for removing EOG and the 60 Hz power supply noise from EEG measured in the frontal lobe and the feedback output control method is implemented for biofeedback. The multi-adaptive filter has been implemented on the TMS320C6711 DSP system and the feedback output control algorithm has been realized by calculating the ratio of alpha wave on the TMS320C31 DSP system with real time performance. Through the experiment using the implemented multi-adaptive filter and feedback output controller, we demonstrate that the proposed adaptive filter effectively removes EOG and the 60 Hz power supply noise from the measured EEG in the frontal lobe and the feedback algorithm controls the level of stimulation by the ratio of the alpha wave.

골반근육강화훈련이 산후 비뇨생식기 회복, 성생활 및 일상생활 불편감에 미치는 효과 (Effects of Intensive Pelvic Floor Muscle Exercise on Recovery of Genitourinary System, Sexual Life and Daily Life after Normal Delivery)

  • This study was designed to evaluate the effect of pelvic floor muscle exercise using biofeedback and electrical stimulation after normal vaginal delivery. The data were collected from November 1999 to April 2000 at a university hospital located in Seoul, Korea. Out of 49 women with normal vaginal delivery, 25 of experimental group(with exercise) and 24 of control group(without exercise) were questioned about lower urinary symptoms, discomfort during sexual intercourse and daily life. The maximum pressure of pelvic floor muscle contraction(MPPFMC) and duration of pelvic floor muscle contraction(DPFMC) were measured at pre-treatment, the end of treatment and 8 weeks after a treatment program. The pelvic floor muscle exercise program(using biofeedback and electrical stimulation) was applied to the experimental group twice a week for 4 weeks at the incontinence clinic and the pelvic floor muscle exercise at home for that time and more 8 weeks. Data were analyzed by t-test, $x^2$-test, Fisher's exact test and the repeated measures ANOVA. The results were as follows; 1) MPPFMC(p=0.000) and DPFMC(p=0.021) were significantly increased in the experimental group. 2) In the lower urinary symptoms, daily frequency(p=0.001), nocturia(p=0.002), incontinence episode(p=0.016), stress incontinence(p=0.012), quantity of incontinence(p=0.026), straining(p=0.041), and strength of stream(p=0.009) were significantly decreased in the experimental group. 3) Discomfort during sexual intercourse had not a significant difference between the two groups, which was not significantly decreased as time passed. 4) In the discomfort during daily life, activity restriction(p=0.042), exercise restriction (p=0.008), interpersonal relationship restriction(p=0.046), and discomfort of general life(p=0.027) showed a significant difference between the two groups, which were not significantly decreased as time passed. In conclusion, it is suggested that the pelvic floor muscle exercise using biofeedback and electrical stimulation might be a safer and more effective program for the improvement of postpartum pelvic muscle contraction.
