A Relationship of Care Time with Functional Status and Patients Characteristics among Patients in Long-term Care Hospitals (장기요양환자에서 환자 특징 및 기능상태와 환자돌봄 시간과의 관련성)

  • Objectives : The aim of this study was to investigate the functional status variables related to the care time of health professionals for patients in long-term care facilities. Methods : The functional stati of 1001 patients in 8 long-term care hospitals were examined by the Resident Assessment Instrument for Long-term Care Facility Version 2.0. The care time of health professionals for patients was calculated using data from a self-reported task survey by nurses, auxiliary nurses, private aides, doctors, physiotherapists and social workers. Results : The average care time per diem was 240.6 minutes. The care time by doctors, nurses and private aides were 11.0, 71.0 and 139.5 minutes, respectively. The lower the function of activities of daily living (ADL) and the greater the symptoms of extensive services, special care and clinical complexity, the more care time was served. On the contrary, the greater the symptoms of nursing rehabilitation, depression, cognitive disorder, behavior problem and psychiatry/mood disorder, the less care time was served. Age and gender were not significantly related to the care time. Conclusions : Developing a case mix classification system for elderly long term care patients may be helpful for both of patients and health care providers. The ADL, extensive services, special care and clinical complexity of variables should be considered in the development of a case mix system for the long term care of patients in Korea.

Study on Developing the Scale of Korea Citizen's Intentionality out of Inborn Ability and Mission (한국 도시민의 목적지향 성향 척도 개발을 위한 연구)

  • The aim of this study was to develop a scale of the Korea citizen's intentionality out of their inborn ability and mission. To set up the concept of the above intentionality, there have been many academic and religious studies for reference in addition to the existing productivity, Korea elderly successful aging scale, and the scale of life qualities for review. The first 57 items for questionnaires were created thereafter. The final 27 items were confirmed, which were constantly modified by a professor of statistics and corresponding author in effective response and in removing the redundant items after the first items had been reduced to 44 preliminary items through the many an item-by-item discussions with pastors, theologians and social workers. A survey was conducted around the metropolitan area and 380 records were collected. Factor analysis to verify the validity of the measurement tool was completed, and showed that there were 6 sub-factors; "recognition of being", "autonomous life", "self-completion", "initiative", "acceptance", and "economic feasibility". In addition, reliability analysis was implemented and found to be reliable because the Cronbach's ${\alpha}$ value was 0.6 or more.

  • This study was designed to inquire into the effect of care worker's empowerment on job satisfaction, organizational commitment, and turnover intention. For this, a survey was carried out on 200 care workers in elderly care facilities located in Chungcheong area from May 16 to May 30, 2011. Analysis showed that their empowerment has a significant impact on the improvement of job satisfaction and organizational commitment, and the reduction of turnover intention. This means that their high empowerment has a high inclination for job satisfaction and organizational commitment, and a low inclination for turnover intention according to the positive job satisfaction and organizational commitment. Consequently, considering their low empowerment, the job productivity and specialty may be eventually raised by improving their empowerment that brings out their own special abilities and merits with individual personality and aptitude, and showing more concerns and consideration for the job.

Life Cycle-based Dietary Guidelines for Koreans - Examination of Historical Changes and Dietitians' Needs - (한국인을 위한 생애주기별 식생활 지침 - 변천 과정 및 영양사 니즈 조사 -)

  • This study examined the historical changes of and dietitians' needs for the Life Cycle-based Dietary Guidelines for Koreans. Content analysis of relevant documents, a survey of 307 dietitians, and in-depth interviews with eight dietitians were conducted. The dietary guidelines published between 2003 and 2004 included one set of common guidelines and several sets of dietary action guides corresponding to six target groups: pregnant and lactating women, infants and toddlers, children, adolescents, adults, and the elderly. The guidelines were revised between 2008 and 2011 and consisted of six sets of guidelines for the target groups without common guidelines. The dietitians considered five or six as appropriate numbers of guidelines for each group. Needs for separate guidelines for women of child-bearing age and male workers were reported. The dietitians preferred one set of common guidelines with specific action guides for each target group and wanted easier and more specific messages to be included in the new guidelines. It is suggested that the Life Cycle-based Dietary Guidelines for Koreans should be revised to reflect such dietitians' needs.

The Unequal Burden of Self-Reported Musculoskeletal Pains Among South Korean and European Employees Based on Age, Gender, and Employment Status

  • Background: The objective of this study was to elucidate the relationships musculoskeletal pains with combined vulnerability in terms of age, gender, and employment status Methods: The fifth European Working Conditions Survey (EWCS) in 2010 (43,816 participants aged 15 years and over) analyzed for European employees and the third Korean Working Conditions Survey (KWCS) in 2011 (50,032 participants aged 15 years and older) analyzed for Korean employees. In this study, three well known vulnerable factors to musculoskeletal pains (older age, female gender, and precarious employment status) were combined and defined as combined vulnerability. Associations of musculoskeletal pains with combined vulnerability were assessed with prevalence ratios (PRs) and 95% confidence intervals (CIs) estimated by Poisson regression models with robust estimates of variance. Results: The prevalences of musculoskeletal pains were lower but the absolute and relative differences between combined vulnerabilities were higher among Korean employees compared with the European employees. Furthermore, the increased risk of having musculoskeletal pains according to combined vulnerability was modestly explained by socioeconomic factors and exposure to ergonomic risk factors, especially in Republic of Korea. Conclusions: The results of this study showed that the labor market may be more unfavorable for female and elderly workers in Republic of Korea. Any prevention strategies to ward off musculoskeletal pains, therefore, should be found and implemented to mitigate or buffer against the most vulnerable work population, older, female, and precarious employment status, in Republic of Korea.

Policy Trends and Utilization of Information Communications Technologies for the Senior Support in Japan (일본의 고령자 지원을 위한 정보통신기술 정책 동향 및 활용)

  • Super aged society, Japan actively uses Information Communication Technology(ICT) as one of solving methods for various social problem. Especially, for Korea where soon will become super aged society, it would be meaningful to examine the current policy trends and utilization of senior support using ICT in Japan. This study explores the policy trends and utilization of senior support using ICT in Japan through literature review, and then proposes some suggestions as follows. Opportunities for practical use of ICT should be widened in the field of senior support, and efforts are needed to build an information linkage network between institutions for senior support because linkage between care sector and medical sector is important for the elderly support. Besides, simplification of paperwork and easing of burden of care providers or workers in the field should be sought by development and dissemination of various robots. Moreover, the spread of technology for emotional support and stability should be actively pursued by using ICT.

A Survey on the Awareness of Occupational Therapists by the Adult Day Care Center Staff in Korea: Focusing on Managers or Center Directors (국내 노인 주간보호시설 종사자의 작업치료사에 대한 인식조사 : 관리자 또는 시설장을 중심으로)

  • Purpose : This study aims to investigate the awareness of occupational therapists among managers and directors of adult day care centers in Korea to explore the cause of the low employment rate of these therapists and offer the results as basic data that can be used to expand occupational therapy at adult day care centers. Methods : This study collected data by conducting an online questionnaire with managers and directors of adult day care centers located in Seoul, Busan, Incheon, Daejeon, Gwangju, and Ulsan. A total of 70 completed questionnaires were divided into two groups a 'group with experience in occupational therapy information (OT experience)' and a 'group with no experience in occupational therapy information (OT inexperience)' and analyzed. Results : First, the 'OT experience' was found to have higher levels of awareness of the scope of work of occupational therapists than the 'OT inexperience'. Second, significant differences were found in the degree of awareness and necessity of occupational therapists and plans to employ occupational therapists later between the 'OT experience' and the 'OT inexperience'. Third, it was shown that there were significant correlations between whether the respondents were familiar with occupational therapy and the degree of awareness and necessity of occupational therapists and plans to employ occupational therapists later. Conclusion : Based on these findings, the following measures are recommended to expand the area of occupational therapy in adult day care centers, the need for occupational therapists should be mentioned in the health and welfare-related education for workers at these adult day care centers, policies such as a medical fee system for occupational therapy at these adult day care centers should be prepared.

A Study on a space utilization plan for screening clinic in public health center by means of the prevention of respiratory infectious disease - Focused on a negative pressured tuberculosis exam room (호흡기 감염병 예방을 위한 보건소 상시 선별진료소 활용방안 연구 - 음압 결핵 검진실을 중심으로)

  • Purpose: Tuberculosis(TB) care unit in public health center should be carefully considered to be re-designed as an infection safety environment for both patient and healthcare workers. So, for the enhancement, this study analyses the facility requirements for co-using the screening clinic as a TB and other respiratory disease care unit. Methods: Not only screening clinic facility guidelines from "A Study for Standard Triage Design and Construction Document" but also the guidelines of TB care and related medical facility are reviewed; KDCA, CDC, ECDC and WHO as a TB care, and FGI and NHS for facility. The facility requirements are summarized space, approach, and mechanical requirement in order. By comparing the summary and screening clinic facility guidelines, supplementations are proposed for TB care unit setting. Results: The result of this study shows that both the space program and mechanical requirement of the screening clinic and that of TB care unit are almost identical and could be share, which include direct airflow or negative air pressure in an exam room. To increase functional and economical efficiency, however, it is necessary to consider a multi-functional negative pressured room, So care process may be re-designed based on a room type; face-to-face room or glass wall inbetween. Implications: The facility guidelines for TB care unit of a public health center should be developed to build a safe environment for infection control by reflecting its medical plan and budget.

Consumer Categorization Based on Perception of Functional Jacket and their Utilization of Functional Information and Hang-tag (기능성의류 인식에 따른 소비자 분류와 이에 따른 제품정보 및 행택 활용도 연구)

  • As a part of integrated study for technical communication between consumer and manufacturer of functional clothing, functional clothing consumers were categorized based on their perception of the functional outdoor jacket and the differences in the source of information and use of hang-tag were examined according to the groups. Questionnaire survey was conducted on 472 males and females in their 20s to 60s, who have experience of purchasing functional outdoor jackets. Perception of functional outdoor jackets was classified into four factors ('functionality', 'price', 'experience/knowledge' and 'brand/design') and the respondents were categorized into three groups according to the factors: 'unconversant/brand & design pursuing group', 'conversant/function pursuing group' and 'high-priced products preferring group'. 'Unconversant/brand & design pursuing group' was composed of high rate of office workers and students in their 20s and 30s who use internet most frequently as source of information while 'conversant/function pursuing group' was composed of high rate of professionals in their 40s and 50s who make the most frequent use of information on hang-tag. 'High-priced products preferring group', which has high rate of the elderly in their 60s and housewives, obtain their information mainly from salespersons. 'Unconversant/brand & design pursuing group' was satisfied with functional information more than other two groups and 'conversant/function pursuing group' concerned for the hang-tag and required more explanation of terminology used in hang-tag. The results effectively explained the features of consumer groups and their different responses toward the information of functional outdoor jacket.
