Objective: The purpose of this case study was to investigate selective region high-frequency diathermy at trigger points with therapeutic exercises on pain, function, balance and gait in older patients with degenerative knee osteoarthritis (DKO). Design: A case report. Methods: The patient who participated in this study was a 71-year-old woman, who had been diagnosed with moderate osteoarthritis with grade II Kellgren & Lawrence grading scale. The intervention consisted of selective region high-frequency diathermy at trigger points, with hip and knee stretching and strengthening exercises. The participant was given assessments before and after every intervention session using the Visual Analogue Scale (VAS), Western Ontario and McMaster Universities Arthritis Index (WOMAC), the Timed Up and Go test (TUG) and the 10 Meter Walk Test (10MWT). The participant performed the intervention 18 times for a total of 30 minutes each. Results: As a result of this study, the patient VAS decreased to 3 points, and the WOMAC decreased to 53 points. In addition, the TUG decreased to 3.25 s and the 10MWT decreased to 1.14 s. Conclusions: The results of this study suggest that selective region high-frequency diathermy at trigger points with therapeutic exercises may be an effective intervention to decrease pain, improve knee function, balance and gait in patients with DKO. The selective region high-frequency diathermy with therapeutic exercises may be feasible and provide potential benefits for rehabilitation of DKO.
Muscle stiffness, pain and discomfort in dally activities are cardinal symtoms on arthritis. To reduce these symtoms, a nonequivalent one-group pre & posttest experiment was carried out at a rural community health post area in Chonbuk province during eight weeks (from 25th of January to 20th of March, 1999). The subjects were sixteen rural old women(65-89yrs) with osteoarthritis. And to reduce the level of pain, discomfort in daily living and to increase that of life satisfaction, 30 minutes of flexibility exercise followed by $15{\sim}20\;minutes$ of local heat therapy were done three times a week. Pain scale from Arthritis Impact Measurement Scales developed by Meenan(1980) for pain, Jette's Daily Activity Discomfort Scales revised by Lee(1988) for discomfort and Life Satisfaction Scales developed by Youn(1982) for life satisfaction were used as research instruments. The data were collected by face-to-face interviews with Investigator and the experiments were done under demonstration and supervision of investigator in community health post. The data were analyzed in percentage and Wilcoxon sign rank sum test by SPSS/PC+ program. The result could be summarized as follows ; 1. The scores of experiment group who were applied hot bag followed by flexibility exercise were tended to be significant lower in the level of pain statistically(Z=4.949, P=0.0001). 2. The scores of experiment group who was applied hot bag followed by flexibility exercise were tended to be significant lower in the level of discomfort in daily living statistically(Z=2.7226, P=0.0279). 3. The scores of experiment group who was applied hot bag followed by flexibility exercise were tended to be higher in the level of life satisfaction, but there was no significant statistical difference(Z=-1.2087, P=0.2171). In conclusion, flexibility exercise with local heat therapy could be identified as an effective therapy to reduce pain and discomfort in daily living of the rural elderly with osteoarthritis. And as a community health practitioner, the investigator concluded that this exercise with local heat therapy could be developed as a health promotion program for the rural aged people.

Purpose: This study was done to identify the effects of a health diary program on fall related outcomes the low-income elderly women. Methods: The study was carried out with a nonequivalent control group with pretest-posttest design. The study was composed of two groups, each made up of 24 subjects: experimental group and control group. The subjects were low-income women aged over 65 years with osteoarthritis and both the experimental and control groups were made up of subjects with the same age profiles. The independent variable was the health diary program, and the dependent variables were fall related outcomes (fear of falling, fall-efficacy, knowledge of fall) difficulty of performing activity, and mood state. The health diary program was performed for 50 minutes each session and twice a week for 8 weeks. Data were collected before the health diary program 10 weeks after the beginning of the program. Results: The experimental group showed significant differences in knowledge of fall, fear of falling, and mood state compared to the control group. However there was no significant differences in difficulty of performing activity and fall efficacy. Conclusion: The results of this study may be used as part of an education to prevent falls for low-income elderly women with osteoarthritis.

Purpose: The purpose of this study was to identify health promoting lifestyle patterns (HPLP) and health perception (HP), and related factors, in elderly people using welfare service centers. Methods: Three hundred elders were interviewed using a structured questionnaire through convenience sampling at two welfare centers. Data were analyzed using the SPSS WIN 12.0 program. Results: Most of the elders had some kind of diseases, such as hypertension, diabetes, or osteoarthritis. The mean HPLP score was 2.53, and significant differences were found according to the elders' general characteristics. The sub-domain nutrition had the highest mean score, and stress management and exercise, the lowest. The mean HP score was 3.38, and significant differences were found according to education level and number of underlying diseases. HPLP showed a correlation with HP. Conclusion: The results indicate that elders have poor practices in stress management and exercise reflecting need for community based stress management programs for welfare service centers elderly clients.

In elderly patients, the vital parameters tend to fluctuate based on the blood volume status, which may cause sudden hypovolemic shock if the postoperative bleeding continues. Particularly, those who undergo surgery for arthritis needs to pay extra attention because the bleeding may persist over the joints after the surgery. Therefore, appropriate pain control is required to reduce the postoperative blood loss. This retrospective chart review study was conducted to assess the postoperative pain control and reduction of blood loss with a single injection of saphenous nerve block (SNB) in elderly patients with osteoarthritis. We reviewed the electronic medical records of patients who underwent knee total arthroplasty with spinal anesthesia between January and May 2016. A total of 51 patients participated in this study. All patients were treated with intravenous patient-controlled analgesia for the postoperative pain control, and additional analgesic agents were administered at a visual analogue scale above a score of 6. In 25 patients, SNB was performed using ultrasound with the administration of 0.75% ropivacaine (15 mL) after the surgery. Patients who received additional analgesics were significantly low in the nerve block group (P=0.009). Additionally, the volume of blood loss from catheter drainage was significantly low at 2 and 3 days postoperatively (P=0.013 and P=0.041, respectively) in the nerve block group. In patients who underwent total knee arthroplasty with osteoarthritis, only a single injection of saphenous nerve block was sufficient for the postoperative pain control and reduced bleeding.
The purpose of this study was to assess risk factors associated with food insecurity in the Korean elderly aged over 65 years using data from the Korea National Health & Nutrition Examination Survey 2013 (KNHANES VI). A total of 1,200 subjects were analyzed among the participants of the KNHANES 2013 by using SPSS statistics complex samples (ver. 21.0). Food insecurity was measured by using the modified US Household Food Security/Hunger Survey Module. Thirteen percent of subjects lived in food insecure households. There were differences in the prevalence of food insecurity according to sex, educational level, income level, and household structure. Mean age of the food insecurity group was significantly higher than that of the food security group. The results of logistic regression analysis showed that food insecurity was significantly associated with alcohol intake (OR: 1.82), prevalence of melancholy (OR: 2.07) and suicidal thoughts (OR: 2.67), and intake deficiency of energy (OR: 1.60), calcium (OR: 1.97), iron (OR: 1.97), potassium (OR: 1.96), riboflavin (OR: 1.76), and niacin (OR: 1.64), while not with smoking, physical activity, chronic diseases including obesity, anemia, diabetes, and osteoarthritis, and deficiency of protein, vitamin A, thiamin and vitamin C. These findings suggest that food insecurity is strongly related to mental health and certain nutrient intakes. Prospective research is needed to establish the effects of food insecurity on chronic diseases.

Purpose : This study was implemented to identify the risk conditions influencing suicidal ideation in a nationally representative sample of elderly South Koreans. Methods : Data from 1,152 men and 1,581 women aged 65 years or older were gathered from the 2013 and 2014 Korea National Health and Nutrition Examination Survey VI. All analyses were performed using SPSS. To determine significant correlations between risk condition and suicidal ideation, a t-test was used. Results : There were differences in suicidal ideation according to the following individual factors: age, educational background, marital status, economic activity, recognition of stress, experience of depression, and economic status. There were differences in suicidal ideation according to the following health-related factors: subjective health status, EQ-5D (EuroQoL-5 Dimensions), hours of sleep, and BMI. There were also differences in suicidal ideation according to the following disease-related factors: HTN, COPD, asthma, stroke, depression and osteoarthritis. Conclusion : The findings indicate that broad intervention programs should be distributed to prevent suicidal ideation. It is also recommended that programs be developed in a way that can help manage the variables identified in this study. Furthermore, follow-up studies should be conducted to verify the program.

PURPOSE: The purpose of this study was to compare the maximum knee strength and endurance of elderly Korean women with Kellgren-Lawrence grade (K-L II) by age. METHODS: In this study, the total number of participants was 50, divided into five groups of 10 by age. Maximum knee muscle strength was measured at a low speed of 60°/sec using Biodex, an isokinetic device for comparison of maximum knee muscle strength, whereas muscle endurance was measured by muscle contraction at a high speed of 180°/sec. The peak torque values of the extensor and flexor muscles in the muscle strength and muscle endurance tests were collected according to each classification. RESULTS: The results of the experiment show that maximum muscle strength and muscle endurance decreased significantly at 60°/sec and 180°/sec during flexion and extension according to age (p<.05). In particular, the age group of 71 to 75 years showed the most significant correlation with other age groups (p<.05). CONCLUSION: The results of this study can be used as basic data for increasing physical activity and muscle strength in women with knee osteoarthritis.

Purpose : The purpose of this study was to investigate the effect of kinematic variables by analysis of a gait in older. Methods : This study selected nine in older adults with osteoarthritis. The Kinematic variables during walk were compared analyzed using motion analysis. Results : The findings of this study are as follows. Stance time showed significant difference within-subject groups and interaction within-subjects and time. The swing and stride time showed a no significant interaction within-subjects and time. Swing time showed a no significant difference according to time and within-subjects. Stride time showed a significant difference according to time. But, stride time showed a no significant difference according to within-subjects. Conclusion : These findings of this study indicate that when the patients with total knee arthroplasty decreased stability. Therefore, stance and stride time showed increase when walking, because to decrease the weight bearing that is delivered to knee. And swing time showed decrease.

Although the atlantoaxial joint is the most common site of rheumatoid arthritis, our patient had no symptoms or signs suggestive of rheumatoid arthritis. The atlantoaxial joint is frequently involved in degenerative osteoarthritis, especially in elderly patients. An 81-year old man presented with severe intermittent electric shock like, lancinating pain from the occipital to the temporal and parietofrontal areas. He also had neck pain and a limited range of motion. After many examinations and laboratory tests, at the department of neurology and neurosurgery, he was diagnosed with idiopathic neuralgia. The diagnosis of atlantoaxial joint syndrome was confirmed, and treated successfully with atlantoaxial joint block.
