노인의 혈청 삼투압과 건강관련 요인, 생화학적 변수들과의 상관성 (Serum Osmolality and Its Association with Health-Related Factors and Biochemical Parameters in the Elderly)

  • This study was designed to determine serum osmolality and to investigate the association among serum osmolality, health-related factors and biochemical indices. Two hundred thirty seven elderly(86 male, 151 female : mean age 73.8) residing in the chungbuk area participated. Sociodemographic data and self-perceived health status were obtained by interview, and biochemical parameters were measured. The mean serum osmolaity was 301.2$\pm$10.1 mOsm/kg(range 240.8~328.9 mOsm/kg) and serum osmolality of the females(302.4 mOsm/kg) was significantly(p<0.05) higher than that of the males(299.0 mOsm/kg). Only 11.0% of the elderly were within the accepted normal range(280~295 mOsm/kg) and the majority of the subjects were in a hyperosmolar state(60.8% : 296~307 mOsm/kg). It was observed that serum osmolality was reduced with increasing age but not statistically significant. There was no significant difference in serum osmolality according to marital status. Serum osmolality was significantly lower(p<0.05) in the elderly with higher self-perceived health scores than the elderly with lower self-perceived health scores, however no significant difference was observed between the presence or absence of disease. Serum osmolality tended to be higher(p=0.06) in the elderly with difficulty in mobility than the elderly who have no problem in mobility. Serum osmolality tended to be higher(p=0.06) among drug users compared to that of non-users. There was a significant association between serum osmolality and serum albumin in females but not in males. Significant associations were shown between serum osmolality and serum transferrin in both sexes. However, there was no significant correlation between serum osmolality and hemoglobin or hematocrit except with hematocrit in females only. The results of this study indicated that the elderly were dehydrated and hydration state seems to influence health status and, the elderly should be encouraged to drink an adequate amount of fluids. Also, the hydration state is required to be assessed and considered for the interpretation of biochemical parameters.

가구형태에 따른 노인의 식품섭취 빈도 비교 - 2008년 경기북부 지역사회건강조사 자료를 이용하여 - (A comparison of food frequency for the elderly regarding different family types - Based on Community Health Survey for 2008 -)

  • This study analyzes the food frequency for the elderly regarding different family types and finds the factors for nutritional risk, offers a basic reference for providing nutritional support for them. The study referred to the dietary behavioral survey data of 3,680 elderly people (1652 male and 2028 female) from 21 regions in the northern Kyeonggi province. The data was collected through the method of one-to-one interviews and was a part of the Community Health Survey for 2008 by the Korea Centers for Disease Control and Prevention (KCDC). We classified family types as a household for elderly people living alone, a household of elderly people with a spouse, a household of the elderly with unmarried children and a household of the elderly with married children, and as for intakes of foods, the frequencies of taking fruits, vegetables, kimchi, rice with mixed cereals, meat, fish, bean tofu soymilk, milk and dairy products, as well as sweet beverages are calculated on a daily basis and skipped meals are calculated on a weekly basis. Elderly women showed lower income, lower education level, higher unemployment rates, and a higher rate of government healthcare subsidies than elderly men. Elderly women tend to live alone and with their children while elderly men tend to live with their spouse. In both males and females, the intake of fruits and vegetables were the least in the elderly living alone, while the elderly with married children ate the most. In both males and females, the household of the elderly living alone ate significantly less amounts of Kimchi than other family types. Elderly people living alone tended to have significantly less meat and fish, especially women. In the case of rice with mixed cereals, the elderly men living alone and the elderly men with unmarried children ate significantly less amounts than the elderly men living with a spouse. The elderly men living alone took significantly less milk and dairy products than the elderly men with unmarried children while the elderly women living with a spouse took significantly less milk and dairy products than the elderly women with married children. With regards to the frequency of meal-skipping, the elderly living alone had the highest frequency for skipping meals. From this result, having various foods is difficult for the elderly living alone. Furthermore, the elderly living with unmarried children demonstrated a low quality of dietary life compared to those of married children. Hence, it can be concluded that social support is important in order for the elderly to have a balanced diet.

완도지역 성인 및 노인의 영양소 섭취 실태조사 (A Study on Nutritional Intakes in Elderly People in Wando Area)

  • The purpose of this study was to investigate nutrients intakes and nutritional quality of Adults and Elderly People in an island area (Wando). A three-day dietary intakes survey, using a 24 hour recall method was obtained from 187 subjects aged 46 to 84 (mean age 65.3) living in an island area (Wando). Nutient intakes were analyzed using CAN-Pro soft program and compared to Korean RDA. The quality of nutrients was assessed by analyzing nutrient adequacy ratio (NAR), mean adequacy ratio (MAR), and index of nutritional quality (INQ). The average daily mean energy intakes were 1869.0 kcal for males and 1943.9 kcal for females, respectively. Daily intakes of protein for males and females were 28.0 and 30.4 g and those of fat were 31.5 and 28.51 g, respectively. Nutrient consumed below $75\%$ of Korean RDA was protein, vitamin A, Ca and Zn in both males and females. Average CPF ratio of males and females were 78.8 : 6.0 : 15.1 and 80.0 : 6.4 13.4, respectively. Energy intake ratio from protein was significantly higher in over 60 years males. Carbohydrate dependency decreased with age. Protein dependency increased with age. Nutrient adequacy ratio (NAR) of energy, protein vitamin A and vitamin E were increased with age in males. The mean adequacy ratio (MAR), an index of overall dietary quality, were not significantly different by age group. Average MAR for males and females was 0.77 and 0.78, respectively. The index of nutritional quality (INQ) showed the tendency to decrease with age. Especially, there were significantly decreases in INQ of all nutrients, except protein, with age. Based on these results, it is evident that people in the island area did not consume enough nutrients. Specially, dietary intake of protein, vitamin A, vitamin E, and Ca were not adequate. 노인의 재혼연구 (A Study on the elderly remarriage)

  • Researches found that the attitude of adult married children played an important role for the satisfaction and stability of the elderly remarriage. Therefore this study focused on the children's perception of the elderly remarriage. The results were as follows: Sex was found to be influential to elderly remarriage. Males were more positive than females. The degree of sex-stereotyping and supporting experience of the elderly parents were found to influence on children's perception. adult children positively perceived elderly remarriage as giving emotional satisfaction mutual-dependence and liveliness or freedom of later life. Meanwile they negatively considered elderly remarriage mainly because of traditional public attitudes toward remarriage difficulties of adaptation with step-familes and financial or legal conflicts. Adult children regarded health character financial independence and children's agreement level as the most considerate factors whereas the elderly the adaptati n among step-family members marital adaptaion and public attitudes toward their remarriage.

가정운동 훈련프로그램이 노인의 근력증진에 미치는 영향 (Effects of Home Exercise Program on the Improvement of Muscle Strength in the Elderly)

  • The purpose of this study was to investigate the effect of home exercise program on the improvement of muscle strength in the elderly. Subjects were forty members living in Daegu (20 males, 20 females), between 65 and 81 years of age. The subjects were divided into two groups; an experimental group and a control group, and each group included 10 males and 10 females. The subjects for the experimental group were participated in the home exercise program for 8 weeks, between April 2001 and June 2001. The results of this study follow: 1.General characteristics statistically significantly affecting for knee extensor strength were weight, height, right sight vision, diastolic blood pressure, pulse rate, obesity and thigh BMD; vision and thigh BMD were for knee flexor strength 2. After the exercise program, knee extensor strength in the experimental group improved 30.8% (p<0.001), and knee flexor strength improved 23.9% (p<0.001). 3. After the exercise program, there was significant difference in knee extensor strength (p<0.05) and knee flexor strength (p<0.05) between the experimental group and the control group. As a continuous health care for the elderly using this home exercise program could be helpful to enhance health of the elderly and promote their quality of life.

간섭전류전기자극이 만성요통 노인 환자의 통증, 균형 및 보행능력에 미치는 즉각적인 효과 (Immediate Effects of Interferential Current Stimulation which Affects Pain, Balance and Walking Ability of Elderly Patients with Chronic Low Back Pain)

  • Purpose: The purpose of this study was to investigate the effects of Interferential current stimulation therapy(ICT) on pain, balance, and walking ability of elderly patients with chronic low back pain. Methods: Twenty participants were randomly assigned to ICT group (2 males and 8 females) and placebo ICT group (3 males and 7 females). Participants were ICT and placebo ICT for 20 minutes, and the pre- and post-VAS, TUG and postural balance were measured. Results: The ICT group showed significantly decreased pain scores in the elderly patients with chronic low back pain (p<.05) than the placebo ICT group. The postural fluctuation with eyes opened and closed was significantly lower than the placebo ICT group(p<.05), and the ICT group showed remarkable improvement (p<.05). Conclusion: ICT was expected to improve decrease pain, improve postureal sway and walking ability remarkably. Therefore, It was expected that the application or ICT would be an effective method for elderly patients with Chronic Low Back Pain.

노인의 성별에 따른 인지기능과 영양관리상태의 차이 (Gender Differences in the Cognitive Function and Nutritional Status in Older Age: A Representative Nationwide Data of Korean Elders)

  • Purpose: The purpose of this study was to identify the gender differences in the cognitive function and nutritional status among elderly people living in the community. Methods: This study used data from the 2014 National Survey on the Elderly and focused on 10,054 respondents who answered that they did not have dementia. The Korean version of Mini-Mental State Examination for Dementia Screening and the Nutrition Screening Initiative were used. IBM SPSS Statistics 23.0 was used to conduct statistical analyses. Results: The cognitive function score was 25.30 for men, which was higher than for women. The percentage of subjects who showed cognitive decline was 25.5% for males and 50.2% for females, which was statistically significant (p<.001). There were statistically significant differences in the nutritional status between males and females (p<.001). There was a statistically significant negative correlation between the cognitive function and nutritional status in both men and women (p<.001, p<.001). Conclusion: These results showed that the intervention to maintain cognitive functioning should be provided to elderly women with a low education level or high number of chronic diseases and medication, and an intervention to prevent the cognitive decline of the elderly should include nutritional management for health and function maintenance.

  • This study was designed to assess the effect of a meal service for home-staying urban elderly people with low-income on their protein nutrition status and serum lipids. One hundred and eighty three subjects, who had already completed the first nutritional survey were assigned to two groups : meal served(served) and non-meal served (non-served). A meal approximately containing one half of the RDA for energy, protein, calcium and iron was served as lunch everyday to served group. After 6 months of meal service, the second nutritional survey was done and changes of parameters were analyzed with paired t-test. Served females showed significantly increased intake of energy while non-served females showed significantly decreased intakes of energy, protein and fat after 6 months. Served males were observed with significantly higher intake of energy, fat, vitamin A, niacin and ascorbic acid, whereas intake of vitamin A was lower in non-served males. Arm circumference of both served and non-served females were significantly lowered after 6 months. Mean DBP of served males was significantly lower, while SBP of served females was significantly higher after 6 months of meal service. Serum total protein, serum albumin, serum cholesterol and LDL-cholesterol were significantly increased within normal range regardless of gender and meal service. But HDL-cholesterol was significantly higher in only served elderly subjects after 6 months. (Korean J Community Nutrition 1(2) : 228-238, 1996)

A Study on the Nutrient Intakes of Hospitalized Elderly Dementia Patients Receiving a Regular Diet

  • The purpose of the present study was to examine the nutrient intake status of elderly dementia patients. We surveyed the dietary intake of 50 demented elderly patients receiving a regular diet, who were hospitalized in a geriatric hospital in Yongin, Gyeonggi-do. The average age of the subjects was $79.6{\pm}6.5$ for the males and $80.5{\pm}6.3$ for the females. The average heights were $169.5{\pm}6.0\;cm$ (males) and $154.6{\pm}5.2\;cm$ (females), and the average weights were $58.5{\pm}7.4$ (males) and $51.7{\pm}8.9\;kg$ (females). Depending on the type of dementia, the male patients showed significant differences in their intakes of vegetable protein, fiber, total iron, and non-heme iron between the Alzheimer's disease (AD) and the vascular dementia (VD) groups, and the female patients showed significant differences in their intakes of total protein, phosphorus, zinc, and vitamin $B_6$ between the two groups. The male patients showed significant differences in their intakes of animal-source calcium and carotene according to ability to self-feed. According to physical activity, the male patients showed significant differences in vegetable fat and vitamin A intake, and the females showed significant differences in their intakes of total protein, animal protein, fiber, ash, total calcium, vegetable-source calcium, animal-source calcium, phosphorous, total iron, non-heme iron, heme iron, sodium, vitamins $B_1$, $B_2$, $B_6$, niacin, vitamin C, folate, and potassium. We found that the patients consumed excessive protein, but low amounts of calcium, vitamin $B_2$, and folate. Also, the patients' physical activity abilities appeared to affect their nutrient intakes.

장수 노인의 신체특성과 수단적 일상생활 수행능력 -경북 성주지역을 중심으로- (Physical Characteristics and Instrumental Activities of Daily Living of the Elderly(85+) in Kyungpook Sung-Ju Area)

  • The purpose of this study was to investigate the physical characteristics and instrumental activities of daily living(IADL) of the elderly (85+). The subjects were three-hundred elderly people over the age of 85, living in Kyungpook Sung-Ju area who have no problems in daily living. The survey on anthropometric measures and IADL were carried out by individual measurement and interviews using measuring devices and questionnaires. The subject group of the study was composed of 27% males and 73% females, the average age being 87 years old. Notable physical features of the subjects include considerably short statues and small physiques compared to the average Korean elder. The mean Body Mass Index(BMI) was 20.9. The average percentage of body fat in male and female was 21.7% and 29.5% respectively, and the average waist/hip ratio(WHR) was 0.9 and 0.85 respectively. The percentage of the subjects who were able to perform all the IADL was 51% in males and 25% in females. The male subjects were more capable than the females in all the IADL except for preparing meals. BMI showed positive correlations withy weight(r=699), WHR(r=0.157), body fat percentage(r=0.57), lean body mass(r=0368) and total body water(r=0.368). WHR also had positive correlation with weight(r=0.184), BMI(r=0.157), lean body mass(r=0.149) and total body water (r=0.148). Body fat percentage had a positive correlation with weight(r=0.147) and BMI(r=0.265) and total body water(r=0.265). In summary, the elderly(85+) in the Sung-Ju area had considerably smaller physiques than the average Korean elderly and showed a normal range of BMI in average. The IADL of the elderly(85+) appeared to have a positive correlation with lean body mass and total body water and a negative correlation with body fat percentage.
