자녀와의 교류가 노인 주거이동에 미치는 영향 분석 (The Effect of Old Korean's Interactions with their Children on Residential Mobility)

  • 초고령사회 진입을 앞두고 우리나라는 노인인구의 급속한 증가로 노인주거 문제가 중요한 이슈로 떠오르고 있다. 특히, 노인인구의 살던 곳에서 노후 맞이하기(Aging in Place: AIP)는 노인복지의 지향점으로 꾸준히 제시되고 있다. 이에 본 연구는 2008-2020년 고령화연구패널 자료와 임의효과 로지스틱 회귀분석 모형을 사용하여 자녀와의 교류가 노인 주거이동에 미치는 영향을 실증 분석하였다. 연구 결과는 다음과 같다. 첫째, 자녀와의 교류가 수도권과 비수도권에 무관하게 노인인구의 주거이동을 촉진시킬 확률이 높다는 점을 발견하였다. 둘째, 연령이 증가할수록 자녀와의 교류는 노인인구의 주거이동을 더욱 촉진시키는 경향을 보였으나, 그 영향력은 연령과 변수에 따라 일정치 않게 나타났다. 셋째, 자녀와의 교류 변수들의 상호작용 효과를 통해 노인인구의 주거이동을 더욱 촉진시킬 가능성이 높아짐을 확인하였다. 본 연구는 자녀와의 교류가 노인인구의 주거 안정성을 증진 시켜 독립적인 정주 환경을 구성할 수 있는지, 즉 노인인구의 살던 곳에서 노후 맞이하기 측면에서 정책적 시사점을 제시한다.

노인의 흡연상태와 나이가 SCE 빈도수로 본 임파구 DNA 손상에 미치는 영향 (Effects of Smoking and Age on SCE Frequency Reflecting DNA Damage of Human Lymphocytes in Elderly Koreans)

  • Sister chromatid exchange (SCE) has recently become a common cytogenic assay system for detecting exposure to chemical mutagens and carcinogens. One application of SCE is the monitoring of populations believed to have been exposed to such agents. A cross-sectional study of SCE frequency in peripheral blood lymphocytes from 45 Koreans aged 61 to 84 years was conducted. The effect of cigarette smoking and age on SCE was assessed by different degrees of smoking status such as smokers (n = 14), ex-smokers (n = 16) and non-smokers (n = 15). Mean spontaneous SCE per cell for the smokers (11.5 $\pm$ 1.1) was significantly higher (p < 0.05) than that for the non-smokers (8.8 $\pm$ 0.3). However, mean SCE frequencies per cell for the ex-smokers (10.3 $\pm$ 0.6) were not significantly different from those of the smokers or the non-smokers. The smokers showed an increased number of high SCE frequency cells (HFCs) when compared to the ex-smokers and non-smokers (p < 0.05). The mean SCE frequencies of the non-smokers showed a statistically significant increase (p < 0.05) with the subject's age. These results show that age and smoking habits contribute a great deal in setting a higher degree of basal DNA damage in elderly Koreans, and smoking appeared to be a more significant damaging factor than age.

단전호흡을 수련하는 성인과 노인의 양생정도와 삶의 질에 관한 연구 (Yangsaeng and Quality of Life (WHOQOL-BREF) in Adult and Elderly Koreans Practicing Dan Jeon Breathing (DJB))

  • Purpose: The purpose of this study was to explore the relationships between Yangsaeng and quality of life (WHOQOL-BREF) of Korean adults and older people practicing Dan Jeon Breathing. Methods: The participants were 133 adults and elderly people practicing Dan Jeon Breathing. Data were collected by self-reported questionnaires, which were constructed to include Yangsaeng and quality of life (WHOQOL-BREF). Frequency, percentage, mean standard deviation, t-test, ANOVA, Scheffe test, Pearson correlation coefficient, and Cronbach ${\alpha}$ value were used with the SPSS 14.0 win program to analyze the data using for finding. Results: Significant differences in Yangsaeng were evident on the basis of age, gender, duration of DJB, regular practice of DJB, and other exercises. There were significant differences in quality of life in terms of existence of spouse and duration of DJB. Also, all subcategories in Yangsaeng were positively related to all domain of quality of life. Conclusion: The results indicate that the quality of life of adult and elderly Koreans who practice DJB is more closely associated with Yangsaeng. Therefore, Dan Jeon Breathing can be utilized as a nursing intervention for Yangsaeng and quality of life of Koreans.

Relationship between Health-Related Quality of Life and Suicide Ideation in a Nationally Representative Sample of Elderly Koreans

  • Purpose : This study was implemented to identify the risk conditions influencing suicidal ideation in a nationally representative sample of elderly South Koreans. Methods : Data from 1,152 men and 1,581 women aged 65 years or older were gathered from the 2013 and 2014 Korea National Health and Nutrition Examination Survey VI. All analyses were performed using SPSS. To determine significant correlations between risk condition and suicidal ideation, a t-test was used. Results : There were differences in suicidal ideation according to the following individual factors: age, educational background, marital status, economic activity, recognition of stress, experience of depression, and economic status. There were differences in suicidal ideation according to the following health-related factors: subjective health status, EQ-5D (EuroQoL-5 Dimensions), hours of sleep, and BMI. There were also differences in suicidal ideation according to the following disease-related factors: HTN, COPD, asthma, stroke, depression and osteoarthritis. Conclusion : The findings indicate that broad intervention programs should be distributed to prevent suicidal ideation. It is also recommended that programs be developed in a way that can help manage the variables identified in this study. Furthermore, follow-up studies should be conducted to verify the program.

도시 지역에 거주하는 노인의 영양상태에 관한 연구 (Nutritional Status in Healthy Elderly Koreans from Urban Households)

  • Nutritional Status of 74 elderly Koreans(35 men and 39 women) from middle-income households in Incheon were evaluated in terms of dietary nutrient intakes, anthropometric measurements and biochemical blood analysis. The mean consumptions of all nutrients were lower than R.D.A except iron, thiamine and niacin. However, 25.7% of men and 33.3% of women consumed less amount of iron than 75% of R.D.A. Average body mass index of women was sightly but significantly higher than that of men (P<0.05). Obesity rate and triceps skinfold thickness of women were much higher than those of men (23.3% vs 5.4% and 14.9mm vs 9.6mm, respectively). By judging from amounts of hemoglobin, hematocrit, serum iron, total iron binding capacity, mean corpuscular hemoglobin concentration and transferrin saturation of the elderly, 5.6-33.3% of men and 5.0-33.3% of women seem to be possibly anemic.

The Association of Plasma HDL-Cholesterol Levels with Dietary, Anthropometric, and Hematological Factors in Elderly Koreans

  • Cardiovascular disease (CVD) is one of the most common causes of death in elderly Koreans, and HDL-cholesterol is known to have a pivotal role in protecting against CVD. This study was undertaken to study the relationships between plasma HDL-cholesterol levels and dietary, anthropometric, and biochemical factors in elderly Koreans. The 102 subjects, who were over 60 years old, were classified into two groups based on their plasma HDL-cholesterol levels: a risk group with plasma HDL-cholesterol < 40mg/dl in men or HDL-cholesterol < 50mg/d1 in women, and a control group with higher HDL-cholesterol levels. The subjects' mean intakes of energy, calcium, zinc, vitamin A, vitamin $B_2$, vitamin E, and folate did not meet the Korean RDA for elderly people. Vitamin $B_2$ and folate intakes were significantly lower (p<0.l) in the risk group compared to the control group. The consumption of seaweed was significantly lower (p<0.05), and fish intake was 33% lower, in the risk group compared to the control group. Subjects in the risk group showed a higher BMI, waist/hip ratio, triceps skinfold thickness, and % body fat, compared to control subjects. Plasma triglyceride levels and values of the atherogenic index were significantly higher (p<0.00l) in risk group subjects. Significant negative correlations between HDL-cholesterol level and plasma triglyceride level (r= 0.37), and values of the atherogenic index (r=-0.74), were found. In summary, subjects with low levels of HDL-cholesterol were found to have relatively low intakes of vitamin B$_2$, folate, and seaweed, and higher levels of the CVD risk factors: body fat, plasma TG, and AI. These results suggest that plasma HDL-cholesterol levels can be modified by dietary, anthropometric, and hematological means.

노인소비자의 학력수준이 외식구매의사결정 과정에 미치는 영향에 관한 연구 (Effect of the Elderly Consumers' Education Level on Eating-Out Decision Making Process)

  • As Korea has approached the aging society, older Koreans have become an important force in restaurant sales today. To succeed with this silver market, it is important for restaurant managers to know who they are and what factors influence older Koreans' eating-out decision making process. The purpose of this study was to investigate the effect of the silver consumers' education level on eating-out decision making process. Data were collected from 178 older consumers above 55 years old and analyzed using the descriptive statistic analysis, MANOVA, and one-way ANOVA. The results showed that the elderly consumers' education level significantly influenced the decision making process in determining where to eat out. Significant differences were found in the Problem Recognition Step(Wilks' Lambda=0.817, F=2.991), Information Search Step(Wilks' Lambda=0828, F=2.218), Alternative Evaluation Step II(Wilks' Lambda=0.741, F=3.596), Purchase Decision Step(Wilks' Lambda=0.859, F=2.223), and the Post-Purchase Behavior(Wilks' Lambda=0.885, F=1.780). The higher education level was, the more directly involved in the eating out decision process. The elderly consumers with university education were likely to 'propose to eat out by themselves'(F=9.346), to obtain restaurant information from the 'printed materials'(F=7.452), to go to 'family restaurant'(F=9.057), 'Japanese restaurant'(F=8.7891) and 'fine dining restaurants'(F=3.936), and to directly express their emotion when they had complaints about restaurant service(F=3.206). In conclusion, older Koreans will become more healthy and wealthy which means the dining out activity will be an important part of their life to socialize with people. Therefore, food service operations should consider the elderly consumers' needs and expectation of restaurant services and actively position themselves for this new market segment.

우리나라 노인의 우울이 삶의 질 및 구강건강과의 관련성: 국민건강영양조사 제8기 1차년도(2019)자료 활용 (Relationship of depression with the quality of life and oral health ofthe elderly Koreans: data from the 8th National Health and Nutrition Examination Survey)

  • Background: This study was aimed at reducing depression and improving the quality of life and oral health of the elderly Koreans by elucidating the effect of depression on their life quality and oral health. Methods: Original data from the 8th National Health and Nutrition Survey (2019) were analyzed and implemented. The elderly (age≥65years) Koreans were enrolled. The oral health status was determined using the Decayed, Missing, Filled Teeth (DMFT) index and subjective self-report. The quality of life was quantified as the sum of score of each item in the Euro Qol-5 dimension (EQ-5D) and Health-related Quality of Life Instrument with 8 Items (HINT-8). The t-test, one-way analysis of variance, correlation analysis, and multiple regression analysis were performed to statistically compare the quality of life, oral health status, and oral health status according to the depression status. Results: EQ-5D and HINT-8 scores differed significantly with the history of depression, current depression, stress perception, depression for 2 weeks, and annual suicidal intention (p≤0.05). The DMFT index score differed significantly with the depression diagnosis and depression for 2 weeks (p≤0.05). EQ-5D, HINT-8, and DMFT index differed significantly with all oral health behavior variables (p≤ 0.05). Conclusions: The results of this study could serve as a basis to formulate oral health programs for the elderly to reduce depression and improve the quality of life.

노인들의 응급의료이용 결정요인과 형평성 (The determinants of Emergency Care Utilization and Equity of Access to Care in Elderly Koreans)

  • 본 연구의 목적은 노인들의 응급의료이용에 영향을 주는 요인들을 알아보고 의료접근의 형평성을 조사하는 데 있다. 2014년 한국의료패널조사 자료를 사용하여 기술분석과 로지스틱회귀분석이 행하여졌으며 인터뷰에 참여했던 1,313명의 노인들이 표본으로 선정되었다. 연령, 성별, 교육수준 등의 인구변수들이 응급의료이용의 중요한 결정요인이었다. 의료요구는 노인 계층 간 응급의료이용의 차이를 충분히 설명하지 못한 걸로 나타났으나 건강상태는 응급의료서비스를 사용하는 노인들의 중요한 결정요인이었다. 의료비지출은 응급의료서비스이용의 계층 간 차이를 개선하지 못했으나 응급의료이용의 중요한 예측요인으로 남았다. 한국에서 의료개혁은 의료요구를 가진 노인들이 효과적인 진료혜택을 받을 수 있도록 전국민 응급의료 보장을 확대하는 방향으로 계속 이루어져야 한다. 앞으로 연구도 75세 이상의 교령노인과 여성 및 교육수준이 낮은 노인, 그리고 높은 의료비 지출을 하는 노인과 같은 인구계층들에게 혹시 있으지 모르는 의료접근의 장애들을 허무는 방향으로 이루어질 필요가 있다.

Social Support and Quality of Life in Domestic Korean and Korean American Elderly People

  • Purpose: The aim of this study was to investigate the relationship between social support and quality of life (QOL) in elderly people and to examine potential cross-cultural differences. Methods: The author surveyed 82 elderly people, over the age of 65, residing in a nursing home in a Korean rural area and 73 Korean immigrants living in Houston Texas USA who were also elderly. The degree of social support was measured with 20 questions that were answered using a 5 grade scale. The quality of life was measured in 5 fields (mobility, self-care, usual activities, pain/discomfort, and anxiety/depression, each scored using a 3 grade scale) with EUROQOL EQ-5D. Results: The mean score of social support was 65.43 for domestic Korean elderly people (range: 26 to 98); it was 73.43 for elderly Korean Americans (range: 26 to 100). The score was lower for domestic Koreans despite the traditional environment (p<0.05). Regarding QOL, 61% of domestic Korean elderly people complained of pain/discomfort and 47.5% appeared to have anxiety/depression, 30.5% complained of limitations in mobility, 20.7% reported limitations in usual activities, and 14.6% had limitations in basic self.care activity such as washing or dressing. A similar proportion of Korean American elderly had limitations in usual activities (19.2%) and self care activity (9.5%). But significantly fewer complained of anxiety/depression (16.5%, p<0.01) or pain/discomfort (38.3%, p<0.01). The major differences in psychological domains of QOL may be related to the degree of social support and the difference in the socio.cultural environment. Conclusion: Social support is a major determinant of QOL, especially for psychological domains, and these relationships are under the influence of the socio.cultural environment.
