I. Background The problem of discharging patients from hospital have been well documented in the literature over the last 20 years. They included poor communication between hospital and community, inadequate notice of discharge, over-reliance on informal support and lack of statutory support, inattention to patients needs before leaving hospital, and wasted or duplicated visits by community nurses. Most patients discharged from hospital are able to return home with little or no support, while others will require a 'package of care' to support them back to good health. Patient with complex care needs, including the frail elderly and those with mental health problems, may require continuing care in special housing, residential, or nursing homes. With this population,effective discharge arrangement is needed and the study on this problem is urgent in Korea because the Medical Reform Project is on suspension of success. II. Results of the Study: 1. Discharge service needs assessed on 360 elderly patients who were hospitalized during the survey period at four university hospitals. Patients want to know the information on disease management after discharge. Follow-up telephone service is the most frequently checked service. 2. Multidisciplinary Discharge Planning is recommended at the hospital level to reduce the readmission and decrease the length of stay. 3. Further research is needed to validate and test the assumption of the solution which is developed in this research.
본 연구는 코로나19 장기화로 다양한 노인돌봄 방식의 변화가 제기되고 있어, 재가의 경증치매 및 신체허약노인을 대상으로 비대면 방식의 돌봄기술을 적용하였다. 본 연구의 목적은 AI 로봇 통합관리프로그램을 적용하여 인지기능, 일상생활활동, 우울의 정도를 비교하기 위한 비동등성 대조군 전·후 유사실험 연구이다. 자료수집은 2021년 6월 4일부터 9월 17일까지이며 실험군 17명, 대조군 18명 대상자의 설문결과를 SPSS 25.0으로 분석하였다. 연구결과, 실험군은 언어기능, 일상생활활동, 우울에서 유의하게 나타났다. 특히 중등도 이상 우울과 경증 우울이 감소되는 결과를 나타냈다. 인지기능은 장기요양등급과, 일상생활활동은 동거가족과 통계적으로 유의하게 나타났다. 따라서 '위드 코로나 시대'에 노인돌봄 현장에 비대면 방식의 돌봄기술을 도입한다면, 노인의 인지기능훈련 및 우울 감소에 기여할 것으로 사료된다.
The purpose of the study was to examine the current status of nutritional management at elderly nursing homes. A survey was performed of 83 nursing homes from January 5, 2011 to January 21, 2011 via mail. A total of 34 nursing homes responded to the survey and 149 elderly subjects were analyzed according to the presence of a dietitian. Among the 34 nursing homes, 70.6% had a dietitian on duty. All of the facilities with a dietitian had the dietitian making the meal plan, whereas 70% of the facilities without dietitian served meals planned by a non-professional person. Overall, however, a low proportion of nursing homes implemented dietetic treatments for residents with diseases. For the nutritional assessment of these residents, a mini nutritional assessment (MNA) was performed. MNA scores were significantly associated with body mass index (BMI), mid-arm circumference (MAC), calf circumference (CC), ingestion problems, and weight loss during the last 3 months (P<0.001). Among the elderly studied, 5.4% were malnourished, and 36.9% were at risk for malnutrition by MNA score. The results of this survey show that the current management of nutrition at nursing homes is insufficient because the elderly who needed dietetic treatment did not receive proper care. Dietetic management is the most important service in all nursing homes. Therefore, to improve the nutritional status of elderly residents in nursing homes, systematic nutrition management by nutrition experts should be implemented.
농촌지역의 과소화에 따라 의료서비스에 대한 공간적 접근성이 제한되고, 노령화에 따른 의료서비스의 수요가 급격히 증가함에 따라, 최근 방문보건서비스와 같은 서비스 제공자가 수요자에게 직접 이동하는 새로운 형태의 의료서비스가 많은 주목을 받고 있다. 이러한 의료환경의 변화는 공공 의료기관의 한정된 의료자원을 효율적으로 배분하고 운영하기 위한 혁신적이고 과학적인 접근법개발에 대한 요구를 증대시키고 있다. 이러한 배경하에서 본 연구의 목적은 방문 의료서비스 제공을 위한 구역 설정 모델을 수립하고, GIS 환경에서 구역 설정 모델을 구현하는 자동구획절차(AZP) 알고리즘을 개발하는 것이다. 구역 설정 모델은 구역 내 이동성, 구역간 업무량 균형, 연속성 등의 조건을 고려한다. 이동성에 대한 세 개의 서로 다른 목적함수를 평가한다; 1) 구역 내 단위 지역간 네트워크 거리의 합 최소화, 2) 구역 내 단위 지역간 공간상호작용 최대화, 3) 구역 내 단위 지역을 순회하는 경로의 길이 최소화. 모델을 위한 AZP는 GIS 환경에서 개발되었고, 농촌의 방문보건 사례에 적용되었다. 결과는 개발된 AZP를 업무량 균형과 연속성 제약조건하에서 각 목적함수에 대하여 상이한 구획체계를 산출할 수 있음을 보여준다.
Before introducing the national long-term care insurance in 2008, the want for long term care service has to be estimated and analysed. This study estimates the demand and analyses what determines the want of long term care service. This study investigated data of 3f6 elderlies, that was collected by age stratified random sampling. The elderies resided in Onyang 4 - dong (urban area) and Dogo-myun (rural area) In the city of Asan. The researchers visited the elderlies and their care giver, and assessed their demand for the long term care service and examined physical, mental, socio-economic status by the assessment tools for Korean Long-Term Care System. $64\%$ of the those who are entitled to be served refuse the long term care service. $26.7\%$ of them wants for home care service and $7.9\%$ want facility care service. It is estimated that the want of home care service are three or four times as much as that of facility care service. The demand for long term care service is 5.155 times higher for those who live in rural area (p=0.000), 3.040 times higher for those who do not have spouse(p=0.057), and 3.356 times higher for the people who is in medicaid than medical insurance(p=0.029). However, income(p=0.782), means(p=0.614), living alone(p=0.223), number of family to live with (p=0.341) and age of the elderly(p=0.420) are not related with the demand of long term care service. The assessment tools for Korean Long-Term Care System for need evaluation of the long term care service can reflect the demand well.(p=0.024) If medical care will cover $80\%$ of total cost, the willingness to pay of the out of pocket money of the people with medical insurance is 67,400 Korean Won(66.77 US$) for the home care service and 182,500 Korean Won(180.78 US$) for the facility care service. There is possibility that long term care demand is still small after Introducing the long term care Insurance due to the care given by family members. When developing service delivery system of long term care insurance, rural area has to be given more consideration than urban area because of the higher demand. The people who do not have spouse or are in medicaid have to be given special consideration as well.
Purpose: The purpose of this research was to examine the degree of care stress and the quality of life of grandmothers caring for their grandchildren while mother was employed. Method: A convenience sample of 107 grandmothers were measured for grandmother's care stress using a revised for of the PSI/SF and the quality of life tool by Andrew and Weinert. Descriptive statistics, t-test, and Pearson correlation coeffcients were used to analyze the data using SPSS/PC 10.0 Results: The results found that grandmothers' care stress was high according to their age and religion. Their quality of life was low according to their age, their present health status, income and sleep pattern. Their subjective care stress was a sense of social isolation, burden of caring for grandchildren, health problems, and conflict with adult children. Care stress and quality of life were found to be negatively correlated. This result was statistically significant. Conclusion: In conclusion, it was found that the care stress was high and affected quality of life. From this study it is proposed that there is a need to develop programs which consider grandmother' techniques in child care and health care to improve their quality of life. Ultimately, a good care environment for children will be good for the children's growth and development.
The purpose of this study was to clarify residential satisfaction of the Living Environment according to responses of staffs in the Horsing Homes. For this, the data about residential satisfaction of the Living Environment of elderly with dementia were taken from the staffs in 2 nursing homes in Tokyo by the questionnaire. Additional information was collected by interviews and observation. It was impossible that the elderly with dementia answered their satisfaction and needs of spaces. For this reason, staffs who care the elderly with dementia during 24 hours and interact with the elderly with dementia were selected the subjects of this survey. Survey question asked for the general features of the staffs, satisfaction, dissatisfactory spaces and the reasons about dissatisfaction of the elderly with dementia. The data were analyzed using the frequency, percentage, crosstab, paired t-test with SPSS 11 program. The result of this study was as follows. First, most of the elderly with dementia were unsatisfied with their bedrooms and bathrooms in bedrooms. Second, the unsatisfied spaces among the living environment were bedroom, bathroom in bedroom, dining room, day corner, special activity room, common bathroom with mechanical bath, and dressing room. The reasons of the unsatisfied spaces were size, comfort, non-existence of the space, and noise in the space.
고령자들이 증가하면서 고령자들을 위한 다양한 IT 홈 시스템에 대한 연구가 활발히 진행 중이다. 하지만 대부분의 시스템들이 위급한 상황이 발생할 경우 데이터를 전송하는 부분에 초점이 맞춰져 있으며 주거환경에서의 에너지 저감 부분을 고려한 시스템은 찾아보기 쉽지 않다. 사실, 기존의 고령자 주거를 위한 시스템들은 시스템의 기능 수행을 위하여 오히려 더 많은 전력의 소비를 촉진한다. 본 논문에서는 에너지 저감기능을 포함한 위험상황을 모니터링 시스템을 연구 및 구현 한다. 본 시스템을 통하여 위급상황을 모니터링 하면서도 저전력 주거환경을 현실화함으로써 고령자 사회와 그린 사회의 두가지 미래 지향적인 요소를 만족할 수 있을 것이다.
The number of residents in elderly institution has been increasing due to the change of the family support system. This study was focused on understanding the process of adjustment of the institutionalized elderly using the Grounded Theory approach. Method: There were seven participants, 4 men and 3 women living in 3 different elderly facilities. The data was collected through in-depth interviews and participant observation from June 20, 1999 to January 10, 2000 and analyzed by the Strauss and Corbin's analysis method. Result: 125 concepts were found and grouped into 30 sub-categories and then grouped into 13 categories. These categories are , , , , , , , , , , , and , which were synthesized into the process of adjustment. being the core category. The adjustment process of the facility elderly consisted of : 1. expressive phase of 'reluctance' 2. control phase of 'reluctance' 3. latent phase of 'reluctance' Conclusion: This study offers better understandings on the adjustment process of the institutionalized elderly and provides more appropriate nursing care to the New Comers of these facilities.
The purpose of this study is to examine demographic characteristics, psychological characteristics and patterns of supportive relationships with their children that affect the successful lives of the elderly. A survey was conducted of 410 people aged between 65 and 85, and 405 samples were used for the analysis. First, the supportive relation between the elderly and their children was categorized into four groups depending on the total amount and direction of support. The four groups consisted of the elderly who exchanged little or no support, those who only received support from their children, those who only gave support, and those who both gave and received support. Second, the more the elderly exchanged support and gave emotional support to their children, the better the chance that the elderly would lead a successful life in later years. Third, subjective health conditions most significantly affected the successful lives of the elderly who exchanged little or no support. The level of education was most significant for the elderly who only received support. The level of housing satisfaction mostly affected the elderly who only gave support. Mutual support primarily affected the elderly who both gave and received. Thus, we should provide health programs for the elderly who exchange little or no support to encourage a healthier lifestyle. In addition, opportunities for continuing education should be provided for the elderly who only receive support to make up any deficiency in education. Because spouses are mostly affected for those elderly who exchange little or no support and the elderly who only receive support, we should provide educational programs about how to be better lifetime companions to each other and also provide medical and in-home care for the remaining spouse even after the other partner is gone. Because housing satisfaction greatly affects the elderly who only give support, improvements in the residential environment are essential. Since the level of mutual support is crucial to the elderly who both give and receive support, programs on the development of human resources are needed for the elderly to interact dynamically and intimately with their children.
